RESUMO

Fibrocystic changes are commonly seen in clinically symptomatic patients and during imaging workup of screening-detected findings. The term "fibrocystic changes" encompasses a broad spectrum of specific benign pathologic entities. Recognition of classically benign findings of fibrocystic changes, including cysts and layering calcifications, can prevent unnecessary follow-ups and biopsies. Imaging findings such as solid masses, nonlayering calcifications, and architectural distortion may require core needle biopsy for diagnosis. In these cases, understanding the varied appearances of fibrocystic change aids determination of radiologic-pathologic concordance. Management of fibrocystic change is typically conservative.

RESUMO

Adenosis is a benign breast condition whose lesions can mimic breast carcinoma and is evaluated for malignancy with the Breast Imaging-Reporting and Data System (BI-RADS). We construct and validate the performance of modality-specific enhancement (MSE)-Breast Net based on multimodal ultrasound images and compare it to the BI-RADS in differentiating adenosis from breast cancer. A total of 179 patients with breast carcinoma and 229 patients with adenosis were included in this retrospective, two-institution study, then divided into a training cohort (institution I, n = 292) and a validation cohort (institution II, n = 116). In the training cohort, the final model had a significantly greater AUC (0.82; P < 0.05) than B-mode-based model (0.69, 95% CI [0.49-0.90]). In the validation cohort, the AUC of the final model was 0.81, greater than that of the BI-RADS (0.75, P < 0.05). The multimodal model outperformed the individual and bimodal models, reaching a significantly greater AUC of 0.87 (95% CI = 0.69-1.0) (P < 0.05). MSE-Breast Net, based on multimodal ultrasound images, exhibited better diagnostic performance than the BI-RADS in differentiating adenosis from breast cancer and may contribute to clinical diagnosis and treatment.

Presentamos un caso de una mujer de 33 años que consultó por una tumoración palpable y dolorosa en mama izquierda de 3 años de evolución. El diagnóstico se realizó de forma casual por el hallazgo quirúrgico macroscópicamente compatible con un quiste hidatídico de mama. La equinococosis quística aislada de mama es una entidad muy rara. Es un reto lograr un diagnóstico preoperatorio y diferenciarla de otras lesiones mamarias. Hay muy pocos casos publicados y en la mayoría, el diagnóstico de certeza se consigue tras la cirugía, dado que resulta muy complicado sospechar esta patología, únicamente, en base a la exploración clínica y la radiología


We report the case of a 33-year old patient who presented a palpable lump in her left breast for three years. The diagnosis was made by chance by the surgical finding macroscopically compatible with a breast hydatid cyst. Cystic echinococcosis of the breast is a very uncommon disease. It is a challenge to achieve a preoperative diagnosis and to differentiate it from other breast lesions. There are limited reported cases in the literature, and in majority of them the certain diagnosis is obtained postoperatively, since it is complicated to suspect this pathology based solely on clinical examination and radiological investigations

RESUMO

La mastopatía diabética es una entidad benigna y poco frecuente. Se asocia a diabetes mellitus tipo i mal controlada y de larga evolución, se manifiesta clínica y radiológicamente como un nódulo mamario, no pudiendo diferenciarlo de la enfermedad oncológica por lo que el estudio histológico de la lesión es fundamental. Presentamos 3 casos clínicos en nuestra unidad y realizamos una revisión de la literatura


Diabetic mastopathy is a benign and infrequent entity. It is associated with long-term and poorly controlled diabetes mellitus type I, manifested clinically and radiologically as a breast nodule that cannot be differentiated from oncological disease. Consequently, histological study of the lesion is essential. We present 3 cases of this entity managed in our unit and provide a review of the literature

La mastopatía diabética es una entidad infrecuente. La presentación más común suele ser en forma de nódulo único o múltiple. Sus características clínicas y radiológicas pueden simular un cáncer de mama, por lo que debe tenerse en cuenta este diagnóstico diferencial en las pacientes jóvenes y con antecedentes de diabetes mellitus. Un correcto diagnóstico puede evitar tratamientos quirúrgicos innecesarios. Presentamos 2 casos que hemos diagnosticado en nuestra área sanitaria así como la revisión de la bibliografía al respecto


Diabetic mastopathy is an uncommon disorder, in which the most frequent presentation is as a single or multiple nodules. It can imitate breast cancer both clinically and radiologically. A differential diagnosis should be done in young patients with a personal history of diabetes, in order to avoid unnecessary surgical interventions. Two cases are presented that were diagnosed in our health area, as well as a review of the literature on this pathology
