RESUMO

Despite significant growth in fiscal expenditure, the overall level of public services in China remains inadequate. One approach to improving government public service efficiency from the perspective of management psychology is to strengthen government competition. However, only a few studies have explored the improvement of public service efficiency through government competition, with even fewer addressing the phenomena of market segmentation and spatial effects that accompany the process of government competition. This paper aims to fill this research gap by examining the effects of government competition and market segmentation on public service efficiency, as well as their spatial disparities. We initially employs the DEA method to assess the efficiency of public services based on inputs and outputs, and examines its spatial variations. Subsequently, a semi-parametric spatial lag panel model is utilized to validate the effects of market segmentation and government competition on public service efficiency. Our findings indicate that inter-provincial market segmentation leads to a decline in public service efficiency. Moreover, the influence of horizontal competition between local governments on public service efficiency varies depending on the degree of positive and negative effects in their competition dynamics. The impact of vertical competition between central and local governments on public service efficiency is influenced by the degree of fiscal decentralization. When the level of fiscal decentralization is below 0.808, vertical competition between central and local governments has a promoting effect on public service efficiency. However, when the degree of fiscal decentralization exceeds 0.08, this promoting effect weakens and gradually transforms into a negative influence. The insights and evidence provided by this study offer valuable guidance for for effectively reshaping the fiscal relations between the central and local governments in China and improving public service efficiency in the context of a new round of fiscal and tax system reforms.

BACKGROUND: Cardiovascular diseases (CVD) have significant health and economic burdens. In South America, the loss of productivity related to these diseases has not yet been well explored. OBJECTIVE: Estimate the potentially productive years of life lost (PPYLL) and loss of productivity related to premature mortality associated with CVD in South America, in 2019. METHODS: Mortality data available from the 2019 Global Burden of Disease Study were used to estimate the burden of disease attributable to CVD. For monetary calculations of productivity loss, a proxy of the human capital approach was used. Data were stratified by sex, in working age groups. RESULTS: The total number of deaths due to CVD in South America in 2019 was 754,324, and the total number of PPYLL was 2,040,973. The total permanent loss of productivity was approximately US$ 3.7 billion and US$ 7.8 billion in purchasing power parity, equivalent to 0.11% of the gross domestic product. The cost per death was US$ 22,904, and the ratio between men and women for the cost per death was 1.45. The variation in scenarios indicates that the estimates are robust, even with important differences between countries. CONCLUSIONS: CVD impose a significant economic burden on countries in South America. The characterization of this burden can support governments in the allocation of resources for the planning and execution of health policies and interventions in promotion, prevention, and recovery.


FUNDAMENTO: As doenças cardiovasculares (DCV) têm ônus sanitário e econômico significativos. Na América do Sul (AS), a perda de produtividade relacionada a estas enfermidades ainda não foi bem explorada. OBJETIVO: Estimar os anos de vida produtiva perdidos (AVPP) e a perda de produtividade relacionados a mortalidade prematura associada as DCV na AS, em 2019. MÉTODOS: Empregou-se dados de mortalidade disponíveis no Global Burden of Disease Study 2019 na estimativa da carga de doença atribuível a DCV. Para os cálculos monetários da perda da produtividade usou-se uma proxy da abordagem de capital humano. Estratificou-se por sexo, nas faixas etárias de trabalho. RESULTADOS: O número total de mortes por DCV na AS no ano de 2019 foi de 754.324 e os AVPP foram 2.040.973. A perda permanente de produtividade total foi de aproximadamente US$ 3,7 bilhões e US$ 7,8 bilhões em paridade do poder de compra, equivalente a 0,11% do produto interno bruto. O custo por morte foi de US$ 22.904, e a razão desse custo por óbito, entre homens e mulheres foi 1,45. A variação dos cenários aponta robustez nas estimativas, mesmo com diferenças importantes entre os países. CONCLUSÕES: As DCV impõem um ônus econômico significativo a este bloco de países. A caracterização deste fardo pode amparar os governos na alocação de recursos destinados ao planejamento e execução de políticas e intervenções sanitárias, sejam de promoção, prevenção ou recuperação.

As an essential material basis and power source for economic and social development, Western China's low energy use efficiency has hindered its sustainable economic development. This study aims to evaluate the total factor energy efficiency of the region and identify its influencing factors. A three-stage DEA model was used to measure the efficiency of 11 provinces from 2006 to 2021, and the Tobit model was employed to investigate internal factors. The findings show that (i) external environmental factors and stochastic perturbations have a significant impact on TFEE in the western region, overestimating integrated efficiency and scale efficiency and underestimating pure technical efficiency. (ii) the study of external influencing factors finds that the level of economic development increases input redundancy; the industrial structure increases capital input and labor input redundancy while decreasing energy input redundancy; and the energy consumption structure increases capital input and energy input redundancy while decreasing labor input redundancy. (iii) the study of internal influencing factors finds that the level of scientific and technological innovation, the level of openness to the outside world, and the TFEE have a positive correlation. In contrast, the intensity of environmental regulation has a negative correlation.

Ports are critical centers of international trade and global logistics now that economic globalization has taken hold. The efficiency performance of port logistics is crucial to building an emerging pattern of development in which domestic and foreign dual cycles are complementary for China. This paper examines the efficiency performance of 19 ports within five major economic circles in China. It explores how their efficiency is distributed, and the configurations of efficiency improvement during the new normal of China's economy. First, the DEA-BCC model is employed to calculate the technical efficiency performance and distribution of each port from 2011 to 2020. Then, fuzzy-set qualitative comparative analysis (fsQCA) was applied to integrate and analyze the influencing factors. The results show as follows: (1) Each port group performed differently on efficiency rankings, as well as regional distributions. Among these, the port groups of the Bohai Rim region, the Yangtze River Delta region, and the Bohai Rim region continue to rank highly. (2) From the perspective of configuration analysis, the results suggest that government support is not necessary for port logistics with better economic endowments. However, it is critical for backward ones. (3) A rational industrial structure can enhance levels of infrastructure, openness, and information technology, improving port performance. The findings can provide theoretical and practical references for better promoting the development of Chinese port management.al references for better promoting the development of Chinese port management.

Judicial improvement and trade structure optimization are important elements in achieving a high level of openness. This paper develops a theoretical framework on the impact of local protectionism on market integration and export technical complexity, and then empirically analyses the effect of judicial improvement on the technical complexity of urban product exports and its mechanism of action based on a quasi-natural experiment with the establishment of circuit courts. The study finds that the establishment of circuit courts significantly increases the technical complexity of product exports, and the findings remain valid after a series of robustness tests. Further analysis shows that the technology enhancement effect of the establishment of circuit courts is more pronounced in the group of cities with higher administrative levels, better location advantages, and less advanced rule of law. Mechanism Analysis shows that a superior institutional environment can have a two-sided effect on different types of firms. Overall, however, the productivity-enhancing effect of low-productivity firms exiting the market is lower than the productivity-reducing effect of higher-productivity firms' increased compliance costs, which in turn weakens the positive promotional effect of circuit courts. The above theoretical and empirical findings provide policy implications for maximizing the technology enhancement effect of the establishment of circuit courts on product exports, thereby promoting a high level of openness in China.

Urbanization leads to dramatic changes in habitat quality, which significantly affects population health. Research on the coupling coordination relationship between new urbanization and health production efficiency is conducive to improving residents' well-being and urban sustainable development. In this article, we adopted the super-efficient SBM model and entropy value method separately to evaluate the spatiotemporal variation characteristics of health production efficiency and new urbanization in China. Then, we used the coupling coordination degree model to investigate the interactive coercing relationship between new urbanization and health production efficiency. Finally, the panel Tobit model is used to analyze the factors influencing the coupled coordination of the two systems. The results showed that the new urbanization levels of 31 provinces in China have all steadily increased from 2003 to 2018. Health production efficiency exhibited a fluctuating but increasing trend, and its regional differences are gradually narrowing. Health production efficiency and new urbanization have developed in a more coordinated direction, with a spatial pattern of "high in the southeast and low in the northwest." Meanwhile, the relative development characteristics between the two systems have constantly changed, from the new urbanization lagged type to the two systems synchronized type and the health production efficiency lagged type. Population density, economic development level, government financial investment, and government health investment positively impact the coupling coordination degree of the two systems. In comparison, individual health investment harms the harmonization of the two systems.

BACKGROUND: Psoriasis is a prevalent, chronic skin disease with a potential impact on work productivity, medical consumption costs, and quality of life. The influence of the extent of skin lesions on these outcomes is not well known. OBJECTIVE: We determined associations of self-reported skin lesions with self-reported work productivity, medical consumption costs, and health-related quality of life in respondents with psoriasis. METHODS: In this cross-sectional study, we included respondents with self-reported psoriasis in the Netherlands in an online questionnaire. We assessed the self-reported percentage body surface area (BSA) of psoriasis lesions. We used validated instruments to assess work productivity (WPAI-PsO), medical consumption costs (iMCQ), and health-related quality of life (EQ-5D-5L and the DLQI). We used ordinal logistic regression to associate BSA categories >1% versus 0-1% with outcomes adjusted for multiple confounders. RESULTS: We included 501 respondents with a mean age of 43 ± 12 years; 64% were men. Median BSA was 2% (interquartile range 1-5%). A higher BSA was associated with higher overall work impairment due to psoriasis (common odds ratio [cOR] 2.44, 95% confidence interval [CI] 1.40-4.29; n = 205), higher medical consumption costs (cOR 2.06, 95% CI 1.45-2.94) and lower health-related quality of life. Associations were strongest with a BSA cutoff of 0% or 1% compared to 2% or higher categories. DISCUSSION: In our study, having few to no lesions in psoriasis was associated with lower overall work impairment due to psoriasis, lower medical consumption costs, and higher health-related quality of life.

This paper utilizes an improved undesirable output DEA model to measure the eco-efficiency of cities in five major urban agglomerations in China during 2006-2020. It employs the Theil Index and Geodetector to investigate the spatial-temporal distribution differentiation characteristics and driving factors of urban eco-efficiency. The main findings are as follows. Firstly, the eco-efficiency of all urban agglomerations showed a fluctuating upward trend, but the eco-efficiency performance of different urban agglomerations in China shows a stratification characteristic. Specifically, the Pearl River Delta urban agglomeration consistently ranks first in China, while the mean values of the Yangtze River Delta urban agglomeration, Beijing-Tianjin-Hebei urban agglomeration, and Chengdu-Chongqing urban agglomeration are lower than the national average. Secondly, the overall differences in the urban eco-efficiency of all sample cities show a consistently fluctuating downward trend. The factor that affects the level differences of eco-efficiency in different cities is the intra-regional differences. Last but not least, the top three factors affecting the spatial distribution difference of urban eco-efficiency in the whole sample are environmental pollution control investments, innovation level, and environmental infrastructure investments. In the end, this paper proposes that reducing the intra-regional differences is the primary task to achieve the coordinated improvement of urban eco-efficiency in urban agglomerations, and then puts forward some policy suggestions to improve eco-efficiency for the five major urban agglomerations.

As digital economy develops, its impact on green innovation and energy efficiency has become the focus of current research. To explore the impact of the current development of the digital economy on the energy industry, this paper selects the parameter of green innovation resilience, analyzes the impact mechanism of green innovation resilience on energy efficiency under the shock of digital economic development, and uses relevant data from 284 cities in China from 2011 to 2019 for empirical testing. It is found that: green innovation resilience promotes energy efficiency; low level of green innovation resilience inhibits the improvement of energy efficiency, while high level of resilience promotes energy efficiency; the initial stage of digital economic development generates resource grabbing and the effect of technological constraints, which weakens the role of green innovation resilience in promoting energy efficiency. The results indicate that the resilience of the green innovation system should be strengthened in order to fully tap the potential for promoting energy efficiency; the policy orientation of "digital greening-energy efficient" should be pursued in the development of digital economy; the rational allocation of resources and the implementation of green standards should be strengthened in the process of digital economic development; and the constraints on energy efficiency improvement in the early stage of digital economic development should be broken through by accelerating the digitalization process.

China is currently confronted with the dilemma of achieving its green development goals while maintaining economic growth. The National Ecological Demonstration Zones (NEDZs) represent an innovative attempt by local governments to balance economic development with ecological civilisation, potentially offering a solution to this issue. This study calculated the Green Total Factor Productivity (GTFP) for 1925 districts and counties from 1999 to 2018. Using the selection of NEDZs as a quasi-natural experiment, a difference-in-differences model was employed to empirically analyse the impact of NEDZs on GTFP. The results indicated that the establishment of NEDZs led to an average increase in GTFP of 0.2175 compared to the control group. The primary mechanisms involved are innovation, structure, and enforcement. Moreover, the green development effects of the NEDZs exhibited regional heterogeneity. Compared to the western regions, the central and eastern regions showed limited green development effects. Areas with smaller populations, lower human capital, and lower administrative levels demonstrated significant improvements after the implementation of the NEDZ. Further analysis reveals a significant spatial agglomeration pattern of GTFP and the NEDZs exert a strong 'siphon effect' on the GTFP of neighbouring areas. This study provides a new perspective on the GTFP research and offers theoretical and practical evidence for assessing the impact of NEDZs.

Rapid urbanization and industrialization promote economic growth as well as bring carbon emissions, which seriously threaten the eco-environment and socioeconomic sustainable development. Facing increasing resource constraints, improving carbon emissions efficiency (CEE) is conducive to promote coordinated development of economy and environmental protection. In recent years, regional economic integration (REI) has rapidly developed. It can not only promote factors flow between regions but also achieve industrial and economic agglomeration. However, few studies have been reported in the literature about the relationship between the REI and CEE. In this study, we first illustrate how the REI influences CEE in theory, then take the Yangtze River Delta (YRD) as a case study to conduct empirical research. The results show that (1) the overall CEE value in the YRD has exhibited an upward trend from 2000 to 2020, and its spatial distribution has revealed a significant auto-correlation pattern. (2) On the whole, the REI act a noteworthy positive impact on CEE. When considering types of cities, it is found to have significant positive impacts for the CEE in economically developed cities, while it exhibits a negative impact in the less-developed ones. (3) Upgrading industrial structure and increasing per capita GDP can promote the CEE, but hinder its growth in surrounding areas. Our findings suggest that the government should formulate a unified overall plan to facilitate REI development and establish a modern industrial system of clean and low-carbon to promote regional sustainable development.

The paper examines how digital finance affects energy efficiency in China using a dynamic panel model and data from 282 cities between 2011 and 2019. The study is based on the hypothesis which is related with digital finance, environmental regulation, and energy efficiency. The results indicate that: (1) Digital finance significantly improves energy efficiency, and this finding is consistent after several tests; (2) Digital finance has a positive effect on energy efficiency in non-resource-based cities, recession and regeneration resource-based cities, and old industrial base cities, but no significant effect on energy efficiency in growth and maturity resource-based cities and non-old industrial base cities; (3) Environmental regulation positively influences how digital finance affects energy efficiency; (4) The impact of digital finance on energy efficiency depends on the degree and tools of environmental regulation. This research offers valuable insights to local governments in China for promoting financial digitization and enhancing energy efficiency.

Previous research has yielded mixed conclusions regarding whether business environment (BE) optimization can enhance carbon emission efficiency (CEE). This study delves into the impact of the BE on CEE using panel data from 30 provinces in China, employing fixed effect, quantile, and mediated effect models. It innovates in three key areas: research perspective, mechanism of action, and heterogeneity analysis. The research found that the BE optimization enhances CEE. Meanwhile, the influence of the BE on CEE exhibits marginal decreasing characteristics. The mechanism analysis reveals that the BE enhances CEE through the industrial structure optimization effect and the progress of green technology, while it diminishes efficiency through the energy rebound effect. Heterogeneity analysis indicates that BE optimization has a stronger impact on improving CEE in provinces with robust government governance, younger governors, and highly educated officials. The policy implication suggests that local governments should continually optimize the BE, enhance government governance capacity, and prioritize the appointment of young and highly educated officials.

The traditional supply-side-driven e-commerce modes provide consumers with massive amounts of structured information about goods and services. Due to the lack of a tailor-made ability to describe, explain, and accurately understand unstructured service demands, existing technologies and service modes still struggle to fully explore, articulate, and meet the personalized, high-quality, and urgent service demands of the disadvantaged groups in e-commerce. This paper innovatively proposes the concept of "integrated services on demand" and develops its C2B2C2B2C-based "Qbnb" e-commerce mode, operation mechanism and intermediary platform architecture. After conducting exploratory operation training, it demonstrates that the Qbnb mode can effectively achieve centralized allocation of idle social surplus service capacity to cater to the needs of disadvantaged groups in e-commerce. Compared to the traditional e-commerce service mode, Qbnb will significantly expand service demand and capacity, achieve higher service efficiency, and create a broader service consumption market, resulting in many more job opportunities.

Scientists are enthusiastically imagining ways in which artificial intelligence (AI) tools might improve research. Why are AI tools so attractive and what are the risks of implementing them across the research pipeline? Here we develop a taxonomy of scientists' visions for AI, observing that their appeal comes from promises to improve productivity and objectivity by overcoming human shortcomings. But proposed AI solutions can also exploit our cognitive limitations, making us vulnerable to illusions of understanding in which we believe we understand more about the world than we actually do. Such illusions obscure the scientific community's ability to see the formation of scientific monocultures, in which some types of methods, questions and viewpoints come to dominate alternative approaches, making science less innovative and more vulnerable to errors. The proliferation of AI tools in science risks introducing a phase of scientific enquiry in which we produce more but understand less. By analysing the appeal of these tools, we provide a framework for advancing discussions of responsible knowledge production in the age of AI.

The development of information technology has created conducive conditions for the digital economy. The digital economy is regarded as a critical pathway for transforming traditional economic models. Green total factor productivity serves as an indicator for assessing the quality of economic development. During pivotal periods of economic transition, the digital economy and green total factor productivity have emerged as two prominent themes for achieving sustainable economic development. But the impact of digital economy on green total factor productivity is less discussed. Innovation environment refers to a confluence of conditions shaped by factors such as talent, funding, cultural atmosphere and government policies, all of which collectively support innovative activities within a region. The institutional environment encompasses the aggregate of economic, political, social, and legal rules. Currently, there is little discussion on bringing innovation environment and institutional environment into the impact of digital economy on green total factor productivity. To fill the research gap, this paper adopts the Slack based measure-Directional distance function model and Malmquist-Luenberger productivity index to measure green total factor productivity in each region based on the panel data collected from 30 provinces in China from 2004 to 2019. Generalized Method of Moments method is constructed to carry out an empirical study on the impact of digital economy on green total factor productivity. This paper constructs a panel threshold model with innovation environment and institutional environment as threshold variables. In further analysis, this paper employs panel quantile regression for the empirical analysis of the impact of the digital economy on green total factor productivity. Further analysis elucidates the evident disparities in the influence of the digital economy on green total factor productivity at various levels. The research results can provide a guide for discussing the green value of the digital economy and its role in fostering the development of a green economy.

BACKGROUND: Various professional groups are involved in the daily work of the central operating room with the aim of providing the best possible treatment for each individual using modern medical technology (sociotechnical system) in a cost-effective manner. Ensuring perioperative patient safety is of particular importance. At the same time, the efficient use of the central operating room is essential for the economic success of a hospital. Preoperative preparation is a complex process with many substeps that are often difficult to manage. Historically, the focus has been on retrospective learning from errors and incidents. More recent approaches take a systemic view. A central idea is to consider the mostly positive course of treatment and the adjustments to daily work that are currently required by the people involved (Safety-II). By taking greater account of how the many components of the system interact, processes can be better understood and specific measures derived. This strengthens the system's ability to adapt to changes and disturbances, thus ensuring that goals are achieved. The functional resonance analysis method (FRAM) is an internationally recognized method for modelling work as done compared to work as imagined. This paper presents the application of FRAM to preoperative preparation in a major regional hospital. OBJECTIVE: Is FRAM suitable for improving process understanding in preoperative preparation? MATERIAL AND METHODS: An interdisciplinary project team identified relevant functions of preoperative preparation through document analysis and walkthroughs. Based on this, more than 30 guided interviews were conducted with functionaries. The results were presented graphically and specific information, such as safety-related statements or reasons for the variability of functions, were also presented textually. In the next phase, statements were evaluated and compared with the target model and the job descriptions. RESULTS: The FRAM revealed the process as a complex network of relationships. During the modelling process, a varying degree of centrality and variability of certain functions became apparent. From the observations, the project team selected those with high relevance for patient safety and for the efficiency of the overall process in order to prioritize starting points for deriving measures to increase resilience. These starting points relate either to single functions, such as surgical site marking or to multiple functions that are variable in their execution, such as delays due to nonsynchronized duty times. CONCLUSION: The FRAM conducted provides valuable new insights into the functioning of complex sociotechnical systems that go far beyond classical linear methods. The awareness of operational processes gained and the resulting dynamic view of interactions within the system enable specific measures to be derived that promote resilient behavior and reduce critical variability, thus contributing to increased patient safety and efficiency.

With increasing internal and external risks to the WEF system, a single emphasis on efficiency or a lopsided pursuit of resilience can lead to difficulties in adapting to complex changes and resource redundancy. Revealing the synergistic evolutionary characteristics between efficiency and resilience of the WEF system is an effective method to deal with systemic internal and external risks. However, the current study of the WEF system lacks a synergistic perspective on resilience and efficiency. Thus, taking Chengdu-Chongqing Economic Circle (CCEC) as the research object and its geospatial boundary as the system boundary, this paper adopted the entropy-topsis model to evaluate the WEF resilience, and applied the super-efficient SBM model to measure the WEF efficiency accurately, which fully considered the non-expected outputs in the process of resource utilization. Then, applying the development coordination degree model, the synergistic relationship between the two was measured. The results indicated that: the average value of WEF resilience in CCEC increased from 0.414 to 0.485 and showed spatial characteristics of west>east>central. The WEF efficiency interval was 0.79-0.93, and cities with average WEF efficiency reaching the effective production frontier accounted for only 37.5%. The clustered distribution of the synergy levels intensified. The number of cities with primary, medium, more advanced, and advanced levels was 6, 6, 1, and 3, respectively, with primary and medium synergy levels dominating. The findings suggest that cities should strengthen regional exchanges and formulate targeted measures based on their own situations. In addition, CCEC should possess a comprehensive understanding of the interdependencies and conflicts that arise between resilience and efficiency throughout the decision-making procedure.

The population of people in Japan over 65 years old is expected to exceed 30% by 2025. As the society ages, there are not only healthy workers and employers, but also an increasing number of diseased or injured workers. Falls, the most common occupational hazard, increase in incidence with age. The management of osteoporosis and prevention of bone fractures from falls are emerging in elderly female workers, to reduce the loss of work productivity. Rheumatoid arthritis is a representative musculoskeletal disease that causes functional decline because of joint damage mainly in working women, but appropriate treatment improves disease activity and work productivity in workers with rheumatoid arthritis. It is also important not only to digitize subjective information by converting it into digital form (digitization), but also to digitalize the physiological information related to health, labor and disease (digitalization). In the future, artificial intelligence (AI) will be able to analyze vast amounts of physiological information (big data) obtained from workers and patients via the Internet of things (IoT), which will improve the information value linked to health promotion and optimal treatment practices, and contribute to the Digital transformation (DX).
