RESUMO

El dolor abdominal es un cuadro de consulta muy frecuente en la edad pediátrica, tanto en atención primaria como en atención especializada, e incluye los servicios de urgencias. En los casos en que el dolor se focaliza en la fosa iliaca derecha, el primer diagnóstico que hay que descartar es la apendicitis aguda. Además, destacamos la importancia de la ecografía para el diagnóstico de dolor abdominal. Presentamos un caso de dolor abdominal localizado en la fosa iliaca derecha en el que, tras descartar una apendicitis aguda mediante ecografía, se descubrió una infestación por Ascaris lumbricoides, entidad de dolor abdominal infrecuente en nuestro medio (AU)


Abdominal pain is a usual reason for consultation in pediatric, both in primary care and hospital, including emergency service. When the pain is located in right lower quadrant, the first diagnosis to discard is acute appendicitis. Besides, the relevance of ultrasonography for diagnosis of abdominal pain is emphasized. We report a case of abdominal pain located in right lower quadrant, where, after discarding acute appendicitis with an abdominal ultrasonography, we discover an Ascaris lumbricoides infestation, which is an unusual cause of abdominal pain in our country (AU)

Background: Helminth infections and allergies are diseases with intense Th2 lymphocytes participation and characterised by a high IgE and Interleukin-(IL) IL-4, IL-5 production and eosinophilia. However, helminths also induce IL-10 production, which may alter the outcome of allergic diseases in infected patients. Objective: This experimental study analyses the relationship between IL-10 production by cell culture from geohelminth infected and non-infected children and specific IgE to Ascaris lumbricoides (Asc) or Blomia tropicalis (BT).MethodsIL-10 content in supernatant from peripheral blood mononuclear cell culture from nine helminth infected and eleven non-infected patients was determined by ELISA after in vitro stimulation with Asc or BT extracts. Results: A positive association was observed between total IgE levels and anti-Ascaris and anti-Blomia tropicalis specific IgE, independent of infection status. For both helminth-infected and non-infected groups, there was no difference in IL-10 production in response to Asc extract, even though anti-Ascaris IgE levels were higher in the latter group. In response to BT stimulus, a lower production of IL-10 by the geohelminth-infected group was observed, but with no relationship between IL-10 production and specific IgE to BT. Conclusion: The results suggest that anti-Ascaris IgE in non-infected patients may be associated to a resistance to parasites. Levels of specific IgE to parasite antigens or B. tropicalis allergen were not impaired by IL-10 production in children from an urban area in which geohelminthiasis is endemic (AU)

RESUMO

La perforación intestinal y la peritonitis secundaria causada por Ascaris lumbricoides, aunque es rara en nuestro medio, debe tenerse en cuenta en los pacientes con clínica compatible procedentes de áreas endémicas. Presentamos el caso de una niña rumana con un cuadro clínico de fiebre, vómitos y dolor abdominal de 15 días de evolución, con colecciones abdominales observadas en las pruebas de imagen, en las que se aprecia una infestación por A. lumbricoides tras la cirugía laparoscópica (AU)


Intestinal perforation with secondary peritonitis caused by Ascaris lumbricoides is rare in our environment. This entity should be taken into consideration in patients who come from endemic areas, and who have suggestive clinical manifestations. We report a case of a Rumanian girl with 15 days of fever, stomach pains and vomiting. Imaging studies evidenced abdominal collections in which Ascaris lumbricoides infestation was found through laparoscopic surgery (AU)

The pathological effects that ascaris producesin man manifest in different locations, in accordance with the location of its different evolutionaryforms. Biliary ascaris is one of the severest complications. We present a case of acute pancreatitis produced by Ascaris infestation

Ascaris lumbricoides es el mayor nematodo intestinal, parásito de la especie humana, y llega a alcanzar 40 cm. Ascaris lumbricoides se encuentra ampliamente distribuido por las regiones tropicales y subtropicales, y se calcula que en el mundo están infectadas hasta 1.000 millones de personas. Casi todas las personas infectadas presentan cantidades pequeñas de gusanos y están asintomáticas. La enfermedad clínica surge por hipersensibilidad pulmonar y complicaciones intestinales. La presencia de parasitosis debe considerarse en el diagnóstico diferencial de cuadros de vómitos y otros síntomas digestivos, especialmente en gestantes procedentes de áreas endémicas. Así, nosotros presentamos un caso clínico de una gestante de 13 semanas, natural de Ecuador y residente en España desde hacía 3 meses, que presentó un cuadro de vómitos incoercibles que resultó deberse a una infección por Ascaris lumbricoides. Se instauró tratamiento para erradicar esta infección


Ascaris lumbricoides is the largest intestinal nematode and can reach up to 40 cm. It is a human parasite. A lumbricoides is widespread in tropical and subtropical regions with an estimated worldwide prevalence of over one thousand million people. Nearly all infected individuals present few worms and are asymptomatic. Symptoms occur when pulmonary hypersensitivity or intestinal complications appear. The presence of parasitosis should be considered in the differential diagnosis of cases of incoercible vomiting and other gastrointestinal symptoms, especially in pregnant women from endemic areas. We present a case of A lumbricoides in a 13-week pregnant woman from Ecuador who had been resident in Spain for 3 months and who presented incoercible vomiting. She received antiparasitic therapy with favorable outcome
