RESUMO

The story of COVID-19 at Montefiore Health System is not really a story about a virus or a pandemic. It is a story about innovation-accelerating innovation. Indeed, the COVID-19 crisis has spurred innovation at healthcare organizations across the country, helping them respond to unprecedented times. The pandemic radically disrupted the entire healthcare sector as demand and supply shocks simultaneously affected established processes and supply chains. Similarly, patients' demands for healthcare drastically changed. Nothing could be taken for granted-not even the health of employees or their ability to come to provide care. In this new reality, digital innovation is fundamental for healthcare organizations as they make their transition into recovery. The Montefiore experience exemplifies how the COVID-19 pandemic can serve as the catalyst for digital innovation.

OBJECTIVE: The aim of this study was to quantify the impact of electronic health record (EHR) workstation single sign-on (SSO) for nurses. BACKGROUND: SSO was implemented in 19 hospitals for expedited EHR access. METHODS: Login durations before and after SSO implementation were compared, and the financial value of nursing time liberated from keyboard was estimated. Stratified analyses show time liberated and financial value by staffing level and system size. RESULTS: First-of-shift login was reduced by 5.3 seconds (15.3%) and reconnect duration was reduced by 20.4 seconds (69.9%). SSO liberated 27,962.4 hours of nursing time from keyboard login per year across 19 facilities, and 1,471.7 hours/year/facility, valued at $52,112/facility and $990,128 for 19 hospitals. Time value ranges from $201,835 per year for a 5-hospital system with 300 nurses per facility to $672,790 per year for a 10-facility system with 500 nurses per hospital. CONCLUSIONS: Nurses gained substantial time liberated from EHR keyboard by SSO for patient care, having significant financial value for the organization.

OBJECTIVE: Analysis of national and foreign trials investigating accumulation of experience in innovative technologies using the learning curves. MATERIAL AND METHODS. S: Earching for Russian-language manuscripts was carried out within the references of the articles and in the ELIBRARY database. Foreign trials were selected from the PubMed database according to the keywords «learning curves in surgical practice¼. The discovered publications were studied for accordance with the objectives of this study. RESULTS: Accumulation of experience in new technologies by using of learning curves is valuable to improve the training, determine duration of development of new technology and the factors affecting its characteristics. CONCLUSION: The method is high-quality for comprehensive analysis of experience accumulation in new surgical technologies.

This study analyses the dynamics of carbon emissions for N-11 countries from 1990 to 2017. We introduce some innovative factors such as financial development, human capital, renewable energy consumption, and gross domestic product as determinants of carbon dioxide (CO2) emissions. The empirical estimations are based on Pesaran (2007) unit root test, common correlated effect mean group, and augmented mean group. The findings reveal a positive relationship between carbon emissions and financial development as well as a gross domestic product. In contrast, technological innovation and renewable energy consumption are adversely related to carbon emissions. These findings have important implications, and we recommend the promotion of technological innovation and the use of renewable energy consumption. This will help in achieving the goals set by COP21.

Methods of previous researches on green technology innovation will have difficulty in finite population. One solution is the use of stochastic evolutionary game dynamic-Moran process. In this paper we study stochastic dynamic games about green technology innovation with a two-stage free riding problem. Results illustrate the incentive and selection strength play positive roles in promoting participant to be more useful to society, but with threshold effect: too slighted strength makes no effect due to the randomness of the evolution process in finite population. Two-stage free riding problem can be solved with the use of inequality incentives, however, higher inequality can make policy achieves faster but more unstable, so there would be an optimal range. In this paper we provided the key variables of green technology innovation incentive and principles for the environmental regulation policy making. Also reminded that it's difficult to formulate policies reasonably and make them achieve the expected results.

Under different technological innovation modes, regional energy consumption may have spatial heterogeneity. Spatial heterogeneity complicates the nexus between environmental regulation and energy consumption. Traditional spatial homogeneity analysis is hard to describe the nonlinear nexus between them. Based on the data of 30 provinces in China from 2007 to 2017, this paper employs the spatial econometric method and the nonlinear econometric method to investigate the spatial effects and nonlinearity of energy consumption, respectively. The results display that under the current level of economic development, per capita energy consumption has a significant spatial spillover effect. Environmental regulation promotes regional per capita energy consumption in the short term. On the contrary, the technological effect of environmental regulation has significantly reduced Chinese per capita energy consumption. Therefore, energy policy should be tailored to local conditions, and policymakers can strengthen the environmental regulatory system and encourage enterprises to implement green technology innovation.

A significant proportion of the current technological developments in pneumology originate from the various areas of information technology. The spectrum ranges from smartphone apps to be used in daily life or in patient care to the use of artificial intelligence in screening and early detection of diseases. The diagnostic accuracy of apps for symptom analysis is currently very limited. Research projects are performed on the integration of symptoms and functional parameters into early detection, but also on mobility measurements as a prognostic marker in COPD. Lung cancer screening using computed tomography represents a major challenge. Here, artificial intelligence can help radiologists to cope with huge amounts of data. However, the quality of the software depends on the sufficient training of the system. Technological developments shape all fields of pneumology. For diagnostic and interventional endoscopy, they offer improved biopsy techniques and microstructural imaging. Advances in lung function measurements allow the differentiated analysis of respiratory mechanical disorders, and they could be transferred to ventilation technology. The translation of basic findings about the lung microbiome into patient care may perspectively help to better understand and treat COPD exacerbations.

This paper introduces a measure of the proximity in ideas using unsupervised machine learning. Knowledge transfers are considered a key driving force of innovation and regional economic growth. I explore knowledge relationships by deriving vector space representations of a patent's abstract text using Document Vectors (Doc2Vec), and using cosine similarity to measure their proximity in ideas space. I illustrate the potential uses of this method with an application to geographic localization in knowledge spillovers. For patents in the same technology field, their normalized text similarity is 0.02-0.05 S.D.s higher if they are located within the same city, compared to patents from other cities. This effect is much smaller than when knowledge transfers are measured using normalized patent citations: local patents receive about 0.23-0.30 S.D.s more local citations than compared to non-local control patents. These findings suggest that the effect of geography on knowledge transfers may be much smaller than the previous literature using citations suggests.

This study aims to explore the direct effect and spillover effect of innovation policy in the distribution and dynamic evolution of the regional innovation from the perspective of innovation motivation using the spatial econometric model. Substantive innovation and strategic innovation in one region could affect innovation in another region. In addition, the direct effect and spatial spillover effect of innovation policy could significantly affect innovation; however, they exert a higher impact on substantive innovation. Considering different policy tools, we found that government subsidies exert a significant positive impact on substantive innovation and strategic innovation, whereas financial institution loans exert a significant negative impact on substantive innovation and strategic innovation. And for the impact range, the government subsidies are higher than that of financial institution loans. Furthermore, this study reveals the leading environmental factors affecting regional innovation and provide a policy basis to promote the construction of an innovation-oriented country.

Since the 21st century, with the development of minimally invasive surgical technology, the update of comprehensive treatment strategies and the progress of clinical research, colorectal surgery has developed rapidly. However, in recent years, some disputable issues still exist in colorectal surgery, such as transanal total mesorectal excision, pelvic cavity lateral lymph node dissection, the "wait and observe" strategy for clinical complete remission of rectal cancer after neoadjuvant therapy, and robotic colorectal surgical operation. In addition, the application of three dimensions imaging, 4K resolution, 5th generation wireless systems, virtual reality, artificial intelligence and other new techniques may provide extensive space and new opportunity for the development of colorectal surgery. The therapic outcome could be optimized by more relevant clinical research and evidence, which contribute to the standardization of surgical treatment of colorectal cancer.

For surgeons, the invention and improvement of a new operation or surgical instrument directly depends on curiosity and exploration. Whether a novel idea or technology can be transformed and used in clinical practice depends on its commercial prospects and the adjustment of macro health policies. The protection of intellectual property and appropriate distribution of interests are the key to ensure sustainable innovation. The transformation of medical science and technology achievements is of great practical significance for improving the medical standard of our country and promoting the health and well-being of the people. However, China's medical science and technology achievements transformation ecosystem is still immature. Each subject in the system has insufficient cognition of their roles, unclear division of responsibilities, and imperfect system guarantees, resulting in the obstacle of scientific and technological achievements that can truly be transformed into actual productivity. In order to further deepen the understanding and thinking of the current status of China's medical science and technology achievement transformation ecosystem, the opportunities and challenges it faces, to find an optimized path that conforms to the situation in China today, we combine our own production, teaching and research achievement transformation practice, summarize the composition and existing problems of the current system subjects, analyze the plight and outlet of doctors, hoping to provide help and reference for the transformation of scientific and technological achievements to our peer doctors.

Surgery for rectal cancer has obtained quick improvement in techniques and concepts in recent years but still has challenging areas. Colorectal surgeons always seek to make operations clearer and easier, so that surgery can be safer and less time-consuming while guaranteeing surgical goals. With this purpose, our team have explored to make innovations in operations for rectal cancer and translate relevant patents from 2009. We summarize our achievements in this article as follows: (1) Reverse Miles operation (perineal operation first then laparoscopic abdominal operation) with two relevant patents-specialized instruments bag for laparoscopic operations (patent number ZL201520442331.0) and accessory spotlight for ultrasound scalpel (patent number ZL20102 0137689.X). (2) Laparoscopic sphincter-saving surgery for low rectal cancer through marker meeting approach with two patents-vacuum rectal drainage tube with functions of irrigation and ventilation (patent number ZL201520374385.8) and sterile sleeve cover of ultrasound scalpel handle (patent number ZL201920648102.2). (3) Laparoscopic radical resection of colorectal cancer and natural orifice specimen extraction. Different methods were designed according to the location of the tumor that classified as 20-40 cm, 10-20 cm and 5-10 cm to anus. Two relevant patents were specialized instruments for natural orifice specimen extraction (patent application number ZL2017101480141) and plastic film sleeve for natural orifice specimen extraction (patent application number ZL 201921169857.0). Reformation of surgical technique and innovation of surgical instruments should be conducted by surgeons with innovative thinking who always seek the way to translate ideas to patents and then real products to promote surgical treatment.

Medical artificial intelligence is a deeply integrated field of production-university-research. The collaborative innovation system of "industry, education and research" has broken down the barriers between various disciplines and has shown great impetus in scientific and technological innovation and achievement transformation. The Affiliated Hospital of Qingdao University where the authors work, relying on Shandong Key Laboratory of Digital Medicine and Computer-Assisted Surgery and Institute of Digital Medicine and Computer-Assisted Surgery of Qingdao University, has jointly established a close "industry-university-research" cooperation mechanism with the State Key Laboratory of Virtual Reality Technology and Systems of Beijing University of Aeronautics and Astronautics and Qingdao Hisense Medical Equipment Co., Ltd., which lasted for 10 years and carried out a series of useful explorations in scientific and technological innovation and achievement transformation in the field of gastrointestinal surgery of artificial intelligence with fruitful results. It mainly comprises of the following: (1) The research and development (R&D) of Hisense computer-assisted surgery (CAS) system has important clinical significance for laparoscopic gastrointestinal surgery guided by surrounding blood vessels that grasps globally the movement and variation of blood vessels in order to accurately select surgical strategies. (2) R&D of an automatic identification system for gastrointestinal diseases (namely image-assisted diagnosis system of artificial intelligence) initially applies deep neural network of artificial intelligence to the identification of lymph node metastases in rectal cancer, which has achieved accurate clinical diagnosis. (3) The virtual endoscopy platform for the gastrointestinal tract developed can successfully provide a new virtual endoscopy view of the intestinal wall eversion and realize the one-way navigation of the intestinal wall. (4) A localized platform of laparoscopic surgery simulation training is established with completely independent intellectual property rights. We realize that clinical requirement is the source of research and development, clinical practice is the source of innovation, and clinicians are practitioners of transformation of achievements. The establishment of artificial intelligence models often requires the support of massive and high-quality clinical data. Therefore, the effectiveness of its promotion and application can only be ensured by adopting a multi-center research approach. In the whole process of scientific and technological innovation and achievement transformation, the issues of medical ethics and intellectual property rights cannot be ignored.

The combination of medicine and engineering is a new interdisciplinary subject, which is a mode of cross integration and collaborative innovation between medical science and engineering. The combination and collaborative innovation of medicine and industry means more about the improvement, innovation and R&D of medical devices. However, the combination of traditional industry with biomedical engineering, modern medical imaging technology, electronic information technology and other high-tech in medical device industry is a reflection of the manufacturing industry and high-tech level of a country. The development mode of medical industry integration and collaborative innovation in China is mainly to merge medical colleges and universities with science and engineering colleges, promote the cross of different departments, and set up biomedical engineering specialty under the support of a series of relevant national policies, relying on large-scale comprehensive hospitals and research institutes, establish numerous research centers of translational medicine, thus achieving a series of achievements. Our team has made some explorations in the practice of the combination of medicine and engineering, including the utility model patent "reusable simple anal expander" and "incision protective cover of transanal multi-channel endoscopic surgery operation platform", which have been authorized by the State Intellectual Property Office, meanwhile the ultra-fine laparoscope, intragastric gasbag and other projects have been demonstrated by relevant research and development teams and are to be transformed into production. On January 10, 2020, with the approval of Guangdong Pharmaceutical Association, the Medical Innovation and Transformation Expert Committee of Guangdong Pharmaceutical Association was established jointly with the representatives of medical colleagues, scientific research institutions and enterprises, who are interested in the combination of medical industry and collaborative innovation. This Committee provides a platform for the exchange of medical colleagues, scientific research institutions and enterprises. We realize that clinical practice is the source of the combination of medical workers and collaborative innovation, and clinicians are the driving force of the combination of medical workers and collaborative innovation. At present, the main problems faced by the development of medical industry integration in China are as follows: insufficient integration of medical industry integration disciplines in the basic research stage; less interaction of clinical application needs in the application research stage; difficult transformation of scientific research achievements; the unconnected whole chain of "production, learning, research and application". If we can increase the investment in scientific research and policy incentives, strengthen the communication and interaction with enterprises, pay more attentions to the social and economic benefits of the promotion of achievements, open the whole process of the combination of medicine and industry, and improve the evaluation mechanism of the innovation ability of such combination, combination of medicine and engineering and collaborative innovation in China will enter the golden period of rapid development.
