Introduction: This article assumes a critical reflection as a personal suggestion to include the dimension of care in healthcare system. The discussion is contextualized in the epidemiological transitions of the 21st Century framework, chronicity, longevity and increasing dependence. Methods: An experimental metaphor for manipulating the genome constituting healthcare systems, based on principles of universality, accessibility, solidarity, ethics, efficacy and efficiency, is held to achieve the minimum necessary change to transform them in favour of humanization, empathy and compassion. Concepts of care and self-care are explored, as an attitude in relation to oneself, with others and with the world, undervalued dimensions of care, usually neglected or invisible. Results: Ten reasons are proposed to include the ethical, moral and pedagogical model of palliative care approach in a transversal way for the whole system and stages of life, as well as the leadership to carry it out. Conclusion: this transformative healthcare system model is offered, as other face of carelessness, dehumanization and anomie, both, to users and healthcare professionals.

OBJECTIVE: to develop and validate with a panel of experts a scenario of maternal-child clinical simulation, related to humanized childbirth and birth. METHOD: methodological study based on the Jeffries framework and standardized guides of the International Nursing Association for Clinical Simulation in Learning, which used analysis with descriptive statistics for general aspects of adherence to the aforementioned guide and inferential statistics for validating the checklist of actions through the Intraclass Correlation Coefficient (ICC). RESULTS: the scenario contains learning objectives, necessary resources, prebriefing and debriefing of guidelines, description of the simulated situation, participants and roles, and checklist of expected actions. The validation obtained an agreement level above 80% in all aspects evaluated by 31 experts, highlighting realism of the environment and setting, vital sign parameters, alignment with scientific literature and encouragement of critical thinking and problem solving. In addition, the checklist of actions was validated with 0.899 agreement among experts, statistically analyzed by the ICC and Cronbach's alpha 0.908 (95% confidence interval). CONCLUSION: the simulated scenario on humanized childbirth and birth can strengthen the articulation between women's and children's health disciplines, and was validated by experts.

Humanbeings are unexpectedly subjected to:1. Happenings: they belong to the plane of the personality, crisis, the Age of the virus appears that had already been announced. The ideological support is not sustained, causing a bankruptcy in the system of our life. The subject acts unconsciously, does not believe in the advent of the catastrophe, does not follow logical rules, is unable to transcend, fear-anguish appears and this requires a lot of protection. Those who remain free from personal decomposition skewer him and put a project called "person" with his sense of life, his lifestyle and life project, which is the conjunction of body and spirit, which with the agony struggles against life itself.2. Events: they belong to the plane of nature, catastrophes. The use of the word catastrophe is not synonymou swith unavoidable natural disaster, accident, although it is used by the health authorities this can have a great impact on those affected by the virus. This word has the final nuance of Greek tragedies. False arrogance appears, true madness. In the case of the Covid-19 there is something else, a dialectical position with oneself. They lived and we lived on a volcano. The example of the Apocalypse and its three plagues, epidemics and post-war neuroses, etc. The most striking neuropsychiatricsymptomatology is the appearance of "ageusia" and "anosmia", abruptly and totally, which disappears slowly, as well as sleep disturbances with ignagogic images post-traumatic stress charts appear.

RESUMO

The global COVID-19 pandemic has become one of the largest clinical and operational challenges faced by emergency medicine, and our EDs continue to see increased volumes of infected patients, many of whom are not only ill, but acutely aware and fearful of their circumstances and potential mortality. Given this, there may be no more important time to focus on staff-patient communication and expression of compassion. However, many of the techniques usually employed by emergency clinicians to provide comfort to patients and their families are made more challenging or impossible by the current circumstances. Geriatric ED patients, who are at increased risk of severe disease, are particularly vulnerable to the effects of isolation. Despite many challenges, emergency clinicians have at their disposal a myriad of tools that can still be used to express compassion and empathy to their patients. Placing emphasis on using these techniques to maximize humanism in the care of COVID-19 patients during this crisis has the potential to bring improvements to ED patient care well after this pandemic has passed.

Directed towards the construction of knowledge by the student, the teaching methods proposed in initial nursing education are based on humanistic values open to the diversity of situations experienced by people being cared for. According to this approach, a nursing training institute in Saône-et-Loire shares its experience on an original teaching approach linked to the theme of disability.

RESUMO

The COVID-19 pandemic is an unprecedented challenge for society. Supporting the mental health of medical staff and affiliated healthcare workers (staff) is a critical part of the public health response. This paper details the effects on staff and addresses some of the organisational, team and individual considerations for supporting staff (pragmatically) during this pandemic. Leaders at all levels of health care organisations will find this a valuable resource.

BACKGROUND: Healer's Art (HA) is a voluntary course offered during medical school. The course aims to address the growing loss of meaning and commitment experienced by doctors through the exploration of compassion, empathy and awe in medicine, and early exposure to a supportive community of practice. This project aimed to evaluate the potential influence of HA on junior doctor graduates. METHODS: Junior doctors who had undertaken HA during their medical studies were interviewed. A thematic analysis was performed on the results of these semi-structured interviews. RESULTS: Ten junior doctors who had undertaken the HA course participated in interviews. All interviewees described the HA as a positive and enlightening experience in their medical education. The thematic analysis identified four major themes: developing empathy in the doctor-patient journey, self-care and self-awareness, the creation of a supportive community, and coping with the challenging medical culture. CONCLUSIONS: HA provides experiential learning that enables participants to explore humanistic medicine. Self-selected junior doctors recall the course as a positive experience, and perceive themselves to be continuing to employ the techniques from HA in the healthcare setting. The concepts taught in the HA course appear to have a lasting personal impact on some junior doctors, who identify the course as influencing their self-reported positive patient-doctor relationships and supportive relationships with medical peers.

RESUMO

OBJECTIVE: to present the term, concept and technique of art of maternal womb painting. METHOD: a theoretical, exploratory and qualitative study. RESULTS: from the results, the concept was elaborated and based on linguistic, philosophical and specialized perspective. The concept and technique of Art of Maternal Womb Painting were guided by humanism and holism. FINAL CONSIDERATIONS: this study can broaden the visibility of this visual art and insert it in the obstetrics, as well as its adoption by nurses, midwives and other health professionals in humanistic and holistic care for maternal health.

Based on fieldwork in a specialized geriatric outpatient clinic in Brazil, this article shows how a humanistic discourse that 'means well' can do good, but can also produce a regime of care that ultimately results in care that is contrary to stated values. These values - such as holistic care, multidisciplinarity, and empathy - that have been at the heart of geriatrics since its more official founding in the 1940s and 1950s, cannot be conceived as only local. The Brazilian data mirrors international geriatric values and norms, which, however, are being applied here in a specific context, in a country perceived as 'young' and with limited resources for elder care. Fallacies of care in this context result preliminary from a translation of more structural factors as individualized (self-)care and from the abstraction and generalization of aging individuals as 'older people'.
