RESUMEN
This report describes an effective cerebral perfusion method using an additional 4th side limb graft sewn to a trifurcated graft in patients undergoing redo aortic root reconstruction combined with total aortic arch replacement. This method is useful in cases where the origin of the innominate artery is unsuitable for clamping or when axillary perfusion is not employed, and assures continuous brain protection and minimizes circulatory arrest time.

We describe a case of a giant right atrial large B-cell lymphoma treated with initial surgical excision followed by postoperative chemotherapy. The patient had no clinical evidence of recurrence through 87 months after her hospital discharge. To our knowledge, this is the first reported case of long-term survival following this method of disease management. The discussion reviews the current status of chemotherapy for cardiac lymphomas and the pros and cons of treatment option algorithms.

The work presents an analysis of the nearest results of treatment of 82 patients with pulsating hematomas, false and true aneurysms of the upper and lower extremity vessels depending on the kond of the pulsating formation, variants of complications in the course of disease, the level and size of defects of the vessel as well as of the state of the distal bed. Most optimum operations are those with using autovenous transplants, patients with false aneurysms excluded. In all the operated patients the adequate blood circulation was restored and the extremity was saved.

Of 28 extremities of the cadavers, the angioarchitectonics of a donor zone of the "saphenous" flap was studied. Three variants of blood supply of the region were revealed, and according to them, the different directions of the flap axis were defined. The neglect of the data of angioarchitectonics can lead to the loss of a considerable party of the vascular bed and affect the results of transplantation.
