INTRODUCTION: Chronic kidney disease is a worldwide public health problem, with a high prevalence of patients on dialysis. mHealth technologies can greatly support the treatment and monitoring of these patients. Thus, this study aimed to evaluate the spontaneous use of the application (app) Renal Health, a previously available technology, for patients on hemodialysis and validate content to support patients undergoing peritoneal dialysis. METHODS: The first stage consisted of evaluating the spontaneous use of the app, and the second stage consisted of methodological research for the development, evaluation, and improvement of a technological instrument for use in clinical practice as a support for patients undergoing peritoneal dialysis (PD). The association between categorical variables was performed using the chi-square test, adopting a significance level of 5%. RESULTS: The app was accessed by 753 users and of these, 34 % accessed the hemodialysis section. Most accesses were in the state of São Paulo/Brazil and performed by women. The records of biochemical tests did not vary according to gender and age group (p > 0.05). The developed and validated PD section enables section control, allowing the user to manage their sessions. The analysis of the technology by the specialists showed good results for the global content validity index (CVI) regarding objectives (CVI = 0.95), structure (CVI = 0.97), and relevance (CVI = 1.0). CONCLUSION: It is concluded that the hemodialysis section of the Renal Health app aroused the interest of the population and that the developed peritoneal dialysis section was validated by specialists.

BACKGROUND: Phase angle (PhA), obtained from the bioimpedance analysis, is widely used in clinical situations and in sports. This study evaluated the association between PhA with body composition and physical performance of handball athletes. METHODS: 43 national-level players (22.19 ± 3.86 years) of both sexes were evaluated regarding anthropometry, body composition, squat (SJ) and countermovement (CMJ) jumps, handgrip strength, and cardiorespiratory fitness. RESULTS: We verified a correlation between PhA of the whole body and fat-free mass (r = 0.511), body mass index (r = 0.307), and body fat % (r = -0.303). There was a positive correlation between PhA of the whole body and SJ (r = 0.376), CMJ (r = 0.419), and handgrip for the dominant hand (r = 0.448). Moreover, PhA of the upper limbs was more strongly correlated with handgrip for the dominant (r = 0.630) and non-dominant hand (r = 0.575) compared to PhA of the whole body considering both sexes. Similarly, segmental PhA had a stronger significant correlation with SJ (r = 0.402) and handgrip for the dominant hand (r = 0.482) in males, as well as CMJ (r = 0.602) in females, compared to PhA of the whole body. CONCLUSION: PhA of the whole body was positively related to fat-free mass, body mass index, body fat %, and lower- and upper-limbs strength in handball athletes. Segmental PhA might be used as a tool for estimating lower and upper limbs performance considering the sex, in preference to the PhA of the whole body.

Introduction Muscle mass assessment of critically ill patients is essential to be part of the nutritional diagnosis in hospital care. Thus, the evaluation of more specific techniques for that purpose is needed. The present study aimed to investigate the association of quadriceps muscle thickness (QMT), measured by ultrasound (US), with the nutritional status of critically ill patients in a referral high-complexity trauma care hospital. Methods A cross-sectional observational study was conducted in the intensive care units (ICUs) in a tertiary hospital in Brazil. The sample comprised 30 critically ill trauma patients admitted between February and March 2022. The methodology involved evaluating muscle mass and comparing nutritional status through mid-upper arm circumference measurements and US assessments. Specifically, the QMT was quantified using US at a predefined site between the iliac crest and the proximal border of the patella. Results The Kruskal-Wallis test indicated variability in QMT between the nutritional status groups, with statistical significance reached after excluding the overweight group (H(2) = 7.532, p = 0.023). The moderate malnutrition group exhibited notably lower QMT. Sensitivity analyses using bootstrap and Monte Carlo methods showed moderate trends toward significance. A positive correlation was found between QMT and mid-upper arm circumference adequacy (p < 0.05), demonstrating fair to moderate correlation (rs = 0.531). Conclusion Significant changes in QMT were detected by ultrasound assessment in moderate malnutrition patients compared to patients of other nutritional statuses. Ultrasound may be a valuable technique for monitoring muscle integrity in critically ill patients.

ABSTRACT Objective To evaluate the association between dietary self-perception and socioeconomic, health, diet, and lifestyle variables of socially vulnerable women in primary health care. Methods This cross-sectional study was conducted in a Primary Health Care Unit in Fortaleza, Ceará, with 158 women beneficiaries of the Bolsa Família (Family Aid) Program. We applied a questionnaire containing socioeconomic and health variables, food knowledge, and self-perception. Pearson's chi-square test, with a 5% significance level, was adopted to investigate possible associations between women's food self-perception and other variables. Results The women had a mean age of 31.2 years. Most had a household income of less than one minimum wage (75.9%), a high school education level (53.8%), were overweight (67.7%), did not engage in physical activity (72.8%), and received dietary guidance (51.9%). Negative dietary self-perception was reported by 57.0% of the sample. Women who did not engage in physical activity had a higher frequency of negative dietary self-perception (p=0.007). Conclusion Most women showed negative dietary self-perception. The variable "physical activity" was positively associated with dietary self-perception. Knowing how women perceive their diet and which factors are associated with their food choices can help health professionals in their daily conduct in health care units.


RESUMO Objetivo Avaliar a associação entre a autopercepção alimentar negativa e variáveis socioeconômicas, de saúde, alimentação e estilo de vida de mulheres em vulnerabilidade social, na atenção primária à saúde. Métodos Estudo do tipo transversal realizado em uma Unidade de Atenção Primária à Saúde de Fortaleza, Ceará, com 158 mulheres beneficiárias do Programa Bolsa Família. Um questionário foi aplicado contendo as variáveis investigadas. O teste Qui-quadrado de Pearson, com nível de significância 5% foi utilizado para investigar possíveis associações entre a autopercepção alimentar das mulheres e as demais variáveis. Resultados As mulheres apresentaram média de idade de 31,2 anos e a maioria tinha renda familiar inferior a um salário-mínimo (75,9%), ensino médio como o nível de escolaridade (53,8%), apresentou excesso de peso (67,7%), não praticava atividade física (72,8%) e recebeu orientações sobre alimentação (51,9%). A autopercepção alimentar negativa foi relatada por 57,0% da amostra. As mulheres que não praticavam atividade física, tiveram maior frequência de autopercepção alimentar negativa (p=0,007). Conclusão A maioria das mulheres apresentou autopercepção negativa da alimentação e a variável "atividade física" mostrou associação positiva em relação à autopercepção alimentar. Conhecer como as mulheres percebem sua alimentação e quais fatores se associam às suas escolhas pode auxiliar os profissionais de saúde em suas condutas no cotidiano dos atendimentos nas unidades de saúde.

OBJECTIVE: To describe the prevalence of underweight and obesity indicators among individuals registered as traditional peoples and communities in the Food and Nutrition Surveillance System, across Brazil, in 2019. METHODS: This was a descriptive study using individual secondary data from participants receiving care in the Primary Health Care within the Brazilian National Health System. RESULTS: In the study population (N = 13,944), there was a higher prevalence of short stature among male children and adolescents (14.2%), when compared to their female counterparts (11.8%); in the adult female population, there was a higher prevalence of obesity (23.0%), when compared to the male population (11.3%); the prevalence of low height-for-age in riverine communities (18.5%) and obesity in the adult faxinalense population (75.1%) stood out. CONCLUSION: Anthropometric disparities between different communities require tailored responses, emphasizing targeted primary health care and programs to ensure food and nutrition security. MAIN RESULTS: It was possible to identify a higher prevalence of low weight and height-for-age in the child population of riverine communities, while faxinalense communities showed a higher prevalence of obesity in adults. IMPLICATIONS FOR SERVICES: Continuous improvement in the implementation of SISVAN protocols is recommended, as well as strengthening the assistance provided to communities with the worst indicators. PERSPECTIVES: Studies assessing the potential impact of interventions to mitigate the effects of malnutrition in communities, through in-depth qualitative research or probabilistic quantitative studies using primary data.

This study aimed to analyze whether community food environments are associated with individual food consumption among nutrition students and newly graduated nutritionists. This cross-sectional study used data from the Nutritionists' Health Study cohort, which included 357 undergraduate nutrition students from the city of Fortaleza, Ceará, Brazil. Exposure to the food environment was defined as the proximity and availability of food outlets within a 500 m buffer from the participants' homes. Food consumption was assessed using a Food Frequency Questionnaire and analyzed according to the NOVA classification. Multi-level linear regression models with fixed effects were used to estimate the presence of food outlets within the buffer and their association with food consumption. The presence of mini-markets in the buffer in the fourth quartile was associated with lower consumption of ultra-processed foods (UPF) when observing socioeconomic and lifestyle conditions (ß = -3.29; 95% CI = -6.39 to -0.19). The presence of bakeries and coffee shops was related to lower consumption of ultra-processed foods among participants when observing socioeconomic conditions (ß = -3.10; 95% CI = -6.18 to -0.02). There was no clear evidence of an association between the type of food outlet and UPF consumption. The community food environment seemed to influence food consumption among study participants, although clearer and more consistent evidence on this subject is needed.

INTRODUCTION: Paroxysmal nocturnal hemoglobinuria (PNH) is a clonal disease of hematopoietic cells with a variable clinical spectrum characterized by intravascular hemolysis, high risk of thrombosis, and cytopenias. To understand the biochemical shifts underlying PNH, this study aimed to search for the dysfunctional pathways involved in PNH physiopathology by comparing the systemic metabolic profiles of affected patients to healthy controls and the metabolomic profiles before and after the administration of eculizumab in PNH patients undergoing treatment. METHODS: Plasma metabolic profiles, comprising 186 specific annotated metabolites, were quantified using targeted quantitative electrospray ionization tandem mass spectrometry in 23 PNH patients and 166 population-based controls. In addition, samples from 12 PNH patients on regular eculizumab maintenance therapy collected before and 24 hours after eculizumab infusion were also analyzed. RESULTS: In the PNH group, levels of the long-chain acylcarnitines metabolites were significantly higher as compared to the controls, while levels of histidine, taurine, glutamate, glutamine, aspartate and phosphatidylcholines were significantly lower in the PNH group. These differences suggest altered acylcarnitine balance, reduction in the amino acids participating in the glycogenesis pathway and impaired glutaminolysis. In 12 PNH patients who were receiving regular eculizumab therapy, the concentrations of acylcarnitine C6:1, the C14:1/C6 ratio (reflecting the impaired action of the medium-chain acyl-Co A dehydrogenase), and the C4/C6 ratio (reflecting the impaired action of short-chain acyl-Co A dehydrogenase) were significantly reduced immediately before eculizumab infusion, revealing impairments in the Acyl CoA metabolism, and reached levels similar to those in the healthy controls 24 hours after infusion. CONCLUSIONS: We demonstrated significant differences in the metabolomes of the PNH patients compared to healthy controls. Eculizumab infusion seemed to improve deficiencies in the acyl CoA metabolism and may have a role in the mitochondrial oxidative process of long and medium-chain fatty acids, reducing oxidative stress, and inflammation.

Patients with SARS-CoV generally require noninvasive treatments including continuous positive airway pressure (CPAP), and appropriate nutritional therapy. Our hypothesis was that a higher body mass index (BMI) would result in greater respiratory difficulties during CPAP treatment and lead to a reduction in energy and protein intake. The study was conducted prospectively and retrospectively in a Brazilian public hospital. The sample comprised all patients (n = 70) hospitalized in COVID-19 wards and using the Elmo System (ELMO) helmet. To ensure proper nutritional support, the hospital established a standard ELMO diet and questionnaire for estimating dietary adherence. The median length of stay was 11 days (interquartile range, 9-20), and 84.3% of the patients received hospital discharge. The median duration of helmet use was 1.5 days (interquartile range, 1-4). From mid-upper arm circumference adequacy measurements, 35 patients (50%) were classified as being overweight or living with obesity and 36 adult patients (66.7%) were considered living with obesity based on their BMI. The prevalence of overweight elderly patients was 81.3%. The median energy and protein intake adequacy percentages were 65% and 72.5%, respectively. Patients with higher BMIs exhibited lower oxygen flows while wearing the ELMO helmet. Younger age and lower energy and protein intake characterized the obesity group. A correlation between oxygen therapy and food intake in COVID-19 patients undergoing CPAP treatment was observed. Patients with higher BMIs and/or mid-upper arm circumference adequacies were more susceptible to energy and protein intake declines, and patients with obesity presented the lowest intake values.

ABSTRACT Introduction: Supplementation with probiotics for patients with chronic kidney disease (CKD) may be associated with decreased systemic inflammation. Objective: To assess the impact of oral supplementation with probiotics for patients with CKD on hemodialysis. Method: This double-blind randomized clinical trial included 70 patients on hemodialysis; 32 were given oral supplementation with probiotics and 38 were in the placebo group. Blood samples were collected at the start of the study and patients were given oral supplementation with probiotics or placebo for three months. The probiotic supplement comprised four strains of encapsulated Gram-positive bacteria: Lactobacillus Plantarum A87, Lactobacillus rhamnosus, Bifidobacterium bifidum A218 and Bifidobacterium longum A101. Patients were given one capsule per day for 3 months. Blood samples were taken throughout the study to check for inflammatory biomarkers. Non-traditional biomarkers Syndecan-1, IFN-y, NGAL, and cystatin C were measured using an ELISA kit, along with biochemical parameters CRP, calcium, phosphorus, potassium, PTH, GPT, hematocrit, hemoglobin, glucose, and urea. Results: Patients given supplementation with probiotics had significant decreases in serum levels of syndecan-1 (239 ± 113 to 184 ± 106 ng/mL, p = 0.005); blood glucose levels also decreased significantly (162 ± 112 to 146 ± 74 mg/dL, p = 0.02). Conclusion: Administration of probiotics to patients with advanced CKD was associated with decreases in syndecan-1 and blood glucose levels, indicating potential improvements in metabolism and decreased systemic inflammation.


Resumo Introdução: A suplementação com probióticos na doença renal crônica (DRC) pode estar associada à redução do processo inflamatório sistêmico. Objetivo: Avaliar a suplementação oral com probióticos em pacientes com DRC em hemodiálise. Método: Ensaio clínico, duplo cego, randomizado com 70 pacientes em hemodiálise, sendo 32 do grupo que recebeu o suplemento de probióticos e 38 do grupo placebo. Inicialmente ocorreu a coleta de sangue e suplementação oral com probióticos ou placebo durante três meses. O suplemento probiótico foi composto pela combinação de 4 cepas de bactérias Gram-positivas encapsuladas: Lactobacillus Plantarum A87, Lactobacillus rhamnosus, Bifidobacterium bifidum A218 e Bifidobacterium longum A101, sendo 1 cápsula do suplemento ao dia, durante 3 meses. Após esse período foram feitas novas coletas de sangue para dosagem dos biomarcadores inflamatórios. Foram analisados os biomarcadores não tradicionais: Syndecan-1, IFN-y, NGAL e cistatina C pelo método ELISA, e os seguintes parâmetros bioquímicos: PCR, cálcio, fósforo, potássio, PTH, TGP, hematócrito, hemoglobina, glicose e ureia. Resultados: Os pacientes que receberam suplemento tiveram diminuição significativa dos níveis séricos de syndecan-1 (de 239 ± 113 para 184 ± 106 ng/mL, p = 0,005). Outro parâmetro que diminuiu significativamente nos pacientes que receberam suplemento foi a glicemia (de 162 ± 112 para 146 ± 74 mg/dL, p = 0,02). Conclusão: O uso de probióticos na DRC avançada esteve associado à redução dos níveis de syndecan-1 e glicemia, sinalizando possível melhora no metabolismo e redução do processo inflamatório sistêmico.

Objective: this study aimed to evaluate if there is an association of intake of coffee and its polyphenols with mammographic findings in women treated at a breast care service unit of the Unified Health System (SUS), Brazil. Research methods and procedures: this was a cross-sectional study with 532 women treated at a health service. The participants were divided according to their mammographic reports into two groups: without and with altered findings. Two 24-h dietary recalls were applied and coffee consumption was categorized into three groups (less than 1 cup, 1 to 3 cups, and more than 3 cups). Phenolic acids were determined using the Phenol Explorer program. The intake of polyphenols was calculated by adding the values obtained from the total amount of coffee consumed during the day. The Multiple Source Method (MSM) was applied to analyze the usual intake. Results: of the 532 women, 178 (33.5 %) had altered mammographic findings. The participants' average daily coffee intake was 193.4 mL. No significant association was found between coffee consumption and mammographic findings. However, it was found that the second tertile of polyphenols was a protective factor for breast changes. Conclusion: Coffee polyphenols are protective against breast changes in the group evaluated and, thus, can help prevent breast cancer. (AU)


Objetivo: este estudio tuvo como objetivo evaluar si existe una asociación de la ingesta de café y sus polifenoles con los hallazgos mamográficos de mujeres tratadas en una unidad de patología mamaria del Sistema Unificado de Salud (SUS), Brasil. Métodos y procedimientos de investigación: este fue un estudio transversal con 532 mujeres tratadas en un servicio de salud. Las participantes se dividieron de acuerdo con sus informes mamográficos en dos grupos: sin y con hallazgos alterados. Se aplicaron dos registros dietéticos de 24 horas y el consumo de café se clasificó en tres grupos (menos de 1 taza, 1 a 3 tazas y más de 3 tazas). Los ácidos fenólicos se determinaron utilizando el programa Phenol Explorer. La ingesta de polifenoles se calculó sumando los valores obtenidos de la cantidad total de café consumido durante el día. Se aplicó el método de fuentes múltiples (MSM) para analizar la ingesta habitual. Resultados: de las 532 mujeres, 178 (33,5 %) tenían hallazgos mamográficos alterados. La ingesta diaria promedio de café de los participantes fue de 193,4 ml. No se encontró una asociación significativa entre el consumo de café y los hallazgos mamográficos. Sin embargo, se encontró que el segundo tercil de polifenoles era un factor protector para los cambios mamarios. Conclusión: los polifenoles del café son protectores contra los cambios mamarios en el grupo evaluado y, por lo tanto, podrían ayudar a prevenir el cáncer de mama. (AU)

The aim of the present study was to estimate the population coverage of recording food intake markers in Brazil's Food and Nutrition Surveillance System (Sisvan) and mean annual percent change (APC) in coverage according to the system used for data entry (e-SUS APS and Sisvan Web). We conducted an ecological time series study of the period 2015-2019. The data were stratified into region and age group. APC in coverage was calculated using Prais-Winsten regression and the correlation between APC and HDI, GDP per capita and primary healthcare coverage was assessed using Spearman's correlation coefficient. Population coverage of recording food intake markers at national level was 0.92% in 2019. Mean APC in coverage throughout the period was 45.63%. The region and age group with the highest coverage rate were the Northeast (4.08%; APC=45.76%, p<0.01) and children aged 2-4 years (3.03%; APC=34.62%, p<0.01), respectively. There was an upward trend in data entry using e-SUS APS, to the detriment of Sisvan Web. There was a positive correlation between APC in coverage using e-SUS APS and HDI and GDP per capita in some age groups. Population coverage of recording Sisvan food intake markers remains low across the country. The e-SUS APS has the potential to be an important strategy for expanding food and nutrition surveillance.


O objetivo deste estudo foi estimar a cobertura populacional e a taxa de incremento anual (TIA) média, em âmbito nacional, do registro de marcadores do consumo alimentar do Sistema de Vigilância Alimentar e Nutricional (Sisvan), entre 2015 e 2019, considerando a entrada dos dados via Estratégia e-SUS APS e Sisvan Web. Trata-se de estudo ecológico de séries temporais, com estratificação por macrorregiões e faixas etárias. A TIA da cobertura foi calculada utilizando regressão de Prais-Winsten e a correlação com IDH, PIB per capita e cobertura da APS foi avaliada por teste de Spearman. A cobertura populacional do registro de marcadores de consumo alimentar no país foi de 0,92% em 2019, com TIA significante de 45,63% desde 2015. As maiores coberturas foram observadas na região Nordeste (4,08%; TIA=45,76%, p<0,01) e em crianças entre 2 e 4 anos (3,03%; TIA=34,62%, p<0,01). Houve tendência crescente para a entrada dos dados via e-SUS APS, em detrimento do Sisvan Web. A TIA da cobertura dos registros via e-SUS APS exibiu correlação positiva com IDH e PIB per capita em alguns recortes etários. A cobertura populacional do registro de marcadores de consumo alimentar do Sisvan ainda se mostra incipiente no país e o e-SUS APS parece ser uma estratégia relevante para sua expansão.

OBJECTIVE: to analyze the temporal trend of Food and Nutrition Surveillance System (Sistema de Vigilância Alimentar e Nutricional - SISVAN) coverage and the nutritional status of older adults, and its correlation with indicators of social inequality in Brazil between 2008-2019. METHODS: this was an ecological study using records from SISVAN, related to the population aged 60 years and older; the temporal trend of coverage and the correlation between indicators of social inequality and increment rate of nutritional status were analyzed; slope index of inequality and concentration index were used to measure absolute and relative inequalities. RESULTS: 11,587,933 records were identified; national coverage increased from 0.1% (2008) to 2.9% (2019), with a statistically significant upward trend; a moderate inverse correlation with an annual increment rate of overweight between human development index and gross domestic product per capita, was found. CONCLUSION: there was an increasing trend in SISVAN coverage; the increase in overweight was associated with social inequality.

RESUMEN

Resumo O objetivo deste estudo foi estimar a cobertura populacional e a taxa de incremento anual (TIA) média, em âmbito nacional, do registro de marcadores do consumo alimentar do Sistema de Vigilância Alimentar e Nutricional (Sisvan), entre 2015 e 2019, considerando a entrada dos dados via Estratégia e-SUS APS e Sisvan Web. Trata-se de estudo ecológico de séries temporais, com estratificação por macrorregiões e faixas etárias. A TIA da cobertura foi calculada utilizando regressão de Prais-Winsten e a correlação com IDH, PIB per capita e cobertura da APS foi avaliada por teste de Spearman. A cobertura populacional do registro de marcadores de consumo alimentar no país foi de 0,92% em 2019, com TIA significante de 45,63% desde 2015. As maiores coberturas foram observadas na região Nordeste (4,08%; TIA=45,76%, p<0,01) e em crianças entre 2 e 4 anos (3,03%; TIA=34,62%, p<0,01). Houve tendência crescente para a entrada dos dados via e-SUS APS, em detrimento do Sisvan Web. A TIA da cobertura dos registros via e-SUS APS exibiu correlação positiva com IDH e PIB per capita em alguns recortes etários. A cobertura populacional do registro de marcadores de consumo alimentar do Sisvan ainda se mostra incipiente no país e o e-SUS APS parece ser uma estratégia relevante para sua expansão.


Abstract The aim of the present study was to estimate the population coverage of recording food intake markers in Brazil's Food and Nutrition Surveillance System (Sisvan) and mean annual percent change (APC) in coverage according to the system used for data entry (e-SUS APS and Sisvan Web). We conducted an ecological time series study of the period 2015-2019. The data were stratified into region and age group. APC in coverage was calculated using Prais-Winsten regression and the correlation between APC and HDI, GDP per capita and primary healthcare coverage was assessed using Spearman's correlation coefficient. Population coverage of recording food intake markers at national level was 0.92% in 2019. Mean APC in coverage throughout the period was 45.63%. The region and age group with the highest coverage rate were the Northeast (4.08%; APC=45.76%, p<0.01) and children aged 2-4 years (3.03%; APC=34.62%, p<0.01), respectively. There was an upward trend in data entry using e-SUS APS, to the detriment of Sisvan Web. There was a positive correlation between APC in coverage using e-SUS APS and HDI and GDP per capita in some age groups. Population coverage of recording Sisvan food intake markers remains low across the country. The e-SUS APS has the potential to be an important strategy for expanding food and nutrition surveillance.

OBJECTIVES: The use of prebiotics has an effect on postprandial glucose and insulin concentrations; however, the interaction between the previous profile of the intestinal microbiota and the effect of supplementation with prebiotics is not clear. Our objective was to evaluate the effect of previous intestinal microbiota profile on the postprandial insulin response to yacon syrup, used as a source of fructooligosaccharides (FOSs), in young women. The product presents high levels of FOS. METHODS: In this double-blind, crossover, randomized clinical trial, 40 adult women were allocated to receive a breakfast containing 40 g of yacon syrup (14 g FOS, intervention A) or a breakfast containing 40 g of placebo (intervention B). On each intervention day, after 12 h of fasting, an aliquot of blood was collected for insulin analysis at 0, 15, 30, 45, 60, 90, and 120 min. The fecal sample was collected before the beginning of the interventions, and the DNA was extracted and quantified, with subsequent amplification of the 16S region, next-generation sequencing, and analysis of sequencing data. RESULTS: The glucose and insulin concentrations were reduced after ingestion of yacon syrup compared with placebo, specifically at the 30 min to insulin. After the receiver operating characteristic analysis, six volunteers who did not respond to the yacon consumption intervention were identified. The abundance of the phylum Actinobacteria (P = 0.021) and the order Bifidobacteriales (P = 0.013) were positively associated with better insulin response. Other main phyla were not associated with intervention response. CONCLUSIONS: The previous profile of the intestinal microbiota has an effect on the postprandial insulin response to FOSs, mainly in the phylum Actinobacteria and Bifidobacteriales order.

Introduction: Objective: this study aimed to evaluate if there is an association of intake of coffee and its polyphenols with mammographic findings in women treated at a breast care service unit of the Unified Health System (SUS), Brazil. Research methods and procedures: this was a cross-sectional study with 532 women treated at a health service. The participants were divided according to their mammographic reports into two groups: without and with altered findings. Two 24-h dietary recalls were applied and coffee consumption was categorized into three groups (less than 1 cup, 1 to 3 cups, and more than 3 cups). Phenolic acids were determined using the Phenol Explorer program. The intake of polyphenols was calculated by adding the values obtained from the total amount of coffee consumed during the day. The Multiple Source Method (MSM) was applied to analyze the usual intake. Results: of the 532 women, 178 (33.5 %) had altered mammographic findings. The participants' average daily coffee intake was 193.4 mL. No significant association was found between coffee consumption and mammographic findings. However, it was found that the second tertile of polyphenols was a protective factor for breast changes. Conclusion: Coffee polyphenols are protective against breast changes in the group evaluated and, thus, can help prevent breast cancer.


Introducción: Objetivo: este estudio tuvo como objetivo evaluar si existe una asociación de la ingesta de café y sus polifenoles con los hallazgos mamográficos de mujeres tratadas en una unidad de patología mamaria del Sistema Unificado de Salud (SUS), Brasil. Métodos y procedimientos de investigación: este fue un estudio transversal con 532 mujeres tratadas en un servicio de salud. Las participantes se dividieron de acuerdo con sus informes mamográficos en dos grupos: sin y con hallazgos alterados. Se aplicaron dos registros dietéticos de 24 horas y el consumo de café se clasificó en tres grupos (menos de 1 taza, 1 a 3 tazas y más de 3 tazas). Los ácidos fenólicos se determinaron utilizando el programa Phenol Explorer. La ingesta de polifenoles se calculó sumando los valores obtenidos de la cantidad total de café consumido durante el día. Se aplicó el método de fuentes múltiples (MSM) para analizar la ingesta habitual. Resultados: de las 532 mujeres, 178 (33,5 %) tenían hallazgos mamográficos alterados. La ingesta diaria promedio de café de los participantes fue de 193,4 ml. No se encontró una asociación significativa entre el consumo de café y los hallazgos mamográficos. Sin embargo, se encontró que el segundo tercil de polifenoles era un factor protector para los cambios mamarios. Conclusión: los polifenoles del café son protectores contra los cambios mamarios en el grupo evaluado y, por lo tanto, podrían ayudar a prevenir el cáncer de mama.

Objective: To analyze the temporal trend in the coverage of the Food and Nutrition Surveillance System (SISVAN) and the nutritional status of the elderly, correlating them with indicators of social inequality. Methods: Ecological study using SISVAN records of the population aged ≥60 years. Correlation analyzes were performed between indicators of social inequality and the rate of increase in nutritional status and analyzes of absolute and relative inequalities to obtain the angular inequality index and the concentration index. Results: 11,587,933 records were identified. National coverage evolved from 0.1% in 2008 to 2.9% in 2019, with a statistically significant upward trend. A moderate inverse correlation was found with the annual increase rate of overweight for HDI and per capita GDP. Conclusion: There was a trend of growth in SISVAN coverage. The increase in overweight was associated with social inequality.


Objetivo: Analizar la tendencia temporal de cobertura del Sistema de Vigilancia Alimentaria y Nutricional (SISVAN), y estado nutricional de adultos mayores, correlacionándolos con indicadores de desigualdad social. Métodos: Estudio ecológico mediante registros SISVAN de la población ≥60 años. Se realizaron análisis de correlación entre indicadores de desigualdad social y la tasa de incremento del estado nutricional y análisis de desigualdades absolutas y relativas para obtener el índice de desigualdad angular y el índice de concentración. Resultados: Se identificaron 11.587.933 registros. La cobertura nacional evolucionó del 0,1% en 2008 al 2,9% en 2019, con una tendencia ascendente estadísticamente significativa. Se encontró una correlación inversa moderada con la tasa de incremento anual de sobrepeso para IDH y PIB per cápita. Conclusión: Hubo una tendencia de crecimiento en la cobertura del SISVAN. El aumento del sobrepeso se asoció con la desigualdad social.


Objetivo: analisar a tendência temporal da cobertura do Sistema de Vigilância Alimentar e Nutricional (SISVAN) e do estado nutricional de idosos, e sua correlação com indicadores de desigualdade social no Brasil, no período 2008-2019. Métodos: estudo ecológico, sobre registros do SISVAN relativos à população na idade de 60 anos ou mais; analisaram-se a tendência temporal da cobertura e a correlação entre indicadores de desigualdade social e taxa de incremento do estado nutricional; os índices angular e de concentração foram utilizados para medir desigualdades absolutas e relativas. Resultados: foram identificados 11.587.933 registros de idosos; a cobertura nacional evoluiu de 0,1% (2008) para 2,9% (2019), com tendência de aumento estatisticamente significativa; foi encontrada correlação inversa moderada com taxa de incremento anual de sobrepeso, para índice de desenvolvimento humano e produto interno bruto per capita. Conclusão: houve tendência de crescimento da cobertura do SISVAN; o aumento de sobrepeso esteve associado à desigualdade social.

OBJECTIVE: to compare different perceptions of Brazilian and Portuguese health students about health promotion in the university. METHOD: cross-sectional, descriptive analytical study that included 669 university students in the health courses of two universities, one Brazilian and the other Portuguese. The Instrument of Assessment for Health Promotion at University was used in both institutions and the comparisons were performed with the Chi-square test and odds ratio estimation. RESULTS: there was a predominance of students aged up to 24 years (89.2%) and females (78.3%). In general, Portuguese university students have better perceptions of the physical activity, eating habits and psychosocial factors domains (p < 0.001). Brazilian university students presented better perceptions in the domains of environmental factors and complementary and alternative therapies (p < 0.001). CONCLUSIONS: the results show that Brazilian and Portuguese students have significant differences in their perception of aspects related to health promotion at university. It is concluded that the university environment is a promising field to carry out educational practices that promote health and that this context differs between countries.

INTRODUCTION: Supplementation with probiotics for patients with chronic kidney disease (CKD) may be associated with decreased systemic inflammation. OBJECTIVE: To assess the impact of oral supplementation with probiotics for patients with CKD on hemodialysis. METHOD: This double-blind randomized clinical trial included 70 patients on hemodialysis; 32 were given oral supplementation with probiotics and 38 were in the placebo group. Blood samples were collected at the start of the study and patients were given oral supplementation with probiotics or placebo for three months. The probiotic supplement comprised four strains of encapsulated Gram-positive bacteria: Lactobacillus Plantarum A87, Lactobacillus rhamnosus, Bifidobacterium bifidum A218 and Bifidobacterium longum A101. Patients were given one capsule per day for 3 months. Blood samples were taken throughout the study to check for inflammatory biomarkers. Non-traditional biomarkers Syndecan-1, IFN-y, NGAL, and cystatin C were measured using an ELISA kit, along with biochemical parameters CRP, calcium, phosphorus, potassium, PTH, GPT, hematocrit, hemoglobin, glucose, and urea. RESULTS: Patients given supplementation with probiotics had significant decreases in serum levels of syndecan-1 (239 ± 113 to 184 ± 106 ng/mL, p = 0.005); blood glucose levels also decreased significantly (162 ± 112 to 146 ± 74 mg/dL, p = 0.02). CONCLUSION: Administration of probiotics to patients with advanced CKD was associated with decreases in syndecan-1 and blood glucose levels, indicating potential improvements in metabolism and decreased systemic inflammation.

Abstract Objective: To describe the prevalence of underweight and obesity indicators among individuals registered as traditional peoples and communities in the Food and Nutrition Surveillance System, across Brazil, in 2019. Methods: This was a descriptive study using individual secondary data from participants receiving care in the Primary Health Care within the Brazilian National Health System. Results: In the study population (N = 13,944), there was a higher prevalence of short stature among male children and adolescents (14.2%), when compared to their female counterparts (11.8%); in the adult female population, there was a higher prevalence of obesity (23.0%), when compared to the male population (11.3%); the prevalence of low height-for-age in riverine communities (18.5%) and obesity in the adult faxinalense population (75.1%) stood out. Conclusion: Anthropometric disparities between different communities require tailored responses, emphasizing targeted primary health care and programs to ensure food and nutrition security.


Resumen Objetivo: Describir la prevalencia de indicadores de bajo peso y obesidad entre individuos registrados como pueblos y comunidades tradicionales en el Sistema de Vigilancia Alimentaria y Nutricional, en todo Brasil, en 2019. Métodos: Estudio descriptivo, con datos secundarios individuales de participantes atendidos en atención primaria de Salud del Sistema Único de Salud. Resultados: En la población estudiada (N = 13.944), hubo mayor prevalencia de talla baja en niños y adolescentes en el sexo masculino (14,2%), cuando se compara con el femenino (11,8%); en la población adulta femenina hubo mayor prevalencia de obesidad (23,0%), en comparación con la masculina (11,3%); se destacó la prevalencia de baja talla para la edad en comunidades ribereñas (18,5%), y obesidad en la población adulta faxinalense (75,1%). Conclusiones: Las heterogeneidades antropométricas entre comunidades requieren respuestas adaptadas, con énfasis en la atención primaria de salud y en garantizar la seguridad alimentaria y nutricional.

Métodos: Estudo descritivo, com dados individualizados secundários de participantes atendidos na Atenção Básica do Sistema Único de Saúde. Resultados: Na população estudada (N = 13.944), houve maior prevalência de altura baixa entre crianças e adolescentes do sexo masculino (14,2%), quando comparados aos do sexo feminino (11,8%); na população adulta do sexo feminino, observou-se maior prevalência de obesidade (23,0%), quando comparada à do sexo masculino (11,3%); destacou-se a prevalência de altura baixa para a idade nas comunidades ribeirinhas (18,5%), e de obesidade na população adulta faxinalense (75,1%). Conclusões: As heterogeneidades antropométricas entre as comunidades exigem respostas adaptadas, com ênfase na atenção primária à saúde e na garantia da segurança alimentar e nutricional.

ABSTRACT Objective The aim of this study was to evaluate whether the depiction of a fat body for health communication on the Instagram social network by the national societies governing obesity management present elements that enhance weight-related prejudice, with the slim body as a reference. Methods We investigated the last publication quintile, totaling 2,155 publications. A total of 72 images were included and 99 bodies were decoded. The bodies were classified according to positive and negative characteristics into four categories (body presentation, clothing, food and moving behavior) in which the negative characteristics have the potential to enhance the prejudice. The chi-square test was applied to test the difference in the proportion of bias elements associated with body weight between the fat body and the thin body. Results The fat body was more represented (p≤0.05 for all) headless (32.3% vs 9.2%), with bare abdomen (17.6% vs 0%), with a focus on the abdomen (11% vs 0% ), with tight clothes (32.3% vs 0%), sad expression (23.5% vs 6.1%), involved with food of low nutritional value (14.7% vs 0%) and in sedentary behavior ( 11% vs 0%) compared to the thin body. Conclusion National societies that govern management of obesity presented the fat body with more negative elements that enhance the prejudice associated with excess weight, which is an important public health problem.


RESUMO Objetivo O objetivo do presente estudo foi avaliar se a representação do corpo gordo nas imagens para a comunicação em saúde em perfis de sociedades nacionais diretivas orientadas ao manejo da obesidade na rede social Instagram apresentam elementos que potencializam o preconceito relacionado ao peso, tendo como referência o corpo magro. Métodos Foi analisado o último quintil de postagens, totalizando 2.155 publicações. Setenta e duas imagens foram incluídas e 99 corpos foram decodificados. Os corpos foram classificados em quatro categorias (apresentação do corpo, vestimenta, alimentação e comportamento de movimento) de acordo com características positivas e negativas, sendo que as negativas possuem potencial para reforçar o preconceito. O teste qui-quadrado foi aplicado para testar a diferença na proporção de elementos de preconceito relacionados ao peso entre o corpo gordo e o corpo magro. Resultados O corpo gordo foi mais representado (p≤0,05 para tudo) sem cabeça (32,3% vs 9,2%), com abdomen nu (17,6% vs 0%), com foco no abdomen (11% vs 0%), com roupa apertada (32,3% vs 0%), expressão triste (23,5% vs 6,1%), envolvido com alimento de baixo valor nutricional (14,7% vs 0%) e relacionado a um comportamento sedentário (11% vs 0%) em comparação ao corpo magro. Conclusão Sociedades nacionais diretivas ao manejo da obesidade apresentaram o corpo gordo com mais elementos negativos que reforçam o preconceito relacionado ao peso, sendo importante problema de saúde pública.
