With Europe's ageing population and rising demand for palliative care, it is crucial to examine the use of palliative care among older adults during their last years of life and understand the factors influencing their access and end-of-life circumstances. This study employed a cohort of SHARE participants aged 65 years or older who had passed away between Wave 6 (2015) and Wave 7 (2017). Information on death circumstances, palliative care utilization, and associated variables were analysed. The study revealed that nearly 13.0% of individuals across these countries died under palliative care, with Slovenia having the lowest rate (0.3%) and France the highest (30.4%). Palliative care utilization in the last 30 days before death was observed in over 24.0% of participants, with the Czech Republic having the lowest rate (5.0%) and Greece the highest (48.8%). A higher risk of using or dying in palliative care was significantly associated with cognitive impairment (low verbal fluency), physical inactivity, and good to excellent self-perceived health. This work highlights the urgent need for enhanced global access to palliative care and advocates for the cross-country comparison of effective practices within Europe, tailored to the unique healthcare needs of older adults.

Considering the multidimensionality of chronic pain, it is crucial to develop comprehensive strategies for its effective management. However, establishing well-defined, evidence-based guidelines for such approaches remains challenging. To overcome this, we present the finding from a 4-month intervention to enhance the management of non-cancer chronic pain in older adults with pre-frailty and frailty. The intervention's core elements comprised a multidisciplinary individualized plan, a case manager, and patient education. This pilot study involved 22 participants (≥65 years). It assessed changes in pain frequency and intensity (pain scale), frailty (Fried frailty phenotype criteria), and medication adherence (Brief Adherence Rating Scale) before and after the 4-month intervention. The results were encouraging: pain frequency and intensity and frailty score tended to decrease, and medication adherence showed significant improvement. This preliminary small-scale pilot study provides a foundation for further research and for exploring the potential scalability of this multidisciplinary patient-centred intervention.

BACKGROUND: Recognizing the need for palliative care (PC) and referral to PC teams improves patients' quality of life. However, in patients with moderate/severe clinical complexity, early recognition of the need for PC may not correspond to referral to specialized PC services. The definition for clinical complexity is still underexplored, as well as the instruments available to assess complexity. This scoping review aims to gather relevant information on the definition of clinical complexity in PC, as well as on the instruments used to objectively assess complexity. METHODS: According to the methodology of a Scoping Review, the keywords: "palliative care", "hospitalization criteria", "complexity criteria", "complexity assessment" and "clinical complexity", were searched in PubMed, Scopus, Cochrane, and b-on databases, during April 2022, for relevant information on the definition and/or approach and/or protocols related to clinical complexity in patients followed in PC, or on the instruments used to assess it, regardless of study design, the language, or year of publication. RESULTS: From the 626 references found, 15 studies were included in the review. According to these studies, complexity may be organized/defined into 3, 4, or 6 domains, generally including the patient, the family, the health system, and the socio-cultural context. Of the 13 instruments mentioned for the objective assessment of complexity, the HexCom, IDC-Pal, and the recent ID-PALL seem to offer the broadest determinations of complexity. CONCLUSIONS: Complexity is a dynamic process, which reflects the reality of patients and families, and patients, families, and health professionals' perceptions, and so it must be systematically adjusted to the stage of the disease. The definition of complexity and the development and use of suitable instruments can help to identify, assess, and improve patients' quality of life, while supporting their family across the grieving process. Yet, this may not always be summarized in a quantitative value by easy-to-use instruments, highlighting the role of PC interdisciplinary teams.

The artery of Percheron (AOP) is an abnormal variant of the arterial supply of the thalamus. Stroke caused by AOP occlusion is seldom reported. AOP leads to bilateral thalamic and rostral midbrain infarct presenting with unspecific manifestations. There are few descriptions of case series of stroke caused by AOP. We sought to review the clinicoradiological characteristics of AOP infarction from Algarve, Southern Portugal. Eight consecutive cases were retrospectively identified by searching the electronic clinical charts, as well as the stroke Unit database (2015-2020). Sociodemographic (age and gender) and clinicoradiological characteristics (etiological classification, admission severity, manifestations, and short- and long-term prognoses) were retrieved. The corresponding frequency of AOP infarction was 0.17% (95% confidence interval: 0.05-0.28). The mean age was 67.1 (range: 60-80) years. The range of stroke severity evaluated assessed by the National Institute of Health Stroke Scale ranged from 5 to 23 (median = 7.5). None of the patients receive acute ischemic stroke reperfusion treatment. AOP patterns were isolated bilateral paramedian thalamic ( n = 2), bilateral paramedian and anterior thalamic ( n = 2), and bilateral paramedian thalamic with rostral midbrain ( n = 4). Two patients (20%) died on the short term (30 days). At hospital discharge, six patients had functional disability of ≤2 on the modified Rankin scale. In the follow-up at 6 months, half ( n = 3) of the survivors had persistent hypersomnia and two had vascular dementia. Stroke from AOP presents with variable clinical and radiological presentations and patients do not receive alteplase. The short-term survivor and the long-term functional independency can be compromised after AOS infarct.

BACKGROUND: Migraine affects more than a billion people all over the world and requires critical employment of healthcare resources. Telemedicine could be a reasonable tool to manage people suffering from headaches, and it received a big push from the COVID-19 pandemic. OBJECTIVE: This review aims to propose a practical approach for the virtual management of these patients. METHODS: To do this, we conducted a literature search, including 32 articles relevant to the topic treated in this review. RESULTS: The most challenging step in telemedicine applied to practical neurology remains the clinical assessment, but through a careful headache history and a recently proposed entirely virtual neurological assessment, this hitch can be easily overcome. Electronic diary compilations and virtual administration of disability-measuring scales, conversely, are the key features of effective long-term follow-up although we do not have apps that met the criteria of scientific reliability. Furthermore, tele-rehabilitation seems to be effective and has demonstrated to be a solution to alternatively treat chronic patients at home, and can be considered part of the remote management of headache patients. Moreover, virtual management of headaches finds an application in specific communities of patients, as pediatric patients and for rural communities of low- and middle-income countries suffer from health disparities, with inadequate resources and knowledge gaps. CONCLUSION: Telemedicine could be promising for patients with no regular or convenient access to headache specialists and seems to be a priority in managing migraine patients to avoid non-urgent hospitalizations.

PURPOSE: To describe a case of fulminant idiopathic intracranial hypertension (IIH) in a child with "malignant" presentation. CASE REPORT: A 16-year-old, previously healthy, girl presented with bilateral visual loss and bilateral global limitation of eye movements in the absence of headache. Extensive laboratory evaluation for infectious, inflammatory, autoimmune, and neoplastic conditions was negative. Magnetic resonance imaging (MRI) of the brain and lumbar puncture findings were consistent with a diagnosis of IIH. Extraocular motility improved in the next few days as well as optic disc edema but visual acuity remained poor. CONCLUSION: The authors believe that the acute, severe, and fulminant ("malignant") presentation with markedly elevated intracranial pressure may produce the unique presentation of severe vision loss and bilateral complete ophthalmoplegia. Interestingly, there was no headache. To our knowledge this is the first such case to be reported in the English language ophthalmic literature.

O abscesso do psoas traduz uma situação rara, de etiologia variada e fisiopatologia complexa, mas ainda não clarificada. O objetivo deste estudo foi relatar um caso de apresentação rara de abscesso do psoas sob a forma de derrame pleural. Paciente do sexo masculino, 63 anos, com quadro de dispneia e deterioração do estado geral, associado à febre (38,3ºC). Apresentava perda ponderal de 10kg, taquipneia e semiologia pulmonar compatível com derrame pleural direito. Os dados laboratoriais revelaram leucocitose associada à alteração da função hepática, proteína C-reativa e velocidade de sedimentação elevadas. Radiografia do tórax revelou derrame pleural direito. A tomografia computadorizada confirmou a presença de derrame pleural e de abscesso do psoas homolateral, tendo sido iniciada antibioterapia empírica com piperacilina/tazobactam e metronidazol. Realizou, posteriormente, drenagem guiada por tomografia computadorizada do abscesso do psoas. O exame cultural foi positivo para Streptococcus anginosus no líquido pleural, abscesso do psoas e sangue. Após drenagem de abcesso do psoas, foi mantida a imagem compatível com derrame pleural, tendo sido realizada nova drenagem torácica do derrame pleural. Após vários dias, retirou-se a drenagem torácica. Obteve alta hospitalar sendo referenciado à consulta de medicina. Devido à originalidade do caso, visto não existirem casos descritos com essa forma de apresentação, nem por esse agente etiológico, realizou-se uma revisão da literatura do diagnóstico e tratamento dessa doença.


Psoas abscess represents a rare situation of varied etiology and complex pathophysiology that has not yet been clarified. This study aimed to report an unusual presentation of psoas abscesso in the form of pleural effusion. Male patient, 63 years old, with signs of dyspnea and deterioration of general condition, associated with fever (100,94ºF). He presented with 10-kg weight loss, tachypnea, and pulmonary symptomatology that was consistent with right pleural effusion. Laboratory data showed leukocytosis with alteration of liver function, elevated C-reactive protein, and high erythrocyte sedimentation rate. Chest radiograph revealed right pleural effusion. Computed tomography confirmed the presence of pleural effusion and ipsilateral psoas abscess. Empirical antibiotic therapy with piperacillin/tazobactam and metronidazole was initiated, and computed tomography-guided drainage of the psoas abscess was performed. Culture was positive for Streptococcus anginosus in pleural fluid, psoas abscess and blood. After drainage of the psoas abscess, the image that was consistent with pleural effusion remained, and new chest drainage of pleural effusion has been held. After several days, the chest tube was removed. The patient was discharged and referred to clinical consultation. Because of the originality of the case, since there are no cases describing this presentation nor these etiologic agent, literature review of the diagnosis and treatment of this pathology has been conducted

Tolosa-Hunt syndrome is a steroid responsive painful opthalmoplegia due to a nonspecific inflammation of the cavernous sinus. Autoimmune hemolytic anemia is caused by antibodies directed against unmodified autologous red cells. They are both rare conditions. Here we describe the simultaneous occurrence of Tolosa-Hunt syndrome and severe hemolytic crisis in the same patient.
