OBJECTIVE: Cognitive insight refers to the ability to distance oneself from and evaluate one's own beliefs and interpretations. Little is known about whether cognitive insight could influence medication adherence in schizophrenia patients. This study examined the role of cognitive insight in medication adherence and how it would interact with neuropsychological functions. METHODS: Ninety clinically-stable schizophrenia patients completed the Beck's Cognitive Insight Scale (BCIS) and tasks measuring prospective (PM) and other neurocognitive functions. Medication adherence was estimated using a multi-axial method comprising interview, clinician-rating, pharmacy refill record and pill counting. Correlational and regression analyses were conducted to examine whether cognitive insight and PM would be associated with mediation adherence. Post-hoc mediational analysis was performed to examine the interplay between cognitive insight, PM and medication adherence. RESULTS: Clinical insight and cognitive insight together significantly influenced participants' medication adherence, after neurocognitive functions and psychopathology were accounted for. Time-based PM, compared with other neurocognitive functions, affected medication adherence more strongly. CONCLUSIONS: Cognitive insight complements clinical insight in affecting medication adherence in schizophrenia patients.

INTRODUCTION: In Hong Kong, persons in custody receive primary medical care within the institutions of the Correctional Services Department (CSD). However, for psychiatric care, persons in custody must attend specialist out-patient clinics (SOPCs), which may cause embarrassment and stigmatisation. The aim of this interventional pilot study was to compare teleconsultations with face-to-face consultations for a group of stable Chinese psychiatric out-patients in custody. METHODS: A total of 86 stable Chinese male out-patients in custody were recruited for psychiatric teleconsultations. They were compared with 249 age-matched Chinese male out-patients in custody attending standard face-to-face psychiatric consultations at other SOPCs. The two groups had comparable baseline characteristics including age, education level, and 12-item Chinese General Health Questionnaire (C-GHQ-12) score. A satisfaction survey of patients towards the teleconsultation was also carried out. RESULTS: Compared with the face-to-face consultation group, the teleconsultation group showed a significantly better result in the difference in C-GHQ-12 scores before and after consultations (P=0.023). The correlation between the first and second teleconsultations also showed a moderate positive relationship (r=0.309). The satisfaction survey showed a favourable response to teleconsultations. No significant adverse events were identified for the teleconsultation group. CONCLUSIONS: The results suggest that teleconsultations are a sustainable and safe alternative to face-to-face consultations for stable Chinese psychiatric out-patients in custody.

We report a facile route for the green synthesis of trimethyl chitosan nitrate-capped silver nanoparticles (TMCN-AgNPs) with positive surface charge. In this synthesis, silver nitrate, glucose, and trimethyl chitosan nitrate (TMCN) were used as silver precursor, reducing agent, and stabilizer, respectively. The reaction was carried out in a stirred basic aqueous medium at room temperature without the use of energy-consuming or expensive equipment. We investigated the effects of the concentrations of NaOH, glucose, and TMCN on the particle size, zeta potential, and formation yield. The AgNPs were characterized by UV-vis spectroscopy, photon correlation spectroscopy, laser Doppler anemometry, transmission electron microscopy, X-ray diffraction, and X-ray photoelectron spectroscopy. The catalytic activity of the TMCN-AgNPs was studied by the reduction of 4-nitrophenol using NaBH4 as a reducing agent. We evaluated the antibacterial effects of the TMCN-AgNPs on Acinetobacter baumannii, Escherichia coli, Pseudomonas aeruginosa, and Staphylococcus aureus using the broth microdilution method. The results showed that both gram-positive and gram-negative bacteria were killed by the TMCN-AgNPs at very low concentration (<6.13µg/mL). Moreover, the TMCN-AgNPs also showed high antibacterial activity against clinically isolated multidrug-resistant A. baumannii strains, and the minimum inhibitory concentration (MIC) was ≤12.25µg/mL.

The combination of increasing ethnic diversity in North America and growing consumer support for local food products may present opportunities for local producers and processors in the ethnic foods product category. Our study examined the ethnic Chinese (pop. 402,000) market for salted and preserved duck eggs in Vancouver, British Columbia (BC), Canada. The objective of the study was to develop a segmentation model using survey data to categorize consumer groups based on their attitudes and the importance they placed on product attributes. We further used post-segmentation acculturation score, demographics and buyer behaviors to define these groups. Data were gathered via a survey of randomly selected Vancouver households with Chinese surnames (n = 410), targeting the adult responsible for grocery shopping. Results from principal component analysis and a 2-step cluster analysis suggest the existence of 4 market segments, described as Enthusiasts, Potentialists, Pragmatists, Health Skeptics (salted duck eggs), and Neutralists (preserved duck eggs). Kruskal Wallis tests and post hoc Mann-Whitney tests found significant differences between segments in terms of attitudes and the importance placed on product characteristics. Health Skeptics, preserved egg Potentialists, and Pragmatists of both egg products were significantly biased against Chinese imports compared to others. Except for Enthusiasts, segments disagreed that eggs are 'Healthy Products'. Preserved egg Enthusiasts had a significantly lower acculturation score (AS) compared to all others, while salted egg Enthusiasts had a lower AS compared to Health Skeptics. All segments rated "produced in BC, not mainland China" products in the "neutral to very likely" range for increasing their satisfaction with the eggs. Results also indicate that buyers of each egg type are willing to pay an average premium of at least 10% more for BC produced products versus imports, with all other characteristics equal. Overall results indicate that opportunities exist for local producers and processors: Chinese Canadians with lower AS form a core part of the potential market.

Consumers' interest in specialty eggs appears to be growing in Europe and North America. The objective of this research was to develop logistic regression models that utilise purchaser attributes and demographics to predict the probability of a consumer purchasing a specific type of table egg including regular (white and brown), non-caged (free-run, free-range and organic) or nutrient-enhanced eggs. These purchase prediction models, together with the purchasers' attributes, can be used to assess market opportunities of different egg types specifically in British Columbia (BC). An online survey was used to gather data for the models. A total of 702 completed questionnaires were submitted by BC residents. Selected independent variables included in the logistic regression to develop models for different egg types to predict the probability of a consumer purchasing a specific type of table egg. The variables used in the model accounted for 54% and 49% of variances in the purchase of regular and non-caged eggs, respectively. Research results indicate that consumers of different egg types exhibit a set of unique and statistically significant characteristics and/or demographics. For example, consumers of regular eggs were less educated, older, price sensitive, major chain store buyers, and store flyer users, and had lower awareness about different types of eggs and less concern regarding animal welfare issues. However, most of the non-caged egg consumers were less concerned about price, had higher awareness about different types of table eggs, purchased their eggs from local/organic grocery stores, farm gates or farmers markets, and they were more concerned about care and feeding of hens compared to consumers of other eggs types.

Tardive dystonia is an iatrogenic complication of dopamine receptor antagonist medication such as first-generation antipsychotics. It occurs in up to 2% of patients and only 10% recover after stopping medication. Deep brain stimulation for primary dystonia has proven to be effective and its application for secondary dystonias is gaining acceptance. We report our experience in treating three ethnic Chinese schizophrenia patients with severe medically refractory tardive dystonia by globus pallidus internus deep brain stimulation. Preoperatively, all required assistance with essential activities of daily living and two were bed-bound. The mean Burke-Fahn-Marsden Dystonia Rating Scale score was 61 (range, 44-80) and mean Global Dystonia Rating Scale score was 47 (range, 40-52). No procedure-related complications were encountered. By 3 months all could return to unassisted living and walk with support with a mean of 77% and 66% improvement in the Burke-Fahn-Marsden Dystonia Rating Scale and Global Dystonia Rating Scale scores, respectively. Quality-of-life assessment performed for two patients using the EuroQol-5 dimensions visual analogue scale showed a mean improvement of 86% at 3 months. On clinical follow-up, the effect was well maintained for a period of 3 to 10 years. Pallidal deep brain stimulation is a safe and highly effective form of symptomatic treatment for patients with medically refractory tardive dystonia.

Ostrich (Struthio camelus) production is a relatively young industry and there has been little research on ostrich welfare during pretransport handling and the transportation process. A heavy body with a high center of gravity makes ostriches' handling and transportation problems different from other livestock. The main objective of this study was to investigate the effects of the pretransport holding time duration on ostrich behavior and physiological responses. A second objective was to identify and validate behavioral indicator(s) that could be used to identify stressed birds during pretransport handling. Prior to shipping, twenty-four 2.5-yr-old ostriches were moved into a holding pen. Birds were then individually restrained, hooded, and walked from the holding pen (approximately 12 min/bird) to a sampling pen (visually isolated from the holding pen) where they were weighed and a 10-mL blood sample obtained. A second blood sample was taken from each bird after a 1,100-km transportation. Blood samples were analyzed for concentrations of blood metabolites, enzymes, corticosterone, and white blood cell and differential counts. Behavioral responses and physical damages of ostriches were also recorded before and after transport. Results indicated that birds that spent longer time in the pretransport holding pen had higher pretransport plasma concentrations of aspartate aminotransferase, alanine aminotransferase, sodium, and packed cell volume. Immobile sitting behavior, observed in 5 out of the last 11 birds handled, was positively correlated with higher pretransport handling stress, higher posttransport aspartate aminotransferase, alanine aminotransferase, creatine phosphokinase, and glucose concentrations, and transport losses. Knowledge of pretransport handling impacts on ostrich stress and availability of behavioral indicators (e.g., immobile sitting response) could be used to improve handing processes, thereby decreasing potential weight loss, injury, and mortality.

Appropriate management of an ostrich's exposure to stressors during preslaughter handling and transport practices can improve its well-being and product quality. Because of the lack of information about ostrich farming and transportation in North America and lack of developed Codes of Practice for ratite transport in Canada and the United States, the first objective of our research was to identify current preslaughter handling and transport practices of the ostrich industry in Canada and the United States, and to identify potential welfare issues based on the current practices. The second objective of this research was to review ostrich transport welfare standards and guidelines from Australia, European Union, New Zealand, and South Africa to investigate if those guidelines are applicable to Canadian and American ostrich production systems. Preliminary producer interviews, on-farm visits, and literature review information sources were used to design a producer questionnaire that was used to survey producers by Internet and mail surveying methods to identify existing ostrich transport norms in Canada and the United States. Based on the results of our producer survey and review of the transport standards and guidelines, we conclude that following factors are potential ostrich handling and transport welfare issues in Canada and the United States: lack of scientific information about welfare of ostriches during handling and transport; unfamiliarity of handlers and birds with handling and transport practices; not considering birds' social bounds, sex, behavior, and physical state in mixing them during handling and transport process; lack of an established specific maximum water and feed withdrawal duration for ostrich transport in Canada and the United States; lack of a specific vehicle designed for ratite transportation in Canada and the United States considering different physical body characteristics of ostriches compared with other species; exposure of birds to natural light during transport inside the trailer; overcrowding; and long transportation in Canada and the United States. Results of this research will contribute toward developing Codes of Practice for preslaughter handling, transportation, and slaughter of ostriches in Canada and the United States.

OBJECTIVES. To evaluate the efficacy and complications of endovascular treatment for ruptured internal carotid artery pseudoaneurysms following irradiation of nasopharyngeal carcinoma. DESIGN. Retrospective case series. SETTING. Tertiary neurosurgical referral unit of a Hong Kong public hospital. PATIENTS. Patients with ruptured radiation-induced internal carotid artery pseudoaneurysms that were treated endovascularly from October 1999 to October 2011 at Queen Elizabeth Hospital were reviewed. Hospital records, imaging, and angiographic data were studied. RESULTS. During the study period, 15 such nasopharyngeal carcinoma patients were treated by endovascular means at Queen Elizabeth Hospital. Ten presented with epistaxis, three with otorrhagia (bleeding from the ear), and two with both. Therapeutic occlusion of the affected internal carotid artery was performed in four patients, and stenting of the artery (with or without coil obliteration of the pseudoaneurysm) was performed in 11. Immediate haemostasis was achieved in all cases. One (7%) of the 15 patients endured symptomatic recurrence of the pseudoaneurysm, and in another an asymptomatic residual pseudoaneurysm was noted in the follow-up angiogram. Three patients suffered clinically significant procedure-related complications, including cerebral infarction (n=2) and brain abscess (n=1). In the angiograms obtained after a mean post-treatment interval of 13 (range, 0.7-60) months, the stent patency rate was 67%. All three patients with occluded stents were asymptomatic. CONCLUSIONS. Ruptured internal carotid artery pseudoaneurysms following radiotherapy is a rare but life-threatening condition. Endovascular treatment by occlusion or reconstruction of the internal carotid artery with stents provides immediate haemostasis and obliteration of the pseudoaneurysms, with a low recurrence rate. Long-term follow-up is necessary to look out for delayed post-treatment complications.

Blood profiling is a helpful tool in detecting the health status, metabolic diseases, nutritional deficiencies, and welfare of animals. Body weights, body temperatures, hematological and serum biochemical parameters, enzymes, and electrolytes in both sexes of farm emus at the beginning of their breeding season (November in Canada), were determined. The reference interval for each analyte was also calculated. Emus have lower body temperatures (37.2 ± 0.2) than other poultry species. There was no significant between-sex difference in BW, body temperature, and all the hematological and enzyme parameters measured. However, females had significantly (P < 0.001) higher serum calcium, phosphorus, albumin, total protein, globulin, and triglyceride levels than males, probably in preparation for egg laying. We also examined our findings in light of their sex-role reversal in incubation and brooding. Contrary to other avian species in which only females incubate and brood, there was no sex difference in the hematological and enzyme parameters measured in emus. We found that emus are similar to other ratite species with respect to the changes in protein, globulin, triglyceride, and calcium levels. The findings from our study contribute to the database for reference emu hematological and serum enzyme, metabolite, and electrolyte values.

Ovaries from Rhode Island Red donors were transplanted orthotopically into White Leghorn recipients. At maturation, recipients were mated with Rhode Island Red roosters to test the origin of their ovaries, using plumage coloration as a marker. A chick with chimeric plumage coloration was produced, indicating mechanisms that produce follicles with both donor and recipient ovarian contents. This study suggests that ovarian transplantation could be useful for in vivo studies of cytological and molecular mechanisms involved in avian folliculogenesis.

Cryopreservation of testicular tissue is a promising method of preserving male reproductive potential for avian species. This study was conducted to assess whether a vitrification method can be used to preserve avian testicular tissue, using the Japanese quail (Coturnix japonica) as a model. A simple vitrification method that included dimethyl sulphoxide, ethylene glycol, and sucrose as cryoprotective agents, and allowed the storage of tissue in a sealed macrotube was applied to the testicular tissue from 1-wk-old Japanese quail. The vitrified tissue was warmed at room temperature or at 40°C. After warming, tissue was implanted onto the chorioallantoic membrane of 8- to 9-d-old chicken embryos and the vascularization of the grafts was evaluated. When compared with fresh tissue, the tissue that had been warmed at 40°C showed no difference in vascularization. The tissue that had been warmed at room temperature was significantly less vascularized than the fresh tissue. Vitrification of testicular tissue and storage in macrotubes provide a promising model for preservation and recovery of male germplasm of avian species.

No drugs have been approved for the treatment of parasitic nematodes in pigeons, but ivermectin, a broad-spectrum endectocide, has been used extra-label by prescription. Producers currently allow for a 2-wk withdrawal time before marketing squabs. However, because its use is extra-label there is no legal maximum residue limit for ivermectin in squab meat. The purpose of this study was to examine the depletion of ivermectin (passed by the parents to the squabs) from the tissues of squab. Adult pigeons brooding squab were treated with ivermectin in their drinking water (3.3 µg/mL) for 3 d. After dosing the parents, the ivermectin concentration of the breast meat and liver of squabs was found to be greater than the maximum residual limits established for livestock, indicating that ivermectin was transferred from the parents to the squabs. However, ivermectin was not detected in either the breast meat or the livers of squabs 1 wk after dosing. These results indicate that there is a rapid decline in tissue levels of ivermectin in squab.

Vein of Galen aneurysmal malformation is a rare entity in the paediatric population. However, it is being recognised with increasing frequency due to better diagnostic techniques. Neonates usually present with congestive heart failure, while in older infants and children it tends to manifest with seizures, hydrocephalus, intracerebral or subarachnoid haemorrhages. We present a case of ruptured vein of Galen aneurysmal malformation in a 3-month-old baby boy treated by transarterial embolisation using Guglielmi detachable coils.

The maintenance of bone strength has been an important issue in the debate over cage use for laying hens. Bone strength depends on adequate mechanical load and cages restrict movement. Four laying crosses (Lohmann White, Lohmann Brown, H&N White, and Rhode Island Red × Barred Plymouth Rock cross hens) were housed in conventional cages or in floor pens equipped with perches and nest boxes to measure the effect of the housing system on bone strength. Approximately 15 hens of each genotype from each housing system were killed at 50 wk of age and the radius and tibia of each were removed for analysis. There were no differences between the Lohmann White and H&N White (White Leghorn) hens, likely because of their similar genetic background. The Lohmann Brown and the cross hens (brown-egg layers) were larger and they had heavier bones, but the bone density was not different from that of the other lines. The radius was heavier for hens kept in floor pens than for those kept in cages, but the tibia was not. When hens were kept in floor pens, both bones had greater cortical bone density and cross-sectional area, but the difference between housing systems in cortical bone cross-sectional area was much greater for the radius than it was for the tibia. Although the movement of hens in cages is limited, they spend a great deal of time standing, which puts a mechanical load on the tibia. Hens in floor pens are able to stretch their wings or fly, in contrast to hens kept in cages, which likely explains why the difference between housing systems in cortical bone was greater for the radius than for the tibia.

Transplantation of ovarian tissue between different breeds of chickens or Japanese quail can result in normal egg production and give rise to donor-derived offspring with a high frequency. The feasibility of interspecific ovarian transplantation was evaluated by transplanting ovarian tissue from Muscovy ducks to Pekin ducks just after hatch, with subsequent treatment of the recipients with an immunosuppressant. Among the 8 mature Pekin ducks that had received ovarian tissue from Muscovy ducklings, 2 produced fertile eggs when mated with Muscovy drakes, and one of these produced only Muscovy ducks, which were derived from the transplanted Muscovy ovary. The other produced mule ducks that were hybrids of the host Pekin duck and the Muscovy drake. Further development of interspecific transplantation of ovaries may provide a simple method for the conservation and propagation of endangered birds.

Microsatellite marker LEI0258 was used as an indicator to examine the variability of the major histocompatibility complex (MHC) region in 2 commercial layer flocks, 1 experimental layer cross, and 5 noncommercial flocks (used for free-run and free-range meat and egg production). We hypothesized that the populations from noncommercial sources may have more diversity in MHC genes than that in the commercial-source populations. Two related parameters, heterozygosity and the number of alleles harbored by a population, were used to assess the genetic variability. The different combinations of the 22 alleles created 66 genotypes in the 8 chicken populations that were studied. The noncommercial populations, except for the Silkies (SK), harbored more alleles than those in the 2 commercial populations, Lohmann Brown and Lohmann White. The observed heterozygosity of the MHC region was high in all of the populations, except for SK. Considering the 2 parameters we have examined, we can generalize that the intensively selected commercial egg-layer varieties seem to have less genetic variability in their MHC regions compared with that of the noncommercial flocks, which are less intensively selected. The LEI0258 variants can be used as markers to detect most of the MHC haplotypes, but in the different populations the same allele size may not always be associated with the same serologically defined haplotype. The information obtained from this study will be useful for genetic resource conservation and the development of breeding stocks that are suitable for free-range production.

The effectiveness of diatomaceous earth (DE) as a treatment against parasites and to increase feed efficiency and egg production of organically raised free-range layer hens was evaluated in 2 breeds of commercial egg layers [Bovan Brown (BB) and Lowmann Brown (LB)] that differ in their resistance to internal parasitic infections. Half the hens of each breed were fed diets supplemented with DE (2%). Their internal parasite loads were assessed by biweekly fecal egg counts (FEC) and by postmortem examination of the gastrointestinal tract. Supplementing DE in diets of LB hens, the more parasite-resistant breed, did not significantly affect their FEC and adult parasite load. However, BB hens treated with dietary DE had significantly lower Capillaria FEC, slightly lower Eimeria FEC, fewer birds infected with Heterakis, and significantly lower Heterakis worm burden than control BB hens. Both BB and LB hens fed the diet containing DE were significantly heavier, laid more eggs, and consumed more feed than hens fed the control diet, but feed efficiency did not differ between the 2 dietary treatments. Additionally, BB hens consuming the DE diet laid larger eggs containing more albumen and yolk than hens consuming the control diet. In a subsequent experiment, the effectiveness of DE to treat a Northern fowl mite (Ornithonyssus sylviarum) infestation was tested. Relative to controls, both breeds of hens that were dusted with DE had reduced number of mites. The results of this study indicate the DE has the potential to be an effective treatment to help control parasites and improve production of organically raised, free-range layer hens.

In addition to regular (white and brown) eggs, alternative types of table eggs (e.g., free-run, free-range, organic) are available in the Canadian market, and their market growth rate has been high during the last decade in British Columbia (BC). The objective of our research was to identify associations between consumers' attitudes, preferences, and demographic characteristics with their consumption of different types of table eggs. An online survey was conducted in June 2009 to gather information from adult BC residents. Sixty-eight percent of the 1,027 randomly selected subjects completed the survey. Our survey indicated that the consumption of cage-free specialty eggs (free-run, free-range, and organic) has strongly increased in BC to 32.9% free-range eggs, 11.93% organic eggs, and 7.6% free-run eggs in 2009 compared with a Print Measurement Bureau consumer survey that showed combined 8% consumption of cage-free specialty eggs in 2007. Results of our survey indicated that, compared with consumers of white regular eggs, consumers of free-range eggs came from smaller households and had a higher education level and income. These consumers indicated that factors of health, nutritional value, environmental issues, and animal welfare were important in egg type selection. Although most consumers rated the specialty eggs as having a higher nutritional value than white regular eggs, price became the most important deciding factor for those consumers who selected white regular eggs. Our findings indicate that increased consumption and increased differentiation exist in the table egg market and this in turn provides support for more research to increase the efficiency of cage-free egg production systems and for better consumer education.

RESUMEN

Intracranial aneurysm in twins is a rare clinical disease entity. Only 15 cases have been described in the literature. We report on a pair of identical twins with intracranial aneurysms. One presented with subarachnoid haemorrhage; digital subtraction angiography showed a left posterior communicating artery aneurysm, which was treated by coiling. The patient's twin sister was called for screening, whereupon digital subtraction angiography revealed a right ophthalmic internal carotid artery aneurysm that was treated conservatively.
