RESUMEN

In-situ API crystallization in carrier matrices has attracted extensive attention in recent years for its advantages over traditional preparation processes. However, due to the lack of systemic research on molecular self-assembly behaviors, the products obtained by in-situ crystallization suffer from the problems of polymorphic transformation and drug expulsion during storage, limiting its industrial application. This paper investigates the in-situ sequential crystallization behavior of tristearin (SSS) and fenofibrate (FEN), utilizing SSS as the carrier and FEN as the API. It was found that the behavior of mixed crystallization significantly differs from single-component crystallization, including direct formation of stable form of SSS and the rapid crystallization of FEN. During the crystallization process, the melting FEN promotes the movement of SSS molecules, while the sliding of SSS lamellae, in turn, provides a mechanical stimulus to enhance the nucleation of FEN. Based on the observed synergistic crystallization behavior, the distribution and stability of the API within FEN solid lipid microparticles (SLMs) during storage were evaluated, while also examining the stability variations in SLMs formulated at different cooling rates and drug loading concentrations. The findings indicate that the initial nucleated FEN results in a decrease in the surrounding molten FEN and the irregularity of the SSS lamellas, thereby preventing the remaining molten FEN from achieving complete crystallization within a brief period. Due to the compatibility between FEN and SSS, some SSS may blend with the molten FEN, potentially resulting in further crystallization during storage and consequently increasing the risk of drug expulsion.

RESUMEN

The prognosis of acromelanomas (AM) is worse. The objective of this study was to investigate the clinical features of distant metastasis of AM and the factors affecting the survival and prognosis of patients. In this study, a retrospective study was conducted to select 154 AM patients admitted to Nanjing Pukou People's Hospital from January 2018 to April 2021 for clinical research. The clinical characteristics of distant metastasis were statistically analyzed, and the survival curve was drawn with 5-year follow-up outcomes. The median survival time of the patients was calculated, and the clinicopathological features and peripheral blood laboratory indexes of the surviving and dead patients were analyzed. Logistic regression model was used to analyze the risk factors affecting the prognosis of AM patients. In this study, 154 patients with AM were treated, including 88 males and 76 females, aged from 27 to 79 years old, with an average age of (59.3 ±â€…11.7) years old. Among them, 90 cases had distant metastasis. The main metastatic sites were lung (47.78%) and lymph nodes (42.22%). Among them, single site metastasis accounted for 41.11% and multiple site metastasis 58.89%. 89 cases survived and 65 cases died. The survival time was 22 months to 60 months, and the median survival time was 48.0 months. The Breslow thickness, stage at diagnosis, distant metastasis, site of metastasis and ulceration were compared between the survival group and the death group (P < .05). serum lactate dehydrogenase (LDH), neutrophil-to-lymphocyte ratio (NLR) and lymphocyte monocyte ratio (LMR) were compared between the survival group and the death group (P < .05). The results of Logistic regression model showed that LDH ≥ 281 U/L, NLR ≥ 2.96, LMR ≤ 3.57, newly diagnosed stage > stage II, distant metastasis, multiple site metastasis and tumor ulcer were independent risk factors for poor prognosis of AM patients (P < .05). Patients with AM had a higher proportion of distant metastasis, mainly lung and lymph node metastasis. Increased LDH, increased NLR, decreased LMR, higher initial stage, distant metastasis, multiple site metastasis, and combined tumor ulcer were closely related to the poor prognosis of patients after surgery.

RESUMEN

Non-alcoholic fatty liver disease (NAFLD), a chronic liver condition and metabolic disorder, has emerged as a significant health issue worldwide. D-mannose, a natural monosaccharide widely existing in plants and animals, has demonstrated metabolic regulatory properties. However, the effect and mechanism by which D-mannose may counteract NAFLD have not been studied. In this study, network pharmacology followed by molecular docking analysis was utilized to identify potential targets of mannose against NAFLD, and the leptin receptor-deficient, genetically obese db/db mice was employed as an animal model of NAFLD to validate the regulation of D-mannose on core targets. As a result, 67 targets of mannose are predicted associated with NAFLD, which are surprisingly centered on the mechanistic target of rapamycin (mTOR). Further analyses suggest that mTOR signaling is functionally enriched in potential targets of mannose treating NAFLD, and that mannose putatively binds to mTOR as a core mechanism. Expectedly, repeated oral gavage of supraphysiological D-mannose ameliorates liver steatosis of db/db mice, which is based on suppression of hepatic mTOR signaling. Moreover, daily D-mannose administration reduced hepatic expression of lipogenic regulatory genes in counteracting NAFLD. Together, these findings reveal D-mannose as an effective and potential NAFLD therapeutic through mTOR suppression, which holds translational promise.

RESUMEN

ObjectiveTo investigate the clinical features and maternal and fetal outcomes of acute pancreatitis in pregnancy (APIP) and the risk factors for disease aggravation, and to establish a predictive model. MethodsA retrospective analysis was performed for 52 APIP patients who were admitted to Affiliated Hospital of Zunyi Medical University from January 2017 to December 2022, and according to disease severity, they were divided into mild acute pancreatitis (MAP) group with 32 patients, moderate-severe acute pancreatitis (MSAP) group with 8 patients, and severe acute pancreatitis (SAP) group with 12 patients. The logistic regression analysis was performed for the clinical data of each group, and the receiver operating characteristic (ROC) curves were plotted to assess the value of risk factors in predicting the severity of APIP. A one-way analysis of variance was used for comparison of normally distributed continuous data between multiple groups, and the least significant difference t-test was used for further comparision between two groups. The Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between multiple groups, and the Wilcoxon rank-sum test was used for further comparision between two groups; the chi-square test was used for comparison of categorical data between groups. ResultsOf all patients in terms of etiology, 26 (50%) had hyperlipidemic pancreatitis, 20 (38.4%) had biliary pancreatitis, and 6 (11.5%) had idiopathic pancreatitis. In terms of gestational week, 1 patient (1.9%) was in early pregnancy, 25 (48.1%) were in mid-pregnancy, and 26 (50.0%) were in late pregnancy. A total of 10 patients (19.2%) had acute respiratory distress syndrome (ARDS), among whom 9 (90%) required respiratory support. There were significant differences between the patients with different severities of APIP in aspartate aminotransferase, alanine aminotransferase, blood urea nitrogen, blood glucose, C-reactive protein (CRP), international normalized ratio (INR), pneumonia, ARDS, sepsis, hepatic insufficiency, and coagulation dysfunction (all P<0.05). The univariate analysis showed that the severity of APIP was associated with blood glucose, blood urea nitrogen, CRP, and pneumonia (all P<0.05), and pneumonia was a risk factor for the aggravation of APIP (odds ratio=18.938, 95% confidence interval: 1.020 ‍— ‍351.747, P=0.048). CRP, blood glucose, blood urea nitrogen, and INR used in combination had a larger area under the ROC curve than each index used alone (0.954 vs 0.778/0.796/0.721/0.801). ConclusionPneumonia is a risk factor for the aggravation of APIP, and the combination of CRP, blood glucose, blood urea nitrogen, and INR can be used to predict the severity of APIP.

RESUMEN

ObjectiveTo investigate the potential mechanisms of Jiawei Jingqin Decoction (加味荆芩汤) in the treatment of rosacea. MethodsFifty 8-week-old female BALB/c mice were randomly divided into normal control group, model group, and Jiawei Jingqin Decoction low, medium, and high-dose groups, with 10 mice in each group. Except for the normal control group, mice in the other groups were injected with 40μl of antimicrobial peptide LL-37 at a concentration of 320 μmol/L on the back, repeated once every 12 hours for a total of 4 injections, to induce the rosacea mouse model. The mice in the normal control group were injected with 40 μl of PBS solution at the same time. After successful modeling, the mice in the Jiawei Jingqin Decoction low, medium, and high-dose groups were orally administered Jiawei Jingqin Decoction at doses of 4.35, 8.71, and 17.42 g/(kg·d) respectively, while the mice in the normal control group and the model group were orally administered 20 ml/(kg·d) of normal saline. All groups were treated once daily for 5 days. The changes in skin lesions were observed, and the erythema area was measured and scored. Skin tissue at the injection site was collected for histopathological examination using HE staining, and the number of mast cells was detected using toluidine blue staining. The levels of interleukin-6 (IL-6), interleukin-1β (IL-1β), and tumor necrosis factor-alpha (TNF-α) in the skin lesion tissues were measured using the ELISA method. The mRNA expression levels of key factors in the innate immune response pathway of rosacea disease mechanism, including Toll-like receptor 2 (TLR2), downstream molecules serine protease kallikrein 5 (KLK5) and matrix metalloproteinase 9 (MMP-9), as well as related inflammatory factors (IL-1β, IL-6, TNF-α), and adaptive immune CD4+ T cell-related factors interferon-gamma (INF-γ) mRNA were detected using RT-qPCR. Immunohistochemical staining was performed to detect the protein expression of KLK5, MMP-9, and the number of CD4+ T-positive cells. Western blot analysis was used to determine the protein expression levels of CD4+ T cell-related factors including IFN-γ, CC chemokine receptor 5 (CCR5), signal transducer and activator of transcription 1 (STAT1), signal transducer and activator of transcription 3 (STAT3). ResultsThe skin at the injection site on the back of mice in the normal control group appeared normal without any skin lesions or erythema, and histopathological examination showed no infiltration of inflammatory cells. The model group had significantly higher erythema scores and larger erythema areas compared to the normal control group (P<0.05), and histopathological examination revealed a significant infiltration of inflammatory cells, with some signs of deep connective tissue damage. Compared to the model group, the Jiawei Jingqin Decoction medium and high-dose groups showed a significant decrease in erythema scores and erythema areas (P<0.05), as well as a reduction in inflammatory exudates and inflammatory cell infiltration. However, the Jiawei Jingqin Decoction low-dose group did not show significant difference (P>0.05), but there was a slight reduction in inflammatory cell infiltration compared to the model group. When compared to the normal control group, the rosacea skin tissues of the model group showed an increase in the number of mast cells, levels of IL-1β, IL-6, TNF-α, mRNA expression of IL-1β, IL-6, TNF-α, TLR2, INF-γ, KLK5, MMP-9, protein expression of KLK5, MMP-9, and the number of CD4+ T-positive cells (P<0.05). Compared to the model group, the Jiawei Jingqin Decoction medium and high-dose groups showed a decrease in the number of mast cells, levels of IL-1β, mRNA expression of IL-1β, TNF-α, and the number of CD4+ T-positive cells. The Jiawei Jingqin Decoction high-dose group showed a decrease in IL-6, TNF-α levels, mRNA expression of IL-6, TLR2, INF-γ, MMP-9, protein expression of MMP-9, KLK5, and the number of CD4+ T-positive cells, as well as a decrease in IFN-γ and STAT1 protein expression (P<0.05) compared to the Model group. ConclusionJiawei Jingqin Decoction has significant improvement effect on the skin symptoms of roseacne in a mouse model. Its mechanism might be related to regulating skin immune inflammatory reactions, thereby reducing the release of pathogenic factors and inflammatory factors.

Methods The data were collected from 4,263 Chinese and English articles on tigecycline resistance in CNKI, Wanfang, VIP and Web of Science (WOS) databases from 2012 to 2022. CiteSpace 5.8.R3 software was used to analyze the cooperative network of authors, the cooperative network of countries and institutions, the total citation times of journals, and keywords included in the literature, to reveal the hotspots and trends of tigecycline resistance research. Results The number of articles published in English literature was higher than that in Chinese literature. China had the largest number of published documents, showing a significant international academic influence in this research field. Countries all over the world were concerned about the resistance of tigecycline, but Chinese literatures focused more on the clinical infection and prevention of tigecycline resistance, while English literatures placed special emphasis on the research about the drug resistance mechanism of tigecycline. Conclusion The research direction at home and abroad is basically the same, but the research focus has gradually shifted from the clinical treatment and monitoring of tigecycline to the molecular level of drug resistance mechanism.

RESUMEN

Background Air quality health index (AQHI) is derived from exposure-response coefficients calculated from air pollution and morbidity/mortality time series, which helps to understand the overall short-term health impacts of air pollution. Objective To study the effects of common air pollutants on respiratory diseases in Urumqi and to develop an AQHI for the risk of respiratory diseases in the city. Methods The daily outpatient volume data of respiratory diseases from The First Affiliated Hospital of Xinjiang Medical University, meteorological data (daily mean temperature and daily mean relative humidity), and air pollutants [fine particulate matter (PM2.5), inhalable particulate matter (PM10), sulfur dioxide (SO2), nitrogen dioxide (NO2), carbon dioxide (CO), and ozone (O3)] in Urumqi City, Xinjiang, China were collected from January 1, 2017 to December 31, 2021. A distributed lag nonlinear model based on quasi-Poisson distribution was constructed by time-stratified case crossover design. Adopting zero concentration of air pollutants as reference, the exposure-response coefficient (β value) was used to quantify the impact of included air pollutants on the risk of seeking medical treatment for respiratory diseases, and the AQHI was established. The association of between AQHI and the incidence of respiratory diseases and between air quality index (AQI) and the incidence of respiratory diseases was compared to evaluate the prediction effect of AQHI. Results Each 10 µg·m−3 increase in PM10, SO2, NO2, and O3 concentrations presented the highest excess risk of seeking outpatient services at 3 d cumulative lag (Lag03) and 2d cumulative lag (Lag02), with increased risks of morbidity of 0.687% (95%CI: 0.101%, 1.276%), 17.609% (95%CI: 3.253%, 33.961%), 13.344% (95%CI: 8.619%, 18.275%), and 4.921% (95%CI: 1.401%, 8.502%), respectively. There was no statistically significant PM2.5 or CO lag effect. An AQHI was constructed based on a model containing PM10, SO2, NO2, and O3, and the results showed that the excess risk of respiratory disease consultation for the whole population, different genders, ages, or seasons for each inter-quartile range increase in the AQHI was higher than the corresponding value of AQI. Conclusion PM10, SO2, NO2, and O3 impact the number of outpatient visits for respiratory diseases in Urumqi, and the constructed AQHI for the risk of respiratory diseases in Urumqi outperforms the AQI in predicting the effect of air pollution on respiratory health.

RESUMEN

BACKGROUND@#Transcription factor (TF) can bind specific sequences that either promotes or represses the transcription of target genes, and exerts important effects on tumorigenesis, migration, invasion. Staphylococcal nuclease-containing structural domain 1 (SND1), which is a transcriptional co-activator, is considered as a promising target for tumor therapy. However, its role in lung adenocarcinoma (LUAD) remains unclear. This study aims to explore the role of SND1 in LUAD.@*METHODS@#Data from The Cancer Genome Atlas (TCGA), Gene Expression Omnibus (GEO), Clinical Proteomic Tumor Analysis Consortium (CPTAC), and Human Protein Atlas (HPA) database was obtained to explore the association between SND1 and the prognosis, as well as the immune cell infiltration, and subcellular localization in LUAD tissues. Furthermore, the functional role of SND1 in LUAD was verified in vitro. EdU assay, CCK-8 assay, flow cytometry, scratch assay, Transwell assay and Western blot were performed.@*RESULTS@#SND1 was found to be upregulated and high expression of SND1 is correlated with poor prognosis of LUAD patients. In addition, SND1 was predominantly present in the cytoplasm of LUAD cells. Enrichment analysis showed that SND1 was closely associated with the cell cycle, as well as DNA replication, and chromosome segregation. Immune infiltration analysis showed that SND1 was closely associated with various immune cell populations, including T cells, B cells, cytotoxic cells and dendritic cells. In vitro studies demonstrated that silencing of SND1 inhibited cell proliferation, invasion and migration of LUAD cells. Besides, cell cycle was blocked at G1 phase by down-regulating SND1.@*CONCLUSIONS@#SND1 might be an important prognostic biomarker of LUAD and may promote LUAD cells proliferation and migration.

RESUMEN

BACKGROUND: Human pluripotent stem cell (hPSC)-derived hepatocyte-like cells (HLCs) can provide a valuable in vitro model for disease modelling and drug development. However, generating HLCs with characteristics comparable to hepatocytes in vivo is challenging. Extracellular matrix (ECM) plays an important role in supporting liver development and hepatocyte functions, but their impact on hepatocyte differentiation and maturation during hPSC differentiation remains unclear. Here, we investigate the effects of two ECM components-Matrigel and type I collagen on hepatic differentiation of human embryonic stem cells (hESCs). METHODS: hESC-derived HLCs were generated through multistage differentiation in two-dimensional (2D) and three-dimensional (3D) cultures, incorporating either type I collagen or Matrigel during hepatic specification and maturation. The resulting HLCs was characterized for their gene expression and functionality using various molecular and cellular techniques. RESULTS: Our results showed that HLCs cultured with collagen exhibited a significant increase in albumin and alpha-1 anti-trypsin expression with reduced AFP compared to HLCs cultured with Matrigel. They also secreted more urea than Matrigel cultures. However, these HLCs exhibited lower CYP3A4 activity and glycogen storage than those cultured with Matrigel. These functional differences in HLCs between collagen and Matrigel cultures closely resembled the hepatocytes of periportal and pericentral zones, respectively. CONCLUSION: Our study demonstrates that Matrigel and collagen have differential effects on the differentiation and functionality of HLCs, which resemble, to an extent, hepatic zonation in the liver lobules. Our finding has an important impact on the generation of hPSC-HLCs for biomedical and medical applications.

RESUMEN

Vehicular ad hoc networks (VANETs) are used for improving traffic efficiency and road safety. However, VANETs are vulnerable to various attacks from malicious vehicles. Malicious vehicles can disrupt the normal operation of VANET applications by broadcasting bogus event messages that may cause accidents, threatening people's lives. Therefore, the receiver node needs to evaluate the authenticity and trustworthiness of the sender vehicles and their messages before acting. Although several solutions for trust management in VANETs have been proposed to address these issues of malicious vehicles, existing trust management schemes have two main issues. Firstly, these schemes have no authentication components and assume the nodes are authenticated before communicating. Consequently, these schemes do not meet VANET security and privacy requirements. Secondly, existing trust management schemes are not designed to operate in various contexts of VANETs that occur frequently due to sudden variations in the network dynamics, making existing solutions impractical for VANETs. In this paper, we present a novel blockchain-assisted privacy-preserving and context-aware trust management framework that combines a blockchain-assisted privacy-preserving authentication scheme and a context-aware trust management scheme for securing communications in VANETs. The authentication scheme is proposed to enable anonymous and mutual authentication of vehicular nodes and their messages and meet VANET efficiency, security, and privacy requirements. The context-aware trust management scheme is proposed to evaluate the trustworthiness of the sender vehicles and their messages, and successfully detect malicious vehicles and their false/bogus messages and eliminate them from the network, thereby ensuring safe, secure, and efficient communications in VANETs. In contrast to existing trust schemes, the proposed framework can operate and adapt to various contexts/scenarios in VANETs while meeting all VANET security and privacy requirements. According to efficiency analysis and simulation results, the proposed framework outperforms the baseline schemes and demonstrates to be secure, effective, and robust for enhancing vehicular communication security.
