RESUMEN

BACKGROUND AND AIMS: Ustekinumab (UST) is an effective biologic for treatment of inflammatory bowel disease (IBD). However, some patients treated with UST have suboptimal clinical response with standard dosing. The aims of this study were to determine the effectiveness of UST dose intensification (DI), identify factors associated with DI, cumulative incidence of DI and persistence of UST among treated patients. METHODS: Clinical data of patients with Crohn's disease (CD) and ulcerative colitis (UC) who received UST from September 2017 to October 2022 in Singapore General Hospital were collected. Primary outcome was defined as achieving corticosteroid-free clinical remission, biochemical remission, endoscopic healing and/or transmural healing (CD). Statistical analysis was performed to identify factors, which are predictive of UST DI and effectiveness of UST DI. RESULTS: Forty-two patients (34 CD and 8 UC) underwent UST DI to either 6-weekly (n = 19, 45.2%) or 4-weekly (n = 23, 35.9%) and the median time to intensification was 31.1 weeks (17.8-65.7). Presence of perianal disease in CD (HR 4.9; 1.47-16.4) was associated with DI. After DI, 16 (38%) patients achieved primary outcome by week 52. The overall drug persistence rates at 1 year and 2 years were 75.7% (95% CI 62.9-84.6) and 63.5% (95% CI 49.9-74.3), respectively. CONCLUSION: Two third of IBD patients underwent DI while on UST treatment and the median time to DI was about 6 months after induction. CD patients with perianal disease is more likely to undergo DI. More than one third of dose-intensified patients achieved remission by week 52.

BACKGROUND: YouTube is a popular online platform where patients often visit for information. However, the validity of the content on spondyloarthritis (SpA) on YouTube is not known. OBJECTIVES: To evaluate the content, reliability, and quality of the most viewed English-language YouTube videos on SpA. METHODS: Keywords "spondyloarthritis," "spondyloarthropathy," and "ankylosing spondylitis" were searched on YouTube on October 7, 2019. The top 270 videos were screened. Videos were excluded if they were irrelevant, in non-English language, or if they had no audio. Total number of views, duration on YouTube (days), video length, upload date, and number of likes, dislikes, subscribers, and comments were recorded for videos. A modified 5-point DISCERN tool and the 5-point Global Quality Scale (GQS) score were used to assess the reliability and quality of the videos. RESULTS: Two hundred videos were included in the final analysis (62% from healthcare professionals, 37% from patients, and 2% from news channels). Useful information, useful patient opinion, misleading patient opinion, and misleading information comprised o60%, 26%, 11%, and 3% of videos respectively. Majority of misleading videos were uploaded by patients (82%). Misleading videos commonly included wrong clinical features and unproven alternative treatments of SpA. Videos by healthcare professionals had more useful information, higher reliability, and GQS scores. CONCLUSIONS: Majority of YouTube videos have useful information on SpA and are important educational sources to patients. However, rheumatologists should be aware that misleading patient opinions on alternative therapies can contain inaccurate information and should hence actively correct these misinformation during their clinic consults Key Points • The majority of videos on Spondyloarthritis found on YouTube are deemed useful and are uploaded by healthcare professionals. • The majority of misleading videos were uploaded by patients and the main theme of misinformation was on clinical features and treatment of spondyloarthritis.

Musculoskeletal ultrasound (MSUS) is gaining popularity among rheumatologists, especially in the context of rheumatoid arthritis (RA) joint assessment, as it is a non-invasive, radiation-free imaging modality that is relatively easy to set up in a clinic setting. Although ultrasonography (US) is often regarded as being operator dependent with associated reproducibility issues, the use of consensus-based scoring system along with standardized definition of joint inflammation in RA has been shown to improve its performance/reliability as an outcome measurement tool. Through this review article, we have (i) gone through the principle US findings in RA joint assessment, (ii) discussed various scoring systems for evaluation of US joint pathologies, (iii) described the literature in the use of US in areas of RA diagnosis and disease prognostication and (iv) examined the findings of recent major randomized controlled trials incorporating US as monitoring tools to help target treatment in RA. By doing so, we hope to provide clinicians with an insight into the role of musculoskeletal US imaging in areas of RA diagnosis, prognosis and disease monitoring.

Markers of cardiac dysfunction such as amino terminal pro-brain natriuretic peptide (NTpro-BNP) and high sensitivity cardiac troponin T (hs-cTnT) may be associated with dementia. However, limited data exist on their association with either pre-dementia stages, that is, cognitive impairment no dementia (CIND), or the burden of cerebrovascular diseases (CeVD).We therefore, examined the association of these biomarkers of cardiac dysfunction with CeVD in both CIND and dementia.A case-control study, with cases recruited from memory clinics and controls from memory clinics and community. All subjects underwent collection of blood samples, neuropsychological assessment, and neuroimaging. Subjects were classified as CIND and dementia based on clinical criteria whilst significant CeVD was defined as the presence of cortical infarcts and/or more than 2 lacunes and/or confluent white matter lesions in two regions of brain on Age-Related White Matter Changes Scale.We included a total of 35 controls (mean age: 65.9 years), 78 CIND (mean age: 70.2 years) and 80 cases with dementia (mean age: 75.6 years). Plasma concentrations of hs-cTnT were associated significantly with CeVD in both CIND (odds ratios [OR]: 9.05; 95% confidence interval [CI]: 1.64-49.79) and dementia (OR: 16.89; 95%CI: 2.02-142.67). In addition, NTpro-BNP was associated with dementia with CeVD (OR: 7.74; 95%CI: 1.23-48.58). These associations were independent of other vascular risk factors.In this study, we showed that plasma NTproBNP and hs-cTnT are associated with dementia and CIND, only when accompanied by presence of CeVD.
