RESUMEN

The adoption and integration of the Internet of Things (IoT) have become essential for the advancement of many industries, unlocking purposeful connections between objects. However, the surge in IoT adoption and integration has also made it a prime target for malicious attacks. Consequently, ensuring the security of IoT systems and ecosystems has emerged as a crucial research area. Notably, advancements in addressing these security threats include the implementation of intrusion detection systems (IDS), garnering considerable attention within the research community. In this study, and in aim to enhance network anomaly detection, we present a novel intrusion detection approach: the Deep Neural Decision Forest-based IDS (DNDF-IDS). The DNDF-IDS incorporates an improved decision forest model coupled with neural networks to achieve heightened accuracy (ACC). Employing four distinct feature selection methods separately, namely principal component analysis (PCA), LASSO regression (LR), SelectKBest, and Random Forest Feature Importance (RFFI), our objective is to streamline training and prediction processes, enhance overall performance, and identify the most correlated features. Evaluation of our model on three diverse datasets (NSL-KDD, CICIDS2017, and UNSW-NB15) reveals impressive ACC values ranging from 94.09% to 98.84%, depending on the dataset and the feature selection method. Notably, our model achieves a remarkable prediction time of 0.1 ms per record. Comparative analyses with other recent random forest and Convolutional Neural Networks (CNN) based models indicate that our DNDF-IDS performs similarly or even outperforms them in certain instances, particularly when utilizing the top 10 features. One key advantage of our novel model lies in its ability to make accurate predictions with only a few features, showcasing an efficient utilization of computational resources.

Metabolic dysfunction-associated fatty liver disease (MAFLD) or metabolic dysfunction-associated steatotic liver disease (MASLD), has become the leading cause of chronic liver disease worldwide. Optimal dietary intervention strategies for MAFLD are not standardized. This study aimed to achieve consensus on prevention of MAFLD through dietary modification. A multidisciplinary panel of 55 international experts, including specialists in hepatology, gastroenterology, dietetics, endocrinology and other medical specialties from six continents collaborated in a Delphi-based consensus development process. The consensus statements covered aspects ranging from epidemiology to mechanisms, management, and dietary recommendations for MAFLD. The recommended dietary strategies emphasize adherence to a balanced diet with controlled energy intake and personalized nutritional interventions, such as calorie restriction, high-protein, or low-carbohydrate diets. Specific dietary advice encouraged increasing the consumption of whole grains, plant-based proteins, fish, seafood, low-fat or fat-free dairy products, liquid plant oils, and deeply colored fruits and vegetables. Concurrently, it advised reducing the intake of red and processed meats, saturated and trans fats, ultra-processed foods, added sugars, and alcohol. Additionally, maintaining the Mediterranean or DASH diet, minimizing sedentary behavior, and engaging in regular physical activity are recommended. These consensus statements lay the foundation for customized dietary guidelines and proposing avenues for further research on nutrition and MAFLD.

Perovskite solar cells (PSCs) hold potential for low-cost, high-efficiency solar energy, but their sensitivity to moisture limits practical application. Current fabrication requires controlled environments, limiting mass production. Researchers aim to develop stable PSCs with longer lifetimes under ambient conditions. In this research work, we investigated the stability of perovskite films and solar cells fabricated and annealed in natural air using four different anti-solvents: toluene, ethyl acetate, diethyl ether, and chlorobenzene. Films (about 300 nm thick) were deposited via single-step spin-coating and subjected to ambient air-atmosphere for up to 30 days. We monitored changes in crystallinity, electrical properties, and optics over time. Results showed a gradual degradation in the films' crystallinity, morphology, and electro-optical properties. Notably, films made with ethyl acetate exhibited superior stability compared to other solvents. These findings contribute to advancing stable and high-performance PSCs manufactured under normal ambient conditions. In addition, we also discuss the possible machine learning (ML) approach to our future work direction to optimize the materials structures, and synthesis process parameters for future high-efficient perovskite solar cells fabrication.

The article details a feasibility study of removing Brilliant Green (BG), a mutagenic dye from an aqueous solution by adsorption using low-cost coriander seed spent as a by-product in the nutraceutical industry. The study includes an analysis of the parameters that affect the adsorption process. The variables that have been identified include pH, dye concentration, process temperature, adsorbent amount, and particle size of the adsorbent. To obtain information on the adsorption process and to design the mechanism of the adsorption system on experimental equilibrium, 10 isotherm models, namely, Langmuir, Freundlich, Jovanovic, Dubinin-Radushkevich, Sips, Redlich-Peterson, Toth, Vieth-Sladek, Brouers-Sotolongo, and Radke-Prausnitz were applied. It was discovered that the experimental adsorption capacity, qe, was roughly 110 mg g-1. The result has a maximum adsorption of 136.17 mg g-1 as predicted by Dubinin-Radushkevich isotherm. Diffusion film models, Dumwald-Wagner and Weber-Morris models, and pseudo-first- and second-order models, were used to determine the adsorption kinetics. It was realized that the adsorption kinetics data fit into a pseudo-second-order model. Thermodynamic analysis with a reduced enthalpy change suggests a physical process. The values of the thermodynamic parameters ΔG0, ΔH0, and ΔS0 demonstrated an endothermic and nearly spontaneous process of adsorption. The small valuation of ΔH0 specifies that the process is physical. FTIR spectroscopy and SEM imaging were used to confirm that the BG dye had been adsorbing on the adsorbent surface. The study concludes that NICSS is an effective adsorbent to extract BG dye from wastewater solutions, offers insights into numerous dye and adsorbent interaction possibilities and indicates that the process can be scaled to fit into the concept of circular economy.


An attempt has been made to link the concept of circular economy through design and execution of the experiments in the laboratory scale. The following highlights will justify the newer approach adopted by the authors.The experiments are designed by intention to suit the concept of circular economy.The use of NICSS, a nutraceutical industrial spent, which has no feed, fertilizer, or fuel value suits the sustainability concept.The reuse of "waste" from the remediation process replaces the "end-of-life" concept in circular economy.

BACKGROUND: Common metabolic diseases, such as type 2 diabetes mellitus (T2DM), hypertension, obesity, hypercholesterolemia, and metabolic dysfunction-associated steatotic liver disease (MASLD), have become a global health burden in the last three decades. The Global Burden of Disease, Injuries, and Risk Factors Study (GBD) data enables the first insights into the trends and burdens of these metabolic diseases from 1990 to 2021, highlighting regional, temporal and differences by sex. METHODS: Global estimates of disability-adjusted life years (DALYs) and deaths from GBD 2021 were analyzed for common metabolic diseases (T2DM, hypertension, obesity, hypercholesterolemia, and MASLD). Age-standardized DALYs (mortality) per 100,000 population and annual percentage change (APC) between 1990 and 2021 were estimated for trend analyses. Estimates are reported with uncertainty intervals (UI). RESULTS: In 2021, among five common metabolic diseases, hypertension had the greatest burden (226 million [95 % UI: 190-259] DALYs), whilst T2DM (75 million [95 % UI: 63-90] DALYs) conferred much greater disability than MASLD (3.67 million [95 % UI: 2.90-4.61]). The highest absolute burden continues to be found in the most populous countries of the world, particularly India, China, and the United States, whilst the highest relative burden was mostly concentrated in Oceania Island states. The burden of these metabolic diseases has continued to increase over the past three decades but has varied in the rate of increase (1.6-fold to 3-fold increase). The burden of T2DM (0.42 % [95 % UI: 0.34-0.51]) and obesity (0.26 % [95 % UI: 0.17-0.34]) has increased at an accelerated rate, while the rate of increase for the burden of hypertension (-0.30 % [95 % UI: -0.34 to -0.25]) and hypercholesterolemia (-0.33 % [95 % UI: -0.37 to -0.30]) is slowing. There is no significant change in MASLD over time (0.05 % [95 % UI: -0.06 to 0.17]). CONCLUSION: In the 21st century, common metabolic diseases are presenting a significant global health challenge. There is a concerning surge in DALYs and mortality associated with these conditions, underscoring the necessity for a coordinated global health initiative to stem the tide of these debilitating diseases and improve population health outcomes worldwide.

RESUMEN

The most common chronic liver illness worldwide is metabolic dysfunction linked to fatty liver disease (MAFLD), which is poorly understood by doctors and patients. Many people with this disease develop steatohepatitis, cirrhosis and its consequences, as well as extrahepatic manifestations; these conditions are particularly common if they are linked to diabetes mellitus or obesity. A breakthrough with numerous benefits is the switch from NAFLD to MAFLD in terms of terminology and methodology. The diagnosis of MAFLD is based on affirmative criteria; unlike NAFLD, it is no longer based on exclusion. The diagnosis of MAFLD and the evaluation of steatosis and fibrosis is achieved using liver biopsy and non-invasive laboratory or radiographic techniques. We briefly address the most recent developments in MAFLD epidemiology and diagnosis.

The humongous increase in carbon emissions in the past few decades presents an environmental challenge to the scientific community. The current study proposes a method of taxation on high-carbon emission fuels. For this purpose, a comparative enviro-economic analysis is carried out on the three most commonly used fuels (gasoline, Liquefied Petroleum Gas (LPG), and Compressed Natural Gas (CNG)). The speed of the test engine varied from 1800 to 4200 Revolution per Minute (RPM) in increments of 400 RPM. Performance parameters (Brake Power (BP), Brake Thermal Efficiency (BTHE), and Brake Specific Fuel Consumption (BSFC)) were measured using a hydro dynamometer. Emission analysis, including Carbon Dioxide (CO2), Carbon Monoxide (CO), Unburned Hydrocarbons (HC), and Nitrogen Oxide (NOx), was conducted using the TESTO 350 analyzer. The application of Weibull distribution with a 95 % confidence interval, on emission data, explained the adequacy of the data. Among test fuels, CNG emerged as an environment-friendly fuel with an emission reduction of 16, 42, and 43 percent for CO2, CO, and HC in comparison to gasoline. Also, BTHE and BSFC of CNG were better than other alternatives. Moreover, the carbon penalty for CNG fuel showed a price reduction of 32 and 20.8 percent in comparison to gasoline and LPG respectively. The study provides a novel approach to assess the environmental impact of fuels by economic analysis based on emitted carbon quantity. In addition, this very idea is novel in promoting the Sustainable Development Goals (SDG) of the United Nations (UN) through carbon taxation.

This research aims to provide the groundwork for smartly categorizing hand movements for use with prosthetic hands. The hand motions are classified using surface electromyography (sEMG) data. In reaction to a predetermined sequence of fibre activation, every single one of our muscles contracts. They could be useful in developing control protocols for bio-control systems, such human-computer interaction and upper limb prostheses. When focusing on hand gestures, data gloves and vision-based approaches are often used. The data glove technique requires tedious and unnatural user engagement, whereas the vision-based solution requires significantly more expensive sensors. This research offered a Deep Neural Network (DNN) automated hand gesticulation recognition system based on electromyography to circumvent these restrictions. This work primarily aims to augment the concert of the hand gesture recognition system via the use of an artificial classifier. To advance the recognition system's classification accuracy, this study explains how to build models of neural networks and how to use signal processing methods. By locating the Hilbert Huang Transform (HHT), one may get the essential properties of the signal. When training a DNN classifier, these characteristics are sent into it. The investigational results reveal that the suggested technique accomplishes a better categorization rate (98.5 % vs. the alternatives).

BACKGROUND: With the implementation of the 11th edition of the International Classification of Diseases (ICD-11) and the publication of the metabolic dysfunction-associated fatty liver disease (MAFLD) nomenclature in 2020, it is important to establish consensus for the coding of MAFLD in ICD-11. This will inform subsequent revisions of ICD-11. METHODS: Using the Qualtrics XM and WJX platforms, questionnaires were sent online to MAFLD-ICD-11 coding collaborators, authors of papers, and relevant association members. RESULTS: A total of 890 international experts in various fields from 61 countries responded to the survey. We also achieved full coverage of provincial-level administrative regions in China. 77.1% of respondents agreed that MAFLD should be represented in ICD-11 by updating NAFLD, with no significant regional differences (77.3% in Asia and 76.6% in non-Asia, p = 0.819). Over 80% of respondents agreed or somewhat agreed with the need to assign specific codes for progressive stages of MAFLD (i.e. steatohepatitis) (92.2%), MAFLD combined with comorbidities (84.1%), or MAFLD subtypes (i.e., lean, overweight/obese, and diabetic) (86.1%). CONCLUSIONS: This global survey by a collaborative panel of clinical, coding, health management and policy experts, indicates agreement that MAFLD should be coded in ICD-11. The data serves as a foundation for corresponding adjustments in the ICD-11 revision.

BACKGROUND AND AIMS: Diagnostic criteria for metabolic dysfunction-associated steatotic liver disease (MASLD) have been proposed but not yet validated. This study aimed to compare the diagnostic accuracy of the MASLD definition with the existing criteria for metabolic dysfunction-associated fatty liver disease (MAFLD) in identifying patients with significant fibrosis. METHODS: The analysis included a total of 8317 individuals who had complete biochemical and liver ultrasonography data from the National Health and Nutrition Examination Survey (2017-2020). In this study, significant fibrosis (≥ F2) was determined by a median liver stiffness of ≥ 8.0 kPa. To identify independent factors associated with significant fibrosis, multivariable logistic regression analyses were applied. RESULTS: MAFLD (OR 3.44; 95% CI 2.88-4.12; P < 0.0001) has a trend for stronger and independent association with significant fibrosis compared to MASLD (OR 2.63; 95% CI 2.22-3.11; P < 0.0001). Non-MASLD MAFLD is independently associated with a 14.28-fold higher odds of significant fibrosis compared to non-MAFLD MASLD. The sensitivity for detecting significant fibrosis for MAFLD and MASLD was 76.23% vs 69.94%, respectively. The performance of MAFLD remains consistent in a sub-analysis of patients with no or mild alcohol intake. CONCLUSIONS: The definition of MAFLD provides a more precise identification of individuals who have both fatty liver and significant fibrosis, assessed by non-invasive tests.

OBJECTIVES: To evaluate the effectiveness of cochlear implantation (CI) in case of far advanced otosclerosis and to evaluate the value of using intraoperative otoendoscopy to facilitate the identification of the round window membrane and the scala tympani without the need to remove the posterior canal wall or to perform a subtotal petrosectomy. STUDY DESIGN: Retrospective case-series study. SETTING: Tertiary academic CI center. PATIENTS AND METHODS: This study was conducted on patients with far advanced otosclerosis who underwent endoscopic-assisted CI between January 2010 and June 2020 at the same CI center. The minimum follow-up period was 2 years after surgery. RESULTS: Fourteen patients were included in the study. Ten patients had undergone a previous stapedotomy. Electrode insertion in the scala tympani was successfully accomplished in all cases included in the study. There was a statistically significant improvement in pure-tone average and speech discrimination scores in all cases of the study group (p < 0.0001). There were no statistically significant differences in postoperative pure-tone average or speech discrimination scores between cases with and without cochlear ossification or between cases with and without a previous stapedotomy (p > 0.05). CONCLUSION: Endoscopic-assisted CI is an effective option for hearing restoration in patients with far advanced otosclerosis. Otoendoscopy can facilitate visualization and access to the scala tympani without the need to remove the posterior canal wall or to perform a subtotal petrosectomy.

Yasser Fouad The profile of liver diseases in Egypt is changing dramatically and viral hepatitis is declining, while the fatty liver disease is increasing dramatically. However, the impact of these changes on the profile of hepatocellular carcinoma (HCC) remains uncertain. Therefore, we determined the temporal trends in the etiologies of HCC in Egypt over a decade. We retrospectively analyzed data from consecutive patients who were diagnosed with HCC over 10 years (2010-2020) in a large center in Upper Egypt. Standard tests were utilized to diagnose hepatitis C virus (HCV) and hepatitis B virus. In the absence of other liver disorders, the presence of obesity, or diabetes in the absence of other risk factors, metabolic dysfunction-associated fatty liver disease (MAFLD) was diagnosed. A total of 1,368 HCC patients were included, in which 985 (72%) had HCV, 58 (4%) had hepatitis B virus, and 143 (10.5%) had MAFLD, 1 patient had hemochromatosis, 1 had autoimmune liver disease, and 180 (13%) patients were with unknown cause. The annual proportions of MAFLD-related HCC were increased significantly between 8.3% in 2010 and 20.6% in 2020 ( p = 0.001), while HCV-related HCC declined from 84.8 to 66.7% ( p = 0.001). Throughout the study period, there were significant increases in the age at diagnosis of HCC, the proportion of female patients, obesity, diabetes, and less severe liver dysfunction at diagnosis ( p < 0.05 for all). With the decline of HCV, MAFLD is becoming a major cause of HCC in Egypt, which has increased substantially over the past 10 years. This study urges the creation of comprehensive action strategies to address this growing burden.

Solid waste management is one of the biggest challenges of the current era. The combustible fractions in the waste stream turn out to be a good energy source if converted into refuse-derived fuel. Researchers worldwide are successfully converting it into fuel. However, certain challenges are associated with its application in gasifiers, boilers, etc. to co-fire it with coal. These include high moisture content, low calorific value, and difficulty to transport and store. The present study proposed torrefaction as a pretreatment of the waste by heating it in the range of 200 °C-300 °C in the absence of oxygen at atmospheric pressure. The combustible fraction from the waste stream consisting of wood, textile, paper, carton, and plastics termed as mixed waste was collected and torrefied at 225 °C, 250 °C, 275 °C, and 300 °C for 15 and 30 min each. It was observed that the mass yield and energy yield decreased to 45% and 62.96% respectively, but the energy yield tended to increase by the ratio of 1.39. Proximate analysis showed that the moisture content and volatile matter decreased for torrefied samples, whereas the ash content and fixed carbon content increased. Similarly, the elemental analysis revealed that the carbon content increased around 23% compared to raw samples with torrefaction contrary to hydrogen and oxygen, which decreased. Moreover, the higher heating value (HHV) of the torrefied samples increased around 1.3 times as compared to the raw sample. This pretreatment can serve as an effective solution to the current challenges and enhance refuse-derived fuel's fuel properties.

In this editorial we comment on the in-press article in the World Journal of Gastrointestinal endoscopy about the role of computed tomography (CT) for the prediction of esophageal variceal bleeding. The mortality and morbidity are much increased in patients with chronic liver diseases when complicated with variceal bleeding. Predicting the patient at a risk of bleeding is extremely important and receives a great deal of attention, paving the way for primary prophylaxis either using medical treatment including carvedilol or propranolol, or endoscopic band ligation. Endoscopic examination and the hepatic venous pressure gradient are the gold standards in the diagnosis and prediction of variceal bleeding. Several non-invasive laboratory and radiological examinations are used for the prediction of variceal bleeding. The contrast-enhanced multislice CT is a widely used non-invasive, radiological examination that has many advantages. In this editorial we briefly comment on the current research regarding the use of CT as a non-invasive tool in predicting the variceal bleeding.
