RESUMEN

OBJECTIVE: To evaluate the perceptions of students and teachers regarding remote teaching modality in comparison with the traditional face-to-face method. METHODS: In this observational, retrospective, comparative, single-center study, questionnaires containing three major assessment domains were sent to two groups: university professors and undergraduate and graduate students. The first domain collected demographic and general data on the platforms used. The second and third domains contained questions that compared the perception of the quality of information offered by the two systems. RESULTS: Between May and September 2020, 162 students and 71 teachers participated in the study. A greater proportion of students demonstrated previous contact with the online method, while professors had presented a greater number of courses. Most participants reported that their expectations regarding the remote teaching method were met (students, 80.3%; teachers, 94.4%). A significant number of students (83.3%) and teachers (88.7%) rated the classes as easier to attend and manage. Despite difficulties, such as concentration retention, most of the participants agree (at least partially) that the format should be maintained. CONCLUSION: The remote teaching methodology, although still incipient in Brazil, has become a reality in light of current health restrictions. Our study demonstrated a high level of overall satisfaction and a high sense of learning from both students and faculty. However, new challenges associated with this system have been identified, such as retention of attention and interference from the external environment. Longitudinal comparative studies that incorporate various aspects of medical education in all cycles are necessary to corroborate the findings of this study. Resumo Objetivo Realizar a tradução para a língua portuguesa falada no Brasil e a adaptação transcultural do questionário Banff para Instabilidade Patelar. Métodos A tradução e adaptação transcultural seguiu o processo de validação linguístico proposto por diretrizes internacionais, que consiste em seis etapas: tradução, síntese, retrotradução, revisão pelo comitê de especialistas, pré-teste, e apresentação do relatório final aos autores do questionário original. Foram incluídos no estudo pacientes alfabetizados, com diagnóstico de instabilidade patelar recorrente, com mais de 12 anos de idade, que assinaram ou tiveram o termo de consentimento livre e esclarecido assinado por um responsável legal. Foram excluídos do estudo pacientes portadores de comorbidades neurológicas ou sistêmicas. Resultados Um total de 62 pacientes (18 homens e 44 mulheres) foram incluídos no estudo. Durante os processos de tradução e harmonização das retrotraduções, foram observadas discrepâncias, que foram modificadas sem necessidade de reformulação. Não foram necessárias substituições na versão do pré-teste. Conclusão O Questionário Banff para Instabilidade Patelar foi traduzido e adaptado transculturalmente para a língua portuguesa falada no Brasil com sucesso, podendo ser utilizado para a avaliação de pacientes portadores de instabilidade patelar falantes desta língua.

Objectives To analyze the results of anterior cruciate ligament (ACL) reconstruction with remnant-preserving versus remnant-resecting technique, concerning the return to pre-lesion activity level. Methods The present retrospective cohort study has assessed adults > 18 years old who underwent ACL anatomical reconstruction between 2010 and 2014. The main outcomes assessed were: level of physical activity (4-point scale), sports participation rate, ACL rerupture defined as documented lesion requiring revision surgery and the numeric pain scale rate (NPSR). Results A total of 83 individuals were included in the study, with a mean age of 31.8 years old and follow-up mean time of 4.2 years after the surgery. A total of 34 patients underwent ACL reconstruction with remnant-preserving technique, and 49 without remnant preservation. No statistically significant difference was found between groups in all outcomes assessed: level of physical activity before the lesion and after the surgery, ACL rerupture rates and postoperative pain level. Subgroup analysis has shown a statistically significant decrease in the activity level in both groups. The most practiced sport was football; 72% of patients in the remnant group have resumed football activity versus 52.6% of the control group. Conclusion Based in these findings, the comparison between ACL reconstruction with remnant preserving technique and remnant resecting technique has shown no differences concerning the return to prelesion activity level.

Resumo Objetivo Analisar os resultados da reconstrução do ligamento cruzado anterior (LCA) com preservação do remanescente, comparada à técnica convencional, no retorno do paciente ao nível de atividade física pré-lesão. Métodos Estudo transversal retrospectivo, que avaliou indivíduos adultos submetidos à reconstrução anatômica do LCA no período de 2010 a 2014. Os desfechos analisados foram: nível de atividade física, taxa retorno ao esporte, relesão do LCA definida como lesão documentada que necessite de nova reconstrução ligamentar, e dor pela escala numérica de dor (EVN). Resultados Foram incluídos 83 indivíduos com média de 31,8 anos de idade e seguimento médio de 4,2 anos após a cirurgia, 34 submetidos à reconstrução do LCA com preservação do remanescente, e 49 à convencional. Não houve diferença estatisticamente significativa entre os grupos na frequência de atividade física pré-lesão e pós-operatória, na taxa de relesão do LCA reconstruído e na intensidade da dor no pós-operatório. Na análise intragrupos, houve uma queda estatisticamente significativa na frequência da prática de atividade física pós-operatória para ambos os grupos em comparação ao nível pré-lesão. O tipo de esporte mais praticado foi o futebol, onde 72% pacientes do grupo remanescente retornaram ao esporte comparado a 52,6% do grupo controle; porém, essa diferença não foi estatisticamente significante. Conclusão Não foi possível observar diferenças entre os pacientes submetidos às técnicas cirúrgicas de reconstrução LCA com e sem a preservação do remanescente em relação ao retorno ao esporte, frequência de atividade física e intensidade da dor. Estudos futuros prospectivos são necessários.

The use of Adipose-Derived Stem Cells (ADSC) has been presented as a new alternative for tendon reconstruction. Have been admitted that ADSCs are related to better outcomes when used in tendon healing. This research was designed to apply the potential of ADSCs in tendon healing. Flexor digitorum superficialis tendon lesion was performed on both legs of eleven New Zealand rabbits and them, at the same time, treated as follows: Suture alone (Group III - Suture, n:10), suture associated with ADSC (Group IV - Suture + ADSC, n:10) or without suture (Group II - SHAN, n:2). At four weeks after the tendon surgery, the animal was euthanized, and the tendon evaluated (biomechanically and macroscopically). We used 5 additional New Zealand rabbits in the control group "Group I - Control, n:10". In the macroscopic evaluation, the group with ADSC presented a more homogeneous gross morphology compared with the group III. Biomechanical testing showed a lower ultimate tensile load, stiffness and a higher cross-sectional area in the group III and IV compared with the control group. The group with ADSC showed a greater ultimate tensile load, a larger cross-sectional area and bigger deformation at the ultimate tensile load when compared to the group without ADSC. In general terms, the use of ADSCs in tendon healing have biomechanical advantages compared to the non-use of ADSCs at 4 weeks after surgery. © 2018 Orthopaedic Research Society. ABSTRACT Objectives: To evaluate the regenerative capacity of gracilis (G) and semitendinosus (ST) tendons, to examine the sensitivity and specificity of signs and symptoms in the assessment of hamstring tendons, and to assess the thickness and insertion site of regenerated tendons. Methods: Thirty sequential knees were subjected to anterior cruciate ligament reconstruction with semitendinosus and gracilis tendons. After surgery, the patients were followed up clinically with physical examination and magnetic resonance imaging (MRI). Results: Overall, 36.66% of the tendons were visible on MRI, whereas 83.33% were palpable. On MRI, the distal insertion site of the regenerated semitendinosus tendon was visible proximal to the landmark of the medial femoral condyle in 28%, at the same level in 16%, and distally in 56% of the cases. Gracilis tendon insertion was visible proximally in 36.66% of cases, at the same level in 10%, and distally in 53.33%. Eleven knees exhibited complete regeneration. Conclusion: Partial or total regeneration of the ST and G tendons was apparent on MRI. Palpation is effective for evaluating regeneration of the ST and G tendons; however, MRI is still the gold standard. ST and G tendons regenerated completely in only a small percentage of patients, limiting reuse as a graft in cases with new ligament injuries of the knee. RESUMO Objetivo: Avaliar a capacidade de regeneração dos tendões Grácil e Semitendíneo; Identificar a sensibilidade e especificidade da manobra semiológica para detecção da presença dos tendões isquiotibiais; verificar espessura e região de inserção dos tendões regenerados. Métodos: 30 pacientes foram submetidos à cirurgia para reconstrução do LCA, com retirada dos tendões do semitendíneo e grácil. Após a cirurgia, os pacientes foram acompanhados clinicamente por exame físico e por RM. Resultados: Observou-se que em 36,66% eles se apresentavam visíveis, enquanto 83,33% os tendões estavam palpáveis. Observou-se à RM, quanto a região da inserção distal dos tendões regenerados: Semitendíneos, 28% proximal ao ponto padronizado do MFC, 16% se apresentaram ao nível e 56% distal. Já quanto aos tendões do Grácil, 36,66% apresentavam-se proximais, 10% ao nível e 53,33% distal. Onze tendões do ST e do G tiveram regeneração completa. Conclusões: Ficaram caracterizadas, por RM, as regenerações parciais ou totais dos tendões ST e G;a palpação, é eficaz para avaliar a regeneração dos tendões, porém RM mantem-se como padrão ouro; apenas parte dos pacientes os tendões ST e G se regeneraram de maneira completa, limitando a sua reutilização como enxerto nos casos de nova lesão ligamentar. OBJECTIVE: To determine a relationship between smoking, alcohol abuse and anabolic steroids and meniscoligamentous injuries. METHODS: A total of 239 patients underwent surgical treatment of isolated anterior cruciate ligament (ACL) lesion, ACL associated with meniscal injury and isolated meniscal injury, and were inquired by a single team of surgeons about their habits: smoking, alcoholism and use of artificial anabolic agents. In addition, quality of life was assessed using the SF-36 questionnaire. RESULTS: It was not possible to establish a direct relationship between habits and meniscal and ligamentous injuries, despite the finding that patients with ACL-associated lesions were more frequently smokers, and that patients with isolated meniscal injuries used more artificial anabolic agents. Regarding quality of life, there were no statistical differences in the habits between the groups; however, except for smokers with isolated meniscal injury, all patients who did not have the habits analyzed presented a better score in the SF-36 questionnaire. CONCLUSION: There were no significant findings correlating smoking, alcohol abuse and artificial anabolic agents with the presence of injuries. In addition, six months after the surgical treatment, there was no difference in the quality of life between the groups evaluated. Level of Evidence III, Prospective case series.


OBJETIVO: Determinar uma relação entre hábito de fumar, abuso de álcool e anabolizantes artificiais com lesões menisco-ligamentares do joelho. MÉTODOS: Um total de 239 pacientes foi submetido ao tratamento cirúrgico de lesão isolada do ligamento cruzado anterior (LCA), do LCA associada à lesão meniscal e lesão meniscal isolada e foi indagado, por uma única equipe de cirurgiões, sobre seus hábitos: tabagismo, alcoolismo e uso de anabolizantes artificiais. Além disso, foi avaliada a qualidade de vida com o questionário SF-36. RESULTADOS: Não foi possível estabelecer uma relação direta entre os hábitos e as lesões menisco-ligamentares, apesar da constatação de que os pacientes com lesões associadas ao LCA eram, com maior frequência, tabagistas e que os pacientes com lesão meniscal isolada usavam mais anabolizantes artificiais. Com relação à qualidade de vida, não houve diferença estatística quanto aos hábitos entre os grupos, porém, exceto tabagistas com lesão meniscal isolada, todos os pacientes que não tinham os hábitos analisados apresentaram pontuação melhor no questionário SF-36. CONCLUSÃO: Não houve achados significativos que correlacionassem os hábitos de fumar, abuso de álcool e anabolizantes artificiais com a presença de lesão. Além disso, seis meses após o tratamento cirúrgico, não houve diferença na qualidade de vida entre os grupos avaliados. Nível de evidência III, Tipo de estudo: série de casos prospectivos .

ABSTRACT Objective: To analyze the number of hospital permits for total knee arthroplasty (TKA) and total hip arthroplasty (THA) in Brazil between 2008 and 2015, and correlate them with regional, national, and international demographic and epidemiological aspects. Methods: Data on demographics, economic level, and TKA and THA were obtained from the website of the Ministry of Health/DATASUS, Brazilian Institute of Geography and Statistics, and the National Health Agency to assess the assistance provided by the Public Health Care System in arthroplasties for elderly Brazilian population without private health care. Results: The South and Southeast had the best care, with 8.07 and 6.07 TKAs/100,000 inhabitants, one TKA per 1811 and 2624 seniors, 17.3 and 10.99 THAs/100,000 inhabitants, and one THA per 923 and 1427 seniors, respectively. The worst rates were found in the North and Northeast, with 0.88 and 0.98 TKAs/100,000, one TKA per 6930 and 10,411 seniors, 0.96 and 3.25 THAs/100,000, and one THA per 6849 and 2634 seniors, respectively. The national average was 4.00 TKAs/100,000, one TKA per 3249 seniors, 8.01 THAs/100,000, and one THA per 1586 seniors. The international average was 142.8 TKAs/100,000 and 191.8 THAs/100,000. Conclusion: The results expressed unsatisfactory results for TKA and THA in Brazil, with greater relevance in the North and Northeast.


RESUMO Objetivo: Analisar o número de autorizações de internação hospitalar para cirurgias de artroplastia total de joelho (ATJ) e quadril (ATQ) no Brasil entre 2008 e 2015 e correlacioná-lo com aspectos demográficos e epidemiológicos regionais, nacionais e internacionais. Métodos: Os dados sobre informativos demográficos, econômicos e sobre ATJ e ATQ foram obtidos no website do Instituto Brasileiro de Geografia e Estatística (IBGE), Agência Nacional de Saúde (ANS) e Ministério da Saúde/Datasus para avaliar o assistencialismo do Sistema Único de Saúde (SUS) em artroplastias para a população idosa brasileira sem planos de saúde privados. Resultados: As Regiões Sul e Sudeste apresentaram a melhor relação assistencial, com 8,07 e 6,07ATJ/100.000 habitantes e uma ATJ para 1.811 e 2.624 idosos e 17,3 e 10,99ATQ/100.000 habitantes e uma ATQ para 923 e 1.427 idosos, respectivamente. Os piores índices foram do Norte e Nordeste, com 0,88 e 0,98 ATJ/100.000 e uma ATJ para 6.930 e 10.411 idosos e 0,96 e 3,25 ATQ/100.000 e uma ATQ para 6.849 e 2.634 idosos, respectivamente. A média nacional foi de 4,00 ATJ/100.000 e uma ATJ para 3.249 idosos e 8,01 ATQ/100.000 e uma ATQ para 1.586 idosos. A média internacional foi de 142,8 ATJ/100.000 e 191,8 ATQ/100.000. Conclusão: Os resultados indicaram resultados assistenciais insatisfatórios para ATJ e ATQ no Brasil, principalmente nas regiões Norte e Nordeste.

RESUMEN

ABSTRACT The authors report a rare case of osteochondritis dissecans of the trochlea. The treatment of these lesions, in which the osteochondral fragment is not viable, is difficult and often limited in Brazil. A clinical case is presented with functional and radiological outcomes after treatment with microfracture technique, bone graft, and collagen membrane coverage.


RESUMO Os autores relatam um caso raro de osteocondrite dissecante de tróclea. O tratamento dessas lesões com inviabilidade do fragmento osteocondral é difícil e muitas vezes limitado no nosso meio. Os autores apresentam resultados clínicos e radiológicos após o tratamento com a técnica de microfratura, enxertia óssea e cobertura com membrana de colágeno.

Resultados: Não foi possível estabelecer uma relação direta entre os hábitos e as lesões menisco-ligamentares, apesar da constatação de que os pacientes com lesões associadas ao LCA eram, com maior frequência, tabagistas e que os pacientes com lesão meniscal isolada usavam mais anabolizantes artificiais. Com relação à qualidade de vida, não houve diferença estatística quanto aos hábitos entre os grupos, porém, exceto tabagistas com lesão meniscal isolada, todos os pacientes que não tinham os hábitos analisados apresentaram pontuação melhor no questionário SF-36. Conclusão: Não houve achados significativos que correlacionassem os hábitos de fumar, abuso de álcool e anabolizantes artificiais com a presença de lesão. Além disso, seis meses após o tratamento cirúrgico, não houve diferença na qualidade de vida entre os grupos avaliados. Nível de evidência III, Tipo de estudo: série de casos prospectivos.

OBJECTIVE: To analyze the number of hospital permits for total knee arthroplasty (TKA) and total hip arthroplasty (THA) in Brazil between 2008 and 2015, and correlate them with regional, national, and international demographic and epidemiological aspects. METHODS: Data on demographics, economic level, and TKA and THA were obtained from the website of the Ministry of Health/DATASUS, Brazilian Institute of Geography and Statistics, and the National Health Agency to assess the assistance provided by the Public Health Care System in arthroplasties for elderly Brazilian population without private health care. RESULTS: The South and Southeast had the best care, with 8.07 and 6.07 TKAs/100,000 inhabitants, one TKA per 1811 and 2624 seniors, 17.3 and 10.99 THAs/100,000 inhabitants, and one THA per 923 and 1427 seniors, respectively. The worst rates were found in the North and Northeast, with 0.88 and 0.98 TKAs/100,000, one TKA per 6930 and 10,411 seniors, 0.96 and 3.25 THAs/100,000, and one THA per 6849 and 2634 seniors, respectively. The national average was 4.00 TKAs/100,000, one TKA per 3249 seniors, 8.01 THAs/100,000, and one THA per 1586 seniors. The international average was 142.8 TKAs/100,000 and 191.8 THAs/100,000. CONCLUSION: The results expressed unsatisfactory results for TKA and THA in Brazil, with greater relevance in the North and Northeast.


OBJETIVO: Analisar o número de autorizações de internação hospitalar para cirurgias de artroplastia total de joelho (ATJ) e quadril (ATQ) no Brasil entre 2008 e 2015 e correlacioná-lo com aspectos demográficos e epidemiológicos regionais, nacionais e internacionais. MÉTODOS: Os dados sobre informativos demográficos, econômicos e sobre ATJ e ATQ foram obtidos no website do Instituto Brasileiro de Geografia e Estatística (IBGE), Agência Nacional de Saúde (ANS) e Ministério da Saúde/Datasus para avaliar o assistencialismo do Sistema Único de Saúde (SUS) em artroplastias para a população idosa brasileira sem planos de saúde privados. RESULTADOS: As Regiões Sul e Sudeste apresentaram a melhor relação assistencial, com 8,07 e 6,07ATJ/100.000 habitantes e uma ATJ para 1.811 e 2.624 idosos e 17,3 e 10,99ATQ/100.000 habitantes e uma ATQ para 923 e 1.427 idosos, respectivamente. Os piores índices foram do Norte e Nordeste, com 0,88 e 0,98 ATJ/100.000 e uma ATJ para 6.930 e 10.411 idosos e 0,96 e 3,25 ATQ/100.000 e uma ATQ para 6.849 e 2.634 idosos, respectivamente. A média nacional foi de 4,00 ATJ/100.000 e uma ATJ para 3.249 idosos e 8,01 ATQ/100.000 e uma ATQ para 1.586 idosos. A média internacional foi de 142,8 ATJ/100.000 e 191,8 ATQ/100.000. CONCLUSÃO: Os resultados indicaram resultados assistenciais insatisfatórios para ATJ e ATQ no Brasil, principalmente nas regiões Norte e Nordeste.

RESUMEN

The authors report a rare case of osteochondritis dissecans of the trochlea. The treatment of these lesions, in which the osteochondral fragment is not viable, is difficult and often limited in Brazil. A clinical case is presented with functional and radiological outcomes after treatment with microfracture technique, bone graft, and collagen membrane coverage.


Os autores relatam um caso raro de osteocondrite dissecante de tróclea. O tratamento dessas lesões com inviabilidade do fragmento osteocondral é difícil e muitas vezes limitado no nosso meio. Os autores apresentam resultados clínicos e radiológicos após o tratamento com a técnica de microfratura, enxertia óssea e cobertura com membrana de colágeno.

OBJECTIVES: To evaluate the regenerative capacity of gracilis (G) and semitendinosus (ST) tendons, to examine the sensitivity and specificity of signs and symptoms in the assessment of hamstring tendons, and to assess the thickness and insertion site of regenerated tendons. METHODS: Thirty sequential knees were subjected to anterior cruciate ligament reconstruction with semitendinosus and gracilis tendons. After surgery, the patients were followed up clinically with physical examination and magnetic resonance imaging (MRI). RESULTS: Overall, 36.66% of the tendons were visible on MRI, whereas 83.33% were palpable. On MRI, the distal insertion site of the regenerated semitendinosus tendon was visible proximal to the landmark of the medial femoral condyle in 28%, at the same level in 16%, and distally in 56% of the cases. Gracilis tendon insertion was visible proximally in 36.66% of cases, at the same level in 10%, and distally in 53.33%. Eleven knees exhibited complete regeneration. CONCLUSION: Partial or total regeneration of the ST and G tendons was apparent on MRI. Palpation is effective for evaluating regeneration of the ST and G tendons; however, MRI is still the gold standard. ST and G tendons regenerated completely in only a small percentage of patients, limiting reuse as a graft in cases with new ligament injuries of the knee. OBJETIVO: Avaliar a capacidade de regeneração dos tendões Grácil e Semitendíneo; Identificar a sensibilidade e especificidade da manobra semiológica para detecção da presença dos tendões isquiotibiais; verificar espessura e região de inserção dos tendões regenerados. MÉTODOS: 30 pacientes foram submetidos à cirurgia para reconstrução do LCA, com retirada dos tendões do semitendíneo e grácil. Após a cirurgia, os pacientes foram acompanhados clinicamente por exame físico e por RM. RESULTADOS: Observou-se que em 36,66% eles se apresentavam visíveis, enquanto 83,33% os tendões estavam palpáveis. Observou-se à RM, quanto a região da inserção distal dos tendões regenerados: Semitendíneos, 28% proximal ao ponto padronizado do MFC, 16% se apresentaram ao nível e 56% distal. Já quanto aos tendões do Grácil, 36,66% apresentavam-se proximais, 10% ao nível e 53,33% distal. Onze tendões do ST e do G tiveram regeneração completa. CONCLUSÕES: Ficaram caracterizadas, por RM, as regenerações parciais ou totais dos tendões ST e G;a palpação, é eficaz para avaliar a regeneração dos tendões, porém RM mantem-se como padrão ouro; apenas parte dos pacientes os tendões ST e G se regeneraram de maneira completa, limitando a sua reutilização como enxerto nos casos de nova lesão ligamentar. Evaluate parylene scaffold feasibility in cartilage lesion treatment, introducing a novel paradigm combining a reparative and superficial reconstructive procedure. Fifteen rabbits were used. All animals had both knees operated and the same osteochondral lesion model was created bilaterally. The parylene scaffold was implanted in the right knee, and the left knee of the same animal was used as control. The animals were euthanized at different time points after surgery: four animals at three weeks, three animals at six weeks, four animals at nine weeks, and four animals at 12 weeks. Specimens were analyzed by International Cartilage Repair Society (ICRS) macroscopic evaluation, modified Pineda histologic evaluation of cartilage repair, and collagen II immunostaining. Parylene knees were compared to its matched contra-lateral control knees of the same animal using the Wilcoxon matched-pairs signed rank. ICRS mean ± SD values for parylene versus control, three, six, nine and twelve weeks, respectively: 7.83 ± 1.85 versus 4.42 ± 1.08, p = 0.0005; 10.17 ± 1.17 versus 6.83 ± 1.17, p = 0.03; 10.89 ± 0.60 versus 7.33 ± 2.18, p = 0.007; 10.67 ± 0.78 versus 7.83 ± 3.40, p = 0.03. Modified Pineda mean ± SD values for parylene versus control, six, nine and twelve weeks, respectively: 3.37 ± 0.87 versus 6.94 ± 1.7, p < 0.0001; 5.73 ± 2.05 versus 6.41 ± 1.7, p = 0.007; 3.06 ± 1.61 versus 6.52 ± 1.51, p < 0.0001. No inflammation was seen. Parylene implanted knees demonstrated higher collagen II expression via immunostaining in comparison to the control knees. Parylene scaffolds are a feasible option for cartilage lesion treatment and the combination of a reparative to a superficial reconstructive procedure using parylene scaffolds led to better results than the reparative procedure alone.

PURPOSE: To compare the outside-in transtibial lateral and medial approaches for posterior cruciate ligament (PCL) reconstruction regarding the guidewires and popliteal artery integrity. METHODS: Twenty-two human cadaveric knees were used. A PCL tibial aimer was arthroscopically placed within the PCL footprint through the anteromedial portal for the medial approach and through the anterolateral portal for the lateral approach. For the medial approach, the drill guide was introduced through the anteromedial tibial cortex and the guidewire was advanced with the reamer beyond the posterior tibial cortex. For the lateral approach, the drill guide was introduced through the anterolateral tibial cortex and the guidewire was advanced with the reamer beyond the posterior tibial cortex. After this, the knee was dissected. The depth distance (DD) was defined as the distance between the popliteal artery and the tibial posterior cortex projected at the tibial level at which the guidewire intersected or passed by the artery. The guidewire travel distance was calculated as the distance the guidewire had to advance beyond the tibial cortex to intersect the popliteal artery or pass by it. RESULTS: With the medial approach, the popliteal artery was intersected in all knees with a mean DD of 12.20 mm and a mean guidewire travel distance of 15.90 mm. With the lateral approach, the popliteal artery was not intersected in any knee; its mean medial distance from the artery was 4.8 mm at a DD of 10.05 mm. There was a significant difference in the popliteal artery intersection incidence and DD between both groups (P < .0001 and P = .0003, respectively). CONCLUSIONS: The transtibial lateral approach for PCL reconstruction was a safer method than the medial approach regarding popliteal artery injury by a guidewire. CLINICAL RELEVANCE: This study presents a slight modification of the most frequently used PCL reconstruction technique, intending to minimize guidewire injury to the popliteal artery.

Computer-assisted surgery was developed to improve the results of conventional total knee arthroplasty (TKA). The authors investigated the preoperative varus/valgus deformity influence on the production of balanced extension and flexion gaps using computer-assisted surgery. This study evaluated data from a prospective case series. A total of 132 patients (107 women and 25 men) underwent navigated TKA. Patients were divided into the following 3 groups according to the degree of the initial varus/valgus deformity: group 1, 0° to 3°; group 2, 3° to 9°; and group 3, greater than 9°. The final lower limb mechanical axis (LLMA) and the final flexion and extension gaps were measured. Knees exhibiting up to 3° of deviation on the frontal plane and a difference of up to 3 mm between the lateral and medial gaps were considered to be aligned and balanced, respectively. Average LLMA deviation decreased from 5.58° (± 4.80°) to 1.87° (± 1.66°). For knees with varus deviation, the percentage of balancing relative to the flexion gaps was 97.8% and that relative to the extension gap was 100% (P>.05). For knees with valgus deviation, the percentage of balancing relative to the flexion gaps was 95.1% and that relative to the extension gap was 97.6% (P>.05). Approximately 92% of the LLMA alignment was achieved in the group with varus deformity, whereas 71.4% was observed in the group with valgus deformity (P<.05). Computer-assisted TKA could attain proper flexion and extension balance regardless of coronal plane malalignment magnitude. Severe valgus and varus knees are more difficult to align using navigation. No difference was found in the balance of flexion or extension gaps in valgus or varus knees, independent of the severity.
