RESUMEN

Dynamics of mean value indices and mean dispersion doesn't exclude the feedback (Mayanskiy D.N., 2008) in process of study the disease according to cooperative processes using system leukocyte-monocyte-lymphocyte (L+M+LM). The authors investigated a dynamic balance between these cell substratums and collagen formation. Acute inflammatory processes in tissues of maxillofacial area accompanied by leukemoid response of peripheral blood in the form of leukocytosis (10 x 10(9)-15 x 10(9)/l and more). The authors completely agree with the opinion of M.M. Solov'yov (2012) and N.K. Artyushenko (2008) that mechanism of this reaction is associated with both the leukocyte redistribution in the inflammation zone and with changes of hematopoietic rostock which aimed to balance the affected compensatory mechanisms of nonspecific resistance.

RESUMEN

The report discusses the results of proton stereotactic irradiation in 27 children, aged 6-15 (mean--12.5 yrs.), and 21 adults, aged 16-47 (mean--23 yrs.), with arteriovenous malformations (AVM) in the brain. The AVM volume ranged 0.2-37.7 cu cm; absorbed dosage at dose field maximum--40-80 Gy. Complete AVM obliteration was attained in 17 (35%) (mean AVM volume--3.1 cu cm); 50% decrease and more--12 (25%)(mean AVM volume--7 cu cm). When AVM volume was cut down to less than 50% (9) (19%) (mean AVM volume--9.2 cu cm) and 10 (21%) (mean AVM volume--19.6 cu cm), no effect was reported.

RESUMEN

Stereotactic radiosurgery was performed in 42 patients (1997-2007). They presented with 47 cavernous angiomas and 44 of them underwent surgery. Single focal dosage varied 3-9 Gy while total one--18-54Gy. After treatment and 2-year followup, neurologic symptoms regressed in 21 (68%), tumors were smaller (MRI) in 10 (32.3%) and severe convulsions abated in 5 patients out of 9. No cerebral hemorrhage was reported within 9 years although 63.7% of tumors were located in deep cerebral structures and hemorrhage occurred as a precursor in 83%.

The authors present the results of proton stereotactic therapy (using continuous double-axis pendulum irradiation) in 27 children with cerebral arteriovenous malformations (AVM). At the moment of therapy, the children's age was 6 to 15 years. The volume of AVM was 0.2 to 31.2 cm3. The absorbed doses in the maximum of a dose field were 40 to 80 Gy. Complete AVM obliteration (the mean volume in the group was 3.03 cm3) was obtained in 11 (41%) patients. A 50% or more reduction in AVM volume (mean volume, 10.6 cm3) was seen in 8 (30%) patients. There was a less than 50% reduction in AVM volumes (mean volume 9.6 cm3) in 2 (7%) patients and no effect could be achieved in 6 (22%) (mean AMV volume 19.7 cm3).

RESUMEN

The aim of the study was to study the count of circulating immune complexes (CIC) in the blood of children with acute concussion of the brain. The fact that CIC at high concentrations that can penetrate into the brain through the blood-brain barrier and cause complications as vasculitis, microangiopathy, proliferative processes in the meninges, enlarged ventricles of the brain, and atrophy of its tissue was borne in mind. The studies revealed a significant progressive CIC increase within 3 weeks. For correction of blood CIC levels, laser exposure was applied to the carotid and vertebral arteries and acupuncture points. For comparison, thymaline in age-specific doses was used. Laser radiation showed a significant fall of CIC at days 19-21, particular when applied to the acupuncture points. Thymaline did not affect blood CIC levels. Laser application to the acupuncture points in children with acute brain concussion should reduce the incidence of complications of brain injury disease.

RESUMEN

The outcomes of intracranial endoscopic operations for occlusive hydrocephalus (65 interventions in 60 children) and intracranial hematomas (11 operations in 10 children) are analyzed. Indications and contraindications for intracranial endoscopic operations, equipment supply, specific features of their performance, and outcomes are discussed. In occlusive hydrocephalus, the outcomes were as follows: hydrocephalus stabilization (38%), complications (15%), mortality (3%), the follow-up lasting as long as 8 years. The dynamic evaluation of the intracranial status (clinical and sound monitoring) is the obligatory condition of intracranial endoscopic operations made for intracranial hematomas. There were no complications in children with intracranial hematomas after planned intracranial endoscopic operations. The application of flexible distal end-controlled endoscopic, ultrasound-stereotactically aimed, and double-maneuver-controlled systems (visual examination through the optic system of an endoscope in combination with intraoperative ultrasonographic monitoring) was shown to be optimal during intracranial endoscopic operations.

RESUMEN

The authors estimated the influence of proton-beam stereotaxic therapy on a course of epileptic fits in cerebral arteriovenous aneurysms (AVA). A stable disappearance of epileptic fits was noted in 16 of 29 patients (56%), in 12 of them (41%) positive angiography results were noted. The calculated D and V fields of irradiation for 16 patients without fits and for 13 patients with epileptic fits after proton-beam stereotaxic therapy have shown that with D being the same, the effect depends upon a fraction of an AVA irradiated area.

RESUMEN

Various methods of surgical intervention were applied differentially in 116 patients with multifocal epilepsy. From analysis of the late-term results it was found that the cortical form of multifocal epilepsy is treated most effectively by open and the cortico-subcortical form--by combined operations (use of stereotaxis during an open operation). The most rational operations in bitemporal epilepsy are bilateral one-stage stereotaxic interventions on the mediobasal temporal structures or successive (at various intervals) performance of stereotaxic operation on one temporal lobe and open operation on the contralateral temporal lobe. The effect was positive in 78.3% of cases.

RESUMEN

Stereotaxic proton beam therapy using narrow beams (5-10 mm at 50% of isodose) at the energy of 100 MeV in single irradiation and the delivery of absorbed doses, the maximum dosage being 40-80 Gy, was provided to 99 patients with arteriovenous brain aneurysms. An analysis of the therapeutic results based on angiography data was performed in 50 patients with arteriovenous malformations with the volume up to 10 cm3. A positive effect was obtained in 31 of 50 patients.

RESUMEN

Echographic examination of 43 patients was conducted during removal of tumors or during diagnostic operations. Brain tumors of supra- and subtontorial localization were detected. The use of an improved ultrasonic transducer and the transmission sonication method helps in identifying more exactly the character, size, and depth of the tumor as well as determining the condition of the ventricles and main arteries of the brain. The application of the improved method ensures the choice of an adequate surgical approach and reduces the traumatic character of the operation.

RESUMEN

A new method of surgical treatment in dealing with severe forms of multifocal epilepsy-stereotaxic intervention on subcortical structures during open craniotomy--is proposed. The method allows stereoelectrographic examinations to be performed during open operative interventions, helps discover subcortical epileptogenic foci, establish functional interrelationship of the chosen subcortical structure and the cortical epileptogenic focus and to undertake a differentiated intervention on the cortical, subcortical and both these foci at the same time.
