RESUMEN

Tolerance, subjective and objective effectiveness of domestic intraoral applicator UPLH-01 were studied in the treatment of snoring and obstructive sleep apnea (OSA). In 44 patients with uncomplicated snoring the above parameters were assessed using questionnaire, in 20 patients with uncomplicated snoring and OSA paired polysomnographic investigations were made. Tolerance of the treatment was 50%. Subjective positive effect was registered in 65% of the patients. The applicator was found effective in the majority of patients with uncomplicated snoring. In mild and moderate OSA the applicator was effective only in a few cases. In severe OSA no effect was found. Patients with chronic disorders of nasal breathing exhibited deterioration of respiration and blood saturation with oxygen. Thus nasal obstruction is a contraindication to applicator use.

RESUMEN

Constant-frequency transesophageal atrial pacing was used in 87 patients with coronary heart disease concurrent with stable angina pectoris of various functional classes (I-IV). This technique, as a bicycle ergometric test, allows one to assess the functional class of patients with angina and to ascertain the antianginal effect of agents, which was exemplified by erinit given in doses of 80 and 120 mg.

The authors summed up the practical experience gained in long-term maintenance treatment with allapinine, a new Soviet antiarrhythmic agent, given to 65 patients with various cardiac arrhythmias in the outpatient setting. The long-term allapinine use yielded good results: its efficacy was proved to be approximately equal (63% and 60%) in patients with ventricular or supraventricular extrasystoles, respectively, but it was 77% in those with paroxysmal atrial fibrillation. There were no decreases in antiarrhythmic activity of allapinine during its long-term therapy. The agent produced no significant effects on heart rate, blood pressure, and myocardial contractility. Its side effects were recorded in 17 (26.2%) patients, however, withdrawal of the agent was required only in 6 cases.

Proceeding from the experience gained in the medical examination of persistent working ability loss, which was based on the results of bicycle ergometric studies in 383 patients with stable angina, the authors developed clinicofunctional criteria allowing the practitioner to establish the degree of physical working capacity limitation (poor, satisfactory, good). The validity of the criteria was confirmed by coronary angiographic evidence and long-term observation results. Over 3 years of the follow-up, 28% of the patients who had been group II invalids and 7% of the Group III died among those recorded by the medical-and-labour examination commission that used the criteria described above. All the individuals who had been regarded as being able to work survived. The occupational activities adequate to the fitness of patients with angina promote the development of adaptive mechanisms, mobilize the functional reserve of the blood circulatory apparatus, enhance their physical performance. It has been concluded that the efficacy of the medical-and-labour examination in angina is determined by an accurate assessment of its severity and closely associated with the application of bicycle ergometry.

Holter's ECG monitoring was used to investigate 140 males showing no clinical signs of coronary disease, while having 1 to 5 coronary risk factors (hypodynamia, arterial hypertension, smoking, hypercholesterolemia). Supraventricular rhythm disorders were recorded in 86%, and ventricular ones, in 69%. Transitory ST depression was detected in 15% of the cases, appearing in response to physical (90%) of mental (9%) stress or spontaneously in 1%. Ninety percent of ischemic attacks were asymptomatic. Ischemic ECG changes were nearly twice as frequent in patients with 3 to 5 coronary risk factors, as compared to those with 1 or 2 risk factors. It is therefore suggested that asymptomatic coronary disease can be suspected in some of the patients with coronary risk factors. This suspicion is confirmed by positive bicycle ergometric test in 53% of the individuals with changed Holter's ECG monitoring data. Combined use of Holter's ECG monitoring and bicycle ergometry increases twofold the detectability of electrocardiographic abnormalities.

The clinical and morbid anatomy data on 295 patients with arterial hypertension were examined. Two hundred patients had moderate hypertension with the diastolic pressure under 110 mm Hg. Of these, 103 patients had received rauwolfia drugs and 13 patients had been given hypothiazide in addition. Eighty patients who died before 1953 had not received any specific hypotensive drugs. Early hypotensive therapy of moderate hypertension was noted to promote a decrease in arterial blood pressure to normal and to raise the mean lifespan by 8 years. Furthermore the number of fatal brain strokes and myocardial infarctions decreased almost 2-fold.

Findings of research into the specific course of ischaemic heart disease (IHD) concurrent with diabetes mellitus are analysed. Diabetic patients were found to have a more severe course of IHD after myocardial infarction, which was more often complicated with arrhythmias, cardiac failure, and unstable angina. The incidence of painless IHD after myocardial infarction was found to be twice higher in diabetics. A consistent relationship between the severity of diabetes and an increase in painless IHD incidence was noted. Diabetes was found to aggravate the IHD course irrespective of the patient's age. A correlation was observed between the severity of IHD and insulinaemia. Certain functional-diagnosis methodologies are shown to be most informative in the differential diagnosis of IHD and diabetic cardiomyopathy. The high efficacy of a complex therapy including beta-blockers and angioprotectors in concurrent IHD and diabetes is demonstrated.

A total of 158 males with coronary risk factors and excessive (4 to 55% above normal) body weight were treated by diet and exercise for hyperlipoproteinemia in an out-patient clinic for 18 months. Blood cholesterol decreased significantly, by 14.4% (p less than 0.001), in 10% of the males who had lost at least 10% of excessive weight. Blood triglycerides decreased by 23% in 22% of the patients, who had lost 5.5% and more of excessive body weight (p less than 0.01). A two-month course of treatment with essential forte in combination with diet and regular exercise reduced blood cholesterol by 15.1% in 62%, and blood triglycerides, by 44.5% in 61.9% (p less than 0.001).

A study of the impact of 10 sessions of hyperbaric oxygenation on the heart's pump function and contractility as evidenced echocardiographically in 46 patients with angina of effort, functional class 2-3, and 36 matched controls demonstrated that hyperbaric oxygenation can normalize hemodynamics in cases of hyper- and hypokinetic circulation, its effect being particularly good in patients with pre-existing left-ventricular dilatation.

A study of the effect of hyperbaric oxygenation (HBO), applied as single sessions or a treatment course, on cardiac rhythm in 82 coronary patients with chronic extrasystole demonstrated positive therapeutic effect to be achieved after the first 5 or 6 HBO sessions in all patients. Further treatment may provoke an aggravation of arrhythmia in some patients. Positive antiarrhythmic effect of a complete HBO course is doubtful, where ectopic activity is increased following the 7th session. There is a correlation between the results of ECG monitoring from the hyperbaric chamber, from session 7 onwards, and 24-hour ECG recording before and after a HBO course. As shown by 24-hour ECG monitoring, positive effect was obtained in 67% of patients with ventricular extrasystole, the number of ventricular extrasystoles remained basically unchanged after a HBO course in 17%, and ectopic activity increased in 15.4%.

A prospective five-year study and medicinal treatment were conducted in 317 coronary patients with stable angina in the absence of any signs of heart failure, and stenosing coronary arterial atherosclerosis as evidenced by selective coronary angiography. Total mortality was 2.8%, and the incidence of documented non-fatal myocardial infarction was 3.8% per year. The mortality was mostly dependent on the severity of angina's functional class and the number of affected major coronary arteries (narrowed by more than 70%). A group of patients with unfavorable prognosis was identified (functional class III to IV, low physical stress tolerance, the involvement of two or three major coronary arteries). The results demonstrate the efficiency of long-term medication in coronary patients with stable angina due to stenosing coronary atherosclerosis.
