RESUMEN

In 26 patients (female 11, males--15) with diabetic angiopathy (necrosis of the foot or toes) local measurement of pO2 by transcutaneous mode was carried out before and during introduction of serotonine adipinate. Simultaneously with evaluation of pO2 in 14 patients electromyography (EMG) was made. Serotonine adipinate was introduced intravenously with a speed of 5.0-10.0 mg per hour. In this group mean age of the patients made up 57.4 years, mean values of initial pO2 25.6 mm Hg, and mean indices of pO2 at the moment of serotonine treatment 38.5 mm Hg. Average increase of pO2 during serotonine infusion was registered as 50.4% in comparison with mean initial data. In all 14 patients, who underwent simultaneous measurement of pO2 and registration of EMG, reinforcement of electrical activity of smooth muscles (SM) of various intensity and increase in tissue pO2 were detected. Improvement of microcirculation (increase of transcutaneous pO2, reinforcement of electrical activity of SM as a result of serotonine introduction) evidences that functional and morphological changes of serotonine receptors of SM are involead in genesis of diabetic angiopathy. The data obtained showed that in diabetes mellitus, besides insulin deficiency, insufficiency of such an endogenous factor as serotonine duclops. Thus, serotonin should be used for prophylaxis and treatment of diabetic angiopathies.

RESUMEN

The authors had 21 patients with a rare disease, Madelung's lipomatosis. Vegetative disorders of segmental and suprasegmental levels were revealed in all patients. The character of neuropathies in Madelung's lipomatosis is discussed in detail. According to the authors, it is a complex syndrome with peripheral, vegetative, and vascular disorders. The accompanying secondary scalenus syndrome produces a peculiar clinical picture of neuropathy. The existence of neurological disorders dictates the need for complex (surgical and medicamentous) treatment of patients with Madelung's lipomatosis.

RESUMEN

Clinicomorphological data on 12 cases of long-term APV (up to 35 days) are reported. The degree of morphological changes was found to vary with CNS compartment. The authors emphasize clinicomorphological alterations in the spinal cord, particularly in the cervical segments, where almost complete absence of cells and reactivity of glial elements were observed.

RESUMEN

The above study is performed in neurons, oligodendrocytes and capillary endotheliocytes of human brain 4-13 hours after death. Brain biopsies after the surgery and early necropsies served as the material. RNA synthesis in some neurons and glial cells is retained to some degree 8-13 hours after death. Oligodendrocytes are more and neurons less resistant. Grave disturbances of the CNS microcirculation is the obstacle to reversibility of destructive processes.

RESUMEN

Analysis of medical and sociopsychological factors in 231 burn victims was provided during 4-year randomized study. Groups of burn-prone (working class males, alcohol abusers, burned drunk, violating safety measures, with concomitant somatic and brain disorders and maladjustment) and non-prone individuals were elucidated. Shifts in Spielberger's state and trait anxiety scores depend on the damage of their somatic and nervous "underground". Beck's depression and trait anxiety high scores in all subgroups were found.

RESUMEN

The article discusses experience in the examination of 109 patients with chronic calculous pancreatitis (CCP), 102 of them were treated by operation (a total of 130 operations were carried out). Calculous cholecystitis was a consequence of alcohol intake in 102 cases, and was due to other causes in 7 cases. In 71% of cases CCP was attended by the development of indirect signs of pancreatic hypertension and in 45.1% by pancreatic cysts. Operations for internal (h = 49) and external-internal (n = 27) drainage were most pathogenetically justified. The mortality after these operations was 2%. The total postoperative mortality was 6.9%, mortality in the late-term periods was 8.8%. Unfavourable late-term results were encountered in individuals who continued drinking alcohol as well as in inadequate drainage of the pancreatic duct system.

RESUMEN

Presented are the results of comprehensive CNS examinations in 53 alcoholics with chronic pancreatitis. Related psychopathological and neurological syndromes are specified. Hepatopancreatoduodenal and CNS abnormalities were found directly correlated. The addition of pancreatitis to alcoholism leads to development of encephalomyelopolyradiculoneurotic syndrome.

RESUMEN

The efficacy of various types of psychotropic therapy was comparatively assessed in patients with burns and borderline neuro-psychic disorders. The efficacy of therapy was determined using standard uniform scales for the evaluation of the effect of psychotropic agents and by analysis of statistical indexes of the heart rhythm and respiratory pattern. Psychotropic therapy significantly reduced psychopathological symptoms and a more marked normalization of autonomous body functions was observed. It has been concluded that the most effective type of psychotropic therapy in patients with burns and borderline neuro-psychic disorders is combined use of phenazepam and amitriptyline.

RESUMEN

A total of 130 patients were treated in a burn unit of a hospital. Three stages of neurosis-like disorders were noted: (1), acute stress reaction close in its structure to prodromal phase of exogenous psychoses and approximately coinciding with the burn shock; (2), formation and dissolution of psychopathologic syndromes (asthenic, depressive and asthenic-depressive), resembling the so-called "transitional" syndromes coinciding with toxemias and septic pyemias; (3), late psychogenesis (mobilization, pseudoeuphoric and hypochondriac).

RESUMEN

The urgent questions of neuropsychic disorders related to thermal lesions are discussed on the basis of an analysis of the literature data and the results of the authors' long-term studies. It is shown that all these disturbances are formed in patients with deep burns affecting over 15% of the body's surface in the presence of marked disturbances of all links of the systemic anti-infection defence, the latter being conducive to the generalization of the infectious process. Thus, these pronounced disorders of the immunologic mechanisms of homeostasis are responsible for the background against which those severely burned develop the organic neuropsychopathological syndrome, burn-associated encephalopathy, which persists for a long time after the trauma.

A total of 73 patients with burn disease were examined. The whole period of the disease was marked by a distinct asthenic symptomatology shown by predominance of physical asthenia, by stable and stereotypic mental disturbances. The asthenic disorders correlated with the somatic state of the patient. Such disorders were seen for a long period of time, and even after recovery. Apart from asthenic symptomatology, all the patients demonstrated disturbances in the intellectual sphere, and in intellectual-mnestic processes, which were also seen in the late periods of burn disease. The changes in the neurologic state were characterized by lesions of the cranial nerves, anizoreflexia of the hemitype, pathological reflexes in the hand and feet, vegetative-trophic disturbances indicating that all the patients who had extensive and deep burns developed encephalopathy.
