RESUMEN

PURPOSE: To explore trans men's access and use of healthcare services in Chile, based on the experiences of the trans men themselves, as well as of healthcare professionals. METHODS: A qualitative study with an ethnographic approach was carried out with 30 participants: 14 trans men and 16 healthcare professionals. Semi-structured one-on-one interviews with open-ended questions were used to collect the data. A thematic analysis was carried out with the NVivo Software. RESULTS: Three main themes were identified: (1) failures in the recognition of trans identity, (2) challenges with patient-centered care, and (3) use of other ("non-trans") health services. CONCLUSIONS: The results suggest that not all transition processes are the same, individuals seek different ways; therefore, it is necessary to consider different body types and identities when planning programs and care for men in transition. Moreover, the accompaniment provided during the gender transition process should contemplate emotional and mental support. CLINICAL RELEVANCE: The study outlines the need for all healthcare professionals to have training and knowledge about the transgender population, regardless of whether they are part of the teams supporting gender transition processes. The role of nurses and the contributions that can be made from nursing discipline in this research field are fundamental.

Burns are a problem of interest in public health since they generate a high rate of morbidity and mortality worldwide, thermal burns are the most prevalent and can alter the anatomical, functional and aesthetic integrity of the skin, fundamental aspects for the patient's self-esteem and their ability to reintegrate into society. At review literature about the treatment of these conditions, we find various treatments, including the use of human amniotic membrane, which has had a significant impact on burn management by functioning as a biological scaffold with regenerative and anti-inflammatory qualities. The present article aims to synthesize the current information that describes the applications of human amniotic membranes in burns. We carry out a systematic exploratory review of the literature from 2010 to 2021.

RESUMEN

INTRODUCTION AND AIMS: Celiac disease (CD) is an autoimmune enteropathy that develops in genetically susceptible individuals. The typical gastrointestinal manifestation is diarrhea but symptoms of dyspepsia, such as epigastric pain, nausea, or satiety, can sometimes appear. Previous studies have reported that the prevalence of CD in patients with dyspepsia can be as high as 7%. The aim of the present study was to evaluate CD seroprevalence in subjects with dyspeptic symptoms and a control group in a Mexican population. MATERIAL AND METHODS: A case-control study was conducted on blood donors that answered the PAGI-SYM questionnaire for dyspepsia and in whom IgA antibodies to tissue transglutaminase 2 (IgA anti-tTG2) and IgG antibodies to deamidated gliadin peptide (IgG anti-DGP) were determined. CD seroprevalence in subjects with dyspeptic symptoms and in asymptomatic subjects was compared. RESULTS: A total of 427 subjects (76.3% men), with a mean patient age of 34 years (range of 18-65 years) were included. Of those participants, 87 (20.3%) had symptoms of dyspepsia (group A) and 340 (79.6%) were asymptomatic (group B). Antibodies were positive in one (1.15%) of the group A subjects (1/87, 95% CI 0.2-6 %), whereas they were positive in 4 (1.18%) of the group B subjects (4/340, 95% CI 0.4-2.9%, p = 0.59). CONCLUSIONS: CD seroprevalence in the study population with dyspeptic symptoms (1%) was not different from that of the control population. Thus, CD screening in Mexican patients with dyspepsia is not justified.

ABSTRACT During interventional procedures that utilize fluoroscopy, patients are exposed to varying levels of radiation doses, which can have short, medium, or long term impacts, as well as varying severities. This depends on the type of radiation used and the radiation sensitivity of the organ(s) being irradiated. At this point, it becomes crucial to determine the organ doses delivered during procedures in order to ensure patient well-being. Since direct measurements cannot be made on the patient's organs, specialized software is used for dosimetric calculations. PCXMC 2.0 software achieves the estimation of effective dose through the deterministic Monte Carlo method, while also incorporating equivalent dose and risk assessment for dosimetric studies. Therefore, the objective of this work was to propose a standardized methodology for the use of the PCXMC 2.0 sofware in the calculation of these two magnitudes.it is important to outline a working method for this software and provide instructions on its operation

RESUMEN

Background: In Mexico less than half of the treated hypertensive patients reach blood pressure (BP) targets. Most hypertensive individuals rely on the standard medical care (SMC) to achieve the BP control goals; however, the efficacy of BP telemonitoring (BPT) to achieve BP targets has been poorly studied. Aim: To compare the efficacy of BPT versus SMC to achieve BP goals in patients with uncontrolled hypertension. Methods: A two-arm, open-label clinical trial was conducted in patients ≥18 years with uncontrolled hypertension. The participants were randomized to 2 arms (BPT vs SMC) and followed for 12 weeks. For the statistical analysis, the chi-squared test and covariance were used. Results: One hundred and seventy-eight participants were included, BPT (n = 94) and SMC (n = 84), after 12 weeks of follow up, we observed a baseline-adjusted reduction in systolic BP with both BPT (-13.5 [1.3] mmHg) and the SMC (-5.9 [1.4] mmHg; p < 0.001) but a greater decrease with BPT (p < 0.001). Likewise, we found a baseline-adjusted reduction of diastolic BP with BPT (-6.9 [0.9] mmHg) and SMC (-2.7 [0.9] mmHg) (p = 0.007) with a more significant percentage change from baseline with BPT (-6.8% [1.0] vs 2.5% [1.1]; p = 0.007). In the BPT arm, a larger proportion of patients achieved the BP target versus SMC (30.5% vs 12.8%; p = 0.005). Conclusion: BPT showed a greater proportion of patients achieving office BP control goals (<140/90 mmHg), compared to standard medical care.

Introduction: Currently there are few studies regarding the evaluation of attitudes and self-perception of knowledge of health professionals towards patients with some mental health condition, and the relationship of these variables with decision making regarding these patients. From experience, its known the difficulty that doctors who are not related to the area of mental health have in dealing with patients with some psychiatric pathology and even more so in emergencies, either because they recognize a lack of knowledge of the subject and instances that are not suitable for evaluation. in addition to the high demand of these services. Methodology: The study consisted of a cross-sectional online survey applied to health professionals belonging to the Dr Hernán Henriquez Aravena Hospital, located in the city of Temuco, Chile. The survey included sociodemographic questions, characterization of experiences associated with mental health diagnoses, and scales dedicated to measuring variables (self-perception, attitudes and decision making). Results: As self-perception of knowledge grows, the effect of attitudes on decision making also increases. Conclusions: Both the attitudes of health professionals towards this type of patients, as well as their self-perception of knowledge, are two fundamental components to ensure good clinical management. It is important that there is a balance in how both are managed within a service, in order to ensure appropriate care focused on the patient's well-being.

Introduction: Gastric cancer in Chile takes the first place as a cause of mortality from malignant tumors, therefore it is relevant to know the demographic description of this disease in the country. Studied years go from 2017-2021 in order to get updated data to the Chilean public health. Methodology: Ecological, observational, cross-sectional and descriptive study on deaths due to gastric cancer in 2017-2021 in Chile, according to sex, age group and regions (n=15350). The death rate was calculated. Data was obtained from the Department of Statistics and Health Information and the National Institute of Statistics. It did not require approval from an ethics committee. Results: During the 2017-2021 period the overall death rate was 21.84, 21,58 for men and 10,68 for women. The highest values were in the 80 and older age group and the lowest values in the 15-19 years age group. The highest death rate overall period was registered in the Metropolitan Region. Discussion: There was a reduction in the hospital death rate between 2017-2021, which could be due to the increase in screening and treatment of Helicobacter pylori. Death rate is higher in men, which could be explained because they are more genetically susceptible to developing cancer. The lowest values for death rate were found in the 15-19 years age group, and the highest values in the 80 years and older group, which is consistent with international literature. Conclusion: More studies are required in Chile to broaden the findings to allow investigation continuity and to determine specific risk factors present in the Chilean population.

RESUMEN

The aim of this study was to analyze the expression of mBD4, mBD3 and CRAMP in joint of mice with type II collagen-induced arthritis/CIA and to explore its possible association with IL-10, IL-4, IFN-γ, IL-17, MMP3, RANK/RANKL/OPG and histological parameters. METHODS: CIA was induced in 44 DBA/1 J mice. The joints from mice were classified into the onset, peak and remission phase of CIA. Histological sections were stained with hematoxylin-eosin and safranin O. The expression of CRAMP, mBD-3, mBD-4, and MMP-3 was evaluated using reverse transcription polymerase chain reaction (RT-PCR) and immunohistochemistry. The expression of IL-10, IL-4, IFN-γ, IL-17, RANK/RANKL/OPG was analyzed by RT-PCR. RESULTS: We observed that inflammation and immunostained cells for CRAMP increased in the peak and remission phases compared to the control group. In addition, increments in relative expressions of CRAMP were detected for the remission phase and in IL-4 and IL-17 in the peak phase compared to the control and onset phase. In addition, an increase in IL-10 in a peak phase compared to the control, as well as the relative expression of IFN-γ in remission phase was higher than in the onset phase. This was accompanied by an increase in cartilage damage in the peak phase compared to the control. Cells immunostained to MMP3 increased in the peak phase compared to the onset and control group, and relative expression of MMP3 was detected in the peak phase compared to the onset, remission, and control group. We observed that the relative expression of RANK and RANKL in the peak phase was higher than in control and onset phase. Finally, the relative expression of OPG in the peak phase compared to the onset, remission, and control group was detected. Regarding CRAMP behavior in the different phases studied, it was positively correlated with IL-4 and RANK, and showed a negative correlation with IFN-γ, IL-17, IL-10, RANKL, OPG and RANKL/OPG ratio in the control group. Also was positively correlated with IFN-γ, IL-17, IL-4, IL-10, as well as with RANK, RANKL, and OPG in the onset and peak phases of the CIA. In the peak phase, CRAMP showed a positive association with MMP3, and we observed a direct correlation between CRAMP and IFN-γ and RANKL/OPG ratio in remission phase. mBD3 correlates positively with IFN-γ, IL-17, IL-10, RANKL, OPG and RANKL/OPG ratio, and showed a negative correlation with CRAMP, MMP3, and RANK in the control group. Also, it was directly associated with IFN-γ, IL-17, IL-4, IL-10 and RANKL in the onset phase while it was inversely associated with CRAMP, MMP-3, RANK, RANKL, and OPG in the peak phase. Finally, mBD3 was inversely correlated with MMP3 in the remission phase and was directly associated with CRAMP, IFN-γ and RANKL/OPG ratio in this phase. mBD4 was directly associated with CRAMP, IFN-γ, IL-17, IL-4, IL-10, RANKL / OPG in the onset phase, and with CRAMP, IFN-γ, IL-17, IL-4, IL-10, MMP3, RANK, RANKL and OPG in the peak phase. Finally, mBD4 was positively associated with mBD3, IFN-γ, IL-17, IL-10, RANK, RANKL OPG and RANKL/OPG in the CIA remission phase. CONCLUSIONS: Our results demonstrate that CRAMP plays an important role in CIA progress and suggest that its abundance is associated with local pro- and anti-inflammatory status. This makes us propose CRAMP as a possible contributor of bone reconstruction in the last stage of CIA.

RESUMEN

INTRODUCTION: Anomalous intracranial venous anatomy is described in patients with syndromic craniosynostosis and is of significant importance when it comes to surgical morbidity. However, it is still controversial its origin, type of circulation in each syndrome, how it behaves over time, when it can be interrupted and wether it needs to be studied. The purpose of this paper is to discuss these issues by reviewing the literature. METHODS: A literature search was performed using the PubMed database with a focus on papers including detailed descriptions of the venous outflow in complex and syndromic craniosynostosis. Search details used were the following: ("veins"[MeSH Terms] OR "veins"[All Fields] OR "venous"[All Fields]) AND ("abnormalities"[Subheading] OR "abnormalities"[All Fields] OR "anomalies"[All Fields]) AND syndromic[All Fields] AND ("craniosynostoses" [MeSH Terms] OR "craniosynostoses"[All Fields] OR "craniosynostosis"[All Fields]). Studies that exposed details of venous anomalies found in syndromic or complex craniosynostosis were selected. RESULTS: Of a total of 211 articles found, 11 were selected for this review. Of these, 5 were case reports, 5 retrospective studies, and only 1 prospective study. From the 6 series of cases presented, 5 discussed the relationship between jugular foramen stenosis (JFS) and collateral venous drainage. The authors discuss data from the literature for each leading question presented: 1-collateral circulation: is it an intrinsic trouble, a consequence of stenosis of the cranial base foramina or related to raised intracranial pressure (ICP)?; 2-what venous anomalies should we search for, and what is the best exam to study them?; 3-collateral circulation changes with time?; 4-can neurosurgeons interrupt the collateral circulation?; 5-should we study all complex types of craniosynostosis? CONCLUSION: The importance of the study of the venous outflow in patients with complex craniosynostosis is evident in the literature. The real relationship between intracranial hypertension, hypoplastic skull base foramen, Chiari I malformation, hydrocephalus, and venous collateral circulation remains unknown. Prospective studies focusing on molecular biology analysis will possibly solve all of these leading questions.

RESUMEN

Approximately 200,000 tons of water contaminated with dyes are discharged into effluents annually, which in addition to infectious diseases constitute problems that afflict the population worldwide. This study evaluated the mechanical properties, surface structure, antimicrobial performance, and methylene blue dye-contaminant adsorption using the non-woven fabrics manufactured by melt-blowing. The non-woven fabrics are composed of nylon 6 (Ny 6) and zinc oxide nanoparticles (ZnO NPs). The polymer nanocomposites were previously fabricated using variable frequency ultrasound assisted-melt-extrusion to be used in melt-blowing. Energy dispersion spectroscopy (SEM-EDS) images showed a homogeneous dispersion of the ZnO nanoparticles in nylon 6. The mechanical properties of the composites increased by adding ZnO compared to the nylon 6 matrix, and sample Ny/ZnO 0.5 showed the best mechanical performance. All fabric samples exhibited antimicrobial activity against S. aureus and fungus C. albicans, and the incorporation of ZnO nanoparticles significantly improved this property compared to pure nylon 6. The absorption efficiency of methylene blue (MB), during 60 min, for the samples Ny/ZnO 0.05 and Ny/ZnO 0.25 wt%, were 93% and 65%, respectively. The adsorption equilibrium data obeyed the Langmuir isotherm.

RESUMEN

AIMS: To evaluate associations between clinicopathological variables and hypercapnia measured in cats with decompensated chronic kidney disease (CKD) on admission to a veterinary hospital. METHODS: This is a retrospective, cross-sectional study of cats (n = 39) that presented to a tertiary veterinary hospital in Argentina between June 2015 and December 2017 with blood creatinine concentrations >140 µmol/L, and abdominal ultrasound results consistent with CKD. Data recorded included venous partial pressure of CO2 (PvCO2), blood pH, haematocrit and concentrations of glucose, potassium, sodium, corrected sodium (Na+c), and ionised calcium in blood. A logistic regression model was used to assess associations between the presence of hypercapnia (PvCO2 ≥ 44.7 mmHg) and the other clinicopathologic variables. The duration of hospitalisation was compared in cats with and without hypercapnia using the Wilcoxon Rank Sum test. RESULTS: The final study population comprised 39 cats. Eleven cats (28.2%) had hypercapnia. In the logistic regression model, two independent variables were associated with the presence of hypercapnia at admission in cats with CKD: the concentration of creatinine in blood (OR = 1.06 (95% CI = 1.016-1.108); p = 0.007) and Na+c (OR = 1.33 (95% CI = 1.08-1.63); p = 0.005). There were no statistically significant differences in the length of hospital stay between the two groups. CONCLUSIONS AND CLINICAL RELEVANCE: There appears to be an association between elevated concentrations of creatinine and Na+c in blood, and hypercapnia in cats with CKD, suggesting careful assessment of blood gas and electrolyte parameters during hospitalisation is required. Further prospective studies are needed to evaluate the mechanisms behind this association and the association of hypercapnia with disease outcome including mortality.

SUMMARY Mycobacterium tuberculosis meningitis is the most lethal extrapulmonary form of active tuberculosis with high prevalence in developing countries, which constitutes a public health problem. The extrapulmonary presentation that compromise the nervous system include meningeal tuberculosis, tuberculous granuloma, tuberculosis abscess and radiculomyelitis. A nonspecific clinical presentation, the lack of rapid tests with high sensitivity and specificity, the high cost and poor affordability of testing in many countries make the diagnosis more complicated. The diagnosis is based on microbiological, molecular, and host response based testing. Microbiological diagnosis is based on Ziehl - Neelsen staining and culture, the latter long considered the gold standard diagnostic test. Molecular diagnosis is more accurate using nucleic acid amplification tests, the chain reaction of polymerase, the GeneXpert MTB / RIF test, and the Xpert MTB / RIF Ultra test. Given the absence of a rapid test, there are scales that help guide the patient's diagnosis, within which the Thwaites Scale and the Marais Scale deserve to be mentioned. The treatment of tuberculosis of susceptible strains includes a structured regimen with isoniazid, rifampicin, pyrazinamide and ethambutol, however, antimicrobial resistance is currently a problem for which the treatment of multidrug-resistant tuberculosis must be individualized, with a long-term treatment regimen.

Bulls in training (n = 17) for rodeo were evaluated for stress interference in testosterone production. Parameter analysis was performed regarding the health of the bulls before training and blood collection. After the exercise, the second blood collection was performed. The samples collected before the training were submitted to the measurement of cortisol and testosterone hormones and the samples after the training only to the cortisol dosage. A new collection sequence was started for dosing and evaluating testosterone. Both hormones were measured by the radioimmunoassay method. The statistical analysis for comparison of cortisol and testosterone, as well as for the scrotal perimeter and testosterone was performed by Pearson correlation with a 5% level of significance through Action 3.1 version R 3.0.2 (Campinas, SP, Brazil). Serum cortisol dosage before and after training resulted in a positive correlation (0.071), showing statistical difference (p = 0.0004) in these two moments evaluated. The comparison between first day testosterone and cortisol before training was negative as was testosterone on the second day with cortisol after training. It was concluded that there was evidence of post-training stress without interference in the production of testosterone
