A cohort of Chinook salmon juveniles was vaccinated, with an autogenous bivalent vaccine against New Zealand RLOs (NZ-RLO1) and Tenacibaculum maritimum. A proportion of the cohort was not vaccinated to act as controls. At smoltification, the fish were challenged with NZ-RLO1, NZ-RLO2, or T. maritimum. We found that challenge with T. maritimum by immersion in (7.5 × 105 cfu/mL of water) did not yield any pathology. Challenge with RLOs produced clinical signs that were more or less severe depending on the challenge route, dose or vaccination status. Survival was significantly higher for vaccinated fish within the groups challenged with NZ-RLO1 by intraperitoneal injection with a relative percent survival (RPS) of 48.84%. Survival was not significantly different between vaccinated and non-vaccinated fish for groups challenged with NZ-RLO2 by intraperitoneal injection or by NZ-RLO1 by immersion. Yet, anecdotally the clinical disease presentation (manifesting as haemorrhagic, ulcerative skin lesions) was more severe for the non-vaccinated fish. This study demonstrates that autogenous vaccine against NZ-RLO is protective against severe disease and death by NZ-RLO1 challenge which warrants implementation and further evaluation under field conditions. Yet, this study also highlights the importance of the route of administration and dose when evaluating pathogenicity and vaccine efficacy.

La administración de norepinefrina por vía periférica es restringida, por la asociación de la extravasación con necrosis tisular. Método: Revisión de alcance con el objetivo de describir los efectos adversos relacionados con la administración de norepinefrina por acceso venoso periférico corto y las características de administración del fármaco en pacientes hospitalizados en servicios de UCI, cirugía y urgencias. Resultados: Se incluyeron 12 estudios de características heterogéneas por tamaño y tipo de población. La proporción de complicaciones asociadas a la administración de norepinefrina por vía periférica fue inferior al 12% en los estudios observacionales y menor al 2% en aquellos que utilizaron dosis menores a 0,13μg/kg/min y concentraciones inferiores a 22,3μg/ml. La principal complicación asociada fue la extravasación y no se presentó ningún caso de necrosis tisular en el sitio de venopunción. El tratamiento farmacológico utilizado para su manejo fue con terbutalina o nitroglicerina tópica; el tiempo de administración del fármaco osciló entre 1 y 528 horas, con una media ponderada de 2,78 horas. Conclusión: El principal efecto adverso fue la extravasación, no se presentaron complicaciones adicionales, la fentolamina y terbutalina parecen ser útiles en estos casos; su disponibilidad es una necesidad para una administración periférica segura. Es necesario que la enfermera realice una valoración estrecha y un cuidado integral en los pacientes que reciben norepinefrina por vía periférica.(AU)

AIMS: To evaluate the fitness of three PCR assays for the detection of Mycoplasma bovis in dilute (extended) bovine semen, and a reverse transcriptase-PCR (RT-PCR) adaptation as a proxy for viability. MATERIALS AND METHODS: Four commercial kit-based methods for nucleic acid extraction were compared to test for the presence of PCR inhibitors in nucleic acid extracted from undiluted and diluted semen. Then, analytical sensitivity, analytical specificity, and diagnostic specificity of two real-time PCR and one conventional PCR were evaluated for the detection of M. bovis DNA in semen and compared against microbial culture. Furthermore, an RT-PCR was adapted to detect RNA only and tested on viable and non-viable M. bovis to establish its ability to discriminate between the two. RESULTS: No significant PCR inhibition was detected from the dilute semen. All DNA extraction methods except one were equivalent, regardless of semen dilution. The analytical sensitivity of the real-time PCR assays was estimated as 45.6 cfu per 200 µL semen straw (2.2 × 102 cfu/mL). The conventional PCR was 10 times less sensitive. No cross-reactivity was observed for the real-time PCR for any of the bacteria tested and the diagnostic specificity was estimated as 100 (95% CI = 94.04-100) %. The RT-PCR was poor in distinguishing between viable and non-viable M. bovis. The mean quantification cycle (Cq) values for RNA extracted from different treatments to kill M. bovis remained unchanged 0-48 hours after inactivation. CONCLUSION AND CLINICAL RELEVANCE: The real-time PCR were fit for the purpose of screening dilute semen for the detection of M. bovis to prevent incursion via importation of infected semen. The real-time PCR assays can be used interchangeably. The RT-PCR could not reliably indicate the viability of M. bovis. Based on the results from this study, a protocol and guidelines have been produced for laboratories elsewhere that wish to test bovine semen for M. bovis.

OBJECTIVE: To report the ocular manifestations in patients with leukaemia. METHODS: This is a retrospective, descriptive and observational study in patients with ocular manifestations of leukaemia. RESULTS: A total of 14 eyes were evaluated corresponding to 8 patients (5 women and 3 men) with ocular manifestations of leukaemia. The mean age at diagnosis was 43 years (31-76 years). Six eyes corresponded to patients with acute myeloid leukaemia (AML), four eyes to acute lymphoid leukaemia (ALL), two eyes to chronic myeloid leukaemia (CML), and the remaining two belonged to patients with hairy cell leukaemia (HCL). The primary ocular findings were choroidal invasion in 12 eyes (85.7%), retinal infiltration in 4 eyes (28.6%), and neuro-ophthalmic disorders in 4 eyes (28.6%). The mean visual acuity improved from 0.689 to 0.449 (logMAR) (P=.012) after the systemic and intrathecal chemotherapy. Of the eight patients, four died from systemic complications of the underlying disease. CONCLUSIONS: This is the first report of multiple ocular manifestations secondary to leukaemia in a Colombian population. It is important to keep in mind that this disease is included within the masquerade syndromes and that the ophthalmological findings that, while subtle, can also be devastating and be signs of a life-threatening disease.

Systemic lupus erythematosus (SLE) patients are at higher risk of developing opportunistic infections such as tuberculosis (TB), especially extrapulmonary forms like osteoarticular TB, compared to the general population. However, tuberculous sacroiliitis has been scarcely reported in these patients. We present a 34-year-old woman with SLE who developed articular tuberculosis simultaneously affecting the right sacroiliac joint and the left knee. The patient was successfully treated with antituberculosis therapy for nine months. In this case, in addition to the immunological abnormalities of lupus, the long-term glucocorticoid therapy at high dosages was the main risk factor for the development of osteoarticular tuberculosis.

Objetivo: la responsabilidad ética del investigador obliga al monitoreo de la seguridad de los participantes a través del estudio y, por ende, se requiere de un comité de monitoreo de datos, cuya tarea principal es el análisis interino que se refiere a la supervisión de variables como beneficios dramáticos, efectos adversos, mortalidad y futilidad, que lleven a la terminación temprana del estudio. El objetivo de esta investigación es determinar los métodos estadísticos más utilizados en el análisis interino en los ensayos clínicos aleatorizados y publicados en 2016 en una revista médica general de alto factor de impacto. Metodología: se realizó un estudio meta-epidemiológico descriptivo, constituido por ensayos clínicos aleatorizados publicados en el The New England Journal of Medicine, desde el 7 de enero hasta el 10 de noviembre de 2016. Resultados: se analizaron 104 artículos con una muestra promedio de 5 531 ± 762 pacientes y un tiempo de seguimiento promedio de 31 ± 45 meses. De los estudios evaluados se encontró que 88 (84.61%) realizaron análisis interino y 16 (15.38%) no lo realizaron. Los métodos estadísticos utilizados en los artículos fueron O'Brien-Fleming en 30 (34.09%), Haybittle-Peto en 13 (14.77%); otros métodos en dos (2.27%) y no especificado en 43 (48.86%) de ellos. Conclusiones: la mayoría de los ensayos clínicos aleatorizados realizó análisis interino y lo describen en su protocolo. Aproximadamente la mitad de los estudios no especificó el método estadístico; sin embargo, el método descrito más utilizado fue O'Brien-Fleming y, en segundo lugar, Haybittle-Peto y en ningún estudio se especificó el método de Pocock.


Objective: The ethical responsibility of the researcher requires that the safety of participants be monitored throughout the study. Thus, a data monitoring committee is required, whose main function is the interim analysis, which monitors variables such as dramatic benefits, adverse effects, mortality and futility that lead to the early termination of a study. The objective of this study is to determine the most frequently used statistical methods in interim analysis in randomized clinical trials published during 2016 in a high impact factor medical journal. Methodology: Meta-epidemiological descriptive study, consisting of randomized clinical trials published in The New England Journal of Medicine from January 7 to November 10 of 2016. Results: 104 articles were analyzed, where the mean sample was 5,531 ± 762 patients and and average follow-up time was 31 ± 45 months. Of the studies evaluated, 88 (84.61%) performed interim analysis, while 16 (15.38%) did not. Regarding statistical methods used, O'Brien-Fleming was used in 30 (34.09%) studies, Haybittle-Peto in 13 (14.77%), other methods in two (2.27%), and the method was not specified in 43 (48.86%). Conclusions: most of the randomized clinical trials reviewed had interim analysis and described it in their protocol. Approximately half of the studies did not specify the statistical method; however, the most commonly described method was O'Brien-Fleming, followed by Haybittle-Peto; no study used the Pocock method.

Age-dependent susceptibility to nervous necrosis virus (NNV) was demonstrated for barramundi (Lates calcarifer). The experiment used juvenile barramundi produced from a single spawning that were challenged consecutively by immersion with a redspotted grouper nervous necrosis virus (RGNNV) isolate. The dose and environmental conditions (35 ppt salinity and 30 °C) were constant. Fish and water were sampled longitudinally for histopathology and RT-qPCR analysis to examine the evolution of the disease, virus replication, immune response and release of virus into water. Viral nervous necrosis (VNN) disease occurred in barramundi challenged at 3 and 4 weeks of age while fish challenged at 5, 7 and 9 weeks of age developed subclinical infection. Replication of NNV occurred faster and the concentration of virus reached higher concentrations in the younger fish with clinical disease. Virus isolation and qPCR tests indicated that infectious NNV was released from carcasses into water when fish were affected with clinical disease but not when NNV infection was subclinical. Based on these observations, we consider that carcasses from clinically infected fish have a potentially important role in the horizontal transmission of NNV, and barramundi juveniles should be protected from exposure to NNV until they are 5 weeks of age and reach the disease resistance threshold.

C-2U is a high-confinement, advanced beam driven field-reversed configuration plasma experiment which sustains the configuration for >5 ms, in excess of typical MHD and fast particle instability times, as well as fast particle slowing down times. Fast particle dynamics are critical to C-2U performance and several diagnostics have been deployed to characterize the fast particle population, including neutron and proton detectors. To increase our understanding of fast particle behavior and supplement existing diagnostics, an E ∥ B neutral particle analyzer was installed, which simultaneously measures H0 and D0 flux with large dynamic range and high energy resolution. Here we report the commissioning of the E ∥ B analyzer, confirm the instrument has energy resolution ΔE/E≲0.1 and a dynamic range Emax/Emin∼30, and present measurements of initial testing on C-2U.

Aunque la mayoría de las enfermas con osteoporosis que observamos en la practica clínica obedecen al grupo de osteoporosis postmenopáusicas o las relacionadas con el envejecimiento, existen algunos casos de osteoporosis en cuyo desarrollo existe alguna enfermedad o factor identificable distinto a la menopausia o al envejecimiento; la mayoría de estas causas están relacionadas con la toma de esteroides, el hipogonadismo, los procesos malignos como el mieloma múltiple, la cirugía gástrica, el alcoholismo, y el tratamiento por anticonvulsivantes. La hemocromatosis hereditaria es otra situación relacionada con la aparición de osteoporosis. Presentamos 4 casos de pacientes con osteoporosis que padecen también hemocromatosis hereditaria, describiendo sus características y la posible relación de la hemocromatosis hereditaria con la patología ósea (AU)


Although most people's osteoporotic conditions treated in clinical practice may be categorized in the postmenopausal osteoporosis group or related to aging, there are some osteoporosis cases linked to the development of some other disease or identifiable factor. Most of these causes are associated with the taking of steroids, hypogonadism, malignant processes such as multiple myeloma, gastric surgery, alcoholism and treatment with anticonvulsant drugs. Hereditary hemochromatosis is another disorder related to the onset of osteoporosis. In this paper, we present 4 cases of patients with osteoporosis who also suffer hereditary hemochromatosis. The latter’s characteristics are described and also its possible relationship with bone disease (AU)

With the imbalance between donation rates and potential recipients growing, transplant programs are increasingly using non-ideal organs from so-called marginal donors. This is the first reported case of the intentional use of a donor heart with ALCAPA. The recipient was aged one yr with restrictive cardiomyopathy who had been supported with BiVAD for over six months. Function of the donor left ventricle was shown to be well preserved, with no obvious signs of ischemia, except for a fibrotic layer on the anterolateral papillary muscle of the mitral valve. To prevent coronary steal, the anomalous left coronary artery ostium from the MPA was oversewn prior to implantation. The transplanted heart spontaneously regained sinus rhythm immediately following cross-clamp release and showed good contractility from the first postoperative echocardiogram. The patient continues to do well 18 months post-transplant, with excellent function on echocardiography, and good flow on coronary angiography.

El proceso de capacitación en tecnología biomédica se enmarca dentro de los requerimientos del decreto # 4725 de 2005 emitido por el ministerio de protección social, y es parte del proceso de adquisición de tecnología el cual es de gran importancia para las instituciones prestadoras de salud en Colombia. La tecnología médica requiere procesos detallados de capacitación. Uno de los aspectos más importantes a considerar más no el único es la clasificación de riesgo de los dispositivos médicos, el cual es un factor muy importante porque puede cambiar el tiempo y costo de la capacitación. Este artículo pretende hacer una revisión de algunos modelos de capacitación en clínicas y hospitales y comprarlos entre sí, Adicionalmente el artículo presenta un modelo que se basa en los criterios más representativos de los modelos analizados.


The training in biomedical technology is part of the requirements of the decree # 4725 of 2005 issued by the Colombian Ministry of Social Protection, and it's part of the technology procurement process which is of great importance to the health institutions in Colombia. Medical technology requires detailed training processes. One of the most important aspects to consider, not the only one, is risk classification of medical devices, which is a very important factor that may affect time and cost of training. This article aims to review and compare some models of training in clinics and hospitals. In addition, the paper introduces a model based on the most representative criteria of the models analyzed.


O treinamento em tecnologia biomédica é parte dos requisitos do decreto # 4725 de 2005 emitido pelo Ministério da Proteção Social, e faz parte do processo de aquisição de tecnologia, que é de grande importância para as instituições de saúde na Colômbia. A tecnologia médica requer processos de treinamento detalhados. Um dos aspectos mais importantes a considerar não é a única classificação de risco de dispositivos médicos, o que é um fator muito importante que pode mudar o tempo e o custo do treinamento. Este artigo tem por objetivo revisar alguns modelos de formação em clínicas e hospitais e compará-los entre si, Além disso, o artigo apresenta um modelo com base nos critérios mais representativos dos modelos analisados.

OBJECTIVES: An experimental piglet model induces avascular necrosis (AVN) and deformation of the femoral head but its secondary effects on the developing acetabulum have not been studied. The aim of this study was to assess the development of secondary acetabular deformation following femoral head ischemia. METHODS: Intracapsular circumferential ligation at the base of the femoral neck and sectioning of the ligamentum teres were performed in three week old piglets. MRI was then used for qualitative and quantitative studies of the acetabula in operated and non-operated hips in eight piglets from 48 hours to eight weeks post-surgery. Specimen photographs and histological sections of the acetabula were done at the end of the study. RESULTS: The operated-side acetabula were wider, shallower and misshapen, with flattened labral edges. At eight weeks, increased acetabular cartilage thickness characterised the operated sides compared with non-operated sides (p < 0.001, ANOVA). The mean acetabular width on the operated side was increased (p = 0.015) while acetabular depth was decreased anteriorly (p = 0.007) and posteriorly (p = 0.44). The cartilage was thicker, with delayed acetabular bone formation, and showed increased vascularisation with fibrosis laterally and focal degenerative changes involving chondrocyte hypocellularity, chondrocyte cloning, peripheral pannus formation and surface fibrillation. CONCLUSIONS: We demonstrate that femoral head AVN in the young growing piglet also induced, and was coupled with, secondary malformation in acetabular shape affecting both articular and adjacent pelvic cartilage structure, and acetabular bone. The femoral head model inducing AVN can also be applied to studies of acetabular maldevelopment, which is less well understood in terms of developing hip malformation. AIM: To compare the removal of biofilm utilizing four irrigation techniques on a bovine root canal model. METHODOLOGY: Fifty dentine specimens (2 × 2 mm) were infected with biofilm. The samples were then adapted to previously created cavities in the bovine model. The root canals were irrigated twice with 2 mL of 6% sodium hypochlorite for 2 min (4 min total). Following initial irrigation, the different treatment modalities were introduced for 60 s (3 × 20 s intervals). The evaluated techniques were needle irrigation, Endoactivator (Dentsply Tulsa Dental, Tulsa, OK, USA), passive ultrasonic irrigation and laser-activated irrigation (photon-induced photoacoustic streaming). The controls were irrigated with distilled water and conventional needle irrigation. Subsequently, the dentine samples were separated from the model and analysed using a scanning electron microscope (SEM). Fifteen operative fields were scanned per block, and SEM pictures were captured. Two calibrated evaluators examined the images and collected data using a four-degree scale. Nonparametric tests were used to evaluate for statistical significance amongst the groups. RESULTS: The group undergoing laser-activated irrigation using photon-induced photoacoustic streaming exhibited the most favourable results in the removal of biofilm. Passive ultrasonic irrigation scores were significantly lower than both the Endoactivator and needle irrigation scores. Sonic and needle irrigation were not significantly different. The least favourable results were found in the control group. CONCLUSIONS: Laser activation of 6% sodium hypochlorite significantly improved the cleaning of biofilm-infected dentine followed by passive ultrasonic irrigation.

AIM: To evaluate the efficacy of a nonthermal plasma (NTP) at atmospheric pressure on ex vivo biofilm in root canals of extracted teeth. METHODOLOGY: Intracanal contents from three teeth with root canal infections were collected, pooled and grown in thirty-five microCT-mapped root canals of extracted and instrumented human teeth. One group of teeth was treated with NTP, another with 6% NaOCl and one set was left untreated. The intracanal contents from twenty-seven teeth (nine teeth in each group) were plated on agar and colony forming units were determined. Parametric test of one-way analysis of variance (anova) was used to analyse statistical significance. The remaining teeth were cut open, stained with LIVE/DEAD(®) and examined with confocal laser scanning microscopy. RESULTS: The untreated root canals were covered with biofilm of varying thickness. Treatment with nonthermal plasma decreased the number of viable bacteria in biofilms by one order of magnitude, whilst the NaOCl control achieved a reduction of more than four magnitudes. Both the NTP and the NaOCl treatment results were significantly different from the negative control (P < 0.05). CONCLUSION: The nonthermal plasma displayed antimicrobial activity against endodontic biofilms in root canals, but was not as effective as the use of 6% NaOCl.

The knee joint is the one of the most common locations for congenital and developmental musculoskeletal abnormalities. Initial imaging of the knee joint should always begin with conventional radiographs. However, evaluation of the bone marrow, cartilaginous, ligamentous, and other soft-tissue components of the knee joint are better characterized with magnetic resonance imaging (MRI). We present the MRI findings of prevalent congenital and developmental abnormalities in the paediatric knee with particular emphasis on the components of the growing skeleton.

This study was designed to empirically examine the effects of iPod device and text-messaging activities on driver distraction. Sixty participants were asked to perform a driving simulation task while searching for songs using an iPod device or text messaging. Driving errors as measured by lane deviations were recorded and analyzed as a function of the distracters. Physiological measures (EEG) were also recorded during the driving phases in order to measure participant levels of cortical arousal. It was hypothesized that iPod use and text messaging would result in a profound effect on driving ability. The results showed a significant effect of iPod use and text-messaging on driving performance. Increased numbers of driving errors were recorded during the iPod and text-messaging phases than the pre- and post-allocation phases. Higher levels of Theta activity were also observed during the iPod and Text-messaging phase than the pre- and post-allocation phases. Implications for in-vehicle systems design, training, and safety are also discussed.

The effect of including artichoke silage in the rations of dairy ewes on milk characteristics and biochemical changes of ripened cheeses was evaluated. Four groups of lactating ewes were fed rations containing 0, 10, 20, or 30% artichoke silage on a dry matter basis. Bulk milk samples were collected 3 times during the feeding period, and semi-hard cheeses were manufactured and sampled during ripening. Milk composition and cheese yield were not affected by diet. Inclusion of 20 and 30% artichoke silage reduced the firmness of the curds at a level only detected by the Gelograph (Gelograph-NT, Gel-Instrumente, Thalwil, Switzerland) probe. Inclusion of artichoke silage in ewes' diet decreased fat and total free fatty acids content of these cheeses and increased total free amino acids content. Despite the effect of diet on cheese ripening characteristics, the overall sensory scores for cheeses corresponding to artichoke silage diets were statistically higher than those for the control cheeses.

Scanning electron microscopy is a useful imaging approach for the visualization of bacterial biofilms in their natural environments including their medical and dental habitats, because it allows for the exploration of large surfaces with excellent resolution of topographic features. Most biofilms in nature, however, are embedded in a thick layer of extracellular matrix that prevents a clear identification of individual bacteria by scanning electron microscopy. The use of confocal laser scanning microscopy on the other hand in combination with fluorescence in situ hybridization enables the visualization of matrix embedded bacteria in multi-layered biofilms. In our study, fluorescence in situ hybridization/confocal laser scanning microscopy and scanning electron microscopy were applied to visualize bacterial biofilm in endodontic root canals. The resulting fluorescence in situ hybridization /confocal laser scanning microscopy and scanning electron microscopy and pictures were subsequently combined into one single image to provide high-resolution information on the location of hidden bacteria. The combined use of scanning electron microscopy and fluorescence in situ hybridization / confocal laser scanning microscopy has the potential to overcome the limits of each single technique.

Las frecuencias anormalmente rápidas sobre un corazón con una estructura menos distensible, rodeado por una coraza que sólo le permite defenderse con la frecuencia cardiaca para sostener el gasto cardiaco, hacen que este sea un corazón con márgenes estrechos de tolerancia frente a las taquiarritmias. Las frecuencias persistentes y superiores a 220 latidos por minuto, generan presiones anormalmente altas sobre todo el corazón y el sistema venoso. Hidrops y edema placentario son secuelas finales antes de la muerte fetal, y aún hasta este punto se pueden controlar y revertir. Aunque en teoría cualquier tipo de arritmias común en la vida extrauterina se podría desencadenar en el feto, 90% de éstas están incluidas en la taquicardia supraventricular reciprocante y en el flutter. Ambos tipos de arritmias pueden responder a la digoxina, aunque algunas son refractarias a este medicamento. La aplicación de ultrasonido sobre el corazón, permite no sólo descartar anomalías cardiacas asociadas, sino realizar protocolos de tratamiento. La medición de la relación de los tiempos ventrículo atrial:atrio ventricular, permite considerar otro tipo de arritmias como la taquicardia ectópica de la unión y la forma permanente reciprocante de la unión, etc., e incluir otros anti-arrítmicos, como amiodarona, flecainida, sotalol, entre otros. La principal vía de uso de los anti-arrítmicos es la transplacentaria. Por lo tanto, no sólo el feto se expone a posibles efectos secundarios, sino también la madre. Un grupo multidisciplinario enfrentará esta situación con el fin de ofrecer el mejor resultado para los dos.


A heart with a not much distensible structure, surrounded by a membrane, and only able to react with the heart rate for maintaining its cardiac output, has a narrow margin of tolerance in front of tachyarrhythmias. Persistent heart rates >200/min generate abnormally high pressure on the heart and the venous system. Hydrops and placental edema are common final outcomes before fetal death, that still at this point can be controlled and reverted. Although theoretically any type of arrhythmias could be developed in the fetus, 90% of fetal tachyarrhythmias correspond to supraventricular reciprocant tachycardia and flutter. Both types of arrhythmias may respond to digoxin, although some are refractory to this drug. Ultrasound application to the heart allows not only to rule out associated heart anomalies, but to realize treatment protocols. The measurement of the relation between ventricular-atrial and atrio-ventricular times allows to consider another type of arrhythmias such as junctional ectopic tachycardia, permanent junctional reprocicant tachycardia, etc., and to include other antiarrhythmics such as amiodarone, flecainide and sotalol among others. The main administration route of antiarrhythmics is the transplacental. Therefore not only the fetus but also the mother is exposed to drug side effects. A multidisciplinary group will confront this situation in order to offer both mother and fetus the best result.
