RESUMEN

The objective of our study was to map county differences in incidence and mortality by cancers and examine their changes over time. Based on the database of National Cancer Registry and Central Statistical Office, age-standardized incidence and mortality rates per 100,000 person-years were calculated for each county for 15 cancer types and 3 time periods. East-West divide was apparent in incidence and mortality of lung cancer, with larger weight in East (Borsod-Abaúj-Zemplén, Heves, Jász-Nagykun-Szolnok, Békés counties). Concentration of lip and oral cavity malignancies was identified in the northeastern periphery (Borsod-Abaúj-Zemplén, Szabolcs-Szatmár-Bereg counties). Breast cancer incidence was the highest in Budapest. As a conclusion, changes in cancer incidence and mortality over time were similar to developed countries; however, values were higher. Differences in spatial distribution follow territorial pattern of social deprivation, which correspond to higher prevalence of health risk factors. Our study contributes to planning of public health programs by pinpointing regional inequalities in different cancer types.

Infertility patients, often in high distress, are entitled to being informed about their mental status compared to normative data. The objective of this study was to revalidate and test the accuracy of the SCREENIVF, a self-reported tool for screening psychological maladjustment in the assisted reproduction context. A cross-sectional, questionnaire-based online survey was carried out between December 2019 and February 2023 in a consecutive sample of female patients (N = 645, response rate 22.9%) in a university-based assisted reproduction center in Hungary. Confirmatory factor analysis and cluster and ROC analyses were applied to test validity, sensitivity and specificity in relation to Beck Depression Inventory (BDI) scores. Model fit was optimal (chi-square = 630.866, p < 0.001; comparative fit index = 0.99; root-mean-square error of approximation = 0.018 (90% CI 0.013-0.023); standardized-root-mean-square-residual = 0.044), and all dimensions were reliable (α > 0.80). A specific combination of cutoffs correctly predicted 87.4% of BDI-scores possibly indicative of moderate-to-severe depression (χ2(1) = 220.608, p < 0.001, Nagelkerke R2 = 0.462, J = 66.4). The Hungarian version of the SCREENIVF is a valid and reliable tool, with high accuracy in predicting BDI-scores. Low response rate may affect generalizability. The same instrument with different cutoffs can serve various clinical goals.

UNLABELLED: Obesity is one of the major health problems at present and it will also be in the future. AIM: To assess the proportion of obesity-related diseases in the Hungarian population above 18 years of age. METHOD: The survey was conducted via internet. 1,000 subjects inform various subregions of Hungary were asked to fill in the questionnaire. The obtained data were grouped according to gender and age in overweight, lean and normal weight subjects. Additional diseases were assessed using odds ratios with 95% probability. Data from overweight subjects were compared to those obtained from subjects with a body mass index lower than 25 kg/m2. RESULTS: 27,746 filled in questionnaires from 2,597 subregions were analysed. 46.3% of the responders were males and 53.7% were females. The disease rate among females was higher compared to males. The occurrence of diseases increased with age. In overweight subjects the odds ratios for several diseases were substantially increased (diabetes, 3.9; hypertension, 2.9; cardiovascular diseases, 1.69; arthralgia, 1.68; and cervical, dorsal and lumbar pain, 1.35). CONCLUSION: Obesity related additional diseases merit a high concern nowadays. Taking into consideration the tendency of obesity in children, there is a need to put higher emphasis to primary prevention.

UNLABELLED: The social and economic burden of falls among elderly is quite high in our time. Due to demographic changes the problem still persists. Across Europe great emphasis is laid on injury prevention among elderly. The authors accomplished a survey regarding the frequencies and causes of falls among the citizens of nursing homes within the frame of a EU program. AIM: The aim of the survey was not only to register the frequency of falls, but to reveal their background, regarding especially medicine consumption. METHODS: A year-long survey was accomplished among 1016 inhabitants of two nursing homes with a standard questionnaire. During this period, the nursing personnel recorded every fall, their circumstances and consequences. Among the causes, beside environmental factors, medicine use is a high risk factor. The latter was evaluated according to effects and side-effects related to falls, based on their trademark labels. Statistical program SPSS 14.0 was used. RESULTS: Among the inhabitants of the two nursing homes 1013 falls happened during the 12 months. Two third of these required some degree of medical care afterwards. The most frequent cases were contusions, bruises or skin injuries (20-24%). Fractures happened in 4.8%, 3% of them at femoral neck, 1.8% at other bones. Almost every inhabitant uses medicine, 19% of them 3 or less sorts, the majority many times more. The maximal kind of medicine taken by one person was 19, the average is 6 types per person. The maximal number of side-effects related to falls per person was 43, the average was 14. The aggregation of side-effects is presented in detail. CONCLUSIONS: The number and proportion of falls among elderly living in nursing homes is remarkable. One probable cause of this should be the elevated medicine consumption, with numerous side-effects which raise the risk of falls.

The 3rd Hungarian National Nutritional Survey was carried out as a part of the National Population Health Study and the data collection was done in November/December of 2003. The survey comprised a sample of adults above 19 year of age, the dietary questionnaires of 1179 people could be evaluated. The results of energy and macro-nutrient intake were published in 2005, and the results of the mineral intake in 2007, in the Hungarian Medical Journal. This paper focuses on the intake of vitamins. For the evaluation of the results authors used the data of the two previous national surveys, data of some dietary studies of other countries, and the home and international intake recommendations. In respect of the data found in the present survey, it is a favourable trend that the intake of vitamins retinol equivalent, B1 , B6 and B12 , and niacin, as well as biotin was sufficient both in case of men and women, together with adequate vitamin E intake for men. However the intake of vitamin B2 , C, D, together with pantothenic acid and folic acid of both men and women did not meet the criteria of the Hungarian recommendations, besides the vitamin E intake of women was insufficient as well. In Hungary the intake of vitamin D, biotin, folic acid and pantothenic acid was measured at first at this National Dietary Survey. The authors vigorously stress the importance of the varied and healthy nutrition in the adequate vitamin supply of the population.

In the course of the 3rd national nutritional survey data were collected in 2003, and the paper on the energy and nutrition intake of a sample group consisting of 1179 people all aged above 19, was published in the Hungarian Medical Journal (Vol. 146, No. 34, 2005). The present paper focuses on one group of micronutrients, namely the minerals' intake. The authors evaluated the results mainly in comparison with the data of the two previous Hungarian national surveys, and the international and national recommendations. The average magnesium intake was considered adequate in both sexes, while iron, zinc and copper intakes also met the recommended level in men. On the other hand, the insufficient calcium and the excessive phosphorus intakes, as well as the disproportionate calcium-phosphorus ratio in both sexes are unfavourable facts. The adverse results include insufficient iron, zinc and copper intake in women and the excessive sodium intake in both sexes, as well. The authors emphasize the importance of spreading the principles of healthy nutrition as well as co-operating with the food industry in order to prevent the occurrence of mineral nutrients deficiency.

The third Hungarian national dietary survey was conducted in 2003-2004. This publication describes the first part of the energy and nutrient intake findings in a sample consisting of a population of 1179 persons over 19 years of age (energy and macro nutrients). Energy and nutrient intake values were calculated based on 3 x 24- hour dietary records filled out by the subjects themselves. The authors evaluated the results in light of the two previous dietary surveys in Hungary and the Hungarian and international reference intake data. The total fat intake found in this survey lower than the previous data marks a favourable development, just as the higher unsaturated fatty acid and lower saturated fatty acid energy percent, and furthermore the lower cholesterol intake level. The favourable developments include further a polyunsaturated/saturated fatty acid ratio considerably higher than that obtained earlier (the values conforming to the international reference data), a higher carbohydrate energy percentage, as well as lower added sugar energy percentage, this latter being in the recommended range. The overweight ratio in men was 58.9%, while the female value was 49.5%, not much different from the previous survey data.
