RESUMEN

To evaluate the influence of the loss of coronal and radicular tooth structure on the biomechanical behavior and fatigue life of an endodontically treated maxillary premolar with confluent root canals using finite element analysis (FEA). An extracted maxillary second premolar was scanned to produce intact (IT) 3D model. Models were designed with an occlusal conservative access cavity (CAC) with different coronal defects; mesial defect (MO CAC), occlusal, mesial and distal defect (MOD CAC), and 2 different root canal preparations (30/.04, and 40/.04) producing 6 experimental models. FEA was used to study each model. A simulation of cycling loading of 50N was applied occlusally to stimulate the normal masticatory force. Number of cycles till failure (NCF) was used to compare strength of different models and stress distribution patterns via von Mises (vM) and maximum principal stress (MPS). The IT model survived 1.5 × 1010 cycles before failure, the CAC-30.04 had the longest survival of 1.59 × 109, while the MOD CAC-40.04 had the shortest survival of 8.35 × 107 cycles till failure. vM stress analysis showed that stress magnitudes were impacted by the progressive loss of coronal tooth structure rather than the radicular structure. MPS analysis showed that significant loss of coronal tooth structure translates into more tensile stresses. Given the limited size of maxillary premolars, marginal ridges have a critical role in the biomechanical behavior of the tooth. Access cavity preparation has a much bigger impact than radicular preparation on their strength and life span.

INTRODUCTION: This study investigated the significance of pericervical dentin after coronal canal flaring on the biomechanical behavior and life span of a maxillary molar using finite element analysis (FEA). METHODS: In addition to the intact tooth (IT) model, 4 experimental FE models were designed: conservative access cavity model (CON), and 3 models with different radicular preparations for the coronal 4 mL considering 3 instruments: ProTaper SX model (SX), OneFlare model (OF), and Gates-Glidden model (GG). Cyclic loading of 50 N was applied on the occlusal surface and number of cycles until failure (NCF) was compared with the IT model. Mathematical analysis was done to evaluate the stress distribution patterns and calculated maximum von Mises (VM) and maximum principal stresses. RESULTS: Access cavity preparation (CON) decreased NCF significantly when compared with the IT model (93.99%). The coronal preparation of the root canal did not have a significant effect even when the preparation was taken to the extreme (GG: 92.02%). VM analysis confirmed apical dispersion of stresses, with maximum value registered on the occlusal surface in the GG model (7.88 MPa), and minimum on the IT model (7.01 MPa). The furcation area showed higher maximum principal stresses, yet stress values remained minimal and distributed over larger surfaces with the progressive enlargement among models. CONCLUSIONS: Within the limitations of this study, coronal canal flaring affects tooth integrity minimally, and when loading conditions lie within normal functional ranges, tooth structure has the capacity to disperse increasing stresses over a wider surface area.

Objective: This study was done to evaluate the antibacterial effect of chitosan nano-particles (CNPs) root canal irrigant as a new alternative to Sodium hypochlorite (NaOCl) for disinfection of root canals inoculated with Enterococcus faecalis, with and without laser activation.Methodology: Sixty single rooted human premolars were decoronated, prepared and had their apical foramina sealed. E. faecalis were incubated in the root canals for 15 days. The teeth were then randomly divided into two experimental groups (n = 30) according to the disinfection protocol used. In Group I: disinfection was performed using the irrigant solutions only (Saline, 2.5% NaOCl, CNPs). Whereas in Group II, disinfection was done using the same irrigants followed by Diode laser at (980-nm) at 2 W output for 5 × 5 s. Intra-canal bacterial samples were taken before and after canal disinfection to determine the CFU count.Results: In group I, 2.5% NaOCl was as effective as CNP in eradication and significantly more effective than Saline (p = 0.008) in eradication of E. faecalis. In Group II, either 2.5% NaOCl or CNP in combination with diode laser irradiation showed a similarly high effect in bacterial eradication.Conclusions: Within the parameters used in this study, a combination therapy consisting of irrigation followed by diode laser irradiation should be utilized as an effective treatment modality for eliminating E. faecalis from root canal systems.

In the present study, GTX and ProTaper as continuous rotating endodontic files were numerically compared with WaveOne reciprocating file using finite element analysis, aiming at having a low cost, accurate/trustworthy comparison as well as finding out the effect of instrument design and manufacturing material on its lifespan. Two 3D finite element models were especially prepared for this comparison. Commercial engineering CAD/CAM package was used to model full detailed flute geometries of the instruments. Multi-linear materials were defined in analysis by using real strain-stress data of NiTi and M-Wire. Non-linear static analysis was performed to simulate the instrument inside root canal at a 45° angle in the apical portion and subjected to 0.3 N.cm torsion. The three simulations in this study showed that M-Wire is slightly more resistant to failure than conventional NiTi. On the other hand, both materials are fairly similar in case of severe locking conditions. For the same instrument geometry, M-Wire instruments may have longer lifespan than the conventional NiTi ones. In case of severe locking conditions both materials will fail similarly. Larger cross sectional area (function of instrument taper) resisted better to failure than the smaller ones, while the cross sectional shape and its cutting angles could affect instrument cutting efficiency.

Resumo As limas rotativas GTX e ProTaper foram comparadas numericamente com as limas reciprocatórias WaveOne pela análise de elementos finitos, com o objetivo de baixar custos, comparação fiel e exata, além de pesquisar o efeito do projeto das limas e de seu material sobre a durabilidade. Dois modelos tridimensionais de análise de elementos finitos foram especialmente elaborados para esta comparação. Utilizou-se um sistema de engenharia CAD/CAM comercial para construir o modelo totalmente detalhado da geometria das estrias das limas. Materiais multi-lineares foram definidos na análise usando dados reais de deformação por estresse de NiTi e M-Wire. Procedeu-se à análise estática não linear para simular a ação do instrumento dentro do canal radicular em ângulo de 45° na região apical, submetido a uma torsão de 0.3 N.cm. As três simulações realizadas no presente estudo demonstraram que o M-Wire é um pouco mais resistente a falhas que o NiTi convencional. Por outro lado, ambos os materiais são bastante similares em condições de travamento severo. No caso da geometria das limas, os instrumentos de M-Wire podem ter vida útil mais longa que os de NiTi convencional. Em condições de travamento severo, ambos os materiais terão falhas similares. Maior área transversal (em função da conicidade do instrumento) resiste melhor a falhas que as áreas transversais menores, ao passo que a geometria da área transversal e seus ângulos de corte podem afetar a eficiência de corte das limas.

INTRODUCTION: The aim of this study was to compare by scanning electron microscopy the presence of smear layer and debris on root canal walls after preparation with the single-file system WaveOne (Dentsply Maillefer, Ballaigues, Switzerland) versus the rotary ProTaper system (Dentsply Maillefer, Ballaigues, Switzerland) under 2 final irrigant regimens. METHODS: Forty freshly extracted single-rooted human teeth were randomly divided into 4 groups (n = 10). The ProTaper and ProTaper and rotary CanalBrush (Coltène Whaledent GmbH+ Co KG, Langenau, Germany) groups were instrumented with the ProTaper system. Groups WaveOne and WaveOne and rotary CanalBrush were instrumented with the WaveOne system. The irrigant in all groups was 2 mL 5.25% sodium hypochlorite (NaOCl) solution, whereas the final irrigation after preparation in the ProTaper and WaveOne groups was 1 mL Smear Clear solution (Sybron Endo, Orange, CA) and then 5.25% NaOCl applied with a plastic syringe, and in the ProTaper and rotary CanalBrush and WaveOne and rotary CanalBrush groups, it was 1 mL Smear Clear solution and then 5.25% NaOCl (rotary CanalBrush agitation). Roots were processed for scanning electron microscopic examination for debris and smear layer scoring. Data were statistically analyzed. RESULTS: All groups showed more efficient smear layer and debris removal coronally than in the middle and apical regions, whereas the mean total debris score and the mean smear layer score in all groups were less in the WaveOne and rotary CanalBrush groups than the ProTaper and rotary CanalBrush and the WaveOne and ProTaper groups. CONCLUSIONS: Using the rotary CanalBrush in canals prepared with WaveOne produced the cleanest canal walls, and the WaveOne system gave superior results compared with the ProTaper system.
