RESUMEN

There was no significant difference in the level of basic immunological indices between those female patients having received a breast prothesis with making use of the polyacrilamide gel (PAAG) interfall and controls, subjects who had not received a breast prothesis, and group of patients in whom the gel was administered by injection, which fact corraborates the conclusion about immunological tolerance of the gel, with no dependence on the way in which it is used being noted.

RESUMEN

Injection implantation of polyacrylamide gel "Interfall" in soft tissues was conducted in 110 patients in 1992-1997 yrs. In 17 patients the mammary glands prosthesis was conducted using biogel. Biogel did not influence the cell subpopulations of the immunity T- and B-systems, the immunoglobulins level and did not cause the organism sensibilization.

RESUMEN

The study was aimed at the evaluation of functional condition of pancreatic islet apparatus in patients with Cushing's disease who underwent adrenal microsurgical transplantation after bilateral adrenalectomy. The examination of 19 patients included immunological tests as well as calculation of insulin level increase, insulin ejection rate and insulin level excess over the baseline after glucose loading. The results suggest that patients with Cushing's disease in active stage reveal increased sensitivity to glucose and functional capacity of pancreatic beta-cells. After unilateral adrenalectomy insulin base level was observed to fall while increased beta-cells responsiveness to glucose remained unchanged. At the same time adrenal autotransplantation on the vascular stem in patients with the Cushing's disease resulted not only in normalization of insulin base level but also in the improvement of the functional indices of the islet cells.

RESUMEN

The paper is devoted to surgical treatment of patients with Itsenko-Cushing disease. Altogether 16 patients after adrenalectomy with subsequent autotransplantation of the gland on a vascular pedicle were investigated. The purpose was to study a morphological picture of the gland in ischemia and the determination of time required for blood flow recovery in the gland. Light and electron microscopy, radiological and clinical methods were employed. Emerging of the removed gland before autotransplantation into a nutrient medium for 30 min. (before blood flow recovery in a graft) was shown to damage neither secretory cells nor the components of the microcirculatory bed. The extracorporal condition of the gland for 1 h was accompanied by an increase in autolytic processes, especially in the medullary substance and deep layers of reticulated cortical zone, but most of the adrenocorticocytes preserved their viability and structural features of steroid activity, which became an indication for effective autotransplantation of the adrenal gland on a vascular pedicle in the treatment of Itsenko-Cushing disease.

RESUMEN

The authors employed for the first time the method of heterotopic autotransplantation of the adrenals on a vascular pedicle in patients with Itsenko-Cushing disease. It was established that heterotopic transplantation of the adrenals results early in a significant reduction of the level of cortical hormones and insignificant change as compared with initial state of the immune status. All terms were characterized by humoral and cellular immune deficit. Results indicate the expediency of using this method. Adrenal transplantation prevents the development of hypocorticism.
