A preliminary study was carried out on the insect fauna of Al-Baha Province, south-western part of Saudi Arabia. A total number of 582 species and subspecies (few identified only to the genus level) belonging to 129 families and representing 17 orders were recorded. Two of these species are described as new, namely: Monomorium sarawatensis Sharaf & Aldawood, sp. n. [Formicidae, Hymenoptera] and Anthrax alruqibi El-Hawagry sp. n. [Bombyliidae, Diptera]. Another eight species are recorded for the first time in Saudi Arabia, namely: Xiphoceriana arabica (Uvarov, 1922) [Pamphagidae, Orthoptera], Pyrgomorpha conica (Olivier, 1791) [Pyrgomorphidae, Orthoptera], Catopsilia florella (Fabricius, 1775) [Pieridae, Lepidoptera], Anthrax chionanthrax (Bezzi, 1926) [Bombyliidae, Diptera], Spogostylum near tripunctatum Pallas in Wiedemann, 1818 [Bombyliidae, Diptera], Cononedys dichromatopa (Bezzi, 1925) [Bombyliidae, Diptera], Mydas sp. [Mydidae, Diptera], and Hippobosca equina Linnaeus, 1758 [Hippoboscidae, Diptera]. Al-Baha Province is divided by huge and steep Rocky Mountains into two main sectors, a lowland coastal plain at the west, known as "Tihama", and a mountainous area with an elevation of 1500 to 2450 m above sea level at the east, known as "Al-Sarat or Al-Sarah" which form a part of Al-Sarawat Mountains range. Insect species richness in the two sectors (Tihama and Al-Sarah) was compared, and the results showed that each of the two sectors of Al-Baha Province has a unique insect community. The study generally concluded that the insect faunal composition in Al-Baha Province has an Afrotropical flavor, with the Afrotropical elements predominant, and a closer affiliation to the Afrotropical region than to the Palearctic region or the Eremic zone. Consequently, we tend to agree with those biogeographers who consider that parts of the Arabian Peninsula, including Al-Baha Province, should be included in the Afrotropical region rather than in the Palaearctic region or the Eremic zone.

Middle lobe torsion is a known complication of right upper or lower lobectomy, whereas torsion of the upper or lower lobe is extremely rare. We report our experience with torsion of the right upper lobe following a middle lobectomy in a 74-year-old man with non-small-cell lung cancer.

The use of video-assisted thoracoscopic surgery (VATS) in dealing with thoracic trauma has been well established. VATS avoids a thoracotomy and offers excellent visualization of the entire pleural cavity. The removal of bullets from the pleural cavity using VATS has also been reported, but, to our knowledge, this is the first time a bullet has been removed from the pericardial cavity using VATS.

Intrathoracic epithelioid mesothelioma commonly presents as a diffuse tumor of the pleura. It rarely occurs as a localized tumor that might be surgically resectable. We report a case of one such localized mesothelioma in a 72-year-old woman, which arose from the wall of the esophagus and had all the presenting features of an esophageal tumor.

Chivosazoles A and F, isolated from Sorangium cellulosum, showed high antiproliferative activity with different mammalian cell lines including human cancer cells. The chivosazoles caused a delay in G2/M phase of the cell cycle, and treated cells often contained two nuclei. By labeling F-actin it was shown that the actin cytoskeleton of the cells starts to break down after a few minutes of treatment. In vitro polymerization assays with purified G-actin revealed that the chivosazoles inhibit actin polymerization and also cause a depolymerization of pyrene-labeled F-actin microfilaments prepared in vitro. Chivosazoles are new tools for the investigation of issues concerning the actin cytoskeleton and they have a different mode of action from the known microfilament-disrupting compounds like rhizopodin and cytochalasin D.

Peripheral arterial injuries after blunt or penetrating trauma commonly follow injuries to adjacent soft tissue and bone. The traditional approach to these injuries is by open exploration, with identification and ligation of the bleeding vessel. We describe the case of a type II respiratory failure patient who had an enormous pectoral muscle haematoma following chest drain insertion, in whom the bleeding was only controlled by angiographic embolisation following failure of surgical exploration.

OBJECTIVE: Neurogenic tumours of the mediastinum are uncommon neoplasms arising from nerve tissues within the thorax. We sought to evaluate and compare the outcome following surgical resection of such tumours by VATS, open thoracotomy, and by either combined with hemilaminectomy. METHODS: From February 1992 to March 2007, 93 patients underwent surgical resection of neurogenic tumours of the mediastinum in our institution. A videothoracoscopic approach was used in 57 cases (61.3%) (group V), of which 44 underwent VATS only and 13 required conversion to open approach. In the remaining 36 cases, 32 patients underwent thoracotomy (group T) and 4 had combined procedure with the neurosurgeons (group N). RESULTS: No postoperative mortality was reported. Postoperative morbidity rate was 23.6% (22/93; 14 of group T, 4 of group N, and 4 of group V; p<0.0001). Histology showed benign neurogenic tumours in all patients. Statistical analysis showed differences between the two groups (group V and T respectively) in mean operative time (111.3+/-58.2 min vs 149.06+/-77.05 min; p: 0.01), median postoperative stay (4 days vs 6 days p: 0.0009) and median postoperative pain on day 1, day 7 and 1 month after surgery (respectively p<0.0001, p<0.0001 and p: 0.001). At a mean follow-up of 73 months no patients showed recurrence of the tumour. CONCLUSIONS: VATS represents the gold standard for the treatment of benign neurogenic tumours of the mediastinum with better results in terms of morbidity, operative time, postoperative stay and postoperative pain compared to open approach. Dumbbell tumours require a combined thoracic and neurosurgical approach.

Post-traumatic lung herniation through a defect in the chest wall is an uncommon injury, with only about 300 reported in the literature. Various methods of treatment and repair have been described, including both purely thoracoscopic to full open techniques. We repaired a case by using a combination of minithoracotomy and video-assistance through the minithoracotomy wound. The patient did well and there was minimal postoperative pain.

We report the case of a 75-year-old male of Asian origin who developed TB constrictive pericarditis less than 6 weeks after CABG. He had no history of active TB, but did show a strong Heaf test reaction in 1987, following a period of weight loss. This was thought to be indicative of previous infection and was not actively treated. Cases of postcardiac surgery constrictive pericarditis have been reported in the literature, but to our knowledge, there is no report of development of TB constrictive pericarditis in such a short period following surgery.
