RESUMEN

The purpose of this study was to examine the effects of vertical trunk supportability improvement on activities. The study participants were 36 people. Trunk function and physical performance were evaluated using the following tests: trunk righting test (TRT), maximal isometric knee extensor strength test, side hop test, triple hop distance test, stabilometry of one-leg standing, and one-leg rebound jump. The measurement was performed three times: pre-trunk training (pre), post-trunk training for 1 month (post), and 2 months after the second measurement (detraining). Details of trunk training: Two sets of 30-s maximal lateral reach exercises on each side, following the four sets of 15-s maximal raising trunk exercises on each side. The results with TRT in post-training were significantly larger than those in pre-and detraining. Similarly, the results with one-leg rebound jump efficiency in post-training were significantly larger than those in pre-and detraining. Our findings indicate that jump efficiency changes in proportion to the change in vertical trunk supportability.

BACKGROUND: This study aimed to compare the bone resection thickness, rate of minimal thickness insert usage, and postoperative range of motion (ROM) between robotic and navigated total knee arthroplasty (TKA). METHODS: Fifty consecutive posterior-stabilised (PS) robotic TKA and 50 case-controlled PS navigated TKA were enroled. Bone resection thickness, rate of minimal thickness insert usage, and postoperative flexion contracture over 5° were statistically compared. RESULTS: The tibial bone resection thickness was significantly thinner in robotic TKA. The insert thickness was significantly lower, and the rate of minimal thickness insert usage was significantly higher in robotic TKA. The ROM at 3 months postoperatively and the rate of postoperative flexion contracture were comparable between the two groups. CONCLUSION: Robotic TKA can reduce the tibial bone resection thickness and increase the rate of minimal thickness insert usage without increasing postoperative flexion contracture as compared with navigated TKA.

BACKGROUND: This study compared the early clinical recovery of total hip arthroplasty (THA) using computer navigation systems (nTHA) and robotic arm-assisted THA (rTHA). METHODS: Thirty prospective subjects who underwent rTHA were clinically compared to 30 subjects who underwent nTHA. Clinical data (surgical time, intraoperative blood loss, pain severity, number of days to independent walking, and Harris Hip Score (HHS) at discharge), and radiographic parameters (inclination and anteversion angles) were statistically compared between the two groups. RESULTS: Follow-up times were 24.3 ± 6.0 and 27.0 ± 7.0 days in the rTHA and nTHA groups, respectively. The surgical time (135.1 ± 13.9 min vs. 146.2 ± 12.8 min, p = 0.002), number of days to independent walking (7.2 ± 2.0 vs. 11.5 ± 3.0 days, p < 0.001), and postoperative pain using a numeric rating scale on postoperative days 7, 10,, and 14 (1.4 ± 0.9 vs. 2.2 ± 1.2, p = 0.005; 1.0 ± 0.8 vs. 1.8 ± 1.1, p = 0.002; 0.3 ± 0.5 vs. 1.1 ± 0.9, p < 0.001; respectively) were significantly reduced in the rTHA group compared to the nTHA group. The rTHA group showed a significantly higher postoperative HHS compared to the nTHA group (85.3 ± .3.2 vs. 81.0 ± 8.5, p = 0.014). No statistically significant difference was observed in radiographic parameters between the groups; however, the incidence of intraoperative target angle changes was significantly lower in the rTHA group than in the nTHA group (0/30 subjects [0%] vs. 11/30 subjects [36.7%], p < 0.001). CONCLUSION: The surgical time, postoperative pain, and number of days to independent walking were significantly shorter, and the HHS at discharge was significantly higher in the rTHA group than in the nTHA group. Thus, compared to the nTHA group, the rTHA group showed improved early clinical recovery.

Traditional Anterior Cruciate Ligament (ACL) reconstruction is commonly performed using an allograft or autograft and possesses limitations such as donor site morbidity, decreased range of motion, and potential infection. However, a biodegradable synthetic graft could greatly assist in the prevention of such restrictions after ACL reconstruction. In this study, artificial grafts were generated using "wet" and "dry" electrospinning processes with a biodegradable elastomer, poly (ester urethane) urea (PEUU), and were evaluated in vitro and in vivo in a rat model. Four groups were established: (1) Wet PEUU artificial ligament, (2) Dry PEUU artificial ligament, (3) Dry polycaprolactone artificial ligament (PCL), and (4) autologous flexor digitorum longus tendon graft. Eight weeks after surgery, the in vivo tensile strength of wet PEUU ligaments had significantly increased compared to the other synthetic ligaments. These results aligned with increased infiltration of host cells and decreased inflammation within the wet PEUU grafts. In contrast, very little cellular infiltration was observed in PCL and dry PEUU grafts. Micro-computed tomography analysis performed at 4 and 8 weeks postoperatively revealed significantly smaller bone tunnels in the tendon autograft and wet PEUU groups. The Wet PEUU grafts served as an adequate functioning material and allowed for the creation of tissues that closely resembled the ACL.

PURPOSE: This study aimed to compare the intraoperative kinematics, especially for mid-flexion femorotibial anteroposterior (AP) stability, between newly developed medial congruent (MC) inserts and cruciate-retaining (CR) inserts in navigated cruciate-retaining total knee arthroplasty (CR-TKA). METHODS: Thirty consecutive patients with varus osteoarthritis undergoing CR-TKA using an image-free navigation system were enrolled. AP kinematics, the AP translation under manual maximum stress to the knee joint at 45° flexion, rotational kinematics, and varus-valgus laxity were evaluated using a navigation system and statistically compared between the MC and CR inserts. RESULTS: AP kinematic analysis showed that the femoral position with the CR insert was significantly anterior at a maximum extension to 45° flexion compared with the MC insert (p < 0.05). The amount of AP translation at 45° flexion with the MC insert was significantly smaller than that with the CR insert (p < 0.05). Rotational kinematics found that the tibial position at maximum extension was significantly externally rotated with the MC inserts than with the CR inserts. Varus-valgus laxity was comparable between the MC and CR inserts. CONCLUSION: The current results showed that greater mid-flexion AP stability was achieved with the MC inserts than with the CR inserts in CR-TKA. Intraoperative kinematics with the MC inserts more closely resembled those with preoperative conditions in CR-TKA. LEVEL OF EVIDENCE: III, prospective comparative study.

BACKGROUND: The pivot-shift test has become more consistent and reliable and is a meaningful outcome measurement after anterior cruciate ligament reconstruction (ACLR). PURPOSE/HYPOTHESIS: The purpose of this investigation was to assess patient-reported outcomes (PROs) and the quantitative pivot shift (QPS) preoperatively, at time zero immediately after anatomic ACLR, and after 24 months as well as the relationship between PROs and the QPS. It was hypothesized that anatomic ACLR would restore rotatory stability measured by the pivot-shift test and that QPS measurements would be positively correlated with PROs. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: The ACL-injured and contralateral uninjured knees from 89 of 107 (83.2%) enrolled patients at 4 international centers were evaluated using a standardized pivot-shift test. Tibial acceleration was assessed with an inertial sensor, and lateral compartment translation was measured using an image analysis system preoperatively, at time zero immediately postoperatively, and at follow-up after 2 years. PROs were assessed at 12 and 24 months postoperatively with the International Knee Documentation Committee (IKDC) subjective knee form, Cincinnati Knee Rating System (CKRS), Marx activity rating scale, and activity of daily living score (ADLS). RESULTS: The mean patient age at surgery was 27 years (range, 15-45 years). A positive pivot shift preoperatively (side-to-side difference in tibial acceleration, 2.6 ± 4.0 m/s2; side-to-side difference in anterior tibial translation, 2.0 ± 2.0 mm) was reduced at time zero postoperatively (side-to-side difference in tibial acceleration, -0.5 ± 1.3 m/s2; side-to-side difference in anterior tibial translation, -0.1 ± 1.0 mm). All PROs improved from preoperatively to final follow-up at 24 months: from 56.5 to 85.5 points for the IKDC (P = .0001), from 28.8 to 32.4 points for the CKRS (P = .04), from 11.2 to 7.9 points for the Marx (P < .0001), and from 75.7 to 91.6 points for the ADLS (P < .0001). Neither preoperative nor time zero postoperative rotatory laxity assessed by the pivot-shift test correlated with PROs at 24-month follow-up. A graft retear was observed in 4 patients (4.5%) within 2 years of follow-up. CONCLUSION: Anatomic ACLR resulted in significantly improved and acceptable PROs at 2-year follow-up and a low failure rate. Anatomic ACLR restored QPS measurements of anterior tibial translation and tibial acceleration to those of the contralateral knee immediately after surgery while still under anesthesia, but there was no correlation between the QPS preoperatively or at time zero after ACLR and PROs at 2-year follow-up.

PURPOSE: To investigate the correlation between intraoperative tibiofemoral anteroposterior changes at 90° of flexion and postoperative maximum flexion angles in navigated cruciate-substituting TKA. The hypothesis of this study was that intraoperative tibiofemoral anteroposterior changes at 90° of flexion indirectly reflect posterior cruciate ligament (PCL) function and associate with postoperative maximum flexion angles. METHODS: Fifty-five consecutive patients with varus osteoarthritis treated with primary TKA were retrospectively analysed. All patients received the same type of implant, placed with an image-free navigation system. The PCL was retained, and cruciate-substituting inserts were used in all cases. The mean follow-up was 44 ± 8 months. The preoperative and postoperative kinematics were measured intraoperatively with a navigation system, and the preoperative and postoperative tibiofemoral anteroposterior positions at 90° of flexion were determined. The correlation between intraoperative anteroposterior position changes and postoperative maximum flexion angles was investigated. The correlation between the change of anteroposterior position and tibiofemoral rotational angles was also assessed. RESULTS: The intraoperative anteroposterior position change was -1.7 ± 3.4 mm (a positive value indicates tibial posterior shift). Flexion angle improvement was negatively correlated with intraoperative change of tibiofemoral anteroposterior position (R2 = 0.17, p < 0.005). Postoperative maximum flexion angles were also negatively correlated with intraoperative change of tibiofemoral anteroposterior position (R2 = 0.09, p < 0.05). The postoperative amount of tibial internal rotation was positively correlated with the preoperative amount (R2 = 0.60, p < 0.0001); however, the intraoperative anteroposterior position change was not correlated with the postoperative amount of tibial internal rotation (n.s.). CONCLUSION: A navigation system may be able to indirectly evaluate PCL function and predict the postoperative flexion angles in cruciate-substituting TKA. Intraoperative posterior movement of the tibia at 90° of flexion predicts worse postoperative flexion angles in cruciate-substituting TKA. Aging-related loss of adult stem cell function contributes to impaired tissue regeneration. Mice deficient in zinc metalloproteinase STE24 (Zmpste24 -/-) exhibit premature age-related musculoskeletal pathologies similar to those observed in children with Hutchinson-Gilford progeria syndrome (HGPS). We have reported that muscle-derived stem/progenitor cells (MDSPCs) isolated from Zmpste24 -/- mice are defective in their proliferation and differentiation capabilities in culture and during tissue regeneration. The mechanistic target of rapamycin complex 1 (mTORC1) regulates cell growth, and inhibition of the mTORC1 pathway extends the lifespan of several animal species. We therefore hypothesized that inhibition of mTORC1 signaling would rescue the differentiation defects observed in progeroid MDSPCs. MDSPCs were isolated from Zmpste24 -/- mice, and the effects of mTORC1 on MDSPC differentiation and function were examined. We found that mTORC1 signaling was increased in senescent Zmpste24 -/- MDSPCs, along with impaired chondrogenic, osteogenic, and myogenic differentiation capacity versus wild-type MDSPCs. Interestingly, we observed that mTORC1 inhibition with rapamycin improved myogenic and chondrogenic differentiation and reduced levels of apoptosis and senescence in Zmpste24 -/- MDSPCs. Our results demonstrate that age-related adult stem/progenitor cell dysfunction contributes to impaired regenerative capacities and that mTORC1 inhibition may represent a potential therapeutic strategy for improving differentiation capacities of senescent stem and muscle progenitor cells.

[Purpose] It is considered that evaluation of the vertical trunk function is important, because humans stand and move with two legs. To evaluate this, a novel method named Trunk Righting Test has been reported. The purpose of this study was to examine the trunk muscle activity during a TRT using electromyogram analysis. [Participants and Methods] This study included 7 healthy volunteer males. The TRT evaluated the supportability of the posture after moving 10 cm laterally from the sitting position using a hand-held dynamometer. The TRT measurements were analyzed separately at the measurement side (ipsilateral side) and at the non-measurement side (contralateral side). The measurements were obtained bilaterally, and the evaluated muscles included the rectus abdominis, internal oblique, external oblique, multifidus, and transversus abdominis. The measured value was expressed as a percentage after comparing with the value at the maximum voluntary contraction (% MVC) for standardization. The changes in the muscle activities in the sitting position and TRT were evaluated. [Results] All the muscle activities significantly increased during the TRT in contrast to that in the sitting posture. [Conclusion] The load support of the trunk on one side during the TRT was significant in all the muscles on both the sides, which increased the muscle activity, in contrast to that in the sitting position.

PURPOSE: The factors affecting anterior tibial subluxation (ATS) have not yet been well examined. To assess the factors affecting ATS in anterior cruciate ligament (ACL)-deficient knees. METHODS: One hundred twenty-four patients with unilateral ACL injuries were included. True lateral views of the ACL-deficient knee and contralateral normal knee were obtained during maximum extension using fluoroscopy under general anesthesia, and ATS was calculated as the side-to-side difference in the tibial position relative to the femur. Patients were divided into four groups according to the time from injury to surgery. To identify the factors affecting ATS, the following possible factors were assessed: (1) the time from injury to surgery, (2) presence of a medial meniscal injury, and (3) posterior tibial slope angle. RESULTS: There was a positive correlation between the ATS ratio and the time from injury to surgery ( r = 0.52). The ratio of the presence of a medial meniscus injury was significantly higher in patients who underwent anterior tibial subluxation reconstruction (ACL-R) more than 12 months after an injury than in other patients who underwent ACL-R within 12 months after an injury. The ATS ratio was significantly higher in patients with a medial meniscus injury than in those without a medial meniscus injury (5.6% vs. 4.1%). CONCLUSIONS: Our results suggested that ACL-R should be performed within 6 months after injury if surgeons prefer to avoid an increase in ATS in maximum knee extension and at the latest within 12 months to avoid medial meniscal injury at the time of ACL-R.

PURPOSE: The loss of nutrient supply is a suspected contributor of intervertebral disc degeneration. However, the extent to which low nutrition affects disc annulus fibrosus (AF) cells is unknown as nutrient deprivation has mainly been investigated in disc nucleus pulposus cells. Hence, an experimental study was designed to clarify the effects of limited nutrients on disc AF cell fate, including autophagy, the process by which cells recycle their own damaged components. METHODS: Rabbit disc AF cells were cultured in different media with varying serum concentrations under 5% oxygen. Cellular responses to changes in serum and nutrient concentrations were determined by measuring proliferation and metabolic activity. Autophagic flux in AF cells was longitudinally monitored using imaging cytometry and Western blotting for LC3, HMGB1, and p62/SQSTM1. Apoptosis (TUNEL staining and cleaved caspase-3 immunodetection) and cellular senescence (senescence-associated ß-galactosidase assay and p16/INK4A immunodetection) were measured. RESULTS: Markers of apoptosis and senescence increased, while cell proliferation and metabolic activity decreased under the withdrawal of serum and of nutrients other than oxygen, confirming cellular stress. Time-dependent increases in autophagy markers, including LC3 puncta number per cell, LC3-II expression, and cytoplasmic HMGB1, were observed under conditions of reduced nutrition, while an autophagy substrate, p62/SQSTM1, decreased over time. Collectively, these findings suggest increased autophagic flux in disc AF cells under serum and nutrient deprivation. CONCLUSION: Disc AF cells exhibit distinct responses to serum and nutrient deprivation. Cellular responses include cell death and quiescence in addition to reduced proliferation and metabolic activity, as well as activation of autophagy under conditions of nutritional stress. BACKGROUND: It is still uncertain how surgical reconstruction of the anterior cruciate ligament (ACL) is able to restore rotatory laxity of the involved joint. The desired amount of restraint applied by the ACL graft, as compared with the healthy knee, has not been fully clarified. PURPOSE: To quantify the ability of single-bundle anatomic ACL reconstruction using hamstring tendons in reducing the pivot-shift phenomenon immediately after surgery under anesthesia. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: An inertial sensor and image analysis were used at 4 international centers to measure tibial acceleration and lateral compartment translation of the knee, respectively. The standardized pivot-shift test was quantified in terms of the side-to-side difference in laxity both preoperatively and postoperatively with the patient under anesthesia. The reduction in both tibial acceleration and lateral compartment translation after surgery and the side-to-side difference were evaluated using the Wilcoxon signed-rank test. Alpha was set at P < .05. RESULTS: A total of 107 patients were recruited for the study, and data were available for 89 patients. There was a statistically significant reduction in quantitative rotatory knee laxity between preoperatively (inertial sensor, 2.55 ± 4.00 m/s2; image analysis, 2.04 ± 2.02 mm) and postoperatively (inertial sensor, -0.54 ± 1.25 m/s2; image analysis, -0.10 ± 1.04 mm) between the involved and healthy joints, as measured by the 2 devices (P < .001 for both). Postoperatively, both devices detected a lower rotatory laxity value in the involved joint compared with the healthy joint (inertial sensor, 2.45 ± 0.89 vs 2.99 ± 1.10 m/s2, respectively [P < .001]; image analysis, 0.99 ± 0.83 vs 1.09 ± 0.92 mm, respectively [P = .38]). CONCLUSION: The data from this study indicated a significant reduction in the pivot shift when compared side to side. Both the inertial sensor and image analysis used for the quantitative assessment of the pivot-shift test could successfully detect restoration of the pivot shift after anatomic single-bundle ACL reconstruction. Future research will examine how pivot-shift control is maintained over time and correlation of the pivot shift with return to full activity in patients with an ACL injury.

BACKGROUND: This study aimed to determine the appropriate administration duration of edoxaban 15 mg (a factor Xa inhibitor) for the prevention of deep vein thrombosis (DVT) after total knee arthroplasty (TKA). METHODS: Our study comprised 202 patients who underwent TKA (excluding bilateral TKA) at our institution between 2014 and 2015. The subjects received edoxaban 15 mg daily for 1 (n = 93) or 2 (n = 109) weeks; group assignment was random. B-mode ultrasonography was performed 7 and 14 days post-TKA for the detection of DVT. We compared the incidence of DVT between the groups and examined for side effects. RESULTS: The demographic data of the patients in the 1- and 2-week administration groups were similar at baseline. DVT incidence did not differ significantly between the groups at 1 week post-TKA. However, it was significantly lower in the 2-week administration group (n = 0) than in the 1-week administration group (n = 7; p = 0.004) at 2 weeks post-DVT. Neither group exhibited symptomatic DVT. A total of six patients withdrew during the study period because of hepatic dysfunction. CONCLUSIONS: Our results show that the administration of edoxaban 15 mg is more effective in preventing DVT after TKA when administered for 2 weeks than for 1 week.

Recent advances in surgical tools such as navigation systems have contributed to accurate implantation in total knee arthroplasty. Although several navigation systems have been developed, reports regarding which navigation system has better accuracy are few. Therefore, this study aimed to compare the accuracy of postoperative coronal alignment among 3 navigation systems. A total of 90 knee prostheses were implanted for 90 patients with osteoarthritis. Thirty patients were enrolled in each of the following 3 navigation groups: Stryker Navigation System II (computed tomography-free navigation; Stryker, Mahwah, New Jersey); OrthoPilot version 4.2 navigation system (computed tomography-free navigation; B. Braun Aesculap, Tuttlingen, Germany); and VectorVision navigation system (computed tomography-based navigation system; BrainLAB, Munich, Germany). Thirty consecutive total knee arthroplasties performed via the conventional method without navigation were selected as a control group for comparison with the navigation groups. Postoperative coronal mechanical axis and femoral and tibial coronal component angles were compared among the groups using long-leg standing radiographs for the rate of outliers beyond 3°. No differences were observed in the mean femoral and tibial component angles among the navigation and conventional groups. However, the proportion of outliers beyond 3° was higher in the conventional group than in the 3 navigation groups. No significant differences in the outlying values were found among the 3 navigation groups. These 3 navigation systems achieved equally accurate coronal mechanical alignment with fewer outliers. BACKGROUND: The importance of medial compartment stability is recognized in total knee arthroplasty (TKA). To manage the medial extension-flexion gap, the posterior reference position can be changed from conventional posterior center to posterior medial in measured resection techniques. This study aimed to compare the intraoperative soft tissue balance and rotational kinematics between the posterior medial and posterior center reference groups. METHODS: We enrolled 57 consecutive patients with varus osteoarthritis undergoing posterior-stabilized (PS) TKA using an image-free navigation system. The detailed surgical plan in both groups and intraoperative kinematics were recorded using navigation, and soft tissue balance measured with an offset-type tensor was statistically compared between groups. RESULTS: Patients were divided into the posterior center reference group (n = 32) and posterior medial reference group (n = 25). The posterior medial and posterior lateral condyles were significantly thicker in the posterior center reference group (P < .05). Although preoperative rotational kinematics were comparable between groups, the tibial rotational position was significantly more externally rotated in the posterior center reference group than in the posterior medial reference group at 45°, 60°, and 90° of flexion (P < .05). The varus angle and joint component gap were significantly smaller in the posterior medial reference group than in the posterior center reference group at 60° and 90° of flexion (P < .05). CONCLUSION: The posterior reference position affects intraoperative kinematics and soft tissue balance in navigated PS TKA. Posterior medial reference PS TKA decreases the excessive tibial external rotation during midflexion and increases the flexion stability compared with conventional posterior center reference PS TKA.

Soft tissue impingements are well-known complications of total knee arthroplasty. The impingements usually occur between the medial or lateral femoral component and tibial insert, and between the patella and femoral components. We report a rare case of impingement of the soft tissue between the femoral intercondylar fossa and post of the polyethylene insert, which caused pain and walking disability. After the surgery for the arthroscopic removal of the soft tissue, the symptoms disappeared. However, prosthetic loosening of the femur occurred several months after the arthroscopic surgery, requiring revision surgery. We would propose to call this symptom as post-cam clunk syndrome.

BACKGROUND: A fluoroscopic guidance method for medial patellofemoral ligament (MPFL) reconstruction has been widely used to determine the anatomic femoral attachment site. PURPOSE: To examine the graft length change patterns in MPFL reconstruction with a fluoroscopic guidance method. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: Forty-four knees of 42 patients who underwent MPFL reconstruction for the treatment of recurrent patellar dislocation were examined prospectively. During surgery, suture anchors were inserted into the proximal one-third and center of the patella. A guide pin for the femoral tunnel was inserted into the position reported by Schöttle et al based on the true lateral view of the knee under fluoroscopic control. Changes in graft length patterns of the proximal and center anchors were examined through 0° to 120° of knee flexion. Favorable changes in length patterns were defined as meeting 2 of 3 criteria: (1) not long during flexion (≤3 mm between 30° and 120° of flexion) and either (2) nearly isometric during flexion between 0° and 90° or (3) slightly long during maximum extension (≤3 mm). Other patterns were considered unfavorable. If the change in length pattern was unfavorable, then the pin for the femoral tunnel was moved to different positions until it was favorable. Knees were separated into the favorable group and the unfavorable group. Differences between the groups regarding radiographic parameters were assessed. Student t test or chi-square test was used for statistical analysis. RESULTS: Of the 44 knees, 31 (70.5%) showed favorable patterns. However, 13 knees (29.5%) showed unfavorable patterns; therefore, the position of the pin was changed. The mean ± SD distance from the original position to the final position was 5.3 ± 1.1 mm distal for 7 patients and 5.2 ± 0.4 mm posterodistal for 6 patients. Technical errors, including a nontrue lateral view and the tip of the wire not being in the determined area, were found for 4 of 13 knees in the unfavorable group. There was no statistical difference in radiographic parameters between the groups. CONCLUSION: The graft length change pattern could be nonphysiologic at the position determined through the fluoroscopic guidance method; thus, caution may be necessary. The change in length pattern should be checked before graft fixation. If the length change pattern is unfavorable, then it is advisable to move it approximately 5 to 7 mm distally or posterodistally from the first position.

PURPOSE: To investigate the tibiofemoral rotational profiles during navigated posterior-stabilized (PS) total knee arthroplasty (TKA) and investigate the effect on post-operative maximum flexion angles. METHODS: Twenty-five subjects, treated with navigated PS TKA, were enrolled, and the effect of posterior cruciate ligament (PCL) resection and component implantation on tibiofemoral rotational kinematics was statistically evaluated. Then, the effect of rotational alignment changes on the post-operative maximum angles was retrospectively examined in 96 subjects using the multiple regression analysis. RESULTS: Tibial internal rotation was significantly increased in full extension (p < 0.01 and <0.001, respectively) and at 60° and 90° flexion (p < 0.05) after PCL resection, which further increased after implantation, compared with that before resection. The amount of tibial internal rotation from 90° flexion to maximum flexion was significantly decreased after PCL resection and implantation, compared with that before resection (p < 0.05). The internal changes in the rotational alignment were independent factors for the minimal improvement in the post-operative maximum flexion angles (R 2 = 0.078, p = 0.0067). CONCLUSION: PCL resection changed the tibial rotational alignment and decreased the amount of tibial internal rotation. The implantation of PS components further increased the internal rotational alignment and could not compensate for the tibiofemoral rotation. Finally, the internal changes in rotational alignment affected the improvement of the maximum flexion angles, suggesting that rotational alignment is an important factor for improving post-operative maximum flexion angles. BACKGROUND: Deep vein thrombosis (DVT) is one of the main complications following total knee arthroplasty (TKA). In this study, oral administration of 15 mg edoxaban (a factor Xa inhibitor) once daily for 14 days efficiently prevented the incidence of DVT. Our hypothesis was that prothrombin time-international normalized ratio (PT-INR) on the third postoperative day could predict the incidence of DVT following TKA. METHODS: In this study, 286 subjects were enrolled and divided into two groups according to the presence or absence of DVT. Several variables [age, body mass index, postoperative D-dimer level, PT-INR, and functional recovery findings (standing)] were analysed to determine the predictors of DVT, and for DVT diagnosis, ultrasonography was performed for seven days after surgery. RESULTS: The PT-INR levels were significantly higher in the group that did not develop DVT (p = 0.01). Further analysis with logistic regression analysis and receiver operating characteristic curve was performed. The PT-INR on the third postoperative day was an independent factor of the incidence of DVT (odds ratio 0.210; p = 0.035). The cut-off PT-INR was calculated to be 1.425. CONCLUSION: PT-INR level is a useful marker in determining whether 15 mg edoxaban administration can prevent DVT after TKA. It is suggested that increment of edoxaban to control PT-INR over the cut-off point might prevent the incidence of DVT.

Endurance exercise generates CO2 via aerobic metabolism; however, its role remains unclear. Exogenous CO2 by transcutaneous delivery promotes muscle fibre-type switching to increase endurance power in skeletal muscles. Here we determined the performance of rats running in activity wheels with/without transcutaneous CO2 exposure to clarify its effect on endurance exercise and recovery from muscle fatigue. Rats were randomised to control, training and CO2 groups. Endurance exercise included activity-wheel running with/without transcutaneous CO2 delivery. Running performance was measured after exercise initiation. We also analysed changes in muscle weight and muscle fibres in the tibialis anterior muscle. Running performance improved over the treatment period in the CO2 group, with a concomitant switch in muscle fibres to slow-type. The mitochondrial DNA content and capillary density in the CO2 group increased. CO2 was beneficial for performance and muscle development during endurance exercise: it may enhance recovery from fatigue and support anabolic metabolism in skeletal muscles.
