A 30-year-old woman with a normal first trimester Down syndrome screening attended our ultrasound unit for a 20-week scan. The most remarkable anomalies were the presence of a right aortic arch along with a dilated cavum septi pellucidi. In addition, the scan showed an atrioventricular canal and bilateral choroid plexus cysts. Fetal karyotype showed the existence of trisomy 21. A novel association between Down syndrome and dilated cavum septi pellucidi is reported and the relationship between DS and vascular rings is discussed.
RESUMEN
A 20-week fetus was diagnosed with tetralogy of Fallot and multicystic kidneys. The postmortem study showed missing müllerian structures with small streak ovaries, external male genitalia, and an abnormal cloacal septation (imperforate anus with a sigmoid colon opening in the bladder). As the observed anomalies were related with septation, a mechanism related with the activation of specific growth factors, we discuss the possibility of a disorder in the function of the bone morphogenetic proteins as a common cause for the widespread anomalies found in this fetus.
RESUMEN
OBJETIVO: Presentamos un caso de un varón de 47 años con un tumor suprarrenal izquierdo heterogéneo de 16x10x7 cm., al que tras una punción-biopsia se le practicó una suprarrenalectomía por vía abdominal. MÉTODO/RESULTADOS: Estudios inmunohistoquímicos con el anticuerpo anti Melan-A A103 nos indican un origen en la corteza suprarrenal de las células tumorales. Los oncocitos son ricos en mitocondrias, y el anticuerpo antimitocondrial mES-13 nos permite ponerlas de manifiesto sin necesidad de recurrir a estudios ultraestructurales. CONCLUSIÓN: En este tipo de tumores sólo los criterios histológicos de necrosis y aumento de las mitosis permiten hacer un diagnóstico de malignidad, pero aún así su comportamiento es indolente (AU)
