RESUMEN

The management of periprosthetic fractures remains challenging and controversial. There continues to be a significant burden of disease and substantial resource implications associated with fractures following total joint arthroplasty. Achieving consensus opinions regarding the prevention and treatment of this problem has important implications given the profound effect on patient outcomes. Multidisciplinary care in the preoperative and postoperative settings is critical, with a specific focus on bone health.

RESUMEN

CASE: A 40-year-old man presented with limited in range of motion, pain, and tenderness over the medial joint line after an open reduction and internal fixation (ORIF) because of a bicondylar tibial plateau fracture (TPF). The cause of his pain was inconclusive on Magnetic Resonance Image (MRI), so arthroscopy was performed and identified an incarcerated medial meniscus at the fracture site. An osteotomy with medial joint elevation was performed followed by a meniscus release, with excellent results at the 1-year follow-up. CONCLUSION: To our knowledge, this is the first case reporting a trapped/incarcerated meniscus in a healed TPF after ORIF.

Surfing the links between Wikipedia articles constitutes a valuable way to acquire new knowledge related to a topic by exploring its connections to other pages. In this sense, Personalized PageRank is a well-known option to make sense of the graph of links between pages and identify the most relevant articles with respect to a given one; its performance, however, is hindered by pages with high indegree that function as hubs and obtain high scores regardless of the starting point. In this work, we present CycleRank, a novel algorithm based on cyclic paths aimed at finding the most relevant nodes related to a topic. To compare the results of CycleRank with those of Personalized PageRank and other algorithms derived from it, we perform three experiments based on different ground truths. We find that CycleRank aligns better with readers' behaviour as it ranks in higher positions the articles corresponding to links that receive more clicks; it tends to identify in higher position related articles highlighted by editors in 'See also' sections; and it is more robust to global hubs of the network having high indegree. Finally, we show that computing CycleRank is two orders of magnitude faster than computing the other baselines.

The emerging grassroots party Barcelona en Comú won the 2015 Barcelona City Council election. This candidacy was devised by activists involved in the Spanish 15M movement to transform citizen outrage into political change. On the one hand, the 15M movement was based on a decentralized structure. On the other hand, political science literature postulates that parties develop oligarchical leadership structures. This tension motivates to examine whether Barcelona en Comú preserved a decentralized structure or adopted a conventional centralized organization. In this study we develop a computational methodology to characterize the online network organization of every party in the election campaign on Twitter. Results on the network of retweets reveal that, while traditional parties are organized in a single cluster, for Barcelona en Comú two well-defined groups co-exist: a centralized cluster led by the candidate and party accounts, and a decentralized cluster with the movement activists. Furthermore, results on the network of replies also shows a dual structure: a cluster around the candidate receiving the largest attention from other parties, and another with the movement activists exhibiting a higher predisposition to dialogue with other parties.

Wikipedia is a huge global repository of human knowledge that can be leveraged to investigate interwinements between cultures. With this aim, we apply methods of Markov chains and Google matrix for the analysis of the hyperlink networks of 24 Wikipedia language editions, and rank all their articles by PageRank, 2DRank and CheiRank algorithms. Using automatic extraction of people names, we obtain the top 100 historical figures, for each edition and for each algorithm. We investigate their spatial, temporal, and gender distributions in dependence of their cultural origins. Our study demonstrates not only the existence of skewness with local figures, mainly recognized only in their own cultures, but also the existence of global historical figures appearing in a large number of editions. By determining the birth time and place of these persons, we perform an analysis of the evolution of such figures through 35 centuries of human history for each language, thus recovering interactions and entanglement of cultures over time. We also obtain the distributions of historical figures over world countries, highlighting geographical aspects of cross-cultural links. Considering historical figures who appear in multiple editions as interactions between cultures, we construct a network of cultures and identify the most influential cultures according to this network.

BACKGROUND: Despite the undisputed role of emotions in teamwork, not much is known about the make-up of emotions in online collaboration. Publicly available repositories of collaboration data, such as Wikipedia editor discussions, now enable the large-scale study of affect and dialogue in peer production. METHODS: We investigate the established Wikipedia community and focus on how emotion and dialogue differ depending on the status, gender, and the communication network of the [Formula: see text] editors who have written at least 100 comments on the English Wikipedia's article talk pages. Emotions are quantified using a word-based approach comparing the results of two predefined lexicon-based methods: LIWC and SentiStrength. PRINCIPAL FINDINGS: We find that administrators maintain a rather neutral, impersonal tone, while regular editors are more emotional and relationship-oriented, that is, they use language to form and maintain connections to other editors. A persistent gender difference is that female contributors communicate in a manner that promotes social affiliation and emotional connection more than male editors, irrespective of their status in the community. Female regular editors are the most relationship-oriented, whereas male administrators are the least relationship-focused. Finally, emotional and linguistic homophily is prevalent: editors tend to interact with other editors having similar emotional styles (e.g., editors expressing more anger connect more with one another). CONCLUSIONS/SIGNIFICANCE: Emotional expression and linguistic style in online collaboration differ substantially depending on the contributors' gender and status, and on the communication network. This should be taken into account when analyzing collaborative success, and may prove insightful to communities facing gender gap and stagnation in contributor acquisition and participation levels.

BACKGROUND: In their 2005 study, Adamic and Glance coined the memorable phrase 'divided they blog', referring to a trend of cyberbalkanization in the political blogosphere, with liberal and conservative blogs tending to link to other blogs with a similar political slant, and not to one another. As political discussion and activity increasingly moves online, the power of framing political discourses is shifting from mass media to social media. METHODOLOGY/PRINCIPAL FINDINGS: Continued examination of political interactions online is critical, and we extend this line of research by examining the activities of political users within the Wikipedia community. First, we examined how users in Wikipedia choose to display their political affiliation. Next, we analyzed the patterns of cross-party interaction and community participation among those users proclaiming a political affiliation. In contrast to previous analyses of other social media, we did not find strong trends indicating a preference to interact with members of the same political party within the Wikipedia community. CONCLUSIONS/SIGNIFICANCE: Our results indicate that users who proclaim their political affiliation within the community tend to proclaim their identity as a 'Wikipedian' even more loudly. It seems that the shared identity of 'being Wikipedian' may be strong enough to triumph over other potentially divisive facets of personal identity, such as political affiliation.

OBJECTIVES: Picky eating is a major source of parental concern, and children with picky eating habits are potentially at risk for nutritional deficits. This research aimed to determine whether picky eating is related to being underweight in young children. METHODS: Participants included 34 children with picky eating behavior who were referred to the Pediatric Feeding and Nutrition Clinic for evaluation and 136 healthy controls.Weight and height measurements were obtained, and weight-to-length data were calculated for each child. RESULTS: In all, 7 of 34 children (20.6%) in the picky eaters group and 9 of 136 children (6.6%) in the control group were underweight (P = .02). Underweight was found in 15 children (14.2%) at or under 36 months and in 1 child (1.6%) older than 36 months (P = .002). CONCLUSIONS: Children with picky eating habits, especially those younger than 3 years of age, are at increased risk of being underweight.

BACKGROUND: The relation between infections in infancy and subsequent cancer risk in children and young adults is controversial. Our aim was to examine this association in the Jerusalem Perinatal Study, a population-based cohort comprising all offspring from western Jerusalem and surroundings born from 1964 to 1976. METHODS: Identity numbers of non-malformed singletons with recorded data about hospital admission in the 1st year of life (n = 24,554) were linked to the Population and Cancer Registries. Person-year incidence rates were calculated for the exposed (admitted for infection) and nonexposed (not admitted for infection) groups from birth to date of cancer diagnosis, death, or December 31, 2004. We used Cox proportional hazards models to adjust for covariates associated with hospitalization. RESULTS: The median follow-up was 36 years. Cancer developed in 283 individuals. Hospitalization for infection was not associated with overall cancer risk [risk ratio (RR), 0.88; 95% confidence interval (95% CI), 0.56-1.37]. The incidence rate for non-Hodgkin's lymphoma was higher in the exposed compared with the nonexposed group (RR, 3.46; 95% CI, 1.38-8.68), remaining unchanged after controlling for birth weight, gender, and maternal education. Leukemia risk was not significantly associated (RR, 0.44; 95% CI, 0.06-3.24) with hospitalization for infection. CONCLUSIONS: Hospital admission in the 1st year of life due to infection is associated with an increased risk of non-Hodgkin's lymphoma. This is consistent with observations that mild immunodeficiencies predispose to lymphoma. Survival of infants with subtle immune defects, who may have previously succumbed to their infection, may contribute to the increased incidence of non-Hodgkin's lymphoma observed over the last 50 years.
