BACKGROUND: The impact of laparoscopic appendectomy (LA) on the incidence of intra-abdominal abscess (IAA) remains controversial. We aimed to identify risk factors for postappendectomy IAA and assess the impact of appendectomy approach in postoperative morbidity. METHODS: A retrospective single-center study including consecutive patients who underwent appendectomy for acute appendicitis between 2015 and 2018 was performed. Demographic, clinical, intraoperative, and perioperative variables were collected. Univariate and multivariate analyses was performed to detect independent risk factors for IAA. Comparison of LA and open appendectomy was conducted, and propensity score model (PSM) was used to overcome differences between groups. Independent risk factors for IAA were identified by univariate and multivariate analyses in the PSM cohort. RESULTS: A total of 532 appendectomies were included. The median age was 35 years. Three hundred two patients (56.7%) underwent LA. The most frequent operative finding was American Association for the Surgery of Trauma (AAST) grade 1 appendicitis in 303 patients (57%). Peritonitis was found in 109 patients (20.5%). Postoperative morbidity and mortality were 14.4% and 0%, respectively. Intra-abdominal abscess rate was 6.2%. An AAST grade of ≥2, a preoperative C-reactive protein level of >100 mg/dL, and diabetes mellitus were identified as independent risk factors for IAA in the multivariate analysis. Comparing LA and open appendectomy, there were no differences in IAA rates. After PSM, LA showed lower morbidity and shorter hospital stay. CONCLUSIONS: Laparoscopic appendectomy is a safe approach and not related to a higher risk of IAA. Patients with an AAST grade of ≥2, a preoperative C-reactive protein level of >100, and diabetes mellitus have higher risk for postappendectomy IAA. Meckel's diverticulum is an infrequent malformation (2% of the population) and the most frequent complication is bleeding followed by diverticulitis. This usually occurs with clinical signs and symptoms of acute appendicitis in adults. The diagnosis requires a high index of suspicion and is based on complementary tests in most cases (mainly CT).

La resección oncológica completa es el único procedimiento que permite la sobrevida a largo plazo en cáncer de páncreas. La afectación de la arteria hepática, tronco celíaco o arteria mesentérica superior constituyen una contraindicación quirúrgica porque se asocia a mal pronóstico y por las dificultades técnicas que implica conseguir la resección oncológica completa. Solo un grupo seleccionado de pacientes con buena respuesta a la quimioterapia y pasibles de resección R0 se benefician de la cirugía de resección del tronco celíaco. A partir de un caso clínico de un adenocarcinoma de páncreas con infiltración de la arteria hepática común y tronco celíaco que tras una buena respuesta a la neoadyuvancia y embolización de la arteria hepática común fue sometido a una esplenopancreatectomía córporo-caudal con resección del tronco celíaco, se realiza una revisión de la literatura sobre el tema y sus aspectos técnicos relevantes. El análisis realizado permite sugerir que en casos debidamente seleccionados la pancreatectomía córporo-caudal con resección del tronco celíaco en bloque es un procedimiento factible, seguro, y con buenos resultados quirúrgicos y oncológicos. En condiciones de respuesta a la quimioterapia neoadyuvante y experiencia del equipo quirúrgico pareciera que esta cirugía podría mejorar el pronóstico y calidad de vida de estos enfermos.


Abstract Complete oncologic resection is the only procedure that enables survival in pancreatic cancer. Compromise of the liver artery, the celiac artery or the superior mesenteric artery constitute a surgical contraidication since it is associated to a bad prognosis and it is technically hard to achieve a complete surgical resection. Only a selected group of patients who respond well to chemotherapy and may be subject to resection benefit from celiac artery resection surgery. A clinical case of adenocarcinoma of the pancreas with infiltration of the common liver artery and the celiac artery underwent a corporeo-caudal pancreatosplenectomy with celiac artery resection after a good response to neoadjuvant therapy and hepatic arterial embolization. Based on this, a review of literature on this issue and its relevant technical aspects was conducted. The analysis performed may suggest that in duly selected cases, corporeo-caudal pancreatosplenectomy with bloc celiac artery resection is a feasible and safe procedure with good surgical and oncologic results. Upon good response to neoadjuvant chemotherapy and an experienced surgical team, this surgery seems to improve prognosis and the quality of life of these patients.


Resumo A ressecção oncológica completa é o único procedimento que permite uma sobrevida em longo prazo a pacientes com câncer de pâncreas. O comprometimento da artéria hepática, tronco celíaco ou artéria mesentérica superior é uma contraindicação cirúrgica porque está associado a um prognóstico ruim e, devido às dificuldades técnicas que implica conseguir a ressecção oncológica completa. Somente um grupo selecionado de pacientes com boa resposta à quimioterapia e que possa ser submetido à ressecção R.0 pode se beneficiar da cirurgia de ressecção do tronco celíaco. A partir de um caso clínico de um adenocarcinoma de pâncreas com infiltração da artéria hepática comum e do tronco celíaco, que depois de apresentar boa resposta à quimioterapia e a embolização da artéria hepática comum, foi submetido a uma esplenopancreatectomia corpo-caudal com ressecção do tronco celíaco, realizou-se uma revisão da literatura sobre o tema e seus aspectos técnicos relevantes. A análise realizada permite sugerir que nos casos devidamente selecionados, a pancreatectomia corpo-caudal com ressecção em bloco do tronco celíaco é um procedimento factível, seguro, com bons resultados cirúrgicos e oncológicos. Quando se reúnem as condições de resposta adequada à quimioterapia neoadjuvante e a experiência da equipe de cirurgia, pareceria que esta intervenção poderia melhorar o prognóstico e a qualidade de vida destes pacientes.

AIM: To determine reproducibility of perioperative chemotherapy for gastric cancer (GC) on our settings by identifying patient's overall survival and comparing them to larger studies. METHODS: Retrospective analysis of our series, where we present our eleven-year's experience on GC managed according to perioperative approach of three preoperative chemotherapy cycles followed by surgery and finally three postoperative chemotherapy cycles. Chemotherapic scheme used was Xelox (Oxaliplatin and Capecitabine). Epidemiologic parameters as well as surgical variables were analysed, presented, and compared to other series with similar approaches. Survival was estimated by Kaplan Meier/log rank method and also compared to these studies. RESULTS: Mean age was 65 years old. Overall survival in our series was 37.7%, similar to other groups using perioperative schemes. Mortality was 4% and morbidity 30%, which are also similar to those groups. Survival curves were compared to larger studies, finding similarities on them. Subgroup survival analysis between chemotherapy responders and non-responders didn't reach statically significant differences. CONCLUSION: Perioperative chemotherapic scheme can be reproduced on our setting with good results and without increasing morbidity or mortality.

Introducción. El cáncer oculto de mama (COM), con una incidencia del 0,3-1%, aún plantea retos diagnósticos y terapéuticos. El objetivo del trabajo fue determinar las características patológicas de nuestra serie de COM, evaluar los resultados de las técnicas diagnósticas y terapéuticas empleadas (cirugía mamaria con linfadenectomía axilar o solo linfadenectomía axilar) y la supervivencia global. Pacientes y métodos. Análisis retrospectivo de 21 pacientes con metástasis ganglionares de cáncer mamario con mamografía y ecografía mamaria negativas. Desde 2003 se incluyó la resonancia magnética nuclear (RMN) en el protocolo de estudio. Se valoró la supervivencia en 2 grupos; grupo A: pacientes con COM tratadas con cirugía mamaria (15 casos), bien mastectomía radical (10 pacientes) bien cirugía conservadora (5 casos), y grupo B: 4 pacientes tratadas quirúrgicamente con linfadenectomía axilar niveles i y ii de Berg. Se asoció radioterapia sobre la mama tras cirugía conservadora (50 Gy) y sobre la axila si > 3 adenopatías axilares tumorales. Resultados. En 9 pacientes (7 de las mastectomías y 2 de las cirugías conservadoras) se identificó histológicamente un carcinoma de mama, de tipo ductal infiltrante en 8 casos y un carcinoma medular. La RMN (6 casos) demostró en 2 pacientes tumor mamario con confirmación histológica de carcinoma. Con una mediana de seguimiento de 87 meses, la supervivencia global fue de 101,2 meses (rango: 12-235 meses) con una supervivencia a 5 años del 69%. La supervivencia media en el grupo A fue de 120 meses, superior a los 41 meses del grupo B (p = 0,05). Conclusiones. En nuestra serie el tratamiento quirúrgico sobre la mama mejora la supervivencia global respecto al grupo tratado solo con linfadenectomía axilar. La RMN ha sido útil para evidenciar nódulos tumorales ante un supuesto COM y permite seleccionar a pacientes para realizar cirugía conservadora de la mama(AU)


Introduction. Occult breast cancer (OBC) has an incidence of 0.3-1% but nevertheless represents a diagnostic and therapeutic challenge. The aim of our study was to determine the pathologic characteristics of patients in our OBC series, compare the results of the different diagnostic and therapeutic techniques available (breast surgery with axillary lymphadenectomy or axillary lymphadenectomy alone), and overall survival. Patients and methods. A retrospective analysis was carried out in 21 patients with axillary lymph node metastases and negative mammography and breast ultrasonography. From 2003 onward, our group included magnetic resonance imaging (MRI) in the study protocol of OBC. Overall survival was determined and 2 groups were compared: Group A consisted of patients with OBC treated with breast surgery (n = 15), either with radical breast mastectomy (n = 10) or breast-conserving surgery (n = 5); and group B consisted of patients (n = 4) treated surgically with Berg level i or ii axillary lymphadenectomy. We added breast radiotherapy (50 Gy) in patients treated with breast-conserving therapy and axillary radiotherapy when there were more than 3 metastatic axillary nodes. Results. In 9 patients (7 mastectomies, 2 breast-conserving interventions), one breast carcinoma, 8 infiltrating ductal carcinomas and one medullar carcinoma were identified by histology. Two breast carcinomas were detected by MRI in 6 patients, which were later confirmed by histologic study. The median follow-up was 87 months, with an overall survival of 101.2 months (range: 12-235 months) and a 5-year survival of 69%. The mean survival was 120 months in group A and 41 months in group B (P = .05). Conclusions. In our series, surgical treatment of OBC improved overall survival compared with lymphadenectomy alone. MRI was useful in the identification of malignant tumors in suspected OBC and allowed patient selection for breast-conserving surgery(AU)
