RESUMEN
The development of short femoral prostheses has the advantage to preserve bone and soft tissues, restore hip geometry, permit mini-invasive techniques and allow quickly return to an active life, but very few studies described bone reaction to these new designed prostheses. The aim of the present study was to evaluate the osseointegration of two different partial neck retained stemless hip prosthesis at one year after surgery, measured by the changes of periprosthetic bone mineral density (BMD) in 5 regions of interest (ROIs) using a dual-energy X ray absorptiometry (DXA) device. The signs of stress-shielding were evaluated by standard radiographs. Thirty-two uncemented primary total hip arthroplasty (THA) patients allocated into 2 groups were evaluated. In the first group (n=19) a Proxima (De-Puy-J&J) hip stem was implanted. In the second group (n=12) a Nanos (Smith & Nephew) hip stem was used. We found that both the implants preserve metaphyseal bone stock and increase periprosthetic BMD. In Nanos prostheses a significant higher BMD values were observed in region of interest (ROI) 3 and 4 (p<0.05). No differences were found in ROIs 1, 2, and 5. Proxima stem seem to produce a physiological strain distribution in the femur. No signs of stress-shielding were present in both the implants. In conclusion, this preliminary DXA analysis showed a physiological integration of both the stems that reproduces the biomechanical stress of proximal femur. New designed short stem implants showed optimal osseointegration after one year, and therefore appears an excellent alternative to traditional long stem hip prostheses.

OBJECTIVE: To examine the effect of pre-operative physiotherapy before hip arthroplasty in patients with end-stage hip osteoarthritis. DESIGN: A prospective randomized controlled study. SETTING: Physical medicine and rehabilitation and orthopaedic departments of Policlinico Gemelli of Rome. SUBJECTS: Twenty-three patients randomized in study and control groups. INTERVENTION: The study group took part in an educational and physiotherapy programme one month before surgery. Both groups took part in the same inpatient rehabilitation programme after surgery. MAIN OUTCOME MEASURES: Both groups were evaluated one month (T0) and the day before arthroplasty (T1), after 15 days (T2), four weeks (T3) and three months (T4) post surgery, using the Barthel Index, the Short Form-36 (SF-36), the Western Ontario and McMaster Osteoarthritis Index (WOMAC), the Hip Harris Score (HHS), visual analogue scale (VAS), the British Medical Research Council (BMRC) measures of hip abductor and quadriceps strength and range of hip abduction and external rotation. RESULTS: There were no significant differences between groups with regard to the Barthel Index, SF-36, WOMAC and HHS at T4. The study group presented significant improvements of the SF-36 physical composite score at T1. The hip external rotation was significantly higher in the study group at each evaluation and the visual analogue scale values were lower at T1, T3 and T4. CONCLUSION: Pre-operative physiotherapy in patients undergoing hip arthroplasty does not improve impairment and health-related quality of life after intervention. Physiotherapy and educational therapy may be useful for end-stage osteoarthritis.

Objetivos: Describir la técnica de la biopsia de tumores retroperitoneales por vía retroperitoneoscópica y analizar los resultados obtenidos en nuestro servicio. Material y método: Entro los años 1996 y 2003 se han realizado 18 biopsias de tumores retroperitoneales por vía retroperitoneoscópica en 16 pacientes ( en un paciente se realizó el procedimiento en 3 oportunidades). La técnica quirúrgica fue con el paciente en decúbito lateral intermedio con elevación del flanco y abordaje retroperitoneal. Se colocaron 2 trocares de 10 y 5mm, exposición del retroperitoneo, apertura de la fascia de Gerota, disección de la masa tumoral y biopsia incisional analizada por congelación. Resultados: se diagnosticaron 12 linfomas (11 no Hodgkin y1 Enfermedad de Hodgkin), 1 mesotelioma, 1 schwannoma, 1 metástasis ganflionar de cáncer de mama y metástasis adrenal de cáncer de pulmón. Se obtuvo diagnóstico anátomo-patológico en todos los pacientes biopsiados. El material permitió efectuar estudios citológicos por impronta, evaluación histológica e inmunofenotipo por citometria de flujo. No se registraron complicaciones y el tiempo de internación fue menor de 24 horas. Conclusiones: La técnica de biopsia de tumores retroperitoneales por retroperitoneoscopia propociona buenos resultados con baja morbilidad, permitiendo obtener el diagnóstico anátomo-patológico en todos los casos. En los kinfomas posibilitó la clasificación de subtipos y repetir la biopsia en caso necesario

La neovejiga ileal ortotópica es una derivación urinaria ampliamente aceptada luego de una cistectomía radical por cáncer de vejiga. Cuando se necesita un reservorio ostómico se utilizan segmentos ileocecales para usar el apéndice cecal o el íleon terminal afinado como mecanismo de continencia, Los reservorios ileales ostómicos continentes como el Kock y Ghohein, son complejos. Se presenta la técnica quirúrgica de un reservorio ileal continente ostómico de simple realización, con el mecanismo de continencia basado en el principio de Nissen para la funduplicatura gástrica en hernia hiatal. La técnica ha sido efectuada en 2 pacientes con buenos resultados de continencia y fácil cateterismo.

After reviewing their series of prosthetic revisions and comparing it with other reports in literature, the authors reassess, where possible, classic conservative intervention as well as the indications for cemented prostheses given the poor long-term reliability of the bone-cement interface. The authors emphasize the always more frequent incidence of prosthetic loosening and the resulting need for surgical revision, especially in patients who were under 65 years of age at the time of primary prosthetic replacement. Finally, the authors present their clinical and surgical experience in revision using cementless implants, which they prefer in this type of operation.

The authors emphasize the importance of the bone scan in long-term follow-ups of total hip arthroplasty and describe some technical details of the examination. The changes revealed in the bone scan are confirmed by the operative report. Even though the number of patients in the study was relatively small, the fact that the bone scan taken of patients with cementless total hip replacements revealed no pathological changes is considered significant.

The authors, after putting forth several biomechanical premises, discuss their experience in total hip arthroplasty (THA) using a PCA-type press fit cementless prosthesis with partial Madreporic covering. Several cases which demonstrate the versatility of this type of replacement are discussed in depth. Even though the short follow-up precludes definitive conclusions, use of cementless total hip replacements must be considered a valid option in the future, especially in cases of early onset of hip pathology.
