Overall sixty patients with hypertensive disease were examined for clinical effectiveness of the prolonged-action inhibitor angiotensine-converting enzyme renitek (enalapril maleate, MJD). The treatment schedule included placebo over two weeks, renitek as monotherapy, 20 mg daily, over 6 weeks, if the patients failed to derive any benefit from the therapy, they were placed on a 4-week combined therapy of renitek plus hydrochlorothiazide. Renitek was found to dispel cardialgias, reduce frequency of attacks of cardiac angina. In the hemodynamic structure, a positive inotropic effect was identified as was reduction of energy expenditure of the myocardium, total peripheral vascular resistance, diminution of post- and preloads on the myocardium, with the mass of the left ventricular myocardium growing less.

Changes of membrane potential of lymphocytes of peripheral blood under the influence of the activation agents: phytohemagglutinin, concanavalin A and Ca(2+)-ionophore A 23187 in 25 patients (men) with hypertensive disease and 8 healthy persons (men) aged 25 to 35 years have been studied. The results obtained testify to the existence of functional disturbances of lymphocytes' membrane and confirmed the wide-spread of the membrane defect in patients with hypertensive disease.

A study is presented of hemodynamic effects and reactions of prostacyclin--thromboxane system in patients with stable (63 pts) and unstable angina pectoris (57 pts) who took aldizem 240 mg daily during 3 week. It was found that the aldizem improved clinical and hemodynamic indices and increased plasma concentration of 6-keto-PGF1 alpha and FPE2, but reduced TxB2 and PGF2 alpha. Also these changes depended on time of therapy: long-term therapy resulted in improvement of the prostacyclin-thromboxane balance.

The effect of human recombinant interleukin-2, one of the potent mediators of the immune system, on the course of emotional hypertension in non-linear white rats has been investigated. A significant and prolonged hypotensive action of a single injection of interleukin-2 in hypertensive rats has been revealed. The data obtained can be a new evidence of participation of immune system in the development of hypertension in experimental animals.

Changes of a number of functions of beta-receptor complex mechanism have been analyzed by means of the microwave-dielectrometry method on native erythrocytes at various stages of cardiovascular pathology. The control scheme of beta-receptor complex has been analyzed in the adenylate cyclase system and the regions have been extracted which are subjected to an attack of pathological factors of the effect.

The values of depressive humoral factors drastically decrease in healthy subjects and patients with hypertensive disease running with crisis in the areas of hot climate in summer. After arresting hypertensive crises, the levels of kallikreinogen, kallikrein, kininogen increase. In the crises, there is a significant inverse correlation between the blood pressure and blood kallikrein and kininogen concentrations. The values of kallikrein-kinin system components clearly characterize the clinical status of patients with hypertensive disease running with crisis and are of predictive value for clarification of the body's protective reserves.

Serum levels of eicosanoids in patients suffering from chronic hepatic diseases were analysed in correlation with morphological characteristics of the hepatic lesion, body reactivity, manifestations of the alternative and immune phases of the inflammation. The results could be contributing to diagnosing and treating chronic inflammation in the liver.

The inhibitor calmodulin (R 24571) was examined for effects on the activity of red blood cell Ca-ATPases in patients with coronary heart disease during the treatment with nitrates, beta-blockers and calcium antagonists. The maximum activity of Ca-ATPase was measured in the erythrocytes perforated with saponine in the presence of endogenous regulators at a concentration of Ca2+ of 3-5 microM. Patients with high and low Ca-ATPase activity were identified. In the control group R24571 failed to affect Ca-ATPase activity. In patients, the calmodulin inhibitor caused both Ca-ATPase activation and inhibition. The effects of R 24571 correlated with the severity of the patients' condition. In effective therapy, the action of the calmodulin inhibitor became lower on Ca-ATPase activity. It was concluded that there was Ca-ATPase regulation imbalance in patients with coronary heart diseases.

The paper provides the results of studying the clinical disease manifestations, the plasma content of fibronectin and fibrin and fibrinogen degradation products in patients with chronic hepatitis (CH). It is shown that they are determined not only by the morphological form of CH but also by the structure of inflammatory infiltrates in liver tissue.

Extracorporeal techniques (hemosorption, plasmapheresis, thrombocytoplasmapheresis, quantum hemotherapy) were studied for their impact on the clinical course of a disease and some hemostatic parameters in 370 patients with cardiovascular diseases (coronary heart disease, hypertensive disease, lower extremity at atherosclerosis obliterans). They were found to be effective in managing patients with atherosclerosis and arterial hypertension who are refractory to the conventional drug therapy. When there was a positive clinical action of therapy, the patients with coronary heart disease were demonstrated to show better parameters of lipid metabolism, immunological status and blood aggregation control system. The hypotensive effect of plasmapheresis in patients with arterial hypertension was followed by a decrease in pressor hormone levels and peripheral vasodilatation.

The examination of 194 patients with coronary heart disease concurrent with clinical manifestations of angina pectoris established that the development of coronary atherosclerosis was accompanied by suppressed prostacyclin (PGI2) synthesis and increased thromboxane A2 (TxA2) leukotriene (LT) synthesis. The activation of synthesis of the vasoconstrictor and proaggregate TxA2 depended on the severity of clinical signs of angina pectoris. The imbalance of PGI2 to- TxA2 metabolite ratios was associated with dyslipidemias and the extent of coronary atherosclerosis. Bicycle ergometry testing resulted in prostanoid imbalance. The determination of the dynamics of prostanoids was used to check the efficiency of antianginal therapy. The concomitant application of inhibitors of thromboxane synthetase and LT synthesis was recommended for secondary prevention of thrombotic complications in patients with coronary atherosclerosis.

Alpha-atrial natriuretic peptide (ANP) levels, plasma renin activity, and plasma aldosterone levels were measured in 103 patients with coronary heart disease (CHD) having signs of various circulatory failures. The parameters were studied in relation to central hemodynamic values, and their dependence on the stage of chronic circulatory failure. Alpha-ANP was found to be one of the factors of the humoral systems implicated in the pathogenesis of chronic circulatory failure in CHD patients. The plasma level of alpha-ANP was found to be related to the state of the renin-angiotensin-aldosterone system both in healthy subjects and CHD patients.

Experiments carried out on 91 dogs have shown that changes in the inflammatory process course result in complicated postinfarction repair of myocardium, disagreement in the development, i.e. in desynchronization of necrotic repair processes in the myocardial infarction zone. It is supposed that regulation of the myocardial infarction healing by inflammation can be one of the efficient methods of its therapy.

As a result of epidemiological and clinico-functional studies occupational peculiarities of ischemic heart disease (IHD) among miners were detected. The role of a number of industrial factors in the development of arrhythmia with unfavourable prognosis was assessed. Persons at risk for development and progress of IHD were identified. A method of dynamic control over their cardiovascular system was suggested. The findings of the study formed the basis for a complex follow-up programme for miners with IHD.
