RESUMEN

Alzheimer's disease (AD) is a leading cause of dementia in the elderly. Mitogen-activated protein kinase phosphatase 1 (MKP-1) plays a neuroprotective role in AD. However, the molecular mechanisms underlying the effects of MKP-1 on AD have not been extensively studied. MicroRNAs (miRNAs) regulate gene expression at the post-transcriptional level, thereby repressing mRNA translation. Here, we reported that the microRNA-429-3p (miR-429-3p) was significantly increased in the brain of APP23/PS45 AD model mice and N2AAPP AD model cells. We further found that miR-429-3p could downregulate MKP-1 expression by directly binding to its 3'-untranslated region (3' UTR). Inhibition of miR-429-3p by its antagomir (A-miR-429) restored the expression of MKP-1 to a control level and consequently reduced the amyloidogenic processing of APP and Aβ accumulation. More importantly, intranasal administration of A-miR-429 successfully ameliorated the deficits of hippocampal CA1 long-term potentiation and spatial learning and memory in AD model mice by suppressing extracellular signal-regulated kinase (ERK1/2)-mediated GluA1 hyperphosphorylation at Ser831 site, thereby increasing the surface expression of GluA1-containing α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptors (AMPARs). Together, these results demonstrate that inhibiting miR-429-3p to upregulate MKP-1 effectively improves cognitive and synaptic functions in AD model mice, suggesting that miR-429/MKP-1 pathway may be a novel therapeutic target for AD treatment.

RESUMEN

Gases such as hydrogen sulfide, nitric oxide and sulfur dioxide have important regulatory effects on the endocrine and physiological processes of the body and are collectively referred to as "gas signaling molecules". These gas signaling molecules are also closely related to Alzheimer's disease, the inflammatory response and depression. In this paper, we introduce the production and metabolic pathways of NO, H2S and SO2 in living organisms and review the regulatory functions of gas signaling molecules in the endocrine system and their mechanisms in relation to their clinical applications. This work will provide a basis for finding targets for intervention and establishing novel prevention and treatment strategies for related diseases.

OBJECTIVES: The combination of growth hormone (GH) with gonadotropin was a prevalent method to improve clinical reproduction in adjuvant for assisted reproduction treatment (ART). However, the contradictory results from previous studies failed to confirm the benefits. The present study is focused on the mechanism analysis of GH-IGF1-gonadal axis in ART and the changes of IGF1 in follicular fluid among different types of patients. MATERIAL AND METHODS: We recruited 136 patients and divided them into eight groups according to their ages and ovarian reserves. The baseline characteristics of the study population were summarized. The therapeutic outcomes in the study population were observed. In the meantime, concentrations of IGF1 in follicular fluids from different types of patients who underwent GH strategy were measured by Western blot. The functional mechanism of GH-IGF1-gonadal axis in ART was also analyzed. RESULTS: We analyzed the baseline characteristics of the study population, the therapeutic outcome of GH-IGF-1-gonadal axis, as well as the relative protein level of IGF1 and IGFBP1 in follicular fluid from different groups. The chemical pregnancy rate was significantly increased in different degrees for groups with GH co-treatment compared to groups without GH co-treatment. The IGF1 in follicular fluid of patients under 35 years' old showed an upward trend compared with groups of poor, normal and high ovarian reserves. After GH induction, IGF1 in follicular fluid was significantly increased in patients over 35 years old. CONCLUSIONS: The study suggested that the application of GH might be beneficial to the pregnancy outcome in patients. GH application in patients older than 35 years might have a beneficial effect on pregnancy outcome via promoting the expression of IGF1. Our study indicates a different mechanism from GH application among younger and older patient in ART and provides a new clue for individual clinical treatment in infernity patients.

OBJECTIVE@#To investigate the clinical efficacy and prognosis of patients with multiple myeloma (MM) treated by long-term maintenance lenalidomide treatment.@*METHODS@#A total of 97 patients diagnosed as MM in the Department of Hematology of First Affiliated Hospital of Guangzhou University of Chinese Medicine from 2012 to 2019 were selected, and the basic clinical characteristics and laboratory indicators of the patients were tested and evaluated. After long-term maintenance lenalidomide treatment for patients with MM, the short-term and long-term clinical efficacy and the incidence of adverse reactions were evaluated, and factors affecting the prognosis of the patients were analyzed.@*RESULTS@#Before maintenance treatment, 47.42% of the patients (46/97) did not achieve complete remission (CR), among 52.58% (51/97) of CR patients, there were 20.62% of the patients showed minimal residual leukemia (MRD) negative. After lenalidomide maintenance treatment, the patients who did not achieve CR were reduced to 24.74% (24/97), among 75.26% (73/97) of the patients with CR, there were 47.42% of the patients showed MRD negative, the difference showed statistically significant (P<0.001). After maintenance treatment, the median pro-gression-free survival of the patients was 58 months, and the 5-year survival rate was 89.69%. The incidence of adverse reactions was 40.21% (39/97), including neutropenia (31/39, 79.49%), fatigue (21/39, 53.85%), thrombocytopenia (17/39, 43.59%) and gastrointestinal reaction (15/39, 38.46%) were the most common. The discontinuation rate was 24.74% (24/97), and the median time for discontinuation was 21 months. The main reasons for discontinuation were neutropenia (12/24, 50.00%) , thrombocytopenia (8/24, 33.33%) and gastrointestinal reactions accounted for 8.33% (2/24). Old age and positive MRD were the risk factors affecting the prognosis of the patients. The adjusted OR was 1.43 (95% CI 1.03-1.76, P=0.034) and 3.78 (95% CI 2.56-9.56, P=0.037), respectively.@*CONCLUSION@#The long-term maintenance lenalidomide treatment shows a good clinical effect on patients with MM, and MRD detection can assist the cilinical judge the prognosis of the patients. During maintenance treatment, the clinical symptoms, especially blood system damage of the patients should be take care, so as to avoid serious adverse reactions.

Objective:To investigate the effect of scar uterus on assisted reproduction treatment strategy.Methods:From January 1, 2017 to December 31, 2017, 109 cases of scar uterus group and 63 cases of vaginal delivery history group who underwent in vitro fertilization-embryo transfer (IVF-ET) assisted pregnancy fresh embryo transfer in our hospital were retrospectively analyzed. Results:⑴ There was no significant difference in the total amount of gonadotropin (Gn) and the total days of GN in patients <35 between scar uterus group and the vaginal delivery history group ( P>0.05), but there were significant differences in the number of oocytes, mature oocytes, two pronucleus (2PN) and excellent embryos between the scar uterus group and the vaginal delivery history group ( P<0.05). When the age was ≥35 years old, there was no significant difference in the total number of GN, the total days of GN, the number of oocytes obtained, the number of mature oocytes, the number of 2PN and the number of excellent embryos between the two groups ( P>0.05). ⑵ There were no statistically significant differences in clinical pregnancy rate and embryo implantation rate between the two groups of patients < 35 years old, no matter single embryo transplantation or double embryos transplantation. When transplant two embryos, the clinical pregnancy rate and embryo implantation rate in the scar uterus group were slightly lower than those in the vaginal delivery history group (57.57% vs 71.05% and 39.39% vs 47.37%, respectively), but with no significant difference ( P>0.05). There were no statistically significant differences in the clinical pregnancy rate and embryo implantation rate between single embryo transplantation and double embryo transplantation in patients≥35 years old ( P>0.05). ⑶ There were no statistically differences in biochemical pregnancy rate, early abortion rate, preterm birth rate and average newborn weight between the scar uterus group and the vaginal delivery history group ( P>0.05). Conclusions:Cesarean section may affect the ovarian function. For patients with previous cesarean section, early evaluation of ovarian function is recommended. Single embryo transfer does not reduce the outcome in IVF-ET. It is recommended to perform single embryo transfer for patients with scar uterus to reduce the risk during pregnancy of twin pregnancy.

Exploiting cells as vehicles combined with nanoparticles combined with therapy has attracted increasing attention in the world recently. Red blood cells, leukocytes and stem cells have been used for tumor immunotherapy, tissue regeneration and inflammatory disorders, and it is known that neutrophils can accumulate in brain lesions in many brain diseases including depression. -Acetyl Pro-Gly-Pro (PGP) peptide shows high specific binding affinity to neutrophils through the CXCR2 receptor. In this study, PGP was used to modify baicalein-loaded solid lipid nanoparticles (PGP-SLNs) to facilitate binding to neutrophils . Brain-targeted delivery to the basolateral amygdala (BLA) was demonstrated by enhanced concentration of baicalein in the BLA. An enhanced anti-depressant effect was observed and The mechanism involved inhibition of apoptosis and a decrease in lactate dehydrogenase release. Behavioral evaluation carried out with rats demonstrated that anti-depression outcomes were achieved. The results indicate that PGP-SLNs decrease immobility time, increase swimming time and climbing time and attenuate locomotion in olfactory-bulbectomized (OB) rats. In conclusion, PGP modification is a strategy for targeting the brain with a cell-nanoparticle delivery system for depression therapy.

Objective To explore the effect of self-designed breathing exercise on the six-minute walking distance and quality of life in stable patients with chronic obstructive pulmonary disease (COPD).Methods 60 patients with COPD were collected and divided into respiratory exercise group,positive control group and negative control group according to the random number table.Three groups of patients were treated with regular medication according to the GOLD (2016) guidelines,and accepted scheduled healthy knowledge education.Positive control group plus the 12-week aerobic exercise training (50%-80% of the intensity of HRmax,30 minutes a day,three times a week).The respiratory exercise group began to carry out self-designed respiratory exercise training after discharge,which ensured that the patients would receive regular breathing exercise training three times a week and continue for last without interruption.On the day of enrollment,the evaluation will be conducted 3 months after the training.The evaluation indexes were six minutes walking distance (6WMD) and scores of COPD Clinical Questionnaire (CCQ).Results In the respiratory exercise group and the positive control group,both 6WMD and CCQ were significantly improved compared with that before rehabilitation (P <0.01).Compared with the negative control group,the 6WMD and CCQ of the respiratory exercise group and the positive control group were both significantly improved (P <0.01).But there was was no significant statistically difference between the respiratory exercise group and the positive control group (P > 0.05),and the improvement degree of 6WMD and CCQ in the two groups was similar.Conclusions Self-designed breathing exercises can improve the walking distance and quality of life of patients with stable chronic obstructive pulmonary disease and achieve the same effect as exercise training.

Objective To study the relationship between phosphodiesterase 4D (PDE4D) gene rs966221 single nucleotide polymorphisms (SNPs) and ischemic stroke (IS) in Guangxi Zhuang population.Methods One hundred and one IS patients from Guangxi Zhuang Autonomous Region served as IS pgroup and 104 healthy subjects undergoing physical ecamination served as control group in this study.Their PDE4D gene rs966221 SNPs were detected by SNaPshot technique.The genotypes and frequencies of alleles were compared between the two groups and the relationship between PDE4D gene rs966221 SNPs and IS was analyzed.Results No significant difference was found in the GG,GA,AA genotypes and in the frequencies of G and A alleles between the two groups (0.99% vs 3.85%,29.70% vs 21.15%,69.31% vs 75.00%,P>0.05;15.84% vs 14.42%,84.16% vs 85.58%,P>0.05).Univariate and multivariate logistic regression analysis showed that the PDE4D gene rs966221 SNPs were not related with the risk of IS in dominant AA vs GG+GA,recessive GG vs AA+GA and additive GG vs AA genetic models (P>0.05).Conclusion The PDE4D gene rs966221 SNPs are not related with IS in Guangxi Zhuang population.

Objective To explore the relationship between methylenetetrahydrofolate reductase(MTHFR)gene C677T position polymorphism and intracerebral hemorrhage(ICH)in southern Chinese Han population.Methods The MTHFR gene polymorphism were detected in 124 ICH patients(ICH group)and 149 control subjects(normal control group)who were southern Chinese Han population by SNaPshot technique.Results Compared with normal control group,the frequencies of T allele in ICH group and nonlobar ICH subgroup were significantly higher(all P<0.05);and there was no statistical significance of frequencies of C allele in ICH group and nonlobar ICH subgroup, and frequencies of T, C allele in lobar ICH subgroup(all P>0.05).There was no statistical significance of genotype frequency in ICH group, nonlobar ICH subgroup, lobar ICH subgroup and normal control group. Unconditional Logistic regression showed MTHFR gene C677T position polymorphism had no correlation with ICH group and its subgroup(all P>0.05).After confounding factors correction, ICH and nonlobar ICH subtype were significantly associated with T alleles(OR=3.77,95%CI:1.40-10.16,P=0.01;OR=4.19,95%CI:1.30-13.46,P=0.02),and lobar ICH subtype had no correlation with T allele(OR=2.31,95%CI:0.37-14.49,P=0.37).Conclusion MTHFR gene C677T position mutation T allele may be an important risk factor for ICH in the southern Chinese Han population.

Objective To evaluate the effects of intra-aortic balloon counterpulsation (IABP) on mortality of patients with acute myocardial infarction (AMI) treated with percutaneous coronary intervention (PCI).Methods Randomized controlled trials (RCTs) of IABP compared with non-IABP control in AMI patients, from January 1970 to May 2015, were searched from MEDLINE, Embase and Web of Science.The data were analyzed with software RevMan 5.0.Results Five RCTs involving 1 450 AMI patients, including 722 treated with IABP (IABP group) and 728 without IABP (non-IABP group), were included for analysis.Compared with non-IABP group, IABP did not significantly decrease the hospital mortality or 30-day mortality (OR=0.92, 95%CI: 0.69-1.25,P=0.61).According to the timing of IABP before or after PCI, it was further divided into IABP-before-PCI subgroup and IABP-after-PCI subgroup.Compared with non-IABP group, the 30-day mortality was not decreased in IABP-before-PCI subgroup or in IABP-after-PCI subgroup (OR=0.64, 95%CI: 0.23-1.78,P=0.39;OR=1.25, 95%CI: 0.42-3.77,P=0.69, respectively).According to complicating with cardiogenic shock (CS) or not, patients were divided to AMI with CS subgroup and AMI with no-CS subgroup;the hospital or 30-day mortality were not significantly decreased in both subgroups (OR=0.96, 95%CI: 0.70-1.32,P=0.80;OR=0.68, 95%CI: 0.28-1.70,P=0.27, respectively).Conclusion IABP does not decrease the 30-day mortality of AMI patients treated with PCI.

Objective To investigate the temporal expression patterns of the related transcription factors and target genes in cardiomyocyte hypertrophy,and provide valuable clues for further researches on cardiomyocyte hypertrophy.Methods Isoproterenol (ISO) was used to induce ventricular myocytes hypertrophy in neonatal mice.The survival rate of cardiomyocyte was detected by CCK-8,and the average diameters and surface areas of cells were measured by computer photograph analysis system.The mRNA and protein expression levels of related genes were respectively measured by RT-PCR and Western blotting.Results The model of cardiomyocyte hypertrophy stimulated by ISO was constructed successfully.The expression levels of GATA4,MEF2C,GATA5,BNP and ANP increased 24 hours after ISO treatment,the expression levels of P300,α-MHC and TBX5 increased 12 hours after ISO treatment,and of SRF and β-MHC mRNA increased 6 hours after ISO treatment (P<0.05).The expression levels of GATA4,α-MHC,β-MHC,SRF and P300 mRNA increased firstly,and then decreased in cardiomyocytes induced by ISO.The expression levels of GATA4,SRF,α-MHC,β-MHC and P300 mRNA were still higher than normal (P<0.05),but of MEF2C decreased to normal (P>0.0S) 72 hours after ISO treatment.The expression levels continuously elevated of GATA5,TBX5,ANP and BNP mRNA than that of controls (P<0.05),while no fluctuation was found in NKX2.5 mRNA expression (P>0.05).The expression of GATA4 protein increased,while of HEY2 protein decreased 48 hours after ISO treatment (P<0.05).Conclusions In hypertrophic cardiomyocytes,the expression pattern of MEF2C is similar to,but the patterns of GATA5,GATA4,TBX5 and SRF are different with that in the development of heart,implying these genes are important during the process from compensatory stage to decompensation stage.The expression patterns of GATA4,MEF2C and SRF are similar to that of acetylase P300,implying the temporal expressions could be regulated by P300 in cardiomyocyte hypertrophy.

Objective To compare the clinical efficacy betweenLing Gui Ba Fa (eight magic turtle techniques) time-based points selection method and ordinary acupuncture in treating diarrhea-predominant irritable bowel syndrome (IBS-D) due to spleen-stomach deficiency.Method Sixty patients were randomized intoLing Gui Ba Fa group and ordinary acupuncture group, 30 cases each. InLing Gui Ba Fa group, Gongsun (SP4) was selected as the host point, Neiguan (PC6) as the guest point, and Dachangshu (BL25), Tianshu (ST25), Shangjuxu (ST37), Sanyinjiao (SP6), Pishu (BL20) and Zusanli (ST36) as the adjunctive points; in the ordinary acupuncture group, Gongsun and Neiguan were selected as the major points, and Dachangshu, Tianshu, Shangjuxu, Sanyinjiao, Pishu and Zusanli were selected as the adjunctive points. The acupuncture treatments were conducted 3 times a week (the practitioner would make appointment with patient at a convenient time if Gongsun had multiple activation time points according toLing Gui Ba Fa), 10 sessions as a course of treatment. Before and after the intervention, the two groups of patients were evaluated by using the traditional Chinese medicine symptoms and syndromes scale and irritable bowel syndrome quality of life questionnaire (IBS-QOL), and clinical efficacy was also observed.Result The control rate was 86.7% inLing Gui Ba Fa group versus 73.3% in the ordinary acupuncture group, and the therapeutic efficacy ofLing Gui Ba Fa group was significantly better than that of the ordinary acupuncture group (P<0.01); after the treatment, the scores in rating symptoms such as diarrhea, abdominal bloating and abdominal pain were significantly lower than those before the treatment in both groups (P<0.01), and the declines of scores inLing Gui Ba Fa group were more significant than those in the ordinary acupuncture group (P<0.01).ConclusionLing Gui Ba Fa time-based points selection method can produce a more significant efficacy than ordinary acupuncture in treating irritable bowel syndrome.

<p><b>OBJECTIVE</b>To observe and evaluate the clinical efficacy of transcutaneous vagus nerve stimulation (taVNS) at auricular concha for primary insomnia (PI) and affective disorder.</p><p><b>METHODS</b>A total of 35 patients who met the diagnosis standard of PI in(5th edition) were included. The self-developed auricular vagus nerve stimulator (TENS-200A) was applied at auricular concha, 30 min per treatment, twice a day, 5 days a week for consecutive 4 weeks. The follow-up visit was conducted at the end of 6th week. The Pittsburg sleep quality index scale (PSQI), 17-items Hamilton depression scale (17HAMD) and Hamilton anxiety scale (HAMA) were applied for evaluation. The PSQI, HAMA and 17HAMD were observed before and after treatment; the safety was also observed.</p><p><b>RESULTS</b>Compared before treatment, the PSQI was significantly decreased to (13.20±3.61) at the end of 2nd week (<0.05); compared before treatment, the 17HAMD and HAMA were significantly decreased at the end of 4th week and 6th week (all<0.05). No adverse reaction was observed.</p><p><b>CONCLUSIONS</b>The taVNS could not only relieve PI symptoms, but also improve the depressive and anxiety symptoms, in addition, it may have positive long-term efficacy and safety.</p>

RESUMEN

To investigate the effect of the combination of gastrodia and uncaria on the pharmacokinetics of gastrodin and rhynchophylline, and determine their pharmacokinetic parameters after administration of the combination of gastrodia and uncaria at the ratio of 12∶9. Compared with uncaria group or gastrodia group, Cmax and AUC of both gastrodin and rhynchophylline were significantly increased, and tmax was retroceded by 1.5 h for rhynchophylline and 0.25 h for gastrodin. The change of tmax resulted in a 1.25 h difference in the peak time between gastrodin and rhynchophylline , which was the same between them. Uncaria shows a more effect in suppressing hyperactive Yang, while gastrodia has a balancing effect by nourishing Yin and suppressing hyperactive Yang. As a result, gastrodia could exert the effect in nourishing Yin and suppressing effect of uncaria, which could avoid the deficiency of Yang affecting Yin due to mono-treatment of uncaria. On one hand, the enhanced AUC and Cmax of gastrodin could increase the average plasma drug concentration of gastrodin, and remedy the losing effect of uncaria at the early stage; On the other hand, the increased AUC and Cmax of rhynchophylline could make up the quick elimination of gastrodia in vivo at the late stage. Their combination could lead to an increased anti-hypertensive effect with the balance of Yin and Yang. They showed unique advantages compared with simple dosage increase of western medicines. The results were consistent with the principle of TCM treatment for the hypertension due to hyperactivity of the liver Yang. In short, this study gives a good pharmacokinetic explanation of the balance of Yin and Yang and TCM treatment for both symptoms and root cause.

RESUMEN

A 61-year-old woman with pulmonary alternariosis and aspergillosis was reported.The patient presented with recurrent hemoptysis and cough for 3 years.Alternaria was identified by fungal culture.Biopsy specimen showed pulmonary aspergillosis.The patient had been treated with voriconazole at 400 mg/d through intravenous guttae for 7 days,and then switched amphotericin B at 25 mg/d through intravenous guttae for 11 days.The patient was treated with voriconazole at 400 mg through oral when she was discharged from hospital.After the treatment,the clinical symptoms ofhemoptysis and cough were recovered,and the lung CT examinations showed normal.

RESUMEN

Primary mediastinal choriocarcinoma in male is not a very common disease,with nonspecific clinical manifestations.Gynecomastia and testicular atrophy are present in some cases.The levels of serum human chorionic gonadotropin are often significantly increased.Giant lump in the mediastinum and bilateral lungs multiple metastases can be seen on the computed tomography for lung.The diagnosis for it depends on pathological biopsy.Current treatment method is a comprehensive,consisting of chemotherapy,radiotherapy and surgery.This paper reported a case of primary mediastinal choriocarcinoma in male,who were diagnosed and treated in the Second Xiangya Hospital of Central South University.He was admitted for cough and hemoptysis,and finally diagnosed by biopsy.The prognosis is very poor.Therefore,it is important to take physical examination regularly because it can be detected and diagnosed early.

RESUMEN

Objective: To fuse many non-homologous medical images based on wavelet transformation, and integrate and stand out complementation information, and strengthen image quality, and reduce redundancy so as to enhance the precision of clinically auxiliary diagnosis and treatment for medical image. Methods: Through studied and researched the relevant knowledge of wavelet theory in the application of image fusion to proposed a improved fusion algorithm by mutual combination for maximum low frequency energy and maximum high frequency variance. The fusion experiment of non-homologous standard medical image, including CT, MRI and multi-focus images, were implemented, and their data were compared. And then, the performance of image fusion was compared and analyzed under different fusion rule and different fusion method. Results: In the two kinds of contrastive analysis experiments, the fusion image which depended on algorithm included more abundantly effective information amount of source image, and the luminance of image was reasonable enhanced. Besides, the mean value, mutual information and information entropy of fusion image were optimal. Conclusion: The fusion image which comes from multimoding medical image algorithm has better visual effects and quantization indicator, and it has strengthener fused performance. Therefore, it can reflect the effectiveness of the method.
