Consulta enfermera de Educación Terapéutica en asma grave / Outpatient nurse clinic on Therapeutic Education for severe asthma

La enfermera de la Unidad de Alergología del Hospital Universitario Puerta de Hierro Majadahonda (Madrid) propuso al equipo crear una consulta monográfica de educación en asma dada la alta prevalencia, consiguiendo el consenso del equipo interdisciplinar, y la acreditación de la Sociedad Española de Alergología e Inmunología Clínica. El proyecto de creación de la nueva consulta surgió al finalizar la pandemia de COVID-19, debido a la necesidad de mejorar la educación y el seguimiento de estos pacientes crónicos para reducir las recidivas. La Consulta de Educación en Asma Grave se puso en marcha en marzo de 2022. La educación terapéutica, como proceso continuo de cuidados centrado en el paciente, es una de las claves para mejorar el control de la enfermedad y la calidad de vida del paciente-familia. La enfermera es el profesional esencial que interviene en todos los procesos asistenciales, que puede conseguir promover el empoderamiento del paciente frente a su enfermedad, así como su rol activo en autocuidados. The nurses from the Allergology Unit of the Hospital Universitario Puerta de Hierro, Majadahonda (Madrid) put forward to the team the idea of creating a specialized outpatient clinic on asthma education, given its high prevalence, and obtained consensus by the interdisciplinary team and accreditation by the Spanish Society of Allergology and Clinical Immunology. The project for creating this new outpatient clinic came up at the end of the COVID-19 pandemic, due to the need to improve the education and follow-up of these chronic patients in order to reduce relapses. The Outpatient Clinic for Education on Severe Asthma was launched in March 2022. Therapeutic education, as an ongoing process of patient-centred care, is one of the keys to improve disease control and the quality of life of patients and their families. The nurse is the key professional involved in all processes of care, who can achieve the promotion of patient empowerment regarding their disease, as well as an active role in self-care. (AU)

Programa ADAPTATEA para mejorar la atención de niños y niñas con autismo en la realización de un electroencefalograma / The adaptatea program to improve care for children with autism undergoing an electroencephalogram

Las dificultades socio-comunicativas que presentan los/as niños/as con trastorno del espectro autista (TEA), y las barreras del entorno sanitario, pueden complicar las pruebas diagnósticas y las intervenciones sanitarias hasta el punto de no poder realizarse. Pueden darse momentos muy complicados para estos niños/as y sus familias, que son vividos con gran ansiedad generándoles gran hostilidad hacia el entorno sanitario. A su vez, los profesionales se ven desbordados e impotentes ante estas situaciones, aumentando los recursos humanos y el tiempo necesarios para atenderles. El personal de Enfermería tiene un papel fundamental en la identificación de las dificultades y barreras, tanto las que presentan los/as niños/as y sus familias, como las del propio personal sanitario implicado, pudiendo actuar como facilitadores desde el primer momento.El Servicio de Neurofisiología realiza electroencefalogramas (EEG) a pacientes con TEA, ya que la epilepsia es un problema frecuentemente asociado al mismo. Las dificultades para efectuar con éxito la prueba han llevado a la creación del programa ADAPTATEA, cuyo objetivo general es mejorar la calidad de la atención y conseguir realizar el EEG en las mejores condiciones posibles, llevando a cabo las adaptaciones sanitarias necesarias. Se basa en el empleo de pictogramas y material audiovisual cuyo objetivo es anticipar al niño o niña y familia lo que se les va a hacer, transformando la prueba en algo más cercano y cotidiano para ellos. Además, propone recomendaciones para la preparación, ejecución de la prueba, y para el mejor manejo de las conductas desadaptativas del niño/a cuando acude al hospital.(AU)

The socio-communicative difficulties presented by children with autism spectrum disorder (ASD) and the healthcare environment barriers can complicate diagnostic tests and healthcare interventions even to the point where these cannot be performed. These can lead to very difficult times for these children and their families, experienced with high anxiety and generating major hostility towards the healthcare environment. At the same time, professionals will feel overwhelmed and powerless when faced with these situations, which require an increase in human resources and time. The Nursing staff plays an essential role in the detection of these difficulties and barriers, presented both by children and their families and by the healthcare staff involved, and they can act as facilitators since the first minute.The Neurophysiology Unit performs electroencephalograms (EEG) to ASD patients, because epilepsy is a problem often associated with this disorder. The difficulties to perform this test successfully have led to the creation of the ADAPTATEA program, with the overall objective to improve the quality of care and being able to perform the EEG in the best possible conditions, by conducting the healthcare adaptations required. This is based on the use of pictograms and audiovisual materials targeted to explaining the child and his/her family what will be done and transforming the test into something closer and more common to them. Besides, it proposes recommendations for the preparation and performance of the test, and for the best management of the maladaptive behaviours of the child when attending hospital.(AU)

Phototherapy with narrow band ultraviolet B is a treatment used in some dermatology units, and is the first choice in some dermatological diseases due to being comfortable and cheap. The aim of this paper is to describe the management and nursing care by grouping more specific diagnoses, following NANDA-NIC/NOC taxonomy, such as the methodology from application, technique, material, and personnel to space-related aspects, with the aim of avoiding the clinical variability and the possible associated risks for the patients, and for the nurses who administer the treatment. The continuity of the same nurse in the follow-up sessions stimulates the relationship between medical personnel and patients, key points for loyalty and therapeutic adherence. This paper examines a consensus procedure with the Dermatology Unit Team and accredited by the Hospital Quality Unit.

Gestión y cuidados de enfermería al paciente dermatológico en tratamiento con fototerapia con rayos ultravioleta B de banda estrecha / Nursing care management in dermatological patient on phototherapy narrow band UVB

La fototerapia con rayos ultravioleta B de banda estrecha es un tratamiento utilizado en algunos servicios de dermatología, siendo de primera elección para algunas dermatosis porque resulta cómodo y económico. El objetivo de este artículo es describir la gestión y cuidados de enfermería agrupando aquellos diagnósticos más específicos según taxonomía NANDA-NIC/NOC, la metodología desde la aplicación, técnica, utillaje y los aspectos relativos al espacio, material, personal, con el fin de evitar la variabilidad clínica y los posibles riesgos tanto para pacientes como para la enfermería que los administra; la continuidad en las sesiones por la misma enfermera favorece la relación de ayuda, clave para la fidelización y la adherencia terapéutica. Este artículo sintetiza un procedimiento consensuado con el equipo del Servicio de Dermatología de la unidad y acreditado por la Unidad de Calidad del Hospital (AU)
