Hemangiomas are most common benign liver tumor. Most patients have an excellent prognosis because of the small size and benign nature of tumor. On some occasions, giant liver hemangioma may cause symptoms and significant challenges due to its complication. We report a case of giant liver hemangioma treated with minimal invasive approach by transarterial embolization (TAE). Following three TAE sessions over a specific timeframe, the patient was successfully managed, addressing that TAE may be a useful alternative to hepatic surgery in such cases.

BACKGROUND: Type 2 diabetes (T2D) is a progressive disease. Many drugs currently being used for the management of T2D have minimal effect on pancreatic beta cells regeneration. Cell-based therapies might provide potential benefits in this aspect. METHODS: A pilot study in five T2D patients with 12 months follow-up was performed to evaluate the effect of autologous bone marrow mononuclear stem cells (BM-MNCs) infusion into pancreatic arteries on the insulin requirement, beta-cell function, insulin resistance, and systemic inflammatory marker (CRP). RESULTS: The primary endpoint, a 50 % reduction of total insulin doses from baseline, was not achieved in this study. However, a trend of increasing fasting C-peptide (p = 0.07) and C-peptide 60' (p = 0.07) and 90' (p = 0.07) after a mixed-meal tolerance test was observed 12 months post-infusion compared to baseline levels. A similar result was observed for the homeostatic model assessment of beta cell function (HOMA1-B), an index for beta cell function. No improvement was observed for insulin resistance measured by homeostasis model assessment of insulin resistance (HOMA1-IR) and systemic inflammatory parameter. CONCLUSION: Intraarterial pancreatic autologous BM-MNCs infusion might potentially improve beta cell function in T2D patients, although further study is needed to confirm this finding.

Percutaneous Nephrolithotomy (PCNL) has been shown as a safe and effective method for treating nephrolithiasis in horseshoe kidney patients. We report the first X-ray Free PCNL with Alken Telescopic Metal Dilator (ATMD) in horseshoe kidney. A 58-Year-Old female was diagnosed with left large staghorn stone and horseshoe kidney treated with X-ray Free PCNL with ATMD. X-ray Free PCNL with ATMD is safe and effective nephrolithiasis lithotripsy for staghorn stone in Horseshoe Kidney. This technique is the first of its kind and possibly has become an excellent alternative for urological centers without access to fluoroscopy.

Tuberculosis is an endemic disease in certain parts of the world. This disease typically presents in the lungs, but it may also appear within the abdomen, such as in the pancreas. There can be challenges in diagnosing isolated pancreatic tuberculosis as it may mimic other diseases radiologically. We present a 33-year-old female with intermittent abdominal pain and weight loss. Chest x-rays showed normal findings while noncontrast abdominal computed tomography (CT) showed a solid cystic mass in the pancreas and in the spleen. Contrast-enhanced CT showed an inhomogeneous cystic mass in the body and tail of the pancreas with peripheral rim enhancement. A laparotomy procedure was performed, and tuberculosis was confirmed histopathologically. In this case report, we highlight the challenging nature of diagnosing isolated pancreatic and splenic tuberculosis due to its presentation, which is similar to other neoplastic processes.

The diagnosing of central obesity requires ethnic-specific cut-offs of waist circumference (WC) and body mass index (BMI). This study aims to develop formulas to predict visceral adipose tissue (VAT) area based on WC and BMI to determine the cut-off points of central obesity in Indonesia. We conducted a cross-sectional study among 32 middle-aged Indonesian men. VAT area was measured using an abdominal CT scan, whereas WC and BMI were assessed through anthropometric measurements. Linear regression analysis was performed to define the formulas to predict VAT area using WC and BMI. Next, the optimal cut-off values of WC and BMI were determined using ROC curve analysis. Strong positive correlations were found between WC and VAT as well as BMI and VAT (r = 0.78; r = 0.67, p <0.001). The formula to predict VAT area from WC was -182.65 + (3.35 × WC), whereas the formula to predict VAT area from BMI was -57.22 + (6.95 × BMI). These formulas predicted WC of 88.5 cm and BMI of 23.9 kg/m2 as the optimal cut-off values for central obesity in middle-aged Indonesian men.

Intra-arterial chemotherapy (IAC) has become the mainstay therapy for retinoblastoma group D and E for the past few decades. However, IAC had never been done in Indonesia before. In this report, we present a case of a 2-year-old girl with recurrent Group D retinoblastoma of the left eye that improved significantly after one session of IAC, the first IAC in Indonesia.

Leiomyomas are benign fibromuscular tumors that arise from smooth muscle cells. Paraurethral leiomyoma accounts for 5% of all paraurethral masses and occurs in 1 in 1000 women. We reported a case of a 43-year-old female presented with a palpable mass at the vaginal introitus with lower urinary tract symptoms. Patient had no pregnancy and sexual history before. A pelvic MRI demonstrated a 1.6 x 1.7 × 2.0 cm mass within the anterior part of the paraurethra. The excision of the mass was done via transvaginal approach and was successful. The final post-operative pathology report was confirmed as leiomyoma.

Bleeding esophageal varices (EV) have the highest mortality rate from all complications of liver cirrhosis (LC). Several Doppler ultrasound (DUS) studies have been done on the splenic or portal vein (PV) to evaluate the hemodynamic of the esophageal vein. Our study focused on finding a better index using the ratio from two parameters correlated with EV, splenic vein flow volume (SFV), and PV flow velocity. In this study, 28 patients with LC were evaluated using DUS to compare the SFV to PV flow velocity/speed (Sv/Ps) index and other measured DUS parameters with the EV degree. Afterward, the receiver operating characteristic (ROC) curve analysis was performed on statistically significant DUS parameters. Mean Sv/Ps index value in the group of nonvarices was 9.89 ± 3.56; 19.50 ± 5.56 in the small esophageal varices (SEV) and 74.12 ± 29.37 in the large esophageal varices (LEV) group with p < 0.001. ROC curve analysis generated an optimal cutoff point of 16.5 (90% sensitivity and 100% specificity) to predict the presence of EV and the cutoff point of 46.7 (100% sensitivity and specificity) to predict the presence of LEV. In conclusion, the Sv/Ps index measured using DUS can be used as a noninvasive method to predict the presence of EV, especially in predicting LEV.

INTRODUCTION AND IMPORTANCE: Renal cell carcinoma (RCC) skin metastasis is a rare disease. However, there are no data on the effect of a Tyrosine Kinase Inhibitor (TKI) on its treatment. CASE PRESENTATION: A 54-year-old male patient with renal cell carcinoma developed subcutaneous metastasis three months after radical nephrectomy and there was no discoloration or pain. Furthermore, an excision biopsy confirmed the metastatic lesion, and pazopanib was initiated as a treatment method. After 1-month of treatment, the patient developed ulceration and subsided after treatment was stopped. Similarly, a follow-up PET scan was performed almost a year after stopping the treatment, which showed improvement over metastatic pulmonary lesions. CLINICAL DISCUSSION: Renal cell carcinoma (RCC) major metastases were observed in pulmonary, costal, and skin. Tumor burden and location of metastasis influences progression free-survival of RCC patients treated with TKI. CONCLUSION: In this case, TKI treatment showed a long-term partial response, despite its lack of continuous therapy.

Radical nephroureterectomy (RNU) with bladder cuff removal is the treatment of choice for upper tract urothelial carcinoma (UTUC). Partial ureterectomy (PU) with ureteroureterostomy in this case has shown a good result. We herein report an elderly woman with adenocarcinoma colon complaining gross intermittent hematuria and solid ureteral mass on NCCT. Patient declined RNU, so we performed PU with ureteroureterostomy. Histology examination showed high-grade infiltrating urothelial carcinoma with negative margin. Four cycles of Gemcitabine and cisplatin were given. Routine follow up and evaluation were done without any mass progression. PU and ureteroureterostomy with adjuvant chemotherapy are an alternative procedure for UTUC.

Seventy percent of anterior prostate cancer cases are diagnosed during rebiopsy. MRI-US fusion transperineal robotic prostate biopsy is an emerging diagnostic method and might be an effective one in diagnosing prostate cancers in difficult sites such as the anterior zone. We report a case of a high grade anterior prostate cancer previously undetected by transrectal biopsy, diagnosed with MRI-US fusion transperineal robotic prostate biopsy. This case report suggests that MRI-US fusion transperineal robotic prostate biopsy might be valuable in diagnosing prostate cancer especially in difficult sites - the anterior region in this case - and might be an imperative diagnostic method in suspicious cases with prior negative biopsy.

INTRODUCTION: Bladder exstrophy is a rare congenital anomaly while, bladder adenocarcinoma mucinous type is a rare type of bladder cancer, with aggressive behavior and inadequate response to radiation and chemotherapy. In extremely rare cases, untreated bladder exstrophy could transform into bladder mucinous adenocarcinoma. CASE PRESENTATION: We report a case of a 41-year-old male with untreated bladder exstrophy that transformed into mucinous adenocarcinoma. The patient also had epispadias and a right inguinal hernia. Joint procedures were conducted to perform radical cystectomy, total penectomy and W-Pouch continent urostomy, inguinal hernia repair, osteotomy, and keystone and scrotal flap by split-thickness skin graft (STSG) for wound closure. The patient progressed well after surgery, two months after initial procedure, nephrostomies were conducted due to pouches stenosis. Due to the government's limited transportation and lockdown policy, as the Covid-19 pandemic occurred, the patient could not come to the hospital for routine follow-up and died nine-month after surgery. CLINICAL DISCUSSION: Bladder exstrophy is one of the risk factors of bladder cancer. Transformation of bladder exstrophy into mucinous adenocarcinoma is extremely rare, as the case is the first case to be discovered in Indonesia. Surgery, followed with a strict follow-up regime, is mainstay of treatment in this type of malignancy. CONCLUSION: Adenocarcinoma of mucinous type is a scarce type of bladder exstrophy malignancies. A multidiscipline approach is mandatory in these cases. Strict and regular follow up are suggested for these cases.

Hepatocellular carcinoma (HCC) is the fourth leading cause of cancer-related deaths in the world, with hepatitis B and C as its main causes. HCC can have fat metamorphosis which leads to a better prognosis, though this is more commonly found in lesions smaller than 3 cm in diameter, and usually contains intravoxel fat. In this case series, we present three cases of HCC with macroscopic fat metamorphosis as examined using CT scan and MRI. Macroscopic fat is seen using CT as a hypodense mass with attenuation of -10 to -100 HU, or MRI using fat-saturation technique. Intravoxel fat can be seen on MRI using fat saturation chemical shift technique, appearing as signal loss during opposed phase and increased signal during in-phase. The differential diagnoses of HCC with fat metamorphosis are angiomyolipoma, hepatic adenoma, nodular steatosis, focal nodular hyperplasia, dysplastic nodule, liposarcoma, and hepatic metastasis. Enhancement patterns of the fat and non-fat component; intra-tumoral fat distribution; the presence of cirrhosis; the presence of atoll sign; and history of viral hepatitis are useful clues for differentiation of HCC with other differential diagnoses.

Diabetes mellitus (DM) remains one of the most important risk factors for peripheral artery disease (PAD), with approximately 20% of DM patients older than 40 years old are affected with PAD. The current standard management for severe PAD is endovascular intervention with or without surgical bypass. Unfortunately, up to 40% of patients are unable to undergo these revascularization therapies due to excessive surgical risk or adverse vascular side effects. Stem cell therapy has emerged as a novel therapeutic strategy for these 'no-option' patients. Several types of stem cells are utilized for PAD therapy, including bone marrow mononuclear cells (BMMNC) and peripheral blood mononuclear cells (PBMNC). Many studies have reported the safety of BMMNC and PBMNC, as well as its efficacy in reducing ischemic pain, ulcer size, pain-free walking distance, ankle-brachial index (ABI), and transcutaneous oxygen pressure (TcPO2). However, the capacity to establish the efficacy of reducing major amputation rates, amputation free survival, and all-cause mortality is limited, as shown by several randomized placebo-controlled trials. The present literature review will focus on comparing safety and efficacy between BMMNC and PBMNC as cell-based management in diabetic patients with PAD who are not suitable for revascularization therapy.

PURPOSE: Ovarian tumours are the second most common cause of death from gynaecological cancer. There are three types of ovarian cancer based on histopathological examination: benign, borderline, and malignant. However, it is difficult to distinguish the borderline and malignant tumours. Several studies used the apparent diffusion coefficient value to distinguish the ovarian tumour types, with various results. This preliminary report focused more on the use of the minimal ADC (mADC) value on the solid component, to differentiate borderline and malignant ovarian tumours. MATERIAL AND METHODS: In 21 cases of borderline ovarian tumours, of which 11 were regarded as malignant and 10 were regarded as borderline following histopathological examination, the mADC value was measured by two different radiologists by using free-hand technique. The intraclass correlation coefficient (ICC) was used to measure the reliability and agreement between the two radiologists. Receiver-operating characteristic (ROC) curves were then calculated to determine the optimum cut-off point. RESULTS: There were statistically significant (p = 0.001) of the mADC value between the borderline and malignant tumours. The intraclass correlation coefficient value showed excellent reliability and agreement between the examiners. The ROC curve showed the optimum cut-off point at 0.628 × 10-3 mm2/s (p = 0.001), which yielded 100% sensitivity and 80% specificity. CONCLUSIONS: The use of free-hand technique to measure the mADC value on the solid component can be valuable in differentiating borderline and malignant ovarian epithelial tumours. This result may assist clinicians in considering further treatment approaches.

Rhabdomyosarcoma of the prostate is rarely found in adults compared to rhabdomyosarcoma in other body regions. In early stage of the tumor, the symptoms are similar to other genitourinary problems, such as dysuria, urinary retention, lower abdominal pain and prostate enlargement. This condition often leads to misdiagnosis and treatment delay because of the tumor's rapid growth. Asides from the histopathological examination, imaging had an important role in determining the diagnosis. MRI is very useful and more preferred than other imaging modalities in determining the diagnosis.

INTRODUCTION: Primary tubal cancer is very rare, most are diagnosed intra and post operatively. Histopathology is vital in determining the cancer origin. Here we present a case of fallopian tube cancer with clinical presentation mimicking endometrial origin. CASE DESCRIPTION: A 74-year old patient came with complaints of intermittent post-menopausal bleeding and pelvic pain. The patient had several investigations using Ultrasonography, Hysteroscopy-guided biopsy, and Magnetic Resonance Imaging. Pre-operative diagnosis was endometrial cancer based on histopathology of endometrial biopsy during hysteroscopy. Explorative laparotomy, total abdominal hysterectomy, bilateral salphingo-oophorectomy, pelvic and para-aortic lymph node dissection were then performed, and the tumor samples were sent to the histopathology laboratory. It was found that the post-operative diagnosis was in fact primary fallopian tube cancer stage IIB. CONCLUSION: For patients with gynecological malignancies, rare cases such as fallopian tube cancer should never be overlooked as a differential diagnosis.

Hepatocellular adenoma is an extremely rare benign tumor of the liver which predominantly in young women. Its rare incidence with estimated 3-4 cases per 1.000.000 annually makes it a diagnostic challenge. Here we present a 30-year-old female patient with hepatocellular adenoma without classic risk factors. A series of work up tools have been performed in order to diagnose the condition. None but excision biopsy from segmental resection had been showed to increase diagnostic confidence. This case illustrates the role of immunohistochemical staining from excision biopsy as the best diagnostic modality of hepatocellular adenoma as well as therapeutic modality to prevent malignant transformation.

Hepatocellular carcinoma (HCC) is the fifth most prevalent and the second highest cause of death among cancer. The treatment of large solitary nodule HCC is still challenging. Transarterial chemoembolization (TACE) and liver resection are two modalities of therapy in HCC management. However, recurrence rate from each therapy is relatively high. We report a case of 46-year-old man diagnosed with large solitary nodule HCC, who was treated with drug eluting bead TACE (DEB-TACE) prior to liver resection. Studies about this combination are still limited and showed various results.

Muscle hematomas are rare complications in dengue hemorrhagic fever (DHF). We report a case of 58-year-old-female admitted with dengue fever who developed spontaneous rectus sheath hematoma complicating DHF. She presented with progressive thrombocytopenia with platelet count reaching 13000/µL at its lowest point. There was evidence of plasma leakage and persistent cough during the course of illness. During the recovery phase, she reported severe abdominal pain and developed hematoma in the right rectus sheath, which was confirmed by abdominal computed-tomography scan and serial magnetic resonance imaging. This complication during convalescent period of DHF needs to be recognized so it can be managed appropriately.
