RESUMEN

Objectives: This study compares the bacterial susceptibility to three different provisional restorations manufactured with different laboratory techniques, i.e., heat cure, CAD/CAM, and 3D printing. Materials and Methods: Totally, 45 health patients with age group of 30-60 years undergoing complete veneer crown treatment were selected for the study and divided into three groups with 15 samples in each as Group I: the crowns were made from polymethyl methacrylate polymer fabricated by the conventional heat cure method, Group 2: the crowns were made from polymethyl methacrylate fabricated by subtractive manufacturing CAD/CAM method, and Group 3: the crowns were made from oligomers fabricated by additive manufacturing 3D printing. The samples were examined for bacterial adherence at scanning electron microscope. The colony-forming units (CFUs) were calculated, and statistically analyzed. Results: It has been determined that the digitally fabricated provisional restorations prove to be superior in terms of surface topography than to the conventionally fabricated provisional restorations. Conclusion: The study concludes that 3D-printed provisional restorations are more precise with reduced bacterial susceptibility than milled ones.

AIM: Implant therapy in partially edentulous patients has become the most adapted and predictable treatment modality. The success rate of implants has been measured in terms of esthetic, biological, and technical factors such as radiographic bone loss, prosthetic complications, and stability. Despite the existence of several indices for the esthetic assessment of implant crowns, a need for functional evaluation of the implant crown with an objective and reproducible score has arisen. The study aims to validate the reproducibility of the functional implant prosthodontic score (FIPS) and the influence exerted by different dental specialties while evaluating posterior single-unit implant crowns. STUDY SETTING AND DESIGN: This was a prospective clinical study. MATERIALS AND METHODS: Fifteen patients with cement-retained single-implant crowns in the posterior region of the jaws were included. Eight examiners, two prosthodontists, two periodontists, two oral surgeons, and two orthodontists evaluated 15 photographs of single-unit implant crowns during the 1-year follow-up examination. The examiners assessed the photographs for FIPS, which includes five parameters for objectively evaluating the single-unit implant crowns. Assessments were performed twice at a gap of 4 weeks. STATISTICAL ANALYSIS USED: Pearson's correlation with a 95% confidence interval was calculated for the intra-examiner and the Kruskal-Wallis test for inter-examiner reproducibility. RESULTS: The mean total FIPS scores for all included examiners were 7.133 for time T1 and 7.074 for time T2, showing a strong Pearson correlation coefficient for intra-examiner reproducibility. No significant difference was analyzed among different specialties with statistically significant values of the Kruskal-Wallis test. CONCLUSION: Intra- and inter-examiner analysis showed very consistent results during the reproducibility assessment of FIPS. The results validated the use of FIPS as a long-term predictive functional evaluation tool for the single-implant crowns in posterior sites irrespective of the effect of different dental specialties. It could be used for risk estimation and prognosis for long-term survival and performance of implant crowns.

AIM: (1) To compare the temperature rise in the pulp chamber with different resin materials used for making provisional fixed partial dentures in anterior and posterior region while using Polyvinylsiloxane impression materials as matrix. (2) To identify a superior provisionalization material based on the amount of heat dissipated suitable for anterior and posterior provisional fixed partial denture fabrication. SETTINGS AND DESIGN: Temporary crowns and bridges are integral to Fixed Prosthodontics. It has been observed that conventional fixed prosthesis temporisation materials release heat due to the exothermic polymerisation reaction. When such a provisional material is directly let to set on a vital tooth, the heat transfer causes irreversible changes in the pulp tissue depending of the degree of change. Hence, this study observes amount of heat generation in various materials during temporisation procedure, by simulating similar conditions. MATERIALS AND METHODS: Two Models were fabricated, one simulating missing lateral incisor (Model A) and another simulating missing first molar (Model B). Intact maxillary central incisors and canine for Model A and intact mandibular Second Premolar and Second Molar were selected to act as abutments. These abutment teeth were fitted with the tip of a K-type Thermocouple inside their pulp chambers and these were connected to a digital thermometer. Five temporisation materials were chosen for fabrication of temporary crowns through Direct technique. (1) polymethy methacrylate (Self Cure acrylic), (2) bisacryl composite (Protemp 4), (3) visible light cure urethane dimethacrylate (Revotec LC), (4) barium glass and fumed silica infused methacrylate (Dentsply Integrity) and (5)nano-hybrid composite (VOCO Structur 3). Ten observations were made for each provisional material on each model. During each observation, temperature rise was recorded at 30s interval from the time of application, through the peak and till a decrease in temperature is observed. Polyvinyl siloxane was used as matrix for all except light cure resin, where polypropylene sheet was used. STATISTICAL ANALYSIS USED: Anova test used for statistical. RESULTS: ANOVA test revealed that there was a significant difference in the temperature changes associated with the provisional restorative materials used. Among the five, polymethy methacrylate (self cure resin) showed the maximum rise in temperature, followed by bisacryl composite (Protemp 4), visible light cure urethane dimethacrylate (Revotec LC), barium glass and fumed silica infused methacrylate (Dentsply Integrity) and nano-hybrid composite (VOCO Structur 3). There was no comparable difference between Model A and B but an overall reduction of temperature rise was observed in model B. CONCLUSION: VOCO Structur 3 showed the least temperature rise in the pulp chamber, and overall temperature rise was less for model B which can be attributed to the residual dentin thickness.

With the advancement in the implant in dentistry, improvement in the implant designs and placement protocol has enhanced the esthetics outcomes in the anterior zone. Yet the preservation of the peri-implant soft tissue and providing an appropriate emergence profile to the implant crown prosthesis, the tissue grafting procedures are necessary to overcome the ridge contour change. However through the socket-shield technique, the bone resorption process is preserved, and the contour of the buccal gingiva is maintained, thereby preventing its collapse and achieving good aesthetic results. This case report describes the placement of an implant in the upper anterior region and rehabilitation with a cement-retained crown prosthesis using the socket-shield technique and the patient being followed up for 6 months with good results.

AIM: The purpose of this study was to compare the stress distribution around various thread design implants with or without platform switching in the maxillary posterior region. MATERIALS AND METHODS: Stress-based performances of four different thread design implants (single, double, triple, and asymmetric thread design each with or without platform switching) were analyzed by the three-dimensional finite element method under a static load of 100 N at 15° oblique direction buccolingually at the central portion of the abutment. A geometric model of the posterior maxillary segment (first molar region) with an implant and abutment was modeled using the CATIA V5R19 software. Type IV bone quality was approximated and complete osseous integration was assumed. RESULTS: The von Mises stresses recorded around the neck of the fourthread design implants without platform switching were greater than the platform switching variety. The single-threaded implant with platform switching showed the lowest amount of von Mises stress. Additionally, total displacement or micromovement of single, triple, and asymmetric thread implants with platform switching was found to be greater than the without platform switching variety. Further, the total displacement of the single-threaded implant without platform switching was lowest. CONCLUSION: Implant surface design, platform switching, and site of placement affect load transmission mechanisms. Due to low crestal resorption, single thread design with platform switching is preferred. The success of an implant in the maxillary molar region is more challenging in terms of the density of bone and the worst load transfer mechanism. With the right kind of implant surface design selection, this can be reduced to a great extent by the preservation of crest of the ridge. CLINICAL SIGNIFICANCE: Crestal bone resorption following implant placement is an important issue. An optimum implant design with a single thread having a platform switch could compensate for this issue to a great extent.

INTRODUCTION: The dentist must be aware of this drug use in their individual patients to: (1) avoid possible contraindications during dental treatment, (2) be aware of the many oral and craniofacial manifestations of such drug use, (3) be able to provide necessary dental treatment to combat the dental/oral ravages of drug use, and (4) be able to refer such patients, if so desired by the patient. AIMS AND OBJECTIVES: The aim of the study is (1) To determine the knowledge of abusive drugs among dentists and (2) To determine the attitude and role of dentists in identifying patients with abusive drugs. SUBJECTS AND METHODS: A modified 27-item questionnaire was formulated and distributed among the study. The participants were to return the filled questionnaire to the investigators within a week. A total of 203 validated entries were collected. Data were entered into Microsoft Excel 2007 and analyzed in SPSS V20. Associations between categorical variables were determined using Chi-square or Fisher's exact test. P < 0.05 was considered statistically significant. RESULTS: Irrespective of the educational qualification drugs are perceived harmful with definite oral manifestations, and dentists should be concerned with identifying individuals with drugs. It is believed that trying drugs once could lead to possible addiction and that dental practitioners should have their skills developed to handle cases and referred to deaddiction centers with modification of treatment plans. CONCLUSIONS: Educating dental graduates and postgraduates about the oral implications of drugs intake and making it a part of the dental curriculum may help us dealing with the global issues of drugs. Even making dental students a part in counseling and part of the behavioral therapies advocated in treating drug addicts.

BACKGROUND: Dental implants are associated with failure such as early or late failure. Systemic conditions such as diabetes, hypertension, and bruxism affect the success rate. The present study was conducted to assess complications in dental implants in bruxism patients. MATERIALS AND METHODS: This 5-year retrospective study was conducted on 450 patients (640 dental implants) who received implants during the period and followed up for 5 years from June 2010 to June 2016. Among these patients, 124 had bruxism habit. Dental radiographs or patients' recalled records were evaluated for the presence of complications such as fracture of implant, fracture of ceramic, screw loosening, screw fracture, and decementation of unit. RESULTS: In 240 males and 210 females, 380 implants and 260 implants were inserted, respectively. The difference was statistically nonsignificant (P = 0.1). A total of 145 screw-type and 130 cemented-type fixations had complications. The difference was statistically nonsignificant (P = 0.5). Complications were seen in single crown (45), partial prostheses (125), and complete prostheses (105). The difference was statistically significant (P = 0.012). The common complication was fracture of ceramic (70) in cemented-type fixation and fracture of ceramic (85) in screw-type fixation. The difference was statistically significant (P = 0.01). Forty-two single crowns showed decementation, 85 partial prostheses had fracture of ceramic/porcelain, and 50 complete prostheses showed fracture of ceramic/porcelain. The failure rate was 42.9%. Survival rate of dental implants in males with bruxism habit was 90% after 1 year, 87% after 2 years, 85% after 3 years, 75% after 4 years, and 72% after 5 years. Survival rate of dental implants in females with bruxism habit was 92% after 1 year, 90% after 2 years, 85% after 3 years, 75% after 4 years, and 70% after 5 years. The difference among genders was statistically nonsignificant (P = 0.21). CONCLUSION: Bruxism is a parafunctional habit which affects the survival rate of dental implants. There is requirement to follow certain specific protocols in bruxism patients to prevent the developing complications.

Eye is a vital organ not only for vision, but also an important component of facial expression, and over-all personality of a person. Loss of eye, apart from leading to impaired vision has a crippling effect on the psychology of the patient. Prosthodontic rehabilitation of such cases includes fabrication of prosthesis by acrylic resin, silicone and implants. However, not all patients are willing to use implants for maxillofacial rehabilitation. Therefore, a custom made orbital prosthesis serves as an affordable and satisfactory alternative.

AIM: This study was conducted to evaluate the antimicrobial activity of 5.25% sodium hypochlorite (NaOCl), 2% chlorhexidine (CHX) and BioPure MTAD when used as a fnal rinse against Enterococcus faecalis. MATERIALS AND METHODS: Sixty single-rooted premolars were biomechanically prepared, inoculated with E. faecalis and divided into various groups. These were then irrigated with the test irrigants and tested microbiologically for growth of E. faecalis immediately after irrigation and after 48 hours. RESULTS: Statistical analysis showed that there was a signifcant difference between the antibacterial activities of BioPure MTAD, 2% CHX and 5.25% NaOCl at 5 minutes; however, the antibacterial activities of the three irrigants were comparable after 2 days of irrigation. CONCLUSION: The present study concludes that BioPure MTAD is as effective against E. faecalis as 5.25% NaOCl and more effective than 2% CHX. CLINICAL SIGNIFCANCE: E. faecalis is one of the most resistant intracanal species and a possible cause of root canal failure. Many authors have stressed the importance of using antimicrobial irrigants during chemomechanical preparation to ensure complete disinfection. Therefore, various irrigating solutions have been used during and immediately after root canal preparation to remove debris and necrotic pulp tissue and to eliminate microorganisms that cannot be reached by mechanical instrumentation.
