INTRODUCTION: Pemphigus is a chronic inflammatory and autoimmune disease which can cause blisters and mucocutaneous erosions. ABO secretor refers to those who secrete ABO blood group antigens based on their blood type in body fluids such as saliva, sweat, tears, semen, and serum. Previous studies showed that nonsecretor people are more prone to certain autoimmune diseases. AIM: The aim of this study was to determine the ABO secretor status in the saliva of patients with pemphigus vulgaris. MATERIALS AND METHODS: This case-control study was conducted on 35 patients with pemphigus vulgaris and 35 healthy controls. The two groups were matched for age and gender. Pemphigus vulgaris diagnosis was confirmed by histopathology and direct immunofluorescence microscopy. ABO blood grouping was done, and 5 ml of unstimulated saliva was collected to determine secretor status. Secretors were recognized from nonsecretors by the Wiener agglutination inhibition test. Results were extracted by using statistical chi-square and Fisher's exact tests. RESULTS: 16 male and 19 female patients aged 49.43 ± .12.37 years were compared with 16 male and 19 female controls aged 46.43 ± 11.88 years. The most frequent blood group among case and control groups was O (54.3% and 60%, respectively). There was no significant difference in blood groups (P=0.73). 90% of the samples were ABO secretors. The patient group included 31 (88.6%) and the control group included 32 (91.4%) ABO secretors; there was no significant difference between the two groups (P=1.000). CONCLUSION: In this study, we observed that the people with nonsecretor status in comparison with the people with secretor status are not more susceptible to develop pemphigus vulgaris.

Introduction: Lasers are becoming the standard of care for many dental procedures, and are being introduced as a high-tech instrument. They are also becoming more routine in dentistry through the advent of office-based lasers, which are also simple to use within the oral cavity. Many studies have shown the competencies of laser technology for the management of benign oral lesions as these techniques allow for painless and bloodless oral surgery. Cases Report: Nine patients attending the Department of oral and maxillofacial Disease, Shahid Beheshti University of Medical Sciences, Tehran, Iran, underwent laser outpatient procedures for the surgical removal of a wide range of benign oral lesions. Regarding the history, present illnesses and clinical figures of all reported cases, the clinician ensured that the lesions were benign and after taking informed consent from every patient, he did complete the excision of lesions with diode lasers. Following the injection of local anesthesia, an 810 nm diode laser was applied for the excisional biopsy of oral lesions. The specimens were sent for histopathological evaluations and the patients were assessed on intraoperative and postoperative complications. The patients were followed up for postoperative complications at one week and 2 weeks post-treatment. Conclusions: According to our findings, a diode laser can be a choice for the outpatient treatment of oral mucosal benign lesions as this technique provides painless and almost bloodless treatment.

BACKGROUND: Candida species are the most common opportunistic fungal infections. Today, cinnamon plants have been considered for anti-Candida properties. AIM: This study aimed to investigate the effectiveness of cinnamaldehyde extract (from cinnamon derivatives) on Candida albicans and Candida glabrata species and comparison with nystatin. MATERIAL AND METHODS: In this study, cinnamaldehyde and nystatin were used. The specimens included Candida albicans and Candida glabrata. Minimum inhibitory concentration (MIC) and minimum fungicidal concentration (MFC) were measured for each one by the microdilution method. This experiment was repeated three times. RESULTS: Cinnamaldehyde extract at a concentration of 62.5 µl/ml was able to prevent the growth of Candida albicans, at a concentration of 93.7 µl/ml, causing Candida albicans to disappear, at 48.8 µl/ml, to prevent the growth of Candida glabrata, and in the concentration of 62.5 µl/ml, causes the loss of Candida glabrata. In comparison, nystatin at 0.5 µg/ml concentration prevented the growth of Candida albicans, at concentrations of 1 µg/ml causing Candida albicans to be destroyed, at 4 µg/ml concentration to prevent the growth of Candida glabrata, and at a concentration of 8 µg/ml causes the loss of Candida glabrata. The results were the same every three times. CONCLUSIONS: Although cinnamaldehyde extract had an effect on fungal growth in both Candida albicans and Candida glabrata with a fatal effect; the effect on these two species was lower than nystatin.

The present study deals with the fabrication of ibuprofen-mesoporous hydroxyapatite (IBU-MHA) particles via the incorporation of ibuprofen (IBU)-as a nonsteroidal anti-inflammatory drug-into mesoporous hydroxyapatite nanoparticles (MHANPs) using an impregnation process, as a novel drug delivery device. MHANPs were synthesized by a self-assembly process using cetyltrimethylammonium bromide (CTAB) as a cationic surfactant and 1-dodecanethiol as a pore expander under basic condition. The focus of the present study was to optimize the incorporation of IBU molecules into MHANPs under different loading conditions. The synthesized MHANPs and IBU-MHA particles were confirmed by X-ray diffraction (XRD), fourier-transform infrared spectroscopy (FTIR), brunauer-emmett-teller (BET), transmission electron microscopy (TEM), and thermal analysis (TGA). Drug loading (DL) efficiency of IBU-MHA particles was determined by ultraviolet-visible (UV-Vis) spectroscopy, and indicated that the optimized IBU-MHA particles with high DL (34.5%) can be obtained at an IBU/ MHANPs ratio of 35/50 (mg/mg), impregnation period of 24 h, and temperature of 40 °C using ethanol as solvent. In-vitro drug release test was carried out to prove the efficiency of IBU-MHA particles as a sustained drug delivery system. A more sustained and controlled drug release was observed for this particles, indicating that it may be have good potential as drug reservoirs for local drug release.

Background: Late diagnosis and improper treatment lead to many avoidable deaths in patients suffering from oral cancer. This study evaluated the knowledge and perceptions of general dentists regarding oral precancerous lesions and oral cancer risk factors with reference to early diagnosis and referral. Materials and Methods: In this descriptive study, 200 private general dental practitioners were randomly selected in Tehran, Iran. Demographic information and years since graduation were recorded; knowledge levels were evaluated with a standardized questionnaire consisting of 15 closed-response item questions about precancerous lesions and oral cavity cancer. Each correct answer got one point. Results: Out of 200 selected dentists, 153 returned completed questionnaires. The average age of participants was 36 and the average achieved score was 7.96 out of 15 (Min 3, Max 13). Most (64.7%) of participants were in the group that graduated within the previous 10 years, 23.5% between 10 and 20 years since graduation, and 11.8% more than 20 years previously. There was a statistically significant difference between the most recently graduated group (< 10 years) and the other two groups. The dentists' awareness was also significantly dependent on age. There was no statistically significant difference in awareness between male and female dentists. Conclusion: As professional oral health care providers, dentists should be aware of all aspects regarding precancerous lesions in order to optimize early diagnosis and referral of oral cancer patients. Training courses and continuing education seem to be an essential strategy to increase and maintain knowledge of this group.

INTRODUCTION: Different modalities have been used to achieve better results in wound healing among which are laser and pharmacological agents. To our knowledge, there is no investigation to compare phenytoin and laser together. In this study, we aimed to evaluate laser and phenytoin efficacy and compare their effect on wound healing. METHODS: We used 60 rats. The animals were divided into 4 groups, 15 rats in each and each group was subdivided into 3 subgroups for which postoperative evaluation was performed on days 3, 7, and 14. After general anesthesia and back hair shaving of each rat, an excisional wound was made. The wounds in group 2 were treated with 1% phenytoin cream. In group 3, the wounds were irradiated with diode laser. The wounds of group 4 were treated with both phenytoin cream and laser administration. The animals of group 1 served as control. For histopathological evaluation, hematoxylin and eosin staining was used. Polymorphonuclear leucocytes (PMNs), re-epithelization, fibroblasts, new vessels and collagen were evaluated. RESULTS: Faster reduction in polymorphonuclear cells after 7 and 14 days and more collagen fibers on the third day in laser treated group was statistically significant (P < .05) .The result for the phenytoin treated group were not better than control group. When both laser and phenytoin were used samples showed more collagen fibers on the third day, which was statistically significant (P < .05). CONCLUSION: It seems that low level laser is effective in skin wound healing and have better results than phenytoin application.

Background and aims. Cigarette smoke can induce oral cancer by its free radicals and oxidative damage. Salivary anti-oxidants system is believed to have an important role in defense mechanisms against oxidative stress. This study was compared total antioxidant capacity (TAoC) of saliva in smokers and nonsmokers. Materials and methods . In this cross-sectional study, 30 male smokers with mean age of 45.23 years and 30 nonsmokers with mean age of 45.30 years participated. Unstimulated whole saliva samples were collected in the morning in two groups by spitting method. TAoC of saliva was measured with the special kit in two groups at the same time. Statistical analysis was performed by covariance test. Results. The mean salivary TAoC in nonsmokers (0.741±0.123 U/ml) was higher than that in smokers (0.529±0.167 U/ml). This difference was statistically significant (P<0.001). Conclusion. Smoking can alter salivary antioxidant capacity.

BACKGROUND AND AIMS: Burning mouth syndrome (BMS) is a chronic pain disorder characterized by a chief complaint of oral burning/pain with no clinically observable oral mucosal lesions. The prevalence of BMS has been reported to be 2.5-5.1% in the general population and several psychological disorders have been reported as associated or predisposing factors for BMS. The aim of this study was to determine the correlation between BMS and anxiety in the elderly residents of sani-taria in Tehran, Iran. MATERIALS AND METHODS: In a cross-sectional analytical study, 50 patients with BMS were included along with 50 healthy individuals as controls. Inclusion criteria were: age over 60 years, burning sensation in the mouth, normal oral mu-cosa, absence of diabetes, satisfactory prosthesis, absence of iron deficiency and other nutrients, and no heavy smoking habits. Similar inclusion criteria were considered for the control group without any oral complaints. Burning/pain severity was assessed by a 100-mm visual analog scale and the severity of anxiety (state, trait, and total) was determined by Cattell's Anxiety Scale. Data was analyzed by t-test and Pearson's correlation coefficient. RESULTS: Individuals with BMS assessed their burning/pain severity to be 27.4 mm (95% CI=25.2 - 29.6 mm). Standard-ized total anxiety scale in individuals with and without BMS (±SE for mean) was 5.9±0.2 and 4.6±0.3 with significant dif-ferences (p=0.001). Similar significant differences were observed in state and trait anxiety between the two groups. CONCLUSION: It seems that both state and trait anxiety are associated with burning mouth syndrome.

Low-power lasers are a group of lasers with a power less than 250 mW and unlike high-power lasers they have no effect on tissue temperature; they produce light-dependent chemical reactions in tissues. These lasers have analgesic features with their ability to trigger reactions that reduce pain and inflammatory mediators. Low-power lasers can also be used instead of needles in acupuncture to decrease pain. Due to these features they have been used in the treatment of orofacial pain, including tooth hypersensitivity, post-operative flare-ups, mucositis, facial myalgia, temporomandibular joint disorders and neuralgia. In this article we review the effects of low-power lasers and their success rate in different studies. As the name implies (LASER: Light Amplification by the Stimulated Emission of Radiation), laser amplifies light by stimulated and excited radiation; in other words, it is amplification of excited light emission. Such radiation usually has some characteristic features, including mono-chromaticity, coherency, high intensity and polarity. There are various classifications for lasers based on their active material (solid, fluid and gas), wavelength, emission type and power.
