Antecedentes y objetivo: El carcinoma de células de Merkel es un tipo de cáncer de piel infrecuente y agresivo. Hay una gran variación en su manejo y las diferentes guías extranjeras que existen cubren parcialmente los problemas identificados como principales. El objetivo de la presente guía es servir de referencia a los dermatólogos españoles para mejorar aspectos controvertidos del diagnóstico, estadificación y tratamiento del carcinoma de células de Merkel. Materiales y métodos: Se empleó el método ADAPTE: se escogió a miembros del Grupo Español de Dermato-Oncología y Cirugía (GEDOC) con experiencia en el tratamiento de estos tumores y con interés en participar en la elaboración de la guía. Tras resumir el proceso de atención y elaborar las preguntas clínicas relevantes, se hizo una búsqueda de guías, que se seleccionaron según su puntuación mediante el instrumento Appraisal of Guidelines for Research and Evaluation (AGREE II). Tras la búsqueda de las respuestas en dichas guías, se elaboraron posteriormente las recomendaciones. Por último, se sometió la guía a revisión externa. Resultados: Las guías con mejor puntuación fueron las de National Comprehensive Cancer Network, la European consensus-based interdisciplinary guideline, Alberta Healthservices Clinical practice guideline, American Cancer Society y Cutaneous Oncology Group of the French Society of Dermatology. Se obtuvieron en total 9 preguntas clínicas, contestadas a partir de estas guías. Conclusiones: Esta guía responde a preguntas habituales en la práctica clínica diaria y sirve a los dermatólogos como referencia en la toma de decisiones, siempre teniendo presentes los recursos y preferencias del paciente

La misión principal de la cirugía dermatológica oncológica es la extirpación completa del tumor cutáneo. Tras ello se procederá a la reconstrucción del defecto intentando preservar la funcionalidad y la estética. Al nivel de las comisuras se entrecruzan las fibras del músculo orbicular de los labios superior e inferior, y el bermellón se inserta en el modiolo, una compleja estructura con forma de cono. La reconstrucción de la comisura oral supone un reto quirúrgico, ya que hemos de respetar la función esfinteriana y la simetría. Presentamos varios casos de defectos quirúrgicos situados en la comisura bucal y la solución reconstructiva por la que optamos, así como una propuesta de algoritmo reconstructivo
The main objective of oncologic dermatologic surgery is the complete excision of skin tumors. During reconstruction of the defect, we must attempt to preserve function and cosmetic appearance. At the labial commissure, fibers from the superior and inferior portions of the orbicularis oris muscle intersect and the vermillion inserts onto the modiolus, a complex, cone-shaped structure at the angle of the mouth. Surgical reconstruction of the labial commissure is challenging because of the need to preserve sphincter function and cosmetic symmetry. We present a number of cases of surgical defects at the labial commissure and describe the reconstruction techniques used. We also propose an algorithm to help determine the most suitable technique for different cases

El colgajo de pedículo subcutáneo en V-Y es útil en la cobertura de defectos de pequeño-mediano tamaño en áreas donde es fácil obtener un buen pedículo subcutáneo (labio superior, región malar, ciliar, punta y alas nasales). La práctica ausencia de tejido subcutáneo en la cara anterior del pabellón auricular condiciona su uso en esta localización. Presentamos 4 casos en los que utilizamos este colgajo para la reparación de defectos quirúrgicos ubicados en hélix, fosa escafoidea y antitrago, con un buen resultado funcional y estético
The subcutaneous pedicled V-Y advancement flap is useful for the repair of small and medium-sized defects in areas where it is easy to obtain a good subcutaneous pedicle (upper lip, cheek, eyebrow, and nasal tip and ala). The almost complete absence of subcutaneous tissue on the anterior aspect of the auricle of the ear can limit the use of this approach in this region. We present 4 patients in whom subcutaneous pedicled V-Y advancement flaps were used to repair surgical defects of the helix, scaphoid fossa, and antitragus, achieving a good functional and aesthetic result in all cases

La mejilla es la subunidad anatómica mayor de la cara. Es una estructura bilateral, por lo que es necesario respetar la simetría. Por otra parte contacta periféricamente con importantes orificios naturales cuya localización debemos respetar en la reconstrucción quirúrgica. Esto es particularmente importante en la subunidad cigomático-medial, cuya delicada unión con el párpado inferior hace que tengamos que ser cuidadosos para evitar un ectropión. Presentamos 5 opciones reconstructivas de defectos quirúrgicos secundarios a la extirpación de otros tantos tumores en esta localización
The cheek is the largest anatomical subunit of the face. It is a bilateral structure and symmetry must therefore be preserved. Peripherally it is related to important natural orifices whose location must also be maintained during surgical reconstructions. This is particularly important in the medial zygomatic subunit, whose delicate junction with the lower eyelid means that care must be taken to avoid ectropion. We present 5 options for the reconstruction of surgical defects secondary to the excision of tumors in this region
