RESUMEN

Objective To investigate the correlation between ADC value and glioma IDH-1/1p19q genotype. Methods The MRI features and molecular pathological results of 69 patients with pathologically confirmed diagnosis of WHO grade Ⅱ/Ⅲ glioma between March 2013 and December 2020 were retrospectively analyzed. The diagnostic performance of ADC values on glioma genotypes (IDH-1, 1p19q) was evaluated using the ROC curve of the subjects' working characteristics. Results The ADCmean, ADCmin, rADCmean, and rADCmin in the IDH-1 mutation group were significantly higher than those in the IDH-1 wild group (P < 0.05, P < 0.01, P < 0.05, P < 0.01). The use of the rADCmin threshold (0.979×103mm2/s) had the highest efficacy (AUC=0.770) for diagnosis of IDH-1 mutant and IDH-1 wild-type gliomas as well as sensitivity and specificity of 84.61% and 59.09%, respectively. Conclusion ADC can be used as an imaging biomarker for noninvasive prediction of IDH-1 mutant and wild-type Ⅱ /Ⅲ gliomas.

RESUMEN

Lymphocyte subsets and tumor-associated macrophages, which are the primary immune cells in the tumor microenvironment, interacts with its released cytokines to form the immunological microenvironment. It has grown to be a significant factor in the recurrence and metastasis of cervical cancer and influences the effectiveness of concurrent radiotherapy and chemotherapy for the disease, which in turn influences the prognosis and outcome of patients. Immunotherapy and targeted therapy for cervical cancer based on the immune microenvironment have grown in popularity as research topics in recent years.

RESUMEN

The occurrence and progression of cervical cancer are influenced by gut flora. Patients with cervical cancer have different gut flora from healthy women, and the detection and evaluation of gut flora can help in the diagnosis, immunotherapy, assessment of radiotherapy efficacy and prognosis prediction of cervical cancer. Regulation of gut flora is of great clinical value in enhancing response to anti-tumor therapy, improving patients' quality of life and improving prognosis.

RESUMEN

Immune checkpoint inhibitors in the treatment of head and neck squamous cell carcinoma (HNSCC) has shown significant clinical benefit. Some studies have shown that radiotherapy combined with immunotherapy can produce synergistic effects, and several phase Ⅰ/Ⅱ clinical trials have suggested that radiotherapy combined with immunotherapy has good safety and preliminary efficacy benefits in locally advanced HNSCC. However, the timing of combination therapy, the selection of radiotherapy dose/fractionation mode and patients are still unclear. This article further discusses the synergistic mechanism, clinical research status and challenges of radiotherapy combined with immunotherapy in the treatment of HNSCC, aiming to guide the clinical practice and improve the prognosis of HNSCC patients.

Gliomas,as the most common central nervous system tumors,still have a poor prognosis for patients after compre-hensive treatment with surgery,radiotherapy,and chemotherapy.The occurrence,development,invasion and treatment resistance of tumors are closely related to the immune infiltrating cells in the tumor microenvironment.Macrophages/microglia are the main immune-infiltrating cells in the microenvironment of gliomas,and significant progress has been made in their research in recent years,provi-ding new ideas for the treatment of gliomas.This article reviews the research progress of macrophages/microglia in the microenviron-ment of glioma.

Quantifying the allocation of photosynthetic products among different carbon (C) pools is critical for understanding and predicting plant C turnover response to climate change. A field experiment with ectomycorrhizal fungi (EMF) and nitrogen (N) was established to investigate the effects on allocation of photosynthetic products in Pinus massoniana (Lamb.) seedlings given increased N deposition. Seedlings were subjected to N addition and symbiosis with EMF, and the short-term allocation of a 13C photosynthetic pulse into leaves, branches, stems, roots, and soil was traced. Photosynthetic rate and root respiration were measured. It was found that N addition changed the allocation pattern of photosynthetic products in various organs of P. massoniana. Furthermore, N addition, mycorrhizal symbiosis, and interaction of N and EMF, all increased the amount of C produced by photosynthesis. N application less than 60 kg N hm-1 a-1 could promote the transfer and allocation of photosynthetic products in P. massoniana organs, which peaks at 60 kg N hm-1 a-1, and the highest N treatment began to decrease at 90 kg N hm-1 a-1. EMF inoculation could expand the absorption area of plant roots to obtain more nutrients and synthesize more C and N compounds for promoting the growth of itself and the host plant, improving the net photosynthetic rate and the distribution of C produced by photosynthesis in various organs. This forms a benign C and N cycle, thereby reducing the effect of high N addition on plants. The optimal N addition concentration was 60 kg N hm-1 a-1, and the optimal EMF was Pt, which provides a theoretical basis for inoculating EMF during increasing N deposition in the future climate change scenario. This enables plants to distribute more photosynthetic products to their roots, thus affecting their own C distribution for promoting growth.

Following oxidative stress, reactive oxygen species are produced and accumulate in glioma cells in large quantities, and to avoid the occurrence of cellular dysfunction, glioma cells can respond adaptively in the biological processes of DNA damage repair, lipid peroxidation and protein modification to produce radiotherapy resistance. The expression of nuclear factor erythroid 2-related factor 2, solute carrier family 7 member 11, glutathione and microRNA, as key regulatory molecules, can regulate reactive oxygen species levels, alter glioma oxidative stress status, and affect radiochemotherapy sensitivity. Further study on the relationship between oxidative stress and sensitivity to radiotherapy and chemotherapy of glioma can provide theoretical basis for precise treatment of glioma.

Glioma is the most common primary brain tumor in adults and is essentially a polygenic abnormal disease. Solute carrier family 7 member 11 (SLC7A11) is a core component of the cystine/glutamate transporter and its expression is regulated at the transcriptional and translational levels. SLC7A11 mediates glioma cells proliferation, invasion and chemoradiotherapy resistance through regulation of oxidative stress and ferroptosis. Deep study of SLC7A11 will provide new theoretical basis and therapeutic targets for the treatment of gliomas.

Objective:In China, although axillary osmidrosis is common in plastic surgery, there is no standard procedure for the treatment of axillary osmidrosis. So we compared the efficacy of 6 axillary osmidrosis surgical treatments with network meta-analysis in order to provide reference for the clinical surgical treatment of axillary osmidrosis in Chinese patients.Methods:From January 2018 to December 2021, Chinese and English databases including Wanfang Database, VIP Chinese Science and Technology Periodical Full-text Database (VIP) and CNKI, PubMed, Cochrane Library, Embase were searched by the Plastic Surgery Department of Zhangqiu People′s Hospital in Jinan, Shandong province. And some relevant studies were collected for network meta-analysis.Results:47 studies including a total of 6543 subjects were identified for meta-analysis. After analysis, 4 of the 15 pairwise comparisons were not statistically significant, and the effect of swelling aspiration + subcutaneous pruning was more significant. According to the surface under the cumulative ranking curve (SUCRA), the efficacy of 6 surgical methods for the treatment of axillary osmidrosis was as follows: swelling aspiration + subcutaneous pruning > subcutaneous scratching+ subcutaneous pruning > subcutaneous pruning > fusiform skin resection > subcutaneous scratching > swelling aspiration.Conclusions:In surgical treatment of axillary osmidrosis, swelling aspiration + subcutaneous pruning is the best. It is worth popularizing in the clinical work of treating axillary osmidrosis patients. Due to the limitations of this study, the conclusion of this network meta-analysis still needs to be further confirmed by a well-designed randomized controlled trial.

RESUMEN

Radiotherapy is an important auxiliary treatment for glioma. Although comprehensive treatment of glioma has made some progress in recent years, the prognosis is still poor. How to improve the radiosensitivity of glioma is one of the hot spots in current glioma research. The changes of energy metabolism in gliomas promote malignant progression and resistance to chemoradiotherapy, but the exact mechanism is still unclear. This paper reviews the research progress of energy metabolism in gliomas and its relation with radiosensitivity of gliomas, in order to lay a foundation for further exploration of improving radiosensitivity of gliomas.

RESUMEN

ObjectiveWe aim to summarize reliable evidences of evidence-based medicine for the treatment and prevention of the 2019 novel coronavirus (2019-nCoV) by analyzing all the published studies on the clinical characteristics of patients with 2019-nCoV. MethodsPubMed, Cochrane Library, Embase, and other databases were searched. Several studies on the clinical characteristics of 2019-nCoV infection were collected for Meta-analysis. ResultsTen studies were included in Meta-analysis, including a total number of 50466 patients with 2019-nCoV infection. Meta-analysis shows that, among these patients, the incidence of fever was 89.1%, the incidence of cough was 72.2%, and the incidence of muscle soreness or fatigue was 42.5%. The incidence of acute respiratory distress syndrome (ARDS) was 14.8%, the incidence of abnormal chest computer tomography (CT) was 96.6%, the percentage of severe cases in all infected cases was 18.1%, and the case fatality rate of patients with 2019-nCoV infection was 4.3%. ConclusionFever and cough are the most common symptoms in patients with 2019-nCoV infection, and most of these patients have abnormal chest CT examination. Several people have muscle soreness or fatigue as well as ARDS. Diarrhea, hemoptysis, headache, sore throat, shock, and other symptoms only occur in a small number of patients. The case fatality rate of patients with 2019-nCoV infection is lower than that of Severe Acute Respiratory Syndrome (SARS) and Middle East Respiratory Syndrome (MERS).

Objective To evaluate the curative effects of modified minimally invasive double eyelid operation and traditional embedding double eyelid operation.Methods Chinese and English databases including Wanfang Science and Technology Periodical Full-text Database,VIP Chinese Science and Technology Periodical Full-text Database (VIP) and CNKI,PubMed,Web of Science,Science Drirect Online and Cochrane Library were searched by computer.Some randomized controlled clinical trials that compared the effects of modified mini-eyelid operation and traditional embedding double eyelid operation were collected for this Meta analysis.Results The postoperative satisfaction of modified minimally invasive double eyelid surgery was higher than that of the traditional embedding double eyelid surgery (Z =6.16,P<0.00001).The success rate of modified minimally invasive blepharoplasty surgery was higher than that of the traditional embedding double eyelid surgery (Z =4.84,P<0.00001).Conclusions The clinical effect of modified minimally invasive double eyelid surgery is better than that of the traditional embedding double eyelid surgery.

Objective To explore the influence factor of the stability of L5S1 fusion and the relationship between the stability of sacrum fusion and the clinical outcome for degenerative lumber scoliosis with long-segment fusion.Metheds Fifty patients with degenerative lumber scoliosis who underwent the long-segment fusion from June 2010 to January 2015 were included in this retrospective study.Based on post-operative L5S1 fusion,patients were divided into two groups.There were 15 patients(30%,9 male and 6 female patients) with a failing sacrum fusion whose average age 60±7.6 years (range from 58 to 78 years).6 patients had the situation at T12-S1,7 at L1-S1 and 2 at L2-S1.There were 35 patients (70%,26 male and 9 female patients) with a satisfying sacrum fusion whose average age 58.4±4.8 years (range from 50 to 80 years).Among them,16 were at T12-S1,12 at L1-S1 and 7 at L2-S1 levels.The interbody fusion rate and accuracy of pedicle screws were evaluated by anteroposterior and lateral spine radiographs in standing and CT scanning during the postoperative follow-up.The age,body mass index (BMI),bone mineral density,Cobb's angel,lumbar lordosis (LL),sagittal vertical axis (SVA),thoracic kyphosis (TLK),sacral slope (SS),pelvic inclination (PI) and pelvic tilt (PT) were compared between the 2 groups to analysis the relevant factor of the failing L5S1 fusion.Oswestry disability index (ODI) and visual analogue scale (VAS) were compared between the 2 groups in the postoperative follow-up.Results There were no statistically significant difference between the 2 groups preoperative in the age,BMI,lumbar Cobb angel,LL,SVA,TLK,SS,PI and PT.But there was a significant difference between the bone mineral density of 2 groups.LL were-40.5°± 8.7° vs.-41.2°±7.9°;TLK were 1.7°±7.4° vs.1.8°±6.7°;SS were 32.1°±5.6° vs.32.4°±5.5°;PT were 18.7°±10.5° vs.19.5°± 10.1°;PI were 42.3°±4.4° vs.40.1°±5.2°;SVA were 9.2±3.5 cm vs.9.5±3.1 cm;T value were-2.7±1.1 vs.-1.2±1.4.There were no statistically significant difference between the 2 groups in the ODI and VAS in the postoperative follow-up.Oswestry disability index were 8.2%±5.2% vs.7.8%±4.5%;VAS value were 3.4±2.6 vs.3.1±2.1.Conclusion The smaller the bone mineral density of the patients with DS,the higher the incidence of the fusion failure L5S1 fusion.And when the failing L5S1 of fusion happened,the clinical outcome will not always be poor.

Objective: To summarize the short-term clinical experience of minimally invasive direct cardiac surgery (MIDCS) with right anterolateral thoracotomy incision by closed cardiopulmonary bypass. Methods: A total of 42 patients received MIDCS in our hospital from 2013-09 to 2015-05 were summarized. There were 18 male and 24 female patients including 16 with atrial septal defect (ASD) repair, 4 with ventricular septal defect (VSD) repair, 16 with mitral valve replacement (MVR), 1 with mitral valve plasty (MVP) and 5 with aortic valve replacement (AVR). Direct cardiac surgery was performed by cardiopulmonary bypass through femoral artery-vein and right jugular vein annulations. A right anterolateral thoracotomy incision (length 3-5 cm) was made to enter the chest and complete the operation. Results: All 42 patients received successful operation, no peri-operative or early post-operative death, no incision infection occurred. Cardiopulmonary bypass time was [98-142 (122.4 ± 23.7) min], aortic cross-clamp time [0-118 (48.3 ± 26.2) min]. Post-operative mechanical ventilation time was [8-76 (17.4±13.1) h], intensive care unit stay time [45-124, (54.6 ± 32.6) h], hospital stay time [6-12, (8.2 ± 1.3) d]. Incision length was [3-7, (4.8 ± 1.5) cm], the draining volume at the 1st post-operative day was (356.9 ± 283.8) ml and there were 27 (64.3%) patients without transfusion. Conclusion: The short-term outcomes for MIDCS were good, it with superior safety, broad application range with minimal invasion and less complication.

Objective To investigate the distribution and antimicrobial resistance profile of clinical gram-negative bacterial isolates in the First Afifliated Hospital of Nanjing Medical University during 2014.Methods Bacteria identiifcation was performed by API system or the VITEK-2 Compact automatic identiifcation system. Disk diffusion susceptibility testing or VITEK-2 Compact automatic identification system was used to determine the susceptibility to antimicrobial agents. All data were analyzed using WHONET 5.6 software.Results Among the total 7 931 clinical isolates in 2014, gram-negative bacteria accounted for 64.2% (5 088/7 931). The top three pathogens wereE. coli,A. baumannii andK. pneumoniae. Notably, during the year 2014, 195 strains of carbapenem-resistantEnterobacteriaceaewere isolated, about 6.9% of all theEnterobacteriaceae isolates. Meanwhile, 613 (66.5%) strains of multiple drug resistantA. baumannii and 197 (28.7%) strains of multiple drug resistantP. aeruginosa were isolated.Conclusion During the year 2014, the resistance of the gram-negative bacteria in this hospital is mainly characterized by carbapenem-resistantEnterobacteriaceae, multiple drug resistant A. baumanniiand multiple drug resistantP. aeruginosa. Surveillance of antimicrobial resistance is beneifcial for rational use of antibiotics.

ObjectiveTo investigate the clinical efficacy of gentle moxibustion in treating old interphalangeal collateral ligament injury.MethodSixty patients were randomly allocated to two groups. The treatment group of 30 patients received gentle moxibustion 20-30 min once daily for two consecutive courses of treatment. The control group of 30 patients received TDP irradiation 20-30 min once daily for two consecutive courses of treatment.ResultThe excellent rate and the total excellent rate were 56.7% and 83.3%, respectively, in the treatment group and 36.7% and 76.7%, respectively, in the control group; there were statistically significant differences between the two groups (P<0.01).ConclusionGentle moxibustion has a marked therapeutic effect on old interphalangeal collateral ligament injury.

Objective To explore the clinical value of 3-dimensional digital subtraction angiography(3D-DSA)in the diagnosis and therapy of intracranial aneurysms.Methods 2-dimensional digital subtraction angiography (2D-DSA)and 3D-DSA was performed respectively on 1 53 patients,including 1 5 1 cases with spontaneous subarachnoid hemorrhage (SAH),1 case with headache and 1 case with cerebral infarction;Meanwhile,to compare the difference in diagnosis of intracranial aneurysms between 2D-DSA and 3D-DSA.Results 1 12 patients(125 pieces)with intracranial aneurysms were detected on 2D-DSA,its detecting rate was 73.2%(1 12/1 53 );136 patients(1 52 pieces)with intracranial aneurysms were detected on 3D-DSA,its detecting rate was 88.9%(136/1 53)(χ2 =12.254,P <0.05).The displaying rate of 3D-DSA for the relationship between carrying tumor artery and aneurysms and the necks of aneurysms was 100%(1 52/1 52),however,the displaying rate of 2D-DSA for that was 61.6%(77/125)(χ2 =63.993,P <0.05). Conclusion 3D-DSA can improved the diagnostic accurate rate of intracranial aneurysms,it is important in chosing the therapeutic regimen.

Objective To study the ego identity,mental health and their relationship among students in athletes.Methods 110 students in athletes and 122 athletes in students were measured with identity status scale and athletes' symptom checklist.Results ①Students in athletes have higher "present serf-commitment" (5.09±1.07)and lower "crisis in the past"(2.19±1.12)than athletes in students.The two demonstrate the obvious difference on the distribution of identity status.The percentage of students in athletes obtaining A is lower than that of athletes in students ,while the percentage of students in athletes obtaining D and MD is higher. ②Comparing with the norm,students in athletes have higher level on the factors of sematizafion (1.98±0.42),compulsion (2.21±0.51),interpersonal sensitivity(2.09±0.53),depression (1.85±0.53),anxiety ( 2.47 ~0.58 ),paranoid (1.98±0.21),psychoticism (1.79±0.22) and total average marks (2.00±0.34),lower level on the factors of hostility (1.43±0.48)and cognitive(1.64±0.31). ③Three domains of ego identity exit obvious negative correlation with the sores of psychological symptoms.ANOVA confined that the more advanced ego identity is,the better mental health is.Conclusion Ego identity and mental health of students in athletes have their own features,and ego i-dentity will influence mental health.
