RESUMEN

The assessment of presence or absence of substance addiction in subjects working in activities potentially harmful for themselves or others, is indeed an important issue and a compulsory activity. Nevertheless, according to Italian laws (outlined in the agreement on the ascertainment of the absence of drug addiction, executed in the session of October 30th 2007), only occupational company doctors are in charge of such a duty. This professional figure should instead be mainly aimed to protect worker's health. Furthermore, private workers, even in settings or jobs at risk (e.g. bus drivers) are automatically excluded from such a verification. In this case, third parties (e.g. bus passengers) are protected by the law only when the worker is employed in a company. The new procedures, recently promulgated by central and regional governments, increase the burden of occupational company doctors, and introduce restrictions for workers of particular activities, in the case of a positive laboratory result for prohibited substances.

Despite the growing evidence that the use of hand-held mobile phone while driving increases the risk of motor vehicle crashes and the banning by law, in many countries, of this kind of use, this behaviour is more and more spreading. Following a survey we have conducted in 2004, in 2005 we have realized in florentine area a monthly monitoring about this incorrect use of phoning while driving. Overall, 15001 vehicles were observed, and the prevalence of mobile phone use while driving was 2.6%, higher if compared with that referred to the previous year (1.8%). The association between mobile, hand-held, phone use and the considered variables (the same of the 2004 study) is always statistically significant and the conditions with the highest probability of meeting a phoning driver are showed by regression analysis. Intervention for reducing the spread of this behaviour is needed, especially if we consider the recent technological innovations introduced in the new models of mobile phones, which offer new distractions for drivers, the uncertainty of the absence of risk in using hand-free phone and the scarcely proved long-term effectiveness of legislation banning this use.

To investigate the use of child restraint and seating position in Florence, in the Centre of Italy, after the introduction of the New Italian road traffic law, a survey has been conducted. Direct observations were made in the areas related to three schools attended by children of different age groups, to determine the presence of child car seats and their positioning as well as child positioning in case of absence of the seat. A total of 328 vehicles have been observed; in 48.5% of the cars, seats for children were present, 39% were used, and 9% were not used. The presence of child restraint and its use differed according to age, school opening and closing time and number of children on board. When restraints were not present, children were mostly (38%) located on the front seat without an accompanying adult. Child seats were prevalently located in the rear. In spite of the introduction of New Italian road traffic law, in the study area the modality of children's transport in cars was frequently not correct, which constitutes a risk for the health of this specific target group.

RESUMEN

In order to experience a method for the choice in Public Health, a list of the problems in occupational health in Florence has been characterized one, using for the definition of the priorities a "Minnesota Department of Health's Community Health Services Planning Manual--Guidelines for Local Public Health Agencies: CHS Plan Cycle 2002-2003". The following priorities problems of health are emerged in order of: exposure to chemistries in agricultural workers, fatal and serious injures among construction and agricultural workers, musculoskeletal disorders by biomechanical overload of the upper limbs, hearing loss. For everyone of they, has been indicated a possible strategy.

Benzene is a well documented carcinogen for the hematic and lymphopoietic system, and experimental research confirms its carcinogenicity for tumors of other sites. This report presents the results of a historical cohort study in a shoe manufacturing plant in Florence where cases of aplastic anemia and leukemia were reported in the 1960s. A total of 1008 men and 1005 women were considered eligible members of the cohort. For total mortality, comparing the rates of the cohort with the national rates, the standardized mortality ratio (SMR) was 79 for the women and 95 for the men. For the men excesses of risk for aplastic anemia [SMR 1566; 95% confidence interval (95% CI) 547-3264] and leukemia (SMR 400, 95% CI 146-870) were observed. The increased risk occurred among workers first employed during the period in which benzene was used, but the expected number of cases in the subsequent period was too small to evaluate whether any reduction in risk had occurred. No increasing pattern with duration of employment was discernible.

RESUMEN

The Authors have studied AST and ALT enzymatic activities in the workers of two firms, the former of which (tannery) with a high and the latter (boot and shoe factory) with a low level of hepatic-toxic risk. The influence of various trouble factors such as age, sex and seniority was eliminated through appropriate statistical techniques. A significant difference was evidenced between AST and ALT levels in two firms, chiefly attributable to the quantity and quality of the substances utilized in the two technological cycles: trichloroethylene, chromium, sulphuric acid, mineral oils, ammonia, N-hexane, pentanes acetone, ciclo hexane, methanol, ethyl acetate, isopropyl acetate, toluene, methylene chloride.
