RESUMEN

Abstract To date, hydroxyurea is the only effective and safe drug that significantly reduces morbidity and mortality of individuals with Sickle cell disease. Twenty years of real-life experience has demonstrated that hydroxyurea reduces pain attacks, vaso-occlusive events, including acute chest syndrome, the number and duration of hospitalizations and the need for transfusion. The therapeutic success of hydroxyurea is directly linked to access to the drug, the dose used and adherence to treatment which, in part, is correlated to the availability of hydroxyurea. This consensus aims to reduce the number of mandatory exams needed to access the drug, prioritizing the requesting physician's report, without affecting patient safety.

We report the successful closure of Phase I clinical trials, comprising Phases Ia and Ib, of the vaccine candidate against human schistosomiasis: the Schistosoma mansoni 14 kDa fatty acid-binding protein (Sm14) + glucopyranosyl lipid A in squalene emulsion (GLA-SE). Shown here are the results of Phase Ib, an open, non-placebo-controlled, standardized-dose immunization trial involving 10 healthy 18-49-year-old women. Fifty micrograms of the Sm14 protein plus 10 µg GLA-SE per dose was given intramuscularly thrice at 30-day intervals. Participants were assessed clinically, biochemically, and immunologically for up to 120 days. In preambular experiments involving vaccinated pregnant female rabbits, we did not find any toxicological features in either the offspring or mothers, and the vaccine induced adaptive immunity in the animals. In women, no adverse events were observed, and vaccination induced high titers of anti-Sm14 serum IgG antibody production. Vaccination also elicited robust cytokine responses, with increased TNFα, IFNγ, and IL-2 profiles in all vaccinees on days 90 and 120. The completion of Phase I clinical trials, which were performed to the highest standards set by Good Clinical Research Practice (GCP) standards, and preclinical data in pregnant rabbits enabled the vaccine candidate to proceed to Phase II clinical trials in endemic areas.

Abstract Introduction and objectives Multimodal Analgesia (MMA) has shown promising results in postoperative outcomes across a broad spectrum of surgeries, including bariatric surgery. We compared the analgesic effect immediately after Laparoscopic Bariatric Surgery (LBS) of the combined effect of MMA and methadone against two techniques that were based mainly on the use of high-potency medium-acting opioids. Methods Two hundred seventy-one patients were retrospectively reviewed. The primary outcome was postoperative pain score > 3/10 measured by the Verbal Numeric Scale (VNS) during the Postanesthetic Care Unit (PACU) stay. The three protocols of intraoperative analgesia were: (P1) sufentanil at anesthetic induction followed by remifentanil infusion; (P2) sufentanil at induction followed by dexmedetomidine infusion; and (P3) remifentanil at induction followed by MMA including dexmedetomidine, magnesium, lidocaine, and methadone. Only P1 and P2 patients received morphine toward the end of surgery. Poisson regression was used to adjust confounding factors and calculate Prevalence Ratio (PR). Results Postoperative VNS > 3 was recorded in 135 (49.81%) patients, of which 93 (68.89%) were subjected to P1, 25 (18.56%) to P2, and 17 (12.59%) to P3. In the final adjusted model, both anesthetic techniques (P3) (PR = 0.10; 95% CI [0.03-0.28]), and (P2) (PR = 0.42%; 95% CI [0.20-0.90]) were associated with lower occurrence of VNS > 3, whereas age range 20-29 was associated to higher occurrence of VNS > 3 (PR = 3.21; 95% CI [1.22-8.44]) in PACU. Postoperative Nausea and Vomiting (PONV) was distributed as follows: (P1) 20.3%, (P2) 31.25% and (P3) 6.77%; (P3 < P1, P2; p< 0.05). Intraoperative hypotension occurred more often in P3 (39%) compared to P2 (20.31%) and P1 (17.46%) (p< 0.05). Conclusion MMA + methadone was associated with higher incidence of intraoperative hypotension and lower incidence of moderate/severe pain in PACU after LBS.

Schistosomiasis, a disease historically associated with poverty, lack of sanitation and social inequality, is a chronic, debilitating parasitic infection, affecting hundreds of millions of people in endemic countries. Although chemotherapy is capable of reducing morbidity in humans, rapid re-infection demonstrates that the impact of drug treatment on transmission control or disease elimination is marginal. In addition, despite more than two decades of well-executed control activities based on large-scale chemotherapy, the disease is expanding in many areas including Brazil. The development of the Sm14/GLA-SE schistosomiasis vaccine is an emblematic, open knowledge innovation that has successfully completed phase I and phase IIa clinical trials, with Phase II/III trials underway in the African continent, to be followed by further trials in Brazil. The discovery and experimental phases of the development of this vaccine gathered a robust collection of data that strongly supports the ongoing clinical phase. This paper reviews the development of the Sm14 vaccine, formulated with glucopyranosyl lipid A (GLA-SE), from the initial experimental developments to clinical trials including the current status of phase II studies.

The deconstruction of sleep quality into its particular components may help to specify how each one of them influences mental health. Thus, to detail the understanding of the relationship between sleep quality and mental health, our study aims to assess the relationship between each component of the Pittsburgh Sleep Quality Index (PSQI) and depression, anxiety and stress. Also, we intend to analyse the moderating effects of gender, age and country on these relationships. The instruments PSQI and Depression Anxiety Stress Scale (DASS-21) were applied to a non-clinical population of 1552 participants from three different countries (Portugal, Spain and Brazil), aged over 18 years old. We estimated a multivariate regression model with AMOS Graphic 21.0 to test the seven proposed research hypotheses. The adjusted model explains 14.0%, 21.0% and 19.3% of the variances of depression, anxiety and stress, respectively. The conclusion of this study demonstrates that, with the exception of subjective sleep quality, all the other six components of the PSQI individually relate to mental health in non-clinical populations and that country is a significant moderator of these relationships.

Neste artigo relata-se a experiência da implantação da gestãoregionalizada na Secretaria de Estado da Saúde do Distrito Federal,com a finalidade de reestruturação das ações e serviços de saúdecom vistas à melhoria do acesso e qualidade e criar espaços degestão compartilhada e solidária. O Programa de Gestão Regionalda Saúde definiu os Acordos de Gestão Regional e Local comoinstrumentos de pactuação de metas entre a Administração Central eas Superintendências Regionais de Saúde e destas com suas unidadesassistenciais. Estes instrumentos de contratualização objetivaminstituir um modelo de gestão por resultados nessa Secretaria.


This article reports on the experience of the implementation of regionalized management in the State Department of Health of the Federal District (SES-DF), with the purpose of restructuring health actions and services with a view to improving access and quality and creating spaces for shared and supportive management. The Regional Health Management Program defined the Regional and Local Management Agreements as instruments of goal agreement between the Central Administration and the Regional Superintendencies of Health and of these with its health care units. These contractual instruments aim to establish a results-based management model at SES-DF.

INTRODUCTION: Sleep quality relates to mental health in clinical and non-clinical populations. However, there is more evidence of this relationship in clinical populations. Therefore, there is lack of evidence on how these variables relate and on which sociodemographic factors influence this relationship in non-clinical populations. In this study we hypothesize that in a non-clinical population sleep quality predicts mental health indicators and that age, country and gender moderate this relationship. METHODS: In a sample of 1552 subjects from Portugal, Spain and Brazil, self-reported sleep quality and mental health indicators were assessed through the Pittsburgh Sleep Quality Index and the Depression, Anxiety and Stress Scale-21, respectively. A multivariate linear regression model was used to test the research hypotheses. RESULTS: This adjusted model explained 10.1%, 12.3% and 13.1% of the variability of Depression, Anxiety and Stress, respectively, suggesting multiple sources of variance. CONCLUSIONS: Our results confirmed that sleep quality predicts mental health in non-clinical populations, and that the variable country is a significant moderator of this relationship.

ABSTRACT Objective: To evaluate the effectiveness of autologous Plasma- rich platelet (PRP) in symptomatic dry eye of diabetic patients. Design A prospective single group interventional study. Participants: Twelve diabetic patients with refractory dry eye disease. Methods Patients were treated with autologous PRP eye drops four times a day for a month. Dry eye symptoms and its frequency, according to DEWS studies, tear film breakup time, improvement of visual acuity lines, and Schirmer test. Results: All the patients had some symptomatic improvement regarding dryness, itching, burning and redness (p=0.002). Five patients, 41.66% (5/12) had improvement of 1 or more lines of visual acuity in both eyes, 50% and 58.33% had no alteration in right and left eye, respectively (p=0.14). Considering Schirmer test, 66.66% (8/12) had improvement in the test value, 25% (3/12) had no alteration in this test value and 8.33% (1/12) had a reduced value in the test after treatment.(p=0.04). Considering the value of BUT test 58.33% (7/12) had improvement in the test value and 41.66% (5/ 12) had no alteration in this test value (p=0.018). Conclusions PRP is safe and an interesting alternative therapy in symptomatic diabetic dry eye. More clinical trials are required to create specific protocols to this treatment.


RESUMO Objetivo: Avaliar a eficácia do colírio de concentrado de plaquetas (CCP) autólogo no olho seco sintomático de pacientes diabéticos. Projeto Um estudo de intervenção único grupo prospectivo. Participantes Doze pacientes diabéticos com doença do olho seco refratário. Métodos Os pacientes foram tratados com colírio de PRP autólogo quatro vezes por dia durante um mês. Sintomas de olho seco e sua frequência, seguindo os critérios de DEWS, tempo de ruptura do filme lacrimal, a melhoria das linhas de acuidade visual e teste de Schirmer. Resultados Todos os pacientes tiveram alguma melhora dos sintomas de ressecamento, coceira, ardor e vermelhidão (p = 0,002). Destes, 41,66% (5/12) tiveram melhora de uma ou mais linhas de acuidade visual em ambos os olhos; 50% e 58,33% não tinham alteração no olho direito e esquerdo, respectivamente (p = 0,14) . Considerando o teste de Schirmer, 66,66% (8/12) tiveram melhora no valor do teste, 25% (3/12) não apresentaram alteração neste valor de teste e de 8,33% (1/12) tiveram um valor reduzido no teste após tratamento. (p = 0,04). Considerando o valor de teste BUT 58,33% (7/12) apresentaram melhora no valor de teste e 41,66% (12/05) não apresentaram alteração neste valor de teste (p = 0,018). Conclusões O CCP é seguro e uma terapia alternativa interessante no olho seco diabético sintomático. Mais ensaios clínicos são necessários para criar protocolos específicos para este tratamento.

A Imunodeficiência comum variável (ICV) é a segunda mais frequente das imunodeficiências primárias e tem como critérios diagnósticos: infecções recorrentes; idade acima de 4 anos; níveis reduzidos de IgG, de IgA e/ou IgM; exclusão de outras causas de hipogamaglobulinemias; ausência de isohemaglutininas e de resposta vacinal. O artigo visa relatar o caso de um paciente com ICV associada a infecções/infestações recorrentes e esplenomegalia. Trata-se de menino de 8 anos com antecedente de oito pneumonias, giardíase recorrente e esplenomegalia com pesquisa de anticorpos anti-HIV negativa. Apresentava IgG=614mg/dl, IgM= 19mg/dl e IgA=24mg/dl; CD4=24% (742 células/mm3), CD8= 70% (2150 células/mm3), CD19 = 247células/mm3 e linfócito B total = 9,2%. A ICV pode se manifestar através de cinco fenótipos clínicos: infecções, autoimunidade, infiltração linfocítica, malignidade e enteropatia. Neste caso, o paciente apresentava infecções/infestações recorrentes, as dosagens de imunoglobulinas encontram-se abaixo do percentil 3 para idade e o de linfócitos B, no limiar mínimo. Reiteramos a importância do diagnostico de ICV em paciente com infecções/infestações recorrentes e esplenomegalia pela importante prevalência na população.

Both undernutrition and hypoxia exert a negative influence on both growth pattern and bone mechanical properties in developing rats. The present study explored the effects of chronic food restriction on both variables in growing rats exposed to simulated high-altitude hypoxia. Male rats (n 80) aged 28 d were divided into normoxic (Nx) and hypoxic (Hx) groups. Hx rats were exposed to hypobaric air (380 mmHg) in decompression chambers. At T0, Nx and Hx rats were subdivided into four equal subgroups: normoxic control and hypoxic controls, and normoxic growth-restricted and hypoxic growth-restricted received 80 % of the amount of food consumed freely by their respective controls for a 4-week period. Half of these animals were studied at the end of this period (T4). The remaining rats in each group continued under the same environmental conditions, but food was offered ad libitum to explore the type of catch-up growth during 8 weeks. Structural bone properties (strength and stiffness) were evaluated in the right femur midshaft by the mechanical three-point bending test; geometric properties (length, cross-sectional area, cortical mass, bending cross-sectional moment of inertia) and intrinsic properties of the bone tissue (elastic modulus) were measured or derived from appropriate equations. Bone mineralisation was assessed by ash measurement of the left femur. These data indicate that the growth-retarded effects of diminished food intake, induced either by food restriction or hypoxia-related inhibition of appetite, generated the formation of corresponding smaller bones in which subnormal structural and geometric properties were observed. However, they seemed to be appropriate to the body mass of the animals and suggest, therefore, that the bones were not osteopenic. When food restriction was imposed in Hx rats, the combined effects of both variables were additive, inducing a further reduction of bone mass and bone load-carrying capacity. In all cases, the mechanical properties of the mineralised tissue were unaffected. This and the capacity of the treated bones to undergone complete catch-up growth with full restoration of the biomechanical properties suggest that undernutrition, under either Nx or Hx conditions, does not affect bone behaviour because it remains appropriate to its mechanical functions.

ABSTRACT Calcium silicate cements have been used as dental materials for more than twenty years; however, their use in restorative dentistry is more recent. Better mechanical properties and shorter curing times make them suitable for a variety of applications in which they are used as a substitute of dentin, including direct/indirect pulp capping and as cavity base/liner. These materials may also be used to restore enamel temporarily. This article seeks to review the available scientific evidence with a focus on their applications in restorative dentistry. The information was gathered by reviewing original scientific research articles and literature reviews published in journals available in databases such as Medline/Pubmed and Scielo, along with technical information provided by the manufacturers of these cements. This article describes the composition, instructions for use, and curing reaction of calcium silicate cements, as well as the scientific evidence on their applications in restorative dentistry.


RESUMEN Los cementos de silicato de calcio se han aplicado como materiales dentales desde hace más de veinte años; sin embargo, su uso en el área de la odontología restauradora es más reciente. Mejores propiedades mecánicas y menores tiempos de endurecimiento le permiten ser indicados para una variedad de aplicaciones en las que este material se utiliza como sustituto dentinario, entre ellas el recubrimiento pulpar directo/indirecto y como base/liner cavitario. A su vez, también se podría utilizar como material para restaurar esmalte de manera temporal. El presente artículo busca revisar la evidencia científica disponible, enfocándola a sus aplicaciones en odontología restauradora. La información se obtuvo a partir de artículos originales de investigación científica y revisiones de literatura, publicados en revistas disponibles en bases de datos como Medline/Pubmed y Scielo, junto a la información técnica otorgada por los fabricantes de estos cementos. El presente trabajo describe la composición, el modo de empleo, la reacción de fraguado y la evidencia científica sobre las aplicaciones de los cementos de silicato de calcio en odontología restauradora.

DESIGN: Safety and immunogenicity of a recombinant 14kDa, fatty acid-binding protein(FABP) from Schistosoma mansoni (rSm14) were evaluated through an open, non-placebo-controlled, dose-standardized trial, performed at a single research site. The vaccine was formulated with glucopyranosyl lipid A (GLA) adjuvant in an oil-in-water emulsion (SE) and investigated in 20 male volunteers from a non-endemic area for schistosomiasis in the state of Rio de Janeiro, Brazil. Fifty microgram rSm14 with 10 µg GLA-SE (rSm14/GLA-SE)/dose were given intramuscularly three times with 30-day intervals. Participants were assessed clinically, biochemically and immunologically for up to 120 days. METHODS: Participants were screened for inclusion by physical examination, haematology and blood chemistry; then followed to assess adverse events and immunogenicity. Sera were tested for IgG (total and isotypes) and IgE. T cell induction of cytokines IL-2, IL-5, IL-10, IFNγ and TNFα was assessed by Milliplex kit and flow cytometry. RESULTS: The investigational product showed high tolerability; some self-limited, mild adverse events were observed during and after vaccine administration. Significant increases in Sm14-specific total IgG, IgG1 and IgG3 were observed 30 days after the first vaccination with specific IgG2 and IgG4 after 60 days. An increase in IgE antibodies was not observed at any time point. The IgG response was augmented after the second dose and 88% of all vaccinated subjects had developed high anti-Sm14 IgG titres 90 days after the first injection. From day 60 and onwards, there was an increase in CD4(+) T cells producing single cytokines, particularly TNFα and IL-2, with no significant increase of multi-functional TH1 cells. CONCLUSION: Clinical trial data on tolerability and specific immune responses after vaccination of adult, male volunteers in a non-endemic area for schistosomiasis with rSm14/GLA-SE, support this product as a safe, strongly immunogenic vaccine against schistosomiasis paving the way for follow-up Phase 2 trials. RESUMO A literacia pré-escolar tem recebido uma crescente atenção. Neste estudo, participaram 70 crianças que iniciaram o 1.º ano de escolaridade no ano letivo 2009/10 e que, na educação pré-escolar, frequentaram três instituições com abordagens diferenciadas de leitura e de escrita. O estudo inclui quatro medidas repetidas no tempo nas áreas da fonologia, reconhecimento de letras, palavras e velocidade de leitura. Os resultados sugerem que a abordagem específica à leitura na educação pré-escolar condiciona o desempenho da leitura na instrução primária. Contudo, essa influência parece ser atenuada com o tempo. Por outro lado, verificou-se que sujeitos que receberam instrução direta de leitura em idade pré-escolar (mas não treino fonológico) obtêm resultados significativamente superiores em tarefas fonológicas aos de sujeitos que receberam treino fonológico prolongado (mas não de leitura).


ABSTRACT The debate regarding kindergarten literacy has received increasing attention and social visibility. In this study, participants are 70 children who attended first grade in 2009/2010. Participants attended three kindergartens with unique approaches to reading. A four wave repeated measure design was conducted to investigate the growth of the children in phonological awareness, letter recognition, word recognition, and reading speed. Results suggest that the approach towards reading and writing in kindergarten affects children's performance in grades 1 and 2 of elementary school. This influence, however, seems to fade over time. Moreover, results show that children who received formal reading instruction in kindergarten, perform significantly better in phonological tasks than children who received direct and extended phonological instruction.

La causa de fracaso más común de los postes de fibra es su desalojo, principalmente debido a la compleja adhesión dentro del conducto radicular. El principal objetivo de este estudio fue determinar si existe diferencia significativa en la resistencia adhesiva mediante el test push-out en la cementación de postes de fibra con RelyX U-200® (U200) y Core Paste XP® (CPX) en diferentes regiones radiculares. MATERIALES Y MÉTODOS Se seleccionaron 40 premolares, se seccionaron a 15 mm del ápice, fueron tratados endodóncicamente y fueron divididos aleatoriamente en 2 grupos de 20, a los cuales se les cementó un poste de fibra translúcido Exacto® con U200 y CPX. Después de 24 h cada muestra fue seccionada transversalmente a nivel cervical, medio y apical. Fueron sometidas al test de push-out mediante la máquina Tinius Olsen HK5-S para cuantificar la resistencia adhesiva en megapascales. Se utilizó el test Shapiro-Wilk, test de ANOVA de 2 factores y Tukey con un nivel de significación del 95%. RESULTADOS No hubo diferencia significativa de ambos cementos (p = 0,457), pero sí hubo diferencia cuando se comparó el tipo de cemento y la región; CPX presentó valor significativamente mayor en el tercio cervical (p < 0,05), mientras que U200 presentó un valor significativamente mayor en el tercio apical (p < 0,05). CONCLUSIONES No hubo diferencia significativa en el valor de adhesión cuando se comparó solo el factor cemento entre U200 y CXP, sin embargo hubo un mayor valor de CXP a nivel cervical y de U200 a nivel apical.


Loosening is the most common failure of fiber posts, mainly due to complex adhesion within the root canal. The main objective of this study was to determine if significant differences in adhesive resistance, using the push-out test, in the cementation of fiber posts with RelyX U-200® (U200) and Core Paste XP® (CPX) in different root regions. MATERIALS AND METHODS A selection of 40 premolars were sectioned 15 mm from the apex, treated endodontically and then randomly divided in two groups of 20, using an Exacto translucent fiber post with U200 and CPX, respectively. After 24 hours, each sample was sectioned into cervical, middle and apical. The test push-out test was performed using a Tinius Olsen HK5-S machine to quantify the adhesive strength in Mega Pascals. The test Shapiro-Wilk, two-factor Anova and Tukey tests were used with a significance level of 95% RESULTS There were no significant differences between the cements (P = .457) but if there was difference when comparing the type of cement and region. CPX showed a value significantly higher in the cervical third (P < .05), while U200 presented a value significantly higher in the apical third (P < .05). CONCLUSIONS There was no significant difference in the value of the adhesive strength in the comparison between U200 and CXP, however there was a higher value of CXP at cervical and U200 at apical level.

RESUMEN

Este artigo tem como objetivo refletir sobre a atuação do enfermeiro em oncologia, sob a perspectiva da genética e da genômica, e sobre seu papel como membro integrante da equipe multiprofissional e interdisciplinar de aconselhamento genético oncológico. Trata-se de uma reflexão, fruto de leitura minuciosa da literatura da área, acrescida da experiência dos autores e discussões em grupo de pesquisa. No transcorrer desse trabalho, foi possível constatar que o enfermeiro precisa considerar o cuidado de saúde baseado em genômica e apropriar-se de competências essenciais. Essas competências abrangem a habilidade de mobilizar recursos genômicos na coleta da história familiar e nas orientações sobre testes genéticos a famílias em risco para síndromes neoplásicas hereditárias. O profissional de enfermagem pode atuar como referência para os demais membros da equipe de saúde, com potencial para integrar seus conhecimentos no cuidado, no ensino e em pesquisas em oncologia, sob a ótica da genética e da genômica.


This study aimed to reflect on oncology nurses' practice from the perspective of genetics and genomics, and their role as a member of the multiprofessional and interdisciplinary cancer genetics counseling team. This reflection is a result of the detailed reading of literature in the area, increased by the authors' experience and research group discussions. In the course of this work, it was verified that the nurse needs to consider genomic-based health care and incorporates essential competencies. These competencies include the ability to mobilize genomic resources in the family history assessment and in the guidelines on genetic testing for families at risk for hereditary neoplastic syndromes. The nursing staff may act as a reference for other members of the health team, with the potential to integrate their knowledge on care, teaching and research in oncology from the viewpoint of genetics and genomics.


Este estudio objetivó reflexionar sobre la práctica del enfermero en oncología en la perspectiva de la genética y genómica, y su papel como miembro del equipo multiprofesional e interdisciplinario del asesoriamento genético oncológico. Esta reflexión es resultado de lectura atenta de la literatura, además de la experiencia de los autores y discusiones del grupo de investigación. En el curso de este trabajo, fue posible constatar que el enfermero debe tener en cuenta el cuidado de salud basado en genómica y se apropiar de competencias esenciales. Estas competencias incluyen habilidad de movilizar recursos genómicos en la colecta de la historia familiar y orientaciones sobre testes genéticos para familias en riesgo de síndromes neoplásicas hereditarias. El profesional de enfermería puede actuar como referencia para los demás miembros del equipo de salud, con posibilidad de integrar sus conocimientos en asistencia, enseñanza e investigación en oncología, desde el punto de vista de la genética y genómica.

Este estudo tevecomo base outros realizados pela World Allergy Organization(WAO) ao estabelecer os critérios mínimos para a formaçãode um especialista em Alergologia e Imunologia2-4 e tevepor objetivo a busca sobre o perfil do serviço que reunisseas condições mínimas para ser credenciado como apto paraoferecer tal capacitação na especialidade, tomando-se comobase os requisitos mínimos para admissão dos especializandos,os critérios de seleção, o tipo de treinamento, o tempo idealde treinamento, bem como o seu modo de certificação.

OBJETIVO: Determinar a imunofenotipagem eritrocitária em doadores de sangue e em pacientes com anemia falciforme (SS) atendidos no Hemocentro de Alagoas e descrever a frequência e os fatores associados à aloimunização eritrocitária. MÉTODOS: Estudo transversal com 102 pacientes SS e 100 doadores de sangue. Realizou-se a fenotipagem eritrocitária, teste de Coombs Direto e Indireto e detecção de anticorpos irregulares por painel de hemácias fenotipadas. Os dados foram comparados por meio do teste de Mann-Whitney, qui-quadrado ou teste exato de Fisher. Para análise dos fatores associados à aloimunização utilizou-se a regressão logística univariada e múltipla. RESULTADOS: Os antígenos mais frequentes entre os pacientes e os doadores foram c, e, M, s, JK(a). Observaram-se diferenças significativas entre as frequências dos fenótipos dos pacientes e dos doadores em relação aos antígenos s, FY(a) e JK(b). Dos 79 pacientes transfundidos, 10 (12,7 por cento) apresentaram Coombs Indireto positivo. Detectaram-se 13 aloanticorpos, sete do sistema Rh, dois do Kell e quatro não identificados. Os fatores associados à aloimunização foram o intervalo de tempo entre a última transfusão e a data do teste e ter recebido mais de dez transfusões de hemácias. Receber mais de dez transfusões representou uma chance 16,39 (IC 95 por cento: 2,23-120,59) vezes maior de ser aloimunizado, em comparação aos que receberam menos que dez. CONCLUSÃO: A prevalência de aloimunização nos pacientes SS foi 12,7 por cento, sendo 70 por cento dos anticorpos encontrados pertencentes a grupos sanguíneos Rh e Kell. Este estudo mostra a importância da fenotipagem eritrocitária em doadores e receptores para diminuir o risco de aloimunização.


OBJECTIVE: To determine erythrocyte phenotyping in blood donors and patients with sickle cell anemia (SS) treated at Hemocentro of Alagoas and describe the frequency and factors associated with erythrocyte alloimmunization. METHODS: Cross-sectional study with 102 SS patients and 100 blood donors. The following tests were performed: erythrocyte phenotyping, Direct and Indirect antiglobulin test, and detection of irregular antibodies by panel of phenotyped red blood cells. Data were compared by Mann-Whitney, qui-square or Fisher’s exact tests. Factors associated with alloimmunization were studied by univariate and multiple logistic regression analysis. RESULTS: The most frequent antigens found in patients and blood donors were: c, e, M, s, JK(a). Significant differences were observed between the frequency of the phenotype of patients and donors in regard to antigens s, FY(a) and JK(b). Of 79 transfused patients, 10 presented positive Indirect Coombs. Thirteen alloantibodies were found, 7 of the Rh system, 2 of Kell and 4 were not identified. Factors associated with alloimmunization were the period of time between the last transfusion and the date of the test and more than 10 red blood cell transfusions. Patients who received more than 10 transfusions were 16.39 (95 percent CI: 2.23-120.59) times more likely to be alloimmunized than patients with fewer transfusions. CONCLUSION: The prevalence of alloimmunization in SS patients was 12.7 percent, with 70 percent of antibodies belonging to the Rh and Kell systems. This study shows the importance of performing erythrocyte phenotyping in blood donors and receptors to decrease the risk of alloimmunization.
