RESUMEN

The pathogenesis of acute myeloid leukemia (AML) involves mutations in genes such as FLT3 and NPM1, which are also associated with the prognosis of the disease. The immune system influences disease progression, but the mechanisms underlying the interaction between the immune system and AML are not clear. In this study, the profiles of lymphocytes and cytokines were described in individuals with AML stratified by molecular changes associated with prognosis. The participants included in this study were newly diagnosed AML patients (n = 43) who were about to undergo chemotherapy. Subtypes of lymphocytes in peripheral blood, including B cells, T cells, and natural killer cells, and serum concentrations of cytokines, including Th1, Th2, and Th17, were studied by flow cytometry assays (BD FACSCanto II). The correlations between lymphocyte subsets, cytokines, and genetic/prognostic risk stratification (based on the FLT3 and NPM1 genes) were analyzed. The differences in B lymphocytes (%), T lymphocytes (%), plasmablasts (%), leukocytes (cells/µl), and tumor necrosis factor (pg/ml) were determined between groups with FLT3-ITD+ and FLT3-ITD- mutations. The presence of mutations in NPM1 and FLT3-ITD and age suggested changes in the lymphocyte and cytokine profile in individuals with AML.

The osseodensification (OD) technique differs from conventional milling for dental implant installation in that it preserves the prepared bone and compacts it toward the apex and lateral walls of the socket, resulting in bone compaction. By enabling autografting, bone expansion, and high implant insertion torques, OD has become an increasingly popular option. The aim of this clinical case report is to demonstrate the predictability of combining OD with guided bone and tissue regeneration (GBR/GTR) techniques for bone expansion in the maxilla with reduced thickness, while avoiding other reconstructive surgeries. The report presents the treatment of a 32-year-old female patient who had cosmetic concerns regarding the anterior maxillary region. The patient was using an adhesive prosthesis with pontic on tooth 13 fixed between teeth 12 and 14. After the case was planned, it was decided that bone expansion in the region would be performed using the OD technique. The implant installation (AR Torque, 3.5 × 11.5 mm, Conexão®) and guided bone regeneration (GBR) were done with the assistance of L-PRF (Stick Bone, associated with L-PRF membrane). Following the osseointegration period, a provisional resin crown was fabricated, and a collagen matrix membrane (Mucoderm®) was used to increase vestibular soft tissue volume and shape the patient's gingival profile. After a period of 120 days, the final crown was created and observed for a span of 5 years. The results showed stability of the case along with maintaining its esthetic and satisfactory function. The use of the osseodensification technique coupled with a connective tissue graft substitute has been anticipated for a long time. It has proven to be an excellent alternative to autogenous grafts.

Arginine is the main amino acid that constitutes the sperm protamine of roosters, named galline, which complexes with sperm DNA, allowing high compaction of its chromatin. Arginine supplementation has positive effects on semen quality in aged roosters, but this supplementation is not known to limit the progressive worsening of sperm chromatin compaction. This work aimed to verify whether L-arginine supplementation in the feed improve or maintain sperm chromatin quality since aging in roosters is usually accompanied by worsening chromatin quality. Four groups of 52-wk-old Ross AP95 lineage roosters were used, of which 6 semen samples per group were evaluated, totaling 24 samples. Another 24 samples, 6 per group, were evaluated after 6 wk of supplementation when one group was not supplemented (control) and the other 3 were supplemented with 1.15 kg (treatment 1), 2.17 kg (treatment 2), and 3.18 kg (treatment 3) of L-arginine/ton of feed. Computer image analysis of semen smears stained with toluidine blue pH 4.0 was used for sperm chromatin evaluation. Sperm chromatin was evaluated for compaction heterogeneity and compaction intensity by percentage decompaction relative to standard heads and by integrated optical density (IOD), which was used for the first time to identify sperm chromatin changes. Sperm head morphology was also evaluated by means of area and length. The IOD proved to be more efficient in identifying changes in rooster sperm chromatin compaction than the percentual decompaction. In general, chromatin compaction was positively influenced by the supplementation with L-arginine, being better in the supplementation with the highest levels tested. This was corroborated by the smaller average of the variables referring to the size of the spermatozoa heads of the animals that received feed with a higher content of L-arginine, since better compacted heads naturally tend to be smaller. Finally, arginine supplementation was able to limit or even improve sperm chromatin decompaction during the experimental period.

BACKGROUND: Brazil is a country with a high burden of tuberculosis (TB). The immunomodulatory effect of biological therapies is associated with an increased risk of infection. This study evaluated the frequency of TB infection in patients with rheumatoid arthritis (RA), ankylosing spondylitis (AS), juvenile idiopathic arthritis (JIA), and psoriatic arthritis (PsA) after the use of biologic agents in a single center of rheumatology. METHODS: In this observational study, 161 consecutive adult patients with RA, JIA, AS, and PsA using biological therapy were followed up during 55 months to evaluate the occurrence of TB infection throughout treatment. All patients were screened for latent TB infection (LTBI), and TB disease was excluded before introduction of biological therapy. Patients with LTBI received prophylaxis with isoniazid before the start of biological treatment. RESULTS: Of 161 patients on biologics, 31 (19.25%) had positive tuberculin skin test (TST) and received LTBI treatment. Eleven (6.8%) cases of TB were detected in patients on biologics, six (54.5%) had AS, one had PsA (9.09%), two had RA (18.18%), and two had JIA (18.18%). Regarding the use of different biologics, six (54.5%) patients received adalimumab, three (27.2%) infliximab, one (9.09%) etanercept, and one (9.09%) tocilizumab. CONCLUSION: In this study, the frequency of TB infection among 161 patients on biologics, during 55 months of follow-up, was 6.8%. Compared with the national registry of patients receiving biologics (BiobadaBrasil - January 01, 2009 to May 31, 2013), a higher incidence of TB (6.8 versus 0.44%) was found in this sample of patients receiving biological therapy. This study highlights that in a country with high TB burden, the possibility of TB infection in a patient receiving biological therapy should always be considered, even after prophylaxis with isoniazid.

Objetivo: Identificar os sentimentos atribuídos quanto à convivência com o idoso com Doença de Parkinson à luz dos cuidadores familiares. Métodos: Trata-se de um estudo qualitativo, do tipo descritivo-exploratório e transversal. Os participantes do estudo foram os cuidadores familiares que conviviam com o idoso portador da Doença de Parkinson. A amostra foi constituída por 20 cuidadores familiares. Para a coleta de dados utilizou-se os seguintes instrumentos: questionário referente ao perfil pessoal e familiar dos participantes e roteiro de entrevista semiestruturado, a estratégia metodológica para análise das entrevistas foi o Discurso do Sujeito Coletivo (DSC). Resultados: Emergiram as seguintes categorias: "dificuldade pela evolução da doença", "convivo de forma tranquila", "compromisso diário", "sentimento de impotência". Conclusão: É de extrema necessidade cuidar do cuidador e intensificar esforços para amparar estas famílias na sua integralidade, no propósito de eliminar as vulnerabilidades que esses cuidadores são expostos e que implicam diretamente na sua qualidade de vida.


Purpose: To identify the feelings attributed to living with elderly people with Parkinson's disease in the light of family caregivers. Methods: This is a qualitative, descriptive-exploratory and cross-sectional study. The study participants were family caregivers who lived with older adults carrier with Parkinson's disease. The sample consisted of 20 family caregivers. For data collection, the following instruments were used: questionnaire regarding the personal and family profile of the participants and a semi-structured interview script, the methodological strategy for the analysis of the interviews was the Discourse of the Collective Subject (DSC). Results: The following categories emerged: "difficulties by evolution of illness", "live together with ease", "daily commitment", and "feeling of impotence". Conclusion: It is of extreme necessity to care for the caregiver and to intensify efforts in order to support these families in their integrality, in the purpose of eliminating the vulnerabilities that these caregivers are exposed and that imply directly in their quality of life.

Objetivo: identificar os sentimentos vivenciados pelo cuidador familiar sob a ótica de cuidar de idoso com doença de Parkinson. Metodologia: estudo qualitativo, descritivo-exploratório e transversal, realizado com 20 cuidadores familiares, utilizou-se dois instrumentos, os dados foram tratados segundo a técnica "Discurso do Sujeito Coletivo" proposta por Lefèvre e Lefèvre. Resultados: os sentimentos vivenciados pelo cuidador familiar emergiram em cinco categorias: sobrecarga física e mental; dificuldade de cuidar; não é difícil cuidar; sentimento de impotência; sentimento de tristeza. Conclusões: efetivamente, pensar na relação entre envelhecimento, doença de Parkinson e cuidador familiar requer compreensão, discussão e principalmente ação. O escopo da ação é aproximar o cuidador familiar com os profissionais de saúde e este mostrar mecanismos que auxiliem os membros familiares a encontrar soluções para uma melhor qualidade de vida e relação interpessoal. Metodología: estudio cualitativo, descriptivo-exploratorio y transversal, con 20 cuidadores familiares, se utilizaron dos instrumentos, los datos fueron tratados según la técnica del "Discurso del Sujeto Colectivo", propuesta por Lefèvre y Lefèvre. Resultados: los sentimientos vivenciados por el cuidador familiar emergieron en cinco categorías: sobrecarga física y mental; dificultad para cuidar; no es difícil cuidar; sentimiento de impotencia; sentimiento de tristeza. Conclusiones: Efectivamente, pensar en la relación entre envejecimiento, enfermedad de Parkinson y cuidador familiar requiere comprensión, discusión y principalmente acción. El alcance de la acción es acercar al cuidador familiar con los profesionales de la salud y éste mostrar mecanismos que ayuden a los miembros familiares a encontrar soluciones para una mejor calidad de vida y relación interpersonal. O objetivo deste estudo é identificar os significados para os familiares de conviver com idosos com sequelas de AVC. Os participantes do estudo foram os familiares que conviviam com idoso do gênero masculino e feminino. A amostra foi constituída por 15 membros familiares. Os instrumentos utilizados para a coleta de dados foram questionário referente à caracterização pessoal e familiar dos participantes e o roteiro de entrevista semiestruturada. Para extração das ideias principais, foi utilizada a Análise de Conteúdo de Bardin. As categorias emergentes do tema 'Significado de conviver' foram 'Experiência dolorosa', 'Mudança de hábito e de vida', 'Uma situação muito difícil', 'Ser resiliente' e 'Dificuldade da pessoa em aceitar a doença'. Conclui-se que conviver com idosos com sequelas de AVC assume caráter com implicações físico-psicológicas e familiares.(AU)


The objective of this study is to identify the meanings for relatives who live with an elderly with stroke sequelae. The study participants were family members cohabiting with elderly men and women with stroke sequelae. The sample consisted of 15 family members. Data was gathered using a questionnaire on personal and family characteristics of the participants and semi-structured interviews. We used the Bardin Content Analysis in order to extract the main ideas. The emerging categories for the theme 'meaning of living together' were 'a painful experience', 'habits and life changes', 'a very difficult situation', 'to be resilient' and 'the difficulty of the individual in accepting the disease'. We conclude that living with elderly individuals with stroke sequelae has physical, psychological and familial implications.(AU)


El objetivo de este estudio es identificar los significados para los familiares de convivir con una persona anciana de la familia con secuelas de AVC. Los participantes del estudio fueron los familiares que convivían con un anciano con secuelas de AVC del genero masculino y femenino. La muestra estaba formada por 15 miembros de la familia. Los instrumentos utilizados para la colecta de datos fueron: cuestionario referente a la caracterización personal y familiar de los participantes y el guión de entrevista semi-estructurada. Para la extracción de las ideas principales se utilizó el Análisis de Contenido de Bardin. As categorías emergentes del tema 'Significado de convivir' fueron: 'Experiencia dolorosa', 'Cambio de hábito y de vida, 'Una situación muy difícil', 'Ser resiliente' y 'Dificultad de la persona para aceptar la enfermedad. Se concluye que convivir con ancianos con secuelas de AVC asume carácter con implicaciones físicas, psicológicas y familiares.(AU)

The objective of this study was to identify meanings that family members attribute to the act of caring for elderly individuals with stroke sequels. The participants were family members that were taking care of a senior with stroke sequels, both male and female. The sample was composed of 15 family members. The instruments used for data collection were: a questionnaire referring to the participants' personal and family profile, in addition to a semi-structured interview script. For extraction of main ideas, Bardin's content analysis was used. The categories that emerged from the 'Meaning of caring' theme were: 'A lot of love and support', 'Hard because of the person's dependence', 'Caring is not hard', 'Responsibility' and 'Role change'. The conclusion is that caring for an elderly person with stroke sequels involves a variety of aspects and situations.


O objetivo deste estudo foi identificar os significados para os familiares em cuidar de um idoso sequelado de AVC. Os participantes do estudo foram os familiares que cuidavam de um idoso sequelado de AVC, tanto do gênero masculino quanto do feminino. A amostra foi constituída por 15 membros familiares. RESUMEN

Chédiak-Higashi syndrome is a rare hematological disease characterized by increased fusion of cytoplasmatic granules. Neurological symptoms occur in approximately half of the patients. We describe the clinical, eletrophysiological, hematological and radiological findings in a girl who had Chédiak-Higashi syndrome and seizures.

RESUMEN

No período de julho de 1989 a setembro de 1992, 21 pacientes foram submetidos a 24 artroplastias totais do quadril, com cúpulas acetabulares rosqueadas do tipo CO-10. Todos foram reavaliados clínica e radiologicamente, com seguimento médio de 54 meses. As avaliaçöes mostraram bons resultados em 96 por cento dos casos.
