BACKGROUND: The increase in the use of psychoactive substances, alcohol and cigarettes in young people has become a public health problem. The identification of factors that increase or reduce the risk of exposure to these substances and the possible relationship between them is essential for planning strategies with a risk approach; hence the reason for this study. The objective was to establish the profile of use of psychoactive substances, alcohol and cigarettes and the factors associated with such use in nursing students of a higher education institution. METHODS: Quantitative, observational, analytical cross-sectional study. RESULTS: We included 310 students from 1â¯st to 9th semester of a Nursing programme from a private higher education institution in Bogotá. The prevalence of psychoactive substance use in the last year was 2.96% (95%CI, 1.36-5.54), with marijuana being the substance most used (55.55%). The prevalence of alcohol and cigarette use during the last 12 months was estimated at 86.64% (95%CI, 83.24-91.0) and 12.16% (95%CI, 8.43-15.88) respectively. A statistically significant association was found between the use of these substances: alcohol use was associated with cigarette use (ORâ¯=â¯3.22; Pâ¯=â¯0.006) and smoking was associated with psychoactive substance use (ORâ¯=â¯15.4; Pâ¯<â¯0.001). CONCLUSIONS: Alcohol use increases the likelihood of smoking cigarettes, and this in turn increases the likelihood of psychoactive substance use, in this university population.

RESUMEN Introducción: El incremento del consumo de sustancias psicoactivas (SPA), alcohol y cigarrillo en población joven se ha convertido en un problema de salud pública. La identificación de los factores que aumentan o disminuyen el riesgo de la exposición a estas sustancias y la relación que puede haber entre ellas resulta fundamental para la planificación de estrategias con enfoque de riesgo, razón por la cual se planteó el presente estudio. El objetivo es establecer el perfil de consumo de SPA, alcohol y cigarrillo y los factores asociados con dicho consumo en estudiantes del programa de Enfermería de una institución de educación superior. Métodos: Estudio cuantitativo, observacional y analítico de tipo corte transversal. Resultados: Se incluyó a 310 estudiantes de los semestres I a IX de un programa de Enfermería de una institución privada de educación superior de Bogotá. La prevalencia de consumo de SPA en el último ario fue del 2,96% (IC95%, 1,36-5,54); la marihuana es la sustancia de mayor consumo (55,55%). Las prevalencias de consumo de alcohol y cigarrillo durante los últimos 12 meses se estimaron en el 86,64% (IC95%, 83,24-91,0) y el 12,16% (IC95%, 8,4315,88) respectivamente. Se encontró una asociación estadísticamente significativa entre los consumos: tomar alcohol se asoció con el consumo de cigarrillo (OR = 3,22; p = 0,006) y fumar se asoció con el consumo de SPA (OR = 15,4; p <0,001). Conclusiones: En esta población universitaria, el consumo de alcohol aumenta la posibilidad de consumo de cigarrillo, y este a su vez aumenta la posibilidad del consumo de SPA.

RESUMEN
OBJECTIVES: To determine the association between parenthood and academic performance and dropout among professional Nursing degree program students. METHODS: A prospective cohort was conducted. 310 students were included, 26 parents (exposed) and 284 nonparents (unexposed), who were followed up during 16 weeks. At the end of this follow-up, student records were reviewed. RESULTS: Academic performance in the exposed and unexposed groups was 3.51/5.0 and 3.64/5.0 respectively. Although it was higher in the latter group, i.e., with a difference of 0.13, the values were not statistically significant (p=0.058). After adjusting for confounding variables, a difference of -0.165 point was seen (p=0.037). The risk of students dropping out of college is 34 % less for exposed compared to those who are unexposed (RR: 0.66; CI: 0.075-5.78; p=0.708). CONCLUSION: The results suggest that parenthood has a modest impact on the academic performance, and has no impact on the risk of dropping out.

Resumen Introducción: La insuficiencia cardíaca (IC) es una patología estructural o funcional del corazón que deteriora la capacidad de llenado, generando síntomas que afectan la calidad de vida y la funcionalidad. Es conocido el impacto de esta enfermedad sobre los sujetos que la padecen, pero ha sido poco explorado su impacto económico sobre los cuidadores. Objetivo: Estimar el impacto económico de la IC desde la perspectiva de los cuidadores informales. Resultados: Estudio cuantitativo, tipo evaluación económica parcial. Fueron incluidos 54 sujetos diagnosticados con IC y un cuidador informal por sujeto. La edad promedio de los sujetos con IC fue 70.61 años, la mayoría hombres (64.8%), con un tiempo de diagnóstico de 84 meses y un puntaje de Barthel de 81.94, que representa una dependencia leve. Respecto a los cuidadores, el 48.1% reportaban un vínculo laboral activo; el ingreso mediano fue de $1260.000 COP/mes. El número de horas de cuidado fue de 4.93 h/día. El impacto económico se reflejó en los ingresos mensuales, causando una pérdida de $644,017.50 por actividades de cuidado y en el tiempo laboral una reducción del 30%. El 29% de los ingresos fueron destinados a labores del cuidado; por lo tanto, generaron gastos que redujeron el 71% de los ingresos. De acuerdo con lo anterior, por cada 10 horas de trabajo se destinan 7 horas al cuidado. Conclusiones: La IC impacta de manera negativa la economía de los cuidadores informales de sujetos con esta patología.
Abstract Introduction: Heart failure (HF) is a structural or functional pathology of the heart that impairs filling capacity of this organ, generating symptoms that affect quality of life and functionality in subjects who suffer this disease. This impact on economic life in caregivers of patients with HF has been poorly studied, therefore, there is a lack of information about this topic. Objective: To estimate the economic impact of HF from the perspective of informal caregivers. Type of study: quantitative study, partial economic evaluation type. Population and sample: 54 subjects diagnosed with HF and one informal caregiver per subject were included. Results: The average age of the subjects with HF is 70.61 years, most of them men (64.8%), with a diagnosis time of 84 months and a Barthel score of 81.94 points, which represents a mild dependency. Regarding caregivers, 48.1% reported an active employment relationship, the median income was $1,260,000 COP/month. The number of hours of care is 4.93 hours/day. The economic impact was reflected in monthly income, causing a loss of $644,017.50 due to care activities and a 30% reduction in working time. 29% of income was allocated to care work, therefore, they generated expenses that reduced 71% of income. According to the above, for every 10 hours of work, 7 hours are spent on care. Conclusions: HF negatively impacts the economy of the informal caregiver of subjects with this pathology.

Tracheostomy is the commonest bedside surgical procedure performed on patients needing mechanical ventilation with traumatic brain injury (TBI). The researchers made an effort to organize a narrative review of the indications, timing, management, complications, and outcomes of tracheostomy in relation to neuronal and brain-injured patients following TBI. The study observations were collated from the published literature, namely original articles, book chapters, case series, randomized studies, systematic reviews, and review articles. Information sorting was restricted to tracheostomy and its association with TBI. Care was taken to review the correlation of tracheostomy with clinical correlates including indications, scheduling, interventions, prognosis, and complications of the patients suffering from mild, moderate and severe TBIs using Glasgow Coma Scale, Glasgow Outcome Scale, intraclass correlation coefficient, and other internationally acclaimed outcome scales. Tracheostomy is needed to overcome airway obstruction, prolonged respiratory failure and as indispensable component of mechanical ventilation due to diverse reasons in intensive care unit. Researchers are divided over early tracheostomy or late tracheostomy from days to weeks. The conventional classic surgical technique of tracheostomy has been superseded by percutaneous techniques by being less invasive with lesser complications, classified into early and late complications that may be life threatening. Additional studies have to be conducted to validate and streamline varied observations to frame evidence-based practice for successful weaning and decannulation. Tracheostomy is a safer option in critically ill TBI patients for which a universally accepted protocol for tracheostomy is needed that can help to optimize indications and outcomes.

BACKGROUND: The hornworn Erinnyis ello is the major pest of natural rubber crops in Colombia, mainly controlled using toxic chemical insecticides. The use of E. ello Betabaculovirus is an environmentally sustainable alternative for its control. The aim of the present work was to characterize a prototype biopesticide formulation and evaluate its efficacy under different conditions. RESULTS: Quality control evaluations of formulated biopesticide revealed that all the parameters evaluated were under the permissible level. The lethal concentrations LC50 and LC90 of the biopesticide were 4.3 × 103 and 5.5 × 104 occlusion bodies (OBs) mL-1 , respectively. Biopesticide efficacies against second and fourth instar larvae under greenhouse conditions were higher than 80%. Evaluation of two application rates in a clonal garden resulted in 84% and 88% efficacy, comparable to that obtained with the chemical. The biopesticide in a commercial plantation showed efficacies between 74% and 82%. Biopesticide post-application persistence was estimated at least in 1 week under field natural conditions. Results allowed selection of the lowest evaluated dose (1 × 1011 OBs ha-1 ) as the basis for further field evaluations. CONCLUSION: Formulated ErelGV showed high efficacy to control the hornworm in rubber crops and high potential to be included in integrated pest management programs, thus it could be an interesting alternative to replace agrochemicals. © 2018 Society of Chemical Industry.

PURPOSE: We describe the implementation process of a computerized physician order entry (CPOE) for outpatient chemotherapy at a Latin American hospital, with the intent of providing other institutions with general guidance and insight through our experience. METHODS: In 2012, under the direction of the Department of Medicine of the Instituto Nacional de Enfermedades Neoplásicas, a multidisciplinary team composed of oncologists, nurses, pharmacists, and informatics engineers was formed to develop software for a CPOE for chemotherapy within a preexistent homegrown electronic medical record system in various phases. This included mapping and redesigning processes in an entirely electronic format, integrating the needs of the user for the development of electronic order sets, developing a checkpoint and a warning system to minimize prescription errors, and finally, training all the staff in implementation of the system. RESULTS: A CPOE for outpatient chemotherapy was successfully implemented in 2016. We have successfully standardized 266 chemotherapy orders, including for both solid tumors and hematologic malignancies, on the basis of appropriate guidelines. The software is linked to laboratory results and allows entry of important details for the patient's safety, such as anthropometric information for an automatic dose calculation and ranges for safe prescription. In addition, it is linked to the nursing plan sheets. Finally, it is possible to assess and continuously monitor the complex process of chemotherapy prescription. CONCLUSION: This is the first report of implementation of a CPOE for chemotherapy in our region. The system was designed by a multidisciplinary team with its own resources. Our experience demonstrates the feasibility of computerizing the chemotherapy prescription process, constituting a tangible example for other institutions with potential impact on patient care.

In high altitude Andean streams an intense solar radiation and coinciding metal pollution allow the persistence of only a few specialized taxa, including chironomids. The aim of the present study was therefore to determine the mechanisms underlying the persistence of chironomids under these multiple stress conditions, hypothesizing that melanin counteracts both the adverse effects of solar radiation and of metals. Melanin was determined in chironomids from reference and metal polluted streams at 3000 and 4000 m altitude, being 2-fold higher at 4000 m compared to 3000 m, and 2-fold higher in polluted streams than in reference streams at both altitudes. The field observations were experimentally verified by assessing the combined effects of Cu and UV-B on the survival and melanin concentration in larvae of the model species Chironomus riparius (Chironomidae, Diptera). In laboratory exposures, the highest melanin concentrations were found in larvae surviving toxic Cu concentrations, but not in those exposed to the highest UV-B radiation. Pre-exposure to UV-B decreased the sensitivity of the larvae to UV-B and to Cu+UV-B. It is concluded that in the field, melanin may protect chironomids partially against both elevated metal concentrations and solar radiation, allowing them to persist under the harshest conditions in high altitude streams.

High altitude creates unique challenging conditions to biota that limit the diversity of benthic communities. Because environmental pollution may add further stress to life at high altitude, the present study explored the effect of metal pollution on the macroinvertebrate community composition in Andean streams between 3,500 to 4,500 meters above sea level (masl) during wet and dry seasons. At polluted sites, showing a high conductivity and a low pH, metal concentrations (e.g., Al, 13.07 mg/L; As, 3.49 mg/L; Mn, 19.65 mg/L; Pb, 0.876 mg/L; Zn, 16.08 mg/L) ranged from 8-fold up to 3,500-fold higher than at reference sites. The cumulative criterion unit allowed quantifying the potential toxicity of metal mixtures at the contaminated sites. Principal component analysis of physical chemical variables showed that reference sites were more likely to be structured by transparency, water discharge, and current velocity, while polluted sites appeared to be determined by metals and conductivity. Canonical correspondence analysis indicated a strong influence of highly correlated metals in structuring invertebrate communities, which were dominated by dipterans, coleopterans, collembolans, and mites at polluted sites. At reference sites crustaceans, ephemeropterans, plecopterans, and trichopterans were the most representative taxa. We concluded that severe metal pollution induced changes in macroinvertebrate community composition in high-altitude Andean streams, with a replacement of sensitive taxa by more tolerant taxa. Yet relatively species-rich communities persisted under harsh conditions.

El método más utilizado en la actualidad para la detección precoz del cáncer del cérvix uterino (CCU) es la toma de citología cervicovaginal (CCV), la cual ha demostrado en forma amplia su utilidad dada su capacidad para detectar las lesiones precursoras del CCU, lo cual ha contribuido a disminuir de modo notable la morbimortalidad por esta neoplasia. La consecuencia inmediata de los programas de muestreo ha sido el incremento significativo del número de biopsias cervicales. Su análisis histológico es importante pues se considera el patrón de referencia (gold standard) en que el clínico se basa para planificar el tratamiento adecuado para estas pacientes. Sin embargo, existen casos en los que el diagnóstico histopatológico preciso es subjetivo y susceptible de opiniones variadas entre los observadores, hecho que ha incitado la búsqueda de técnicas alternativas como la inmunohistoquímica, para ayudar a establecer un valor pronóstico más confiable en las lesiones preneoplásicas del cérvix. Es por esto que nuestro estudio tiene como objetivo principal determinar la expresión IHQ de antìgenos de VPH (subtipo de alto riesgo) y el índice de proliferación celular en lesiones intraepiteliales escamosas de bajo grado (LIEBG) y lesiones intraepiteliales escamosas de alto grado (LIEAG), utilizando los marcadores biomoleculares Ki67, P16ink4a y Viroactiv®.
Cytology-based screening is currently the most used strategy for early detection of cervical cancer. Cytology has proved to be significantly useful due to its ability to detect precancerous lesions, thus, contributing to a substantial reduction of morbidity and mortality due to this neoplasia. The imme-diate consequence of screening programs has been a significant increase on the number of cervical biopsies performed. The importance of histological analyses of these samples lies on the fact that they are considered the gold standard in which the clinicians base adequate treatment plans for patients. Nevertheless, in some cases a precise histology/pathology interpretation is subjective and susceptible to various opinions among observers. Due to the latter, finding alternative diagnostic techniques suchas immunohistochemistry (IHC) has been encouraged to help establish a more reliable prognostic value in cervical preneoplastic lesions. For this reason, the main purpose of our study is to determi-ne the IHQ expression of HPV antigens (high-risk sub-type) and cell proliferation rate in low-grade squamous intraepithelial lesions (LSIL) and high-grade squamous intraepithelial lesions (HSIL), using biomolecular markers such as Ki67, Pl6ink4a and Viroactiv ®.
