The intraoperative hemodynamics was evaluated in 42 relative kidney donors. Electromagnetic flowmetry was used for the evaluation. The rate of blood flow was calculated with the formula of Hunt. The comparison of the existing rate of blood flow with a proper one has demonstrated the existence of intrakidney angiospasm. The donors over 50 years of age and the female donors are more predisposed to angiospasm. It was noted that in harvesting the right kidney the intraorganic angiospasm is expressed much more strongly than in harvesting the left kidney. The phenomena of intrakidney angiospasm in harvesting kidneys in live donors is of practical importance. Special measures should be undertaken to prevent this complication.

Electromagnetic flowmetry was performed in 53 recipients of kidneys from related donors to investigate intraoperative hemodynamics and its relationships with such factors as the degree of histocompatibility between the donor and recipient, of blood relationship, renal laterality. The real total blood flow was compared to proper flow according to Inke formula. The result of this comparison gave evidence for angiospasm or vasodilation. The range of real flow was much wider in the recipients than in donors. This reflected much more frequent angiospasms and vasodilation in the recipients: the transplants displayed angiospasms in 26.2% of the cases. The angiospastic patients developed rejection crises 2 times more frequently than those without the spasms. It is concluded that intraoperative vasoconstriction denotes greater probability to develop rejection crisis in early postoperative period.

The functional capacity of the lymph vessels in their various pathological conditions was determined from 78 studies of the endolymphatic pressure in patients with lymphatic edema of the extremities. Four types of pressure curves were determined according to the maintenance of the contractility of the wall of the lymph vessels. It was found that marked hypertension develops in the lymphatic channel in obstruction of lymph drainage from the extremity. Endolymphatic pressure in hyperplasia of the lymph vessels and blocking of lymph drainage exceeds venous pressure several times, which confirms the competence of operation for the formation of direct lymph-vein anastomoses.

The authors analyze the causes of gastrointestinal hemorrhages in 64 children aged 11 months to 15 years. 45 children had ulcerous and 19 non-ulcerous hemorrhages. The presence of gastrointestinal hemorrhages is an absolute indication for emergency esophagogastroduodenoscopy which appeared helpful in visualization of the source of hemorrhage in 60 out of the 64 patients (93.7%). 8 children underwent surgery, whereas in the remaining ones, hemorrhage was arrested by conservative treatment methods.

A method of dosed compressive loads with a simultaneous registration of linear blood flow rate in the femoral artery was used in 32 healthy people and 30 patients with different localization of atherosclerotic process in major vessels with the help of an ultrasound apparatus of Bi-Doppler class and specific blood filling of lower extremities by the method of tetrapolar rheography. It was shown that most sensitive and objective index for the assessment of major and collateral blood circulation is a decrease or absence of the retrograde blood flow.

The results of the treatment of 102 children at the age of from 25 days to 14 years with the Meckel's diverticulum (uncomplicated--in 37, complicated--in 65) were analysed. Tactics of treatment depended on the state and age of the children, history of the disease, findings of roentgenocontrast and radionuclide study of the stomach and intestine, laparoscopy, macro- and microscopic picture, state of the abdominal cavity at the moment of operation. In most patients, the favourable results of treatment were achieved. The lethality was 3%.

A total of 510 cases of primary and secondary lymphoedema in upper and lower extremities have been examined with two thirds of primary lymphoedema cases among them. Radical mastectomy combined with irradiation has led to the development of obstructive lymphoedema of upper limbs in most patients. 230 patients with primary and secondary lymphoedema have been subjected to microlymphatic surgery and lymphaticovenous anastomoses have been carried out with microsurgical techniques. Limb volume measurement, the results of direct lymphangiography and intralymphatic pressure reading were taken into account to select patients suitable for microlymphatic surgery. Most favourable results have been obtained in secondary obstructive lymphoedema cases (total 81.2%) due to well-marked hypertension in lymphatic vessels that contributes to better functioning of the lymphaticovenous anastomoses made in a patient.

Eighty-seven autovenous shunts were examined using ultrasonic dopplerography to assess the patency of aorto-coronary shunts in 32 patients. B-scan was used to visualize the shunts and measure their inner diameter. Liner blood flow rate was measured with a Doppler adapter distinguishing the direction of the flow. Methodologic aspects of the application of this method to the assessment of the patency and function of aorto-coronary shunts are outlined. To verify the method, selective angiography was performed in 18 patients. A significant correlation was demonstrated between the two techniques. Ultrasonic Doppler scanning measuring linear blood flow rate is a safe, non-invasive and reliable method for the assessment of patency of aorto-coronary shunts. It can be used on an out-patient basis.

Clinical findings in central hemodynamics of patients with postinfarction aneurysm of the heart are reported. Hemodynamics and myocardial contractility were examined during surgery in 92 patients. Aneurysmectomy was shown to normalize, while still in the surgery, residual volumes of heart cavities and increase significantly myocardial performance. A method for recording hemodynamic information and results of surgical treatment, with blood pressure and flow rate presented as diagrams, is discussed.

The indices of central hemodynamics and myocardial contractile function were studied in 35 dogs before and in different periods after the administration of drugs which block beta-receptors: propranolol, pindolol, and talinolol. The drugs blocking the beta-adrenergic receptors were administered against the background of an intact myocardium to 15 dogs and against the background of acute coronary insufficiency to another 15; acute ischemia was induced in 5 dogs to which the drugs were not given. It was established that beta-adrenergic blocking agents have a beneficial effect in the acute stage of myocardial ischemia; they exert a marked influence on the consumption of oxygen by the myocardium, intramyocardial tension, and the contractile capacity and rhythm of the heart. Talinolol produced the most favourable effect.
