Craniopharyngiomas are supra/parasellar lesions that often present with general, unspecific symptoms. Similarly, internal carotid artery (ICA) bifurcation giant aneurysms may also produce calcified, heterogeneous, parasellar expansive lesions, posing a relevant differential diagnosis due to their inherently different surgical strategies and risks. We report the case of a 54-year-old female presenting with progressive disorientation and apathetic behavior. CT and MRI reports described a suprasellar heterogenous mass with calcifications associated with an adjacent, laterally located fluid collection suggestive of a craniopharyngioma. During the surgical procedure, perfuse and unexplained arterial bleeding ensued, prompting the surgical team to review a previous contrast-enhanced CT scan. Careful inspection revealed an image suggestive of vascular pathology, with an area of continuous hyperdensity along the right ICA bifurcation. The Sylvian fissure was dissected, and an aneurysmal neck was encountered and successfully clipped. Giant intracranial aneurysms are rare but essential differential diagnoses to be considered during the workup and surgical approach toward parasellar mass lesions. This case illustrates the importance of performing a CT angiogram (CTA) for skull base lesions, even when the size is more suggestive of tumor pathology.

BACKGROUND: Despite mainly benign, exophytic subcutaneous cranial masses present with a myriad of differential diagnosis possibilities, ranging from simple, superficial lesions to complex lesions involving the central nervous system. Although the gold standard imaging modality for the diagnosis of these lesions is magnetic resonance imaging, Doppler Ultrasonography can be a useful, inexpensive, and available tool for evaluation of lesions that could potentially be safely treated in the primary care setting, and lesions that would demand advanced neurosurgical care. CASE DESCRIPTION: This patient presented with a complex exophytic plasmocytoma that was first diagnosed and erroneously approached as a subcutaneous lipoma with surgical resection in an outpatient surgical setting. This interpretive approach resulted in the failure of the procedure due to significant hemorrhage. The patient was immediately referred to neurosurgical care and transferred to our center. Admission doppler ultrasound imaging revealed absence of the frontal bone, the enriched and profuse vascularization, allowing further and proper diagnostic approach and treatment. CONCLUSION: Ultrasound could be a reliable, fast, and simple imaging method aiding practitioners to perform a better workup for patients with exophytic subcutaneous cranial masses.

BACKGROUND: Ependymoma is a slowly growing benign neoplasm that constitutes 3-9% of all neuroepithelial spinal cord tumors.[3,4] They rarely involve the cervicomedullary junction where they both compress the distal brainstem and upper cervical cord. Due to the critical contiguous structures, gross total resection of these lesions may result in significant morbidity/mortality.[1,2] Utilizing intraoperative neuromonitoring can help limit the risks of removing these lesions. Not when considering the risk/complications of partial versus total resection, the surgeon should keep in mind that they are benign slow growing tumors with relatively good long-term survivals following partial removals. This surgical video shows the surgical strategy and management of a giant cervicomedullary ependymoma performed in a 23-year-old female. CASE DESCRIPTION: A 23-year-old female presented with cervical pain and quadriparesis of 1-year's duration. The MR with/without gadolinium showed a large intradural, intramedullary cervical spinal cord tumor that severely expanded the spinal cord. It contained a significant cystic component, extending from the lower brain stem to the inferior aspect of C7. The lesion was hyperintense on T1 and T2 sequences and demonstrated minimal contrast enhancement. Surgery warranted a posterior cranio-cervical midline approach consisting of a suboccipital craniectomy with laminotomy. The pathological diagnosis was consistent with an ependymoma (WHO I). Fifteen days postoperatively, the patient was discharged with a minimal residual quadriparesis that largely resolved within 6 postoperative months. Three months later, the MRI confirmed complete tumor removal of the lesion. Notably, longer-term follow-up is warranted before complete excision can be confirmed. If there is a recurrence, repeat resection versus stereotactic radiosurgery may be warranted. CONCLUSION: This video highlights a safe and effective surgical technique for the resection of a giant cervicomedullary ependymoma.

Intraorbital foreign body is a rare condition, especially when extending into the intracranial compartment. When facing this scenario in the ER, the neurosurgeon must carefully choose the optimal point of surgical access in order to reduce morbidity. The authors hereby report the case of a 66 year-old male with a penetrating trauma to the orbit reaching the anterior cranial base through the orbital roof and associated with an intracerebral hematoma. The removal of the foreign body was performed by a dual approach: an 'eyebrow' supraorbital keyhole craniotomy and an intra-orbital extra-ocular exploration, with later microsurgical drainage of the hematoma and evisceration of the eye 48 hours later. The patient developed a pseudomeningocele, which was treated with lumbar puncture and compressive dressing. After proper intravenous antibiotic prophylaxis, the patient was discharged 21 days after hospital admission.

Genitalia-related complications of ventriculoperitoneal shunts, such as scrotal migrations, are rare and most frequently presenting during the first year of the system placement, usually in the pediatric population, due to several factors, including vaginal process patency and increased abdominal pressure. Despite being typically benign, hernias, hydroceles, perforations, and catheter migration to the scrotum can lead to permanent disabilities and lethal complications, such as ventriculoperitoneal shunt dysfunction. We report a case of a late-onset, atraumatic, ventriculoperitoneal shunt fracture and catheter migration to the scrotum in a 22-year-old male, six years after its implantation, presenting in the emergency department due to acute hydrocephalus symptoms.

OBJECTIVE: It was to observe whether a microsurgical gross total removal (GTR) of a parasagittal meningioma (PSM) outside the sinus is a safe and efficient procedure. METHOD: We identify 58 parasagittal meningiomas patients. Medical charts, operative reports, imaging studies and clinical follow-up evaluations were reviewed. RESULTS: GTR of the mass was achieved in 45 (77.7%) instances. The surgical mortality rate was 1.7%. The median follow-up time was 63 months. CONCLUSION: The surgical approach used in this group of patients afford that the great majority of tumors could be totally removed with low mortality, proving to be safe and effective.

Objetivo: Foi observar se a ressecção microcirúrgica extrassinusal dos meningiomas parasagitais é uma técnica segura e efetiva. Método: Foram estudados 58 pacientes portadores de meningiomas parasagitais. Quadros médicos, relatórios de operações, exames de imagem e seguimento clínico foram revisados. Resultados: A remoção total da lesão, fora do seio, ocorreu em 45 casos (77,7%). A taxa de mortalidade cirúrgica observada nesta série foi de 1,7%. O tempo médio de seguimento foi de 63 meses. Conclusão: A estratégia microcirúrgica empregada propiciou a remoção total dos meningiomas na maioria dos pacientes, com baixa mortalidade e morbidade, mostrando ser segura e efetiva. .
