RESUMEN

Angiodysplasia of the gastrointestinal tract is thought to be one of the most common causes of lower gastrointestinal bleeding in the elderly, and, in the majority of cases, lesions are located in the cecum or ascending colon. The authors report an extremely rare case of appendicular angiodysplasia. A 76-yr-old woman was hospitalized with massive recurrent red anal bleeding. Selective superior mesenteric arteriography revealed an extravasation of contrast material from the appendicular artery, and this finding proved to be bleeding from an angiodysplasia of the appendix. An appendectomy was performed, and anal bleeding did not recur postoperatively. A review of the literature revealed this to be an extremely rare case of angiodysplasia.

RESUMEN

Angiomyolipoma of the liver is an extremely rare type of tumor and reported cases are few and far between. This case study reports on a 37-year-old woman who suffered from multiple angiomyolipomas of the liver. Ultrasonography (US) and computed tomography (CT) initially revealed the existence of two tumors located in the right lobe of the liver and measuring 20 x 15 mm and 8 x 7 mm in size. The patient underwent partial resections of the liver. Histopathologic examination of both the resected specimens revealed angiomyolipoma. This is the first reported case of multiple angiomyolipomas of the liver. Furthermore, the lesions of our case study were small in size and rich in fat component. Under such conditions, preoperative examination resulting in an angiomyolipoma diagnosis is difficult even with the aid of imaging modality and needle biopsy techniques.

RESUMEN

We report on a case of resected cholesterol polyp with typical ultrasonic findings that became atypical, with a 40% increase in diameter, over a period of about 10 months. The resected specimen revealed a black polyp with a large-nodular appearance. The pathological diagnosis was cholesterol polyp with marked glandular proliferation, and the glandular cavity showed cystic dilatations filled with dark brown fluid. It is suspected that the aggregation of large numbers of these dilated glands explained the black color, the rapid growth, and the development leading to the atypical ultrasonic findings.

The purpose of this study is to clarify positional relationship between the bile duct of the caudate lobe and the bile duct of the hepatic hilum through endoscopic retrograde cholangiogram (ERC). Until December 1984 all 100 ERC that met the following three conditions were studied which at Yachio Hospital. 1. The first cholangiogram. 2. Without obstructive jaundice. 3. No existence of lesion at the hepatic hilum. Bile duct of the caudate lobe were classified into 4 separate types: 1) Ducts from the cranialis of the right caudate lobe which pass via the inferior vena cava to the hepatic hilum named Blr, were confirmed in 32. 2) Ducts from the cranialis of the left caudate lobe to the hepatic hilum named Bl l s, were confirmed in 19. 3) Ducts from the left lateral part of the left caudate lobe to the hepatic hilum named Bl l i were confirmed in 37. 4) Ducts from the caudate process to the hepatic hilum named Blc, were confirmed in 5. One or more of the above bile ducts were confirmed in 53 cases. All four of the above were confirmed in only 2 cases out of 53 cases. Three were confirmed in 7 out of 53 cases, two were confirmed in 20 cases out of 53 cases and one was confirmed in 24 out of 53 cases. Studies for the bile duct of the caudate lobe have been conducted anatomically, but there has been no research by clinical examination.(ABSTRACT TRUNCATED AT 250 WORDS)

RESUMEN

A 63-year-old man complained of hunger epigastralgia. X-ray and endoscopic examination demonstrated Borrmann II type gastric carcinoma. Radical gastrectomy was performed on October 20, 1982. The histological findings revealed moderately differentiated tubular adenocarcinoma. In May 1983, right hydronephrosis was recognized by ultrasonography. A papillary tumor was visualized in the right ureter in retrograde pyelography, and percutaneous transrenal urinary tract drainage was performed. Cytological findings revealed class V. Total nephro-uretectomy was performed on June 16, 1983. The histological findings revealed transitional cell carcinoma.
