RESUMO Introdução: A competência dos profissionais de saúde em radiologia não apenas desempenha um papel fundamental no controle de custos, mas também na prevenção de erros médicos decorrentes da inexperiência durante a prática clínica. Objetivo: Este estudo realiza uma revisão sistemática com o objetivo de investigar as estratégias pedagógicas empregadas no ensino de diagnóstico por imagem nas instituições médicas brasileiras, bem como analisar o nível de conhecimento adquirido pelos estudantes de Medicina ao longo de sua graduação. Método: A revisão sistemática da literatura seguiu as diretrizes do Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) e abrangeu as bases de dados PubMed, LILACS/BVS, Scopus, Web of Science, Embase e ScienceDirect. A busca visou identificar artigos que abordassem metodologias e estratégias de disseminação do conhecimento em imagiologia médica. A avaliação do nível de evidência foi conduzida utilizando a abordagem GRADE. Resultado: Dos 152 artigos inicialmente identificados, 13 foram selecionados após a remoção de duplicatas e análise de títulos e resumos. Durante a leitura completa, excluíram-se oito artigos por causa de metodologias inadequadas ou desalinhamento com os objetivos da pesquisa. Os estudos elegíveis envolveram 461 alunos de Medicina em 49 instituições brasileiras, principalmente privadas, caracterizadas por um sistema híbrido de ensino e uma ênfase particular em módulos de radiologia médica. Salas de aula foram destacadas como os principais ambientes de ensino. No contexto do ensino de diagnóstico por imagens, muitos alunos ainda enfrentam dificuldades na avaliação de exames, independentemente do período letivo. O estudo aponta para a falta de uniformidade nos benefícios das políticas governamentais, resultando em segregação regional e financeira no acesso ao ensino. Conclusão: A despadronização do ensino contribui para uma formação deficiente em radiologia, deixando os profissionais despreparados para a interpretação adequada de exames. Diante da escassez de estudos e das disparidades identificadas, torna-se imperativo estabelecer parâmetros mínimos para os currículos em radiologia médica, proporcionando uma base estruturada para as instituições de ensino nacionais e visando à melhoria da qualidade do ensino e da formação profissional.


ABSTRACT Introduction: The competence of healthcare professionals in radiology not only plays a fundamental role in cost control but also in preventing medical errors resulting from inexperience during clinical practice. Objective: This study conducts a systematic review aiming to investigate the pedagogical strategies employed in teaching imaging diagnostics at Brazilian medical institutions, as well as to analyze the level of knowledge acquired by medical students throughout their education. Method: The systematic literature review followed the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) and covered the databases PubMed, LILACS/BVS, Scopus, Web of Science, Embase, and ScienceDirect. The search aimed to identify articles that addressed methodologies and strategies for disseminating knowledge in medical imaging. The evaluation of the level of evidence was conducted using the GRADE approach. Results: Of the 152 articles initially identified, 13 were selected after the removal of duplicates and analysis of titles and abstracts. During the full reading, eight articles were excluded due to inadequate methodologies or misalignment with the research objectives. The eligible studies involved 461 medical students across 49 Brazilian institutions, primarily private, characterized by a hybrid teaching system and a particular emphasis on medical radiology modules. Classrooms were highlighted as the main teaching environments. In the context of teaching imaging diagnostics, many students still face difficulties in evaluating exams, regardless of the academic term. The study points to the lack of uniformity in the benefits of government policies, resulting in regional and financial segregation in access to education. Conclusion: The lack of standardization in education contributes to inadequate training in radiology, leaving professionals unprepared for the proper interpretation of exams. Given the scarcity of studies and the identified disparities, it becomes imperative to establish minimum standards for medical radiology curricula, providing a structured foundation for national educational institutions and aiming to improve the quality of education and professional training.

Tuberculosis (TB) is a chronic infection caused by Mycobacterium tuberculosis (Mtb) with high incidence and mortality. Studies reported that host genetic variants might be associated with the risk of tuberculosis. The aim of this study was to perform an association study between 26 single nucleotide polymorphisms (SNPs) and tuberculosis and evaluate whether these SNPs may confer risk factors to tuberculosis in the Amazon population. There were 52 males and 126 females, with total of 178 healthy controls. Genotyping was performed using TaqMan Open Array Genotyping. Ancestry-informative markers were used to estimate the ancestral proportions of the individuals in the case and control groups. The results indicated that the SNPs rs10035440 (DROSHA), rs7372209 (miR26-a1), rs1834306 (miR100), rs4919510 (miR608), and rs10739971 (pri-let-7a-1) were significantly associated with high risk and rs3746444 (miR499) and rs6505162 (miR423), with low risk of developing tuberculosis in the Amazon population. Our study concluded that seven miRNA polymorphisms were associated with tuberculosis. Our study contributes to a better understanding of TB pathogenesis and may promote the development of new diagnostic tools against M. tuberculosis infection.

The role of regulatory elements such as small ncRNAs and their mechanisms are poorly understood in infectious diseases. Tuberculosis is one of the oldest infectious diseases of humans and it is still a challenge to prevent and treat. Control of the infection, as well as its diagnosis, are still complex and current treatments used are linked to several side effects. This study aimed to identify possible biomarkers for tuberculosis by applying NGS techniques to obtain global miRNA expression profiles from 22 blood samples of infected patients with tuberculosis (n = 9), their respective healthy physicians (n = 6) and external healthy individuals as controls (n = 7). Samples were run through a pipeline consisting of differential expression, target genes, gene set enrichment and miRNA-gene network analyses. We observed 153 altered miRNAs, among which only three DEmiRNAs (hsa-let-7g-5p, hsa-miR-486-3p and hsa-miR-4732-5p) were found between the investigated patients and their respective physicians. These DEmiRNAs are suggested to play an important role in granuloma regulation and their immune physiopathology. Our results indicate that miRNAs may be involved in immune modulation by regulating gene expression in cells of the immune system. Our findings encourage the application of miRNAs as potential biomarkers for tuberculosis.

INTRODUÇÃO: A tuberculose (TB) é uma doença de caráter infectocontagiosa, de evolução crônica, causada pela Mycobacterium tuberculosis, Uma vez considerada como doença crônica entre idosos por poder gerar sequelas e resultar em comprometimento da capacidade pulmonar, e desta forma contribuir para um quadro clínico de pior prognóstico é essencial estudar a relação da TB com a síndrome da fragilidade e a capacidade funcional em idosos. OBJETIVO: Analisar a capacidade funcional de idosos com seqüela de TB Pulmonar e investigar a ocorrência de fragilidade nesta população. MATERIAIS E MÉTODOS: Trata-se de um estudo transversal com método quantitativo-correlacional, realizado no período de agosto a novembro de 2017, no ambulatório de fisioterapia do Hospital Universitário João de Barros Barreto, Belém/PA. Avaliou-se a fragilidade pelo instrumento Tilburg Frailty Indicator (TFI) e a capacidade funcional pelo "Teste de AVD-Glittre" (TGlittre). Utilizou-se a "correlação de Spearman" entre TFI e TGlittre. Adotou-se p-valor≤0,05. RESULTADOS: Avaliaram-se 24 idosos com diagnóstico clínico de TB Pulmonar, apresentando alterações e/ou sequelas pulmonares. A média de idade foi de 68,37 ± 6,01anos, 66,7% era do sexo feminino. A presença de fragilidade foi de 70,5% (IC95% 48,9-87,4), com predomínio feminino (82,4%). A média do tempo no TGlittre foi de 5,77 ± 1,71 minutos. Houve correlação moderada positiva entre TFI e TGlittre (p=0.42, p= 0.03). CONCLUSÃO: Este estudo identificou a fragilidade e comprometimento funcional em idosos com sequela de TB pulmonar, com prevalência alta de fragilidade se comparada aos outros estudos brasileiros com a população idosa, além de apresenta correlação moderada positiva entre a capacidade funcional e fragilidade, mostrando que essa população merece atenção especial nos programas de reabilitação para a manutenção ou recuperação funcional.


INTRODUCTION: Tuberculosis (TB) is an infectious and contagious disease, with a chronic evolution that is caused by Mycobacterium tuberculosis, as it is considered a chronic disease among the aged because it can cause sequelae and result in impaired lung capacity and thus contribute to a clinical picture of worse prognosis it is interesting to study the relationship between TB and frailty syndrome and functional capacity in the old age. OBJECTIVE: To analyze the functional capacity of elderly people with pulmonary TB sequelae treated at a pulmonology service in Belém-PA, and to investigate the occurrence of frailty in this population. MATERIALS AND METHODS: This is a crosssectional study with the quantitative-correlational method, conducted from August to November 2017 at the physiotherapy outpatient clinic of the João de Barros Barreto University Hospital, Belém/PA. Frailty by the Tilburg Frailty Indicator (TFI) instrument and functional capacity by the "ADL-Glittre Test" (TGlittre) were evaluated. The "Spearman correlation" between TFI and TGlittre was used. P-value was adopted≤0.05. RESULTS: The average age was 68,37 ± 6.01years, 66.7% were female, all individ-uals were literate, the majority with incomplete primary education (45.8%). The presence of frailty was 70.5% (95% CI 48.9-87.4), with a female predominance (82.4%). The average time on TGlittre was 5,77 ± 1,71 minutes. There was a moderate positive correlation between TFI and TGlittre (p=0.42, p= 0.03). CONCLUSION: This study identified frailty and functional impairment in the elderly with pulmonary TB sequelae, with a high prevalence of frailty compared to other Brazilian studies with the elderly population, besides presenting a moderate positive correlation between functional capacity and frailty, suggesting that these aspects de-serve special attention in rehabilitation programs for functional maintenance or recovery in this profile of patients.

In recent years, the incidence of tuberculosis (TB) has declined worldwide, although this disease still occurs at relatively high rates in Amerindian populations. This suggests that the genetic ancestry of Amerindians may be an important factor in the development of infections, and may account for at least some of the variation in infection rates in the different populations. The present study investigated the potential influence of Amerindian genetic ancestry on susceptibility to tuberculosis in an Amazon population. The study included 280 patients diagnosed with tuberculosis and 138 asymptomatic hospital employees with no history of TB, but who were in contact with bacterially active TB patients. Ancestry analysis was run on a set of 61 Ancestry-Informative Markers to estimate European, African, and Amerindian genetic ancestry using STRUCTURE v2.2. The TB group had significantly higher Amerindian ancestry in comparison with the control group, and significantly lower European ancestry. Amerindian ancestry in the 20-60% range was found to be the principal risk factor for increased susceptibility to TB. The results of the study indicate that Amerindian ancestry is an important risk factor for susceptibility to TB in the admixed population of the Brazilian Amazon region.

OBJECTIVES: The treatment of tuberculosis (TB) remains a challenge owing to the high incidence of drug-induced hepatotoxicity. The aim of this study was to examine the effect of two gene polymorphisms, one in the CYP2B6 (rs3745274) gene and one in the CYP3A5 (rs776746) gene, on the development of hepatotoxicity in patients treated with anti-TB drugs in a Brazilian Amazon population. METHODS: TB patients who were treated with anti-TB drugs were examined for hepatotoxicity, an adverse effect that is characterized by liver damage. The genotype frequencies of the CYP2B6 and CYP3A5 genes examined in this study were assessed using RT-PCR. RESULTS: Thirty-one of the 220 subjects (14.1%) included in this study developed drug-induced hepatotoxicity. The result was significant when the TT homozygous mutant of the CYP2B6 gene was analyzed with additional key variables (p=0.046; odds ratio (OR) 0.063, 95% confidence interval (CI) 0.004-0.955), which may explain the hepatotoxicity results in this study. Using a univariate statistical model to associate the CYP3A5 gene A6986G polymorphism with the examined drugs, the results did not differ between samples from individuals with and without hepatotoxicity (p=0.176; OR 0.562, 95% CI 0.255-1.238). CONCLUSIONS: The G516T polymorphism in the CYP2B6 gene is a key predictor of the therapeutic response to treatment in TB patients.

RESUMEN

O refluxo gastroesofagiano (RGE) pode ser responsável pelo surgimento de manifestações cardiorrespiratórias tão comuns como a asma brônquica, infecções respiratórias de repetição ou dor retro esternal. A medida do pH esofagiano a nível ambulatorial, através de metodologia simples, permite determinar se o refluxo gastroesofagiano é responsável pelos sintomas apresentados pelo paciente. Esta técnica consiste na introdução de um cateter esofagiano conectado a um microprocessador que registra as variações de pH. Os dados são posteriormente analisados pro um microcomputador. A duração ideal do exame é de 24 horas, pois, permite a monitorização do pH esofagiano durante condições fisiológicas como o período pós-prandial e durante o decúbito, assim como durante as condiçãoes de trabalho ou esforço físico
