Achromobacter spp. are opportunistic pathogens of environmental origin increasingly isolated in patients with underlying conditions like cystic fibrosis (CF). Despite recent advances, their virulence factors remain incompletely studied, and siderophore production has not yet been investigated in this genus. The aim of this study was to evaluate the production of siderophores in a large collection of Achromobacter spp. and evaluate the variability according to the origin of the strain and species. A total of 163 strains were studied, including 128 clinical strains (CF and non-CF patients) and 35 strains of environmental origin. Siderophores were quantified by the liquid chrome azurol-sulphonate assay. Species were identified by nrdA gene-based phylogeny. Strains were assigned to 20 species, with Achromobacter xylosoxidans being the most represented (51.5% of strains). Siderophore production was observed in 72.4% of the strains, with amounts ranging from 10.1% to 90% siderophore units. A significantly higher prevalence of siderophore-producing strains and greater production of siderophores were observed for clinical strains compared with strains of environmental origin. Highly variable observations were made according to species: A. xylosoxidans presented unique characteristics (one of the highest prevalence of producing strains and highest amounts produced, particularly by CF strains). Siderophores are important factors for bacterial growth commonly produced by members of the Achromobacter genus. The significance of the observations made during this study must be further investigated. Indeed, the differences observed according to species and the origin of strains suggest that siderophores may represent important determinants of the pathophysiology of Achromobacter spp. infections and also contribute to the particular epidemiological success of A. xylosoxidans in human infections. IMPORTANCE: Achromobacter spp. are recognized as emerging opportunistic pathogens in humans with various underlying diseases, including cystic fibrosis (CF). Although their pathophysiological traits are increasingly studied, their virulence factors remain incompletely described. Particularly, siderophores that represent important factors of bacterial growth have not yet been studied in this genus. A population-based study was performed to explore the ability of members of the Achromobacter genus to produce siderophores, both overall and in relevant subgroups (Achromobacter species; strain origin, either clinical-from CF or non-CF patients-or environmental). This study provides original data showing that siderophore production is a common trait of Achromobacter strains, particularly observed among clinical strains. The major species, Achromobacter xylosoxidans, encompassed both one of the highest prevalence of siderophore-producing strains and strains producing the largest amounts of siderophores, particularly observed for CF strains. These observations may represent additional advantages accounting for the epidemiological success of this species.

The prevalence of various pathotypes of Escherichia coli was investigated during a case-control study conducted in children diarrhoea in Yaounde. Isolates obtained from the stools samples of children aged 6 months to 5 years were selected on phenotypic basis; and identified by virulence genes detection using polymerase chain reactions. The most prevalent pathotype was enteroaggregative Escherichia coli (25.8). Enteropathogenic Escherichia coli (3.6); enterotoxigenic Escherichia coli (1); and enteroinvasive Escherichia coli (0.2) followed. No shiga toxin-producing Escherichia coli were identified. Enteroaggregative Escherichia coli was not associated with diarrhoea (cases 26.1; controls 25.5; P=0;887); unlike enteropathogenic Escherichia coli (cases 6.7; controls 1; P=0.003). Investigations into documented potentials of enteroaggregative Escherichia coli in causing diarrhoea and other related pathologies indicated that it could be a major public health threat in Cameroon despite the fact that it was not found associated with clinical diarrhoeal cases in this study

All cephalo-spinal fluid (CSF) samples sent to the Yaoundé Pasteur Centre in Cameroon, between July 1999 and June 2000, were cultured and tested for soluble antigens. The percentage of positive samples was 10.4%. The main etiological agents detected were Streptococcus pneumoniae (56.2%), Haemophilus influenzae (18.5%) et Neisseria meningitidis (13.4%). Most of these cases of bacterial meningitis were children (86.7%). The susceptibility of the isolates to antibiotics was investigated. The streptococci and meningococci were mostly susceptible to beta-lactams, but the Gram-negative bacteria were not. Combinations of amino-penicillin and aminoside or chloramphenicol continue to be effective, as are third generation cephalosporins used alone. Meningococci appear to have emerged during the year 1999/2000, with 23 such isolates identified, of which 17 were serogroup A, subtype P1-9, clone III-1, which was responsible for the second pandemic. It should also be noted that 4 strains of N. meningitidis of serogroup W135 were isolated in Yaoundé.

A blood culture from a 65-year-old febrile man undergoing hemodialysis revealed, 5 days after inoculation, an unusual gram-negative fusiform rod with darting motility. During another episode of fever 21 days later, this Campylobacter-like organism was again recovered from three blood cultures and subcultured under an H2-enriched microaerobic atmosphere. The organism was catalase negative and oxidase positive and hydrolyzed urea rapidly. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis analysis of whole-cell proteins was indistinguishable from that of "Flexispira rappini" LMG 8738 described by Archer et al. in 1988 (J. R. Archer, S. Romero, A. E. Ritchier, M. E. Hamacher, B. M. Steiner, J. H. Bryner, and R. F. Schell, J. Clin. Microbiol. 26:101-105, 1988). The analysis of the 16S ribosomal DNA sequence revealed a similarity of 99.3% between the two strains. The patient recovered completely after a 4-week course of meropenem therapy. This is the first reported case of a recurrent "F. rappini" bacteremia in an adult patient, which confirms that this organism may be an invasive pathogen in immunocompromised patients, like other newly described Helicobacter species.
