RESUMEN

Las últimas evidencias científicas y la incorporación de nuevos fármacos al arsenal terapéutico de la rosácea hacen necesario revisar y actualizar los criterios y estrategias de tratamiento. Con este fin, un grupo de 15 dermatólogos expertos en esta enfermedad aportaron y discutieron acerca de las diferentes terapias y los criterios de respuesta y cambio de tratamiento. Partiendo de la revisión crítica de la bibliografía y de la exposición de los hábitos de los dermatólogos españoles en su práctica clínica, se formularon distintas propuestas que fueron debatidas teniendo en consideración tanto la experiencia profesional como las preferencias de los pacientes o los criterios de equidad. Una vez validadas las propuestas, se formularon las recomendaciones finales que, junto con la evidencia aportada por las principales guías y estudios internacionales, dieron lugar al presente documento. El objetivo de este consenso es ofrecer al dermatólogo un enfoque práctico para abordar la rosácea

RESUMEN

ANTECEDENTES: La Academia Española de Dermatología y Venereología (AEDV) ha planteado la necesidad de definir un plan estratégico para adaptarse y anticiparse al nuevo escenario. OBJETIVOS: 1) Posicionar a la AEDV como una sociedad con capacidad de influencia en todo lo relacionado con la dermatología; 2) contribuir al desarrollo de la especialidad reforzando el prestigio de la misma y de sus profesionales; y 3) introducir en el seno de la Academia un modelo de funcionamiento y de pensamiento estratégico que permita anticiparse a los retos del futuro y que se transmita a las sucesivas juntas directivas. MÉTODOS: Análisis de las tendencias del sistema sanitario, diagnóstico de la situación actual de la AEDV y de la dermatología mediante un análisis interno a través de encuestas y entrevistas a los académicos, análisis de los puntos fuertes y débiles, así como las amenazas y oportunidades, declaración de la misión, y finalmente, la identificación, el desarrollo y la realización de un mapa estratégico con la priorización de las líneas de acción estratégicas. RESULTADOS: Se definió un mapa estratégico con 16 objetivos generales agrupados en 4 ejes (alcanzar la visión, clientes internos y externos, procesos internos e innovación), encuadrados en un plan de acción con 19 iniciativas y acciones concretas para cada una de ellas. La monitorización de su desarrollo la realizará el comité de seguimiento del plan estratégico, formado por la junta permanente y los responsables de los 9 comités técnicos encargados de llevar a cabo cada una de las iniciativas. Comentario: El plan funcional debe guiar la gestión de la AEDV hasta 2017 y su implantación le permitirá posicionarse como referente en cuanto a su modelo funcional y contribuir al desarrollo y prestigio de la especialidad

RESULTS: The strategy map set out 16 general goals grouped into 4 main topics (achieving the vision, internal and external customers, internal processes, and innovation) and detailed in an action plan with 19 initiatives, each with specific actions. The plan will be monitored by the Strategic Plan Monitoring Committee, which is made up of the members of the Standing Committee and the chairs of the 9 Technical Committees responsible for implementing the initiatives. Comment: The Functional Plan should guide the management of AEDV until 2017, and its implementation will enable the association to contribute to the development and prestige of the specialty and position itself as a reference in terms of its functional model

RESUMEN

Syringomas localized in the genital areas are unusual. There are only three cases reported describing syringomas on the penis. Owing to this atypical localization it is necessary to consider a differential diagnosis involving other papular lesions more frequent found on the penile skin.

RESUMEN

Subacute cutaneous lupus erythematosus (SCLE) is a recently described distinct subset of lupus erythematosus (LE) having characteristic clinical, serological and genetic features. The clinicopathological and serological findings of 5 patients are reported. From a clinical point of view, we describe two types of cutaneous lesions: psoriasiform and annular pattern. ANA were present in 80% of the cases, anti-Ro was present in 60%, and anti-La in 20%; anti-RNP was positive in 1 patient. Circulating immune complexes were detected in 2 patients, and low levels of complement factors (C3, C4) in another 2 patients. The B- cell alloantigens HLA-A2, HLA-Bw4 or HLA-DR3 were present in 60% of the cases. The cutaneous histopathology revealed important changes in the epidermis. Our findings are similar to those described by most other authors. Also, we would like to emphasize the low disease activity and benign course of our patients.

RESUMEN

We present the case of a 64 year-old-male fur trader from Lerida (Spain), who consulted us because of a 2-week history of night fever and the presence of non-pruritic purpuric lesions on the distal third of the lower limbs. Laboratory findings demonstrated positive serology for brucellosis and cutaneous biopsy showed leukocytoclastic vasculitis. The pathogenesis of the cutaneous process is discussed. We also suggest that brucellosis should be included among the infections taken into account in patients suffering from leukocytoclastic vasculitis coming from brucella-endemic areas.

RESUMEN

We report a 69-year-old man with disseminated superficial actinic porokeratosis (DSAP) who developed a hyperkeratotic lesion on the distal third of the right forearm with the characteristic histopathological findings of squamous cell carcinoma and DSAP. A review of the literature revealed only four similar cases.

RESUMEN

The subepidermal calcified nodule (SNC), included under the idiopathic cutaneous calcifications, is an infrequent cutaneous process. We report a new case localized on the chin of an 18 month old child. We review the concept gathered from previous literature and we post the possible role which the mast cell may play in the genesis of the said deposits.

RESUMEN

The clinical and pathological findings in five patients with dermatomyositis are described. The diagnostic value of the cutaneous lesions in the disease, indicating the histological patterns of each cutaneous sign is stressed. Based in our experience and in the reviewed literature, we consider that Gottron's papules and poikilodermatomyositis present histological aspects that allow us to confirm the diagnosis suspected by clinical findings. Likewise we review some special difficult aspects of the disease such as infantile forms, association with neoplasias and overlap syndromes.

RESUMEN

We present a case of keratoacanthoma type II in a 38-year-old female patient, which developed in the thigh over the donor site of a laminar skin graft, six weeks after the procedure. The graft was utilized to cover the defect secondary to the resection of a melanoma. The possible role played by mechanical trauma in the development of this tumor, rare under such circumstances, is discussed. Similar previously published cases are reviewed.

RESUMEN

A description is given of an epithelioid sarcoma observed in a male patient of 71 years of age. This two-year growth was located in the right leg and presented a clinical picture of a hard walled ulcer. After having been removed surgically the tumour reappeared after seven months at which point amputation was practised at the middle thigh. The patient died twelve months after amputation suffering from pleuropulmonary metastasis. An necropsy was not carried out. The finding of massive arterial neoplastic thrombosis in the histological study confirms the fatal outcome of the patient. The clinical, histological, evolutive and therapeutic characteristics of the epithelioid sarcoma are reviewed.
