Objective: Non-suicidal self-injury (NSSI), defined as a deliberate destruction of one's own body without a suicidal intent, is a global public health issue. Adverse childhood events (ACEs) have been shown to be associated with various mental illnesses; however, to date the impact of such events on NSSI in youth has not been reviewed. Methods: We conducted a systematic review, searched 5 databases for published articles evaluating ACE and NSSI in youth less than or equal to 21 years of age. After screening 247 articles, we included 21 unique articles in this systematic review. Results: Increasing ACE score, physical, sexual or emotional abuse, parental neglect and substance use, parental separation or dysfunctional family, and death of a close family member had statistically significant correlation with NSSI. Conclusion: Non-suicidal self-injury is an impairing diagnosis with far reaching psychiatric manifestations and repercussions. Practitioners having high clinical suspicion for ACEs in youth with NSSI must intervene early by administering the ACEs questionnaire. Effective treatment of NSSI in those with ACEs with psychotherapy significantly improves outcomes and prevents suicide in youth.

The exponential increase in diabetes mellitus (DM) poses serious public health concerns. In this review, we focus on the role of leptin in type 2 DM. The peripheral actions of leptin consist of upregulating proinflammatory cytokines which play an important role in the pathogenesis of type 2 DM and insulin resistance. Moreover, leptin is known to inhibit insulin secretion and plays a significant role in insulin resistance in obesity and type 2 DM. A literature search was conducted on Medline, Cochrane, Embase, and Google Scholar for relevant articles published until December 2023. The following search strings and Medical Subject Headings (MeSH terms) were used: "Diabetes Mellitus," "Leptin," "NPY," and "Biomarker." This article aims to discuss the physiology of leptin in type 2 DM, its glucoregulatory actions, its relationship with appetite, the impact that various lifestyle modifications can have on leptin levels, and, finally, explore leptin as a potential target for various treatment strategies.

Background Hereditary cardiomyopathies are commonly occurring myocardial conditions affecting heart structure and function with a genetic or familial association, but the etiology is often unknown. Cardiomyopathies are linked to significant mortality, requiring robust risk stratification with genetic testing and early diagnosis. Hypothesis We hypothesized that health care disparities exist in genetic testing for hereditary cardiomyopathies within clinical practice and research studies. Methods In a narrative fashion, we conducted a literature search with online databases such as PubMed/MEDLINE, Google Scholar, EMBASE, and Science Direct on papers related to hereditary cardiomyopathies. A comprehensive analysis of findings from articles in English on disparities in diagnostics and treatment was grouped into four categories. Results Racial and ethnic disparities in research study enrollment and health care delivery favor White populations and higher socioeconomic status, resulting in differences in the development and implementation of effective genetic screening. Such disparities have shown to be detrimental, as minorities often suffer from disease progression to heart failure and sudden cardiac death. Barriers related to clinical genetic testing included insurance-related issues and health illiteracy. The underrepresentation of minority populations extends to research methodologies, as testing in ethnic minorities resulted in a significantly lower detection rate and diagnostic yield, as well as a higher likelihood of misclassification of variants. Conclusions Prioritizing minority-based participatory research programs and screening protocols can address systemic disparities. Diversifying research studies can improve risk stratification strategies and impact clinical practice.

PURPOSE OF REVIEW: This narrative review discusses the significance of probiotic therapy in the postoperative care of patients with esophageal cancer and its role as an adjunct therapy to other treatment modalities for esophageal cancer. RECENT FINDINGS: As such, there is an emerging need to address any malnutrition and gastrointestinal problems occurring in these patients which tend to have a strong negative impact on their prognosis. Probiotic effects on esophageal cancer biomarkers suggest that there is a positive correlation between these two factors. However, the beneficial effects remain controversial and warrant further investigation. Probiotics, now being widely utilized as postoperative therapy in some carcinomas of the gastrointestinal tract such as gastric cancer and colorectal cancer, have been shown in some clinical studies to positively impact the nutritional status of patients with esophageal cancer. Postoperative care among patients suffering from esophageal cancer is a very crucial aspect in the survival of these patients.

Terlipressin is an analogue of vasopressin and is often used in the treatment of bleeding esophageal varices and also in the treatment of hepatorenal syndrome associated with liver cirrhosis. Although terlipressin is a safe drug, but it has been rarely associated with potentially serious adverse effects like ischemic necrosis of skin involving the abdominal skin, extremities, and scrotal skin. We present one such rare case where terlipressin-induced skin necrosis in bilateral lower extremities in a 48-year-old male while we were managing hepatorenal syndrome in the same.

Posterior reversible encephalopathy syndrome (PRES) is a clinical condition that is characterized by intense headache and neurological deficits such as vision loss which are attributed to the vasogenic edema that occurs in the posterior cerebral cortex involving the occipital and parietal lobes. Although the classical demographies that are affected by the PRES are middle-aged postpartum females and those with renal dysfunction, rarely it is also seen in patients with collagen vascular disorders such as systemic lupus erythematosus (SLE). We report a case of PRES in a 32-year-old SLE patient.

Fahr's disease is a rare neurodegenerative disorder first described by Karl Theodor in 1930, defined by abnormal calcified deposits in the basal ganglia and cerebral cortex. Fahr's disease commonly affects young to middle-aged adults with various clinical presentations, including endocrinologic, dermatologic, and neurologic problems, with extrapyramidal symptoms being the most common manifestation. In this case report, we present a case of an epileptic seizure as the first manifestation of Fahr's disease in a 45-year-old male.

Infective endocarditis is a life-threatening disease of the endocardium of the heart and cardiac valves, caused by several infectious agents, ranging from Streptococci to rickettsia. It has continued to be a serious cause of cardiac infection, with a rise in the incidence globally over the last 5 years. Despite diagnostic and therapeutic advances, its prognosis remains poor, and a surge in morbidity and hospital mortality highlights the significance of the swift diagnosis of this disease. Given its complexity, patients with infective endocarditis need rapid response and prompt diagnosis from a multidisciplinary team with a timely diagnostic protocol and treatment in the critical phase. Still, there is a lack of evidence regarding the timing of surgery or the role of antibiotic prophylaxis in the management. In this review, we detail the epidemiological transition, challenges in the diagnosis, and the newer avenues in the medical and surgical management of infective endocarditis.

Agricultural product (insecticides and pesticides) poisoning is common in the rural Indian setup, and in most cases, it occurs due to suicidal attempts rather than accidental intake. Furthermore, most cases of agrochemical poisoning are organophosphate poisoning rather than other less commonly used pesticides. Lambda-cyhalothrin is a less commonly used insecticide in India, and there have been a few instances in the literature where lambda-cyhalothrin poisoning and its subsequent management have been described. In this case report, we describe accidental lambda-cyhalothrin poisoning in a 26-year-old female and its management at our center.
