RESUMEN

OBJECTIVES: The goal of this study is to investigate the relation between indicators of osteoporosis (i.e., bone mineral density (BMD), and Cortical Index (CI)) and the complexity of a fracture of the proximal humerus as a result of a low-energy trauma. METHODS: A retrospective chart review of 168 patients (mean age 67.2 years, range 51 to 88.7) with a fracture of the proximal humerus between 2007 and 2011, whose BMD was assessed at the Fracture Liaison Service with Dual Energy X-ray Absorptiometry (DXA) measurements of the hip, femoral neck (FN) and/or lumbar spine (LS), and whose CI and complexity of fracture were assessed on plain anteroposterior radiographs of the proximal humerus. RESULTS: No significant differences were found between simple and complex fractures of the proximal humerus in the BMD of the hip, FN or LS (all p > 0.3) or in the CI (p = 0.14). Only the body mass index was significantly higher in patients with a complex fracture compared with those with a simple fracture (26.9 vs 25.2; p = 0.05). CONCLUSION: There was no difference in BMD of the hip, FN, LS or CI of the proximal humerus in simple compared with complex fractures of the proximal humerus after a low-energy trauma. Factors other than the BMD and CI, for example body mass index, may play a more important role in the complexity of this fracture.Cite this article: J.W.A.M. den Teuling, B.S. Pauwels, L. RESUMEN

Preoperative planning in total knee arthroplasty with intramedullary guiding systems requires the measurement of the femoral mechanical-anatomical angle (FMAA) for optimal alignment correction. The main goal of this study was to assess the agreement between two digital FMAA measurements and the analog FMAA measurement. Overall 41 anteroposterior weight-bearing hip-to-ankle radiographs of patients undergoing total knee arthroplasty were used for the measurements of the FMAA. The analog method (gold standard, GS) was compared with two new digital methods (DIG1 and DIG2) using intraclass correlation (ICC) and Bland-Altman plots, measured by three blinded raters. The ICC for measurements of the FMAA comparing the GS and DIG1 was 0.48 (95% confidence interval [CI] 0.20-0.68), and 0.53 (95% CI 0.26-0.73) for comparing GS and DIG2. The ICC between raters for DIG1 was 0.79 (95% CI 0.68-0.88) and 0.88 (95% CI 0.80-0.93) for DIG2. Bland-Altman plots showed a mean difference between the GS and DIG1 of -0.44 degrees, with 95% limits of agreement from 1.21 to -2.09 degrees. The mean difference between the GS and DIG2 was -0.68 degrees with 95% limits of agreement from 0.99 to -2.35 degrees. It was concluded that the digital FMAA measurement is less reliable than analog measurement in total knee arthroplasty.

RESUMEN

A previously unknown side effect of biphosphonate use is emerging. In a specific patient group on long term biphosphonate therapy stress femur fractures seem to occur. The typical presentation consists of prodromal pain in the affected leg and/or a discrete cortical thickening on the lateral side of the femur in conventional radiological examination or the presentation with a spontaneous transverse subtrochanteric femur with typical features. We present three cases of this stress fracture in patients on bisphosphonate therapy. One of these patients suffered a bilateral femur fracture of the same type. In our opinion, in patients on bisphosphonate therapy who present with a spontaneous femur fracture, seizing therapy is advisable. In bilateral cases preventive nailing should be considered.
