RESUMEN

ABSTRACT: Dysplastic nevi are an important subset of melanocytic nevi with atypical clinical, histopathologic, as well as genomic features compared with common acquired nevi. Dysplastic nevi are characterized histologically by both cytologic atypia and architectural disorder. The established criteria for cytologic atypia used to distinguish between low-grade and high-grade dysplastic nevi are often subjective, although there is a dearth of more objective, reproducible features of architectural disorder (eg, pagetoid scatter) that have been validated to differentiate between low-grade and high-grade dysplastic nevi. In this study, we sought to determine whether the presence and degree of follicular extension differ between low-grade and high-grade dysplastic nevi. We retrospectively examined the histopathologic features of 90 dysplastic nevi: 60 cases of low-grade dysplastic nevi (average age of 47.2 ± 18.1 years; 62.7% female) and 30 cases of high-grade dysplastic nevi (average age of 47.4 ± 19.8 years; 60.0% female). After examination, 50% of the cases of dysplastic nevi (n = 45) had hair follicles within the lesion, for which the presence and degree of follicular extension was then determined. Low-grade and high-grade dysplastic nevi do not differ significantly regarding the presence of follicular extension, average depth of follicular extension, and confluence of nevus cells along the follicular epithelium. Both low-grade and high-grade dysplastic nevi in our study demonstrated follicular extension that was superficial, that is, above the level of isthmus of hair follicles (insertion of sebaceous gland into hair follicle). Future studies are warranted to confirm these preliminary findings.

BACKGROUND: Dysplastic nevi are among the most important cutaneous melanoma simulators. They are important risk markers for this neoplasia and can be its potential precursors. Some authors found a statistically significant relationship between the degree of dysplasia and the risk for developing melanoma. However, reproducibility of grading criteria ranged from poor to fair in the researched articles. OBJECTIVE: To test the reproducibility of the grading criteria proposed by Sagebiel et al. regarding dysplastic nevi. METHODS: Histological specimens of 75 dysplastic nevi were graded, independently and in a blinded fashion, according to preestablished criteria, by a panel of 10 pathologists with different levels of experience. Diagnostic agreement was calculated using weighted kappa and intraclass correlation coefficients. RESULTS: The average of weighted kappa values was 0.13 for all observers, 0.12 for dermatopathologists, 0.18 for general pathologists and 0.05 for residents. Intraclass correlation coefficient values were 0.2 for all observers, 0.18 for dermatopathologists, 0.33 for general pathologists and 0.15 for residents. CONCLUSIONS: Histopathological grading for dysplastic nevi was not reproducible in this Brazilian series, so the criteria used are not a helpful histopathological parameter for clinicopathological correlation.

FUNDAMENTOS: Nevos displásicos estão entre os mais importantes simuladores de melanoma. São marcadores de risco para o desenvolvimento dessa neoplasia e podem ser seus precursores. Alguns autores observaram uma relação estatisticamente significativa entre o grau de displasia e o risco de desenvolvimento de melanoma. No entanto, a reprodutibilidade dos critérios para graduação variou de ruim a razoável nos artigos consultados. OBJETIVO: Testar a reprodutibilidade da graduação proposta por Sagebiel et al. para os nevos displásicos. MÉTODOS: Seções histológicas de setenta e cinco nevos displásicos foram graduadas, de forma independente e anônima, segundo critérios pré-estabelecidos, por um painel de 10 patologistas com diferentes níveis de experiência. A concordância diagnóstica foi calculada usando os coeficientes de kappa ponderado e de correlação intraclasse. RESULTADOS: A média dos valores de kappa ponderado foi de 0,13 para todos os observadores, de 0,12 para os dermatopatologistas, de 0,18 para os patologistas gerais e de 0,05 para os residentes. Os valores dos coeficientes de correlação intraclasse foram 0,2 para todos os observadores, 0,18 para os dermatopatologistas, 0,33 para os patologistas gerais e 0,15 para os residentes. Atypical Mole Syndrome is the most important phenotypic risk factor for developing cutaneous melanoma, a malignancy that accounts for about 80% of deaths from skin cancer. Because the diagnosis of melanoma at an early stage is of great prognostic relevance, the identification of Atypical Mole Syndrome carriers is essential, as well as the creation of recommended preventative measures that must be taken by these patients.

RESUMEN

O nevo atípico (displásico) é considerado um fator importante associado com o risco aumentado de desenvolvimento do melanoma cutâneo. Acredita-se que nevos atípicos sejam lesões precursoras do melanoma cutâneo. Podem estar presentes em pacientes com múltiplos nevos melanocíticos (síndrome do nevo atípico) ou isolados e em poucas quantidades em um contexto não familial. Aparecem, geralmente, na puberdade e prevalecem em indivíduos jovens. Têm predileção por áreas expostas ao sol, especialmente, o tronco. O grande desafio em relação ao nevo atípico reside na controvérsia em se definir sua nomenclatura, diagnóstico clínico, critérios dermatoscópicos, diagnóstico histopatológico e aspectos moleculares. Esta revisão tem por objetivo trazer o conhecimento, facilitar o entendimento e responder às questões duvidosas concernentes ao nevo atípico.

Reed nevus or pigmented spindle-cell nevus may mimic cutaneous melanoma; however, its dermoscopic and histopathological characteristics are different. This case report describes three patients with distinct clinical, dermoscopic and histopathological presentations, which were correlated to enable a differential diagnosis to be made between melanoma and Spitz nevus.

O nevo de Reed ou nevo de células fusiformes pigmentado pode ser um simulador do melanoma cutâneo. Apresenta, entretanto, diferentes características dermatoscópicas e histopatológicas. Trata-se de relato de três pacientes com apresentações clínicas, dermatoscópicas e histopatológicas distintas, correlacionando-as no auxílio diagnóstico deste com o melanoma e nevo de Sptiz.

RESUMEN

Dermoscopy is a very useful technique for the analysis of pigmented skin lesions. It represents a link between clinical and histological views, permitting an earlier diagnosis of skin melanoma. It also helps in the diagnosis of many other pigmented skin lesions, such as seborrheic keratosis, pigmented basal cell carcinoma, hemangioma, blue nevus, atypical nevus, and mole, which can often clinically simulate melanoma. In this article, dermoscopy is reviewed from its history to the basic concepts of the interpretation of dermoscopic images. The goal is to introduce this subject to those not yet familiar with it, in order to instigate and encourage the training and practice of this technique of growing importance for everyday usage.

São bastante conhecidos pelos patologistas o desafio e a dificuldade que algumas lesões melanocíticas apresentam no momento de seu diagnóstico. A falta de uniformidade na maneira de aplicar os critérios diagnósticos é, sem dúvida, uma das causas principais dos altos índices de discordância descritos na literatura. O objetivo deste trabalho é tentar diminuir esta discordância e, para isso, foram selecionados os 12 critérios considerados mais importantes para o diagnóstico de melanoma: 1) tamanho; 2) simetria; 3) delimitação lateral; 4) maturação; 5) disseminação pagetóide; 6) necrose/ulceração; 7) infiltrado inflamatório; 8) regressão; 9) atipias celulares; 10) mitoses; 11) melanização; 12) proliferação de células isoladas. Foram expostas as características de cada um deles e sua aplicação no diagnóstico dos melanomas, lembrando sempre as possíveis exceções, onde estes critérios podem estar presentes em lesões melanocíticas benignas. Posteriormente, em três tabelas, estabeleceu-se o diagnóstico diferencial entre melanoma e três condições melanocíticas benignas que costumam apresentar maiores dificuldades no diagnóstico diferencial. Como nenhum critério deve ser considerado isoladamente, a aplicação rigorosa do conjunto de dados aqui fornecido pode ajudar substancialmente o patologista cirúrgico generalista (não-especialista em lesões melanocíticas) a resolver alguns problemas na sua rotina diária.

Pretende-se neste artigo, rever os aspectos históricos da definição do nevo melanocítico atípico (displásico), abordando a sua evolução conceitual com especial ênfase nos aspectos anatomopatológicos. Propõe-se critérios diagnósticos estritos, com base nas resoluções de consensos da Organização Mundial da Saúde e do National Institute of Health (NIH). Estes incluem, 1) critérios maiores (obrigatórios): hiperplasia melanocítica contígua (ou lentiginosa) e atipia melanocítica focal (nunca difusa, pois caracterizaria melanoma in situ) e 2) critérios menores (pelo menos dois): extensão do componente névico juncional lateralmente ao componente dérmico (ombro) quando se tratar de nevo melanocítico composto; fusão de cones epiteliais pelos ninhos da células névicas; fibrose lamelar e concêntrica ao redor dos cones epiteliais; discreto infiltrado inflamatório linfocitário perivascular superficial.

El nevo de Clark, antes llamado nevo ®displásico¼, es una de las lesiones melanocíticas más controvertidas y difíciles de diagnosticar. Representa también un precursor del melanoma. Con el fin de mejorar nuestro conocimiento acerca de sus características clínicas y de contar con mejores recursos para su diagnóstico, se llevó a cabo un estudio prospectivo en el que se pusieron en juego los criterios clínicos, los criterios al alcance del microscopio de superficie y los criterios histológicos. Se colectaron 70 lesiones pertenecientes a 48 pacientes. Los criterios clínicos de asimetría y pigmento bajo el microscopio de superficie, que en la histología se corresponden con clavas interpapilares más o menos elongadas en las que existen grupos de melanocitos. Es factible diagnosticar melanomas incipientes dentro del nevo de Clark. Se logró detectar a seis portadores del síndrome de los nevos de Clark, tres con melanoma y tres sin esta lesión. Puede concluirse que la identificación y el diagnóstico del nevo de Clark, con o sin melanoma asociado, es susceptible de mejorarse considerablemente con el uso del microcopio de superficie

RESUMEN

We present the analysis of a series of 100 Clark's nevus collected during 37 months in three different laboratories of pathology in Mexico City. The purpose has been to know how common is it and what are the clinical features in a sample of the mexican population. We found that Clark's nevus is not so common but it is seen mostly in fair skin people and can be recognized by the trained clinician, we observed four cases associated with melanoma but no patient had the criteria to be considered as having the so called dysplastic nevus syndrome, which may be rare in the mixed (mestizo) population of Mexico. We also reviewed the current concepts about this peculiar melanocytic nevus.
