MANDAT: À la demande du fabricant Takeda Canada Inc., l'Institut national d'excellence en santé et en services sociaux (INESSS) a procédé à l'évaluation du produit du système du sang GlassiaMC, un inhibiteur de l'alpha1-protéinase humain. Au Canada, GlassiaMC est indiqué pour le traitement d'augmentation et d'entretien de longue durée chez les adultes présentant un emphysème cliniquement manifeste attribuable à un déficit héréditaire sévère en inhibiteur de l'alpha1-protéinase (AAT), aussi appelé déficit en alpha1- antitrypsine (AAT). L'indication demandée à l'INESSS est la même. L'INESSS a réalisé les évaluations des produits ProlastinMC-C Liquid, ZemairaMC et GlassiaMC, tous des alpha1-antitrypsine plasmatiques humains, en simultané. Les avis pour ces 3 produits sont publiés au même moment. DÉMARCHE: d'évaluation Une revue des données issues de la littérature et de celles fournies par le fabricant a été réalisée afin de documenter l'efficacité, l'innocuité et l'efficience de GlassiaMC. Des données contextuelles et expérientielles issues de la consultation sont également présentées. Des analyses d'efficience et d'impact budgétaire ont été élaborées par l'INESSS. DIMENSION: populationnelle Le déficit en inhibiteur de l'alpha1-protéinase, ou déficit en alpha1-antitrypsine (DAAT), est une condition génétique rare à présentation variable qui peut entraîner des symptômes pulmonaires (emphysème, bronchite chronique et bronchectasie) et hépatiques sévères et dont la progression est souvent lente. En raison des manifestations cliniques hétérogènes et souvent tardives et de la découverte de nouveaux variants pathogéniques associés à la maladie, le DAAT est une condition sous-diagnostiquée. Les traitements usuels visent l'atténuation des symptômes respiratoires et incluent les médicaments inhalés, la réhabilitation pulmonaire et, pour certains patients, la thérapie d'augmentation qui consiste en l'administration intraveineuse hebdomadaire d'alpha1-antitrypsine (AAT) dérivé du plasma. La thérapie d'augmentation a comme objectif de ralentir la progression de l'emphysème chez les individus atteints d'un DAAT. Présentement, seul le produit ProlastinMC-C est disponible au Québec et son remboursement public n'est possible que par la mesure du patient d'exception. Des traitements qui interrompent ou ralentissent la progression de l'emphysème et la détérioration des fonctions pulmonaires et hépatiques répondraient aux besoins de santé actuels, surtout s'ils permettaient d'améliorer la qualité de vie des individus atteints et de leurs proches. Une facilitation du processus d'accès à la thérapie d'augmentation est également souhaitable. EFFICACITÉ: Chez les individus atteints de DAAT, le produit d'AAT plasmatique humain GlassiaMC est considéré comme bioéquivalent au ProlastinMC puisqu'il affiche un profil pharmacocinétique comparable à ce dernier. Aucune donnée sur la capacité de GlassiaMC à ralentir la progression de l'emphysème chez les individus atteints d'un DAAT n'a été soumise par le fabricant ou répertoriée dans la littérature. Innocuité. Le profil d'innocuité de GlassiaMC est jugé acceptable et comparable à celui du ProlastinMC. Dimension organisationnelle: La couverture des AAT plasmatiques humains est présentement réalisée par la RAMQ via la mesure du patient d'exception et les régimes d'assurance privés. Dorénavant, les AAT plasmatiques devront être inscrits à la Liste des produits du système du sang du Québec et remporter un appel d'offres d'Héma-Québec pour pouvoir être distribués. Lors de ce changement de gestion, il serait prudent d'éviter les interruptions de traitement et minimiser les conséquences qui pourraient s'y lier. DIMENSION ÉCONOMIQUE: Analyse d'efficience: Au prix soumis, GlassiaMC permettrait des économies de XX $ par semaine comparativement à Prolastin-CMC, dont l'efficience n'a pas été évaluée avant cette présente évaluation. Analyse d'impact budgétaire: Advenant l'ajout de GlassiaMC à la Liste des produits du système du sang du Québec, une augmentation du nombre de patients peut être attendue en raison des patients utilisant actuellement ProlastinMC-C à travers le régime privé d'assurance médicaments qui poursuivraient leur traitement par inhibiteur d'AAT à travers le régime public. Cette hausse de personnes couvertes par le système public (XX %) se traduirait par des coûts supplémentaires estimés à plus de 8 M$ sur 3 ans. Dimension socioculturelle Le Québec s'est doté en 2022 d'une politique visant à optimiser l'accès à des soins et à des services de santé de qualité qui sont adaptés aux besoins particuliers des patients atteints de maladies rares, et culturellement sensibles. Certains experts apprécient que le Québec soit à l'avant-garde pour la prise en charge de plusieurs maladies rares, dont le DAAT, par rapport à d'autres provinces canadiennes.

BACKGROUND: Eucalyptol is a monoterpenoid oil present in many plants, principally the Eucalyptus species, and has been reported to have anti-inflammatory and antioxidative effects. HYPOTHESIS/PURPOSE: Since the potential effect of eucalyptol on mouse lung repair has not yet been studied, and considering that chronic obstructive pulmonary disease (COPD) is the fourth leading cause of death worldwide, the aim of this study was to investigate eucalyptol treatment in emphysematous mice. STUDY DESIGN: Male mice (C57BL/6) were divided into the following groups: control (sham-exposed), cigarette smoke (CS) (mice exposed to 12 cigarettes a day for 60 days), CS + 1 mg/ml (CS mice treated with 1 mg/ml eucalyptol for 60 days), and CS + 10 mg/ml (CS mice treated with 10 mg/ml eucalyptol for 60 days). Mice in the CS and control groups received vehicle for 60 days. Eucalyptol (or the vehicle) was administered via inhalation (15 min/daily). Mice were sacrificed 24 h after the completion of the 120-day experimental procedure. METHODS: Histology and additional lung morphometric analyses, including analysis of mean linear intercept (Lm) and volume density of alveolar septa (Vv[alveolar septa]) were performed. Biochemical analyses were also performed using colorimetric assays for myeloperoxidase (MPO), malondialdehyde (MDA), and superoxide dismutase (SOD) activity, in addition to using ELISA kits for the determination of inflammatory marker levels (tumor necrosis factor alpha [TNF-α], interleukin-1 beta [IL-1ß], interleukin 6 [IL-6], keratinocyte chemoattractant [KC], and tumor growth factor beta 1 [TGF-ß1]). Finally, we investigated protein levels by western blotting (nuclear factor (erythroid-derived 2)-like 2 [Nrf2], nuclear factor kappa B [NF-κB], matrix metalloproteinase 12 [MMP-12], tissue inhibitor of matrix metalloproteinase 1 [TIMP-1], neutrophil elastase [NE], and elastin). RESULTS: Eucalyptol promoted lung repair at the higher dose (10 mg/ml), with de novo formation of alveoli, when compared to the CS group. This result was confirmed with Lm and Vv[alveolar septa] morphometric analyses. Moreover, collagen deposit around the peribronchiolar area was reduced with eucalyptol treatment when compared to the CS group. Eucalyptol also reduced all inflammatory (MPO, TNF-α, IL-1ß, IL-6, KC, and TGF-ß1) and redox marker levels (MDA) when compared to the CS group (at least p < 0.05). In general, 10 mg/ml eucalyptol was more effective than 1 mg/ml and, at both doses, we observed an upregulation of SOD activity when compared to the CS group (p < 0.001). Eucalyptol upregulated elastin and TIMP-1 levels, and reduced neutrophil elastase (NE) levels, when compared to the CS group. CONCLUSION: In summary, eucalyptol promoted lung repair in emphysematous mice and represents a potential therapeutic phytomedicine in the treatment of COPD.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is characterized by irreversible airflow obstruction and inflammation. Natural products, such as monoterpenes, displayed anti-inflammatory and anti-oxidant activities and can be used as a source of new compounds to COPD treatment. Our aim was to evaluate, in an elastase-induced pulmonary emphysema in mice, the effects of and underlying mechanisms of three related natural monoterpenes (p-cymene, carvacrol and thymol) isolated from essential oil from leaves Lippia sidoides Cham. (Verbenaceae). METHODS: Mices received porcine pancreatic elastase (PPE) and were treated with p-cymene, carvacrol, thymol or vehicle 30 min later and again on 7th, 14th and 28th days. Lung inflammatory profile and histological sections were evaluated. RESULTS: In the elastase-instilled animals, the tested monoterpenes reduced alveolar enlargement, macrophages and the levels of IL-1ß, IL-6, IL-8 and IL-17 in bronchoalveolar lavage fluid (BALF), and collagen fibers, MMP-9 and p-65-NF-κB-positive cells in lung parenchyma (p < 0.05). All treatments attenuated levels of 8-iso-PGF2α but only thymol was able to reduced exhaled nitric oxide (p < 0.05). CONCLUSION: Monoterpenes p-cymene, carvacrol and thymol reduced lung emphysema and inflammation in mice. No significant differences among the three monoterpenes treatments were found, suggesting that the presence of hydroxyl group in the molecular structure of thymol and carvacrol do not play a central role in the anti-inflammatory effects.

Background. Elastase mediates important oxidative actions during the development of chronic obstructive pulmonary disease (COPD). However, few resources for the inhibition of elastase have been investigated. Our study evaluated the ability of the recombinant plant derived Bauhinia bauhinioides Kallikrein proteinase Inhibitor (rBbKI) to modulate elastase-induced pulmonary inflammation. Methods. C57Bl/6 mice were given intratracheal elastase (ELA group) or saline (SAL group) and were treated intraperitoneally with rBbKI (ELA-rBbKI and SAL-rBbKI groups). At day 28, the following analyses were performed: (I) lung mechanics, (II) exhaled nitric oxide (ENO), (III) bronchoalveolar lavage fluid (BALF), and (IV) lung immunohistochemical staining. Results. In addition to decreasing mechanical alterations and alveolar septum disruption, rBbKI reduced the number of cells in the BALF and decreased the cellular expression of TNF-α, MMP-9, MMP-12, TIMP-1, eNOS, and iNOS in airways and alveolar walls compared with the ELA group. rBbKI decreased the volume proportion of 8-iso-PGF2α, collagen, and elastic fibers in the airways and alveolar walls compared with the ELA group. A reduction in the number of MUC-5-positive cells in the airway walls was also observed. Conclusion. rBbKI reduced elastase-induced pulmonary inflammation and extracellular matrix remodeling. rBbKI may be a potential pharmacological tool for COPD treatment.

Bronchial responsiveness during methacholine (MCh) challenge was analysed in an elastase-induced mouse model of emphysema to explore the magnitude of the response in this model. Swiss mice were intratracheally instilled with saline or elastase (0.3 or 0.6 U). Twenty days afterward, mechanical ventilation data were collected from the closed and opened thorax of baseline and MCh (vehicle, 50 and 100 mg/mL) challenged mice. The lungs were prepared for morphometric analysis. In the 0.6 U group, airway resistance (Raw) and tissue elastance (H) were decreased, and hysteresivity (η) was increased (closed thorax). MCh increased Raw, G and H in all groups, but this increase was attenuated in the elastase-induced emphysema groups, the largest attenuation was observed in the 0.6 U (closed thorax condition). Elastase increased hyperinflation of the alveoli, alveolar collapse and the Lm and reduced the normal area. MCh reduced respiratory mechanics in elastase-induced emphysema, and this reduction was modulated by the collapsed and/or hyperinflated areas, which increased the heterogeneity of the lungs.

METHODS: A population-based study conducted in the city of Pelotas, Brazil, involving 3,670 subjects ≥ 10 years of age, evaluated with a questionnaire. RESULTS: Approximately 10% of the sample reported at least one of the respiratory diseases studied. Among those individuals, 59% reported respiratory symptoms in the last year, and, of those, only half reported using inhalers. The use of inhalers differed significantly by socioeconomic status (39% and 61% for the lowest and the highest, respectively, p = 0.01). The frequency of inhaler use did not differ by gender or age. Among the individuals reporting emphysema and inhaler use, the use of the bronchodilator-corticosteroid combination was more common than was that of a bronchodilator alone. Only among the individuals reporting physician-diagnosed asthma and current symptoms was the proportion of inhaler users higher than 50%. CONCLUSIONS: In our sample, inhalers were underutilized, and the type of medication used by the individuals who reported emphysema does not seem to be in accordance with the consensus recommendations.

OBJETIVO: Avaliar as características dos usuários de dispositivos inalatórios e a prevalência de uso desses em adolescentes e adultos com diagnóstico médico autorreferido de asma, bronquite ou enfisema. MÉTODOS: Estudo de base populacional realizado em Pelotas, RS, incluindo 3.670 indivíduos com idade ≥ 10 anos, avaliados com um questionário. RESULTADOS: Aproximadamente 10% da amostra referiram pelo menos uma das doenças respiratórias investigadas. Entre esses, 59% apresentaram sintomas respiratórios no último ano, e, desses, apenas metade usou inaladores. O uso de inaladores diferiu significativamente de acordo com o nível socioeconômico (39% e 61% entre mais pobres e mais ricos, respectivamente; p = 0,01). Não houve diferença na frequência de uso de inaladores por sexo ou idade. Entre indivíduos com enfisema, o uso da combinação broncodilatador + corticoide inalatório foi mais frequente que o uso isolado de broncodilatador. Somente entre os indivíduos que referiram diagnóstico médico de asma e sintomas atuais, a proporção de uso de inaladores foi maior que 50%. CONCLUSÕES: Em nossa amostra, os inaladores foram subutilizados, e o tipo de medicamento usado por aqueles que referiram enfisema parece não estar de acordo com o preconizado em consensos sobre essa doença. .

Chronic inhalation of cigarette smoke (CS) induces emphysema by the damage contributed by oxidative stress during inhalation of CS. Ingestion of açai fruits (Euterpe oleracea) in animals has both antioxidant and anti-inflammatory effects. This study compared lung damage in mice induced by chronic (60-day) inhalation of regular CS and smoke from cigarettes containing 100mg of hydroalcoholic extract of açai berry stone (CS + A). Sham smoke-exposed mice served as the control group. Mice were sacrificed on day 60, bronchoalveolar lavage was performed, and the lungs were removed for histological and biochemical analyses. Histopathological investigation showed enlargement of alveolar space in CS mice compared to CS + A and control mice. The increase in leukocytes in the CS group was higher than the increase observed in the CS + A group. Oxidative stress, as evaluated by antioxidant enzyme activities, mieloperoxidase, glutathione, and 4-hydroxynonenal, was reduced in mice exposed to CS+A versus CS. Macrophage and neutrophil elastase levels were reduced in mice exposed to CS + A versus CS. Thus, the presence of açai extract in cigarettes had a protective effect against emphysema in mice, probably by reducing oxidative and inflammatory reactions. These results raise the possibility that addition of açaí extract to normal cigarettes could reduce their harmful effects.

Cigarette smoke (CS) induces emphysema by tissue destruction through the production of oxidants and metalloproteinases [matrix metalloproteinases (MMPs)]. The possibility of lung repair after emphysema remains unclear. Our aim was to study the effects of vitamins C and E on mouse lung repair evaluated by catalase (CAT), superoxide dismutase (SOD) and MMP-9 activities; by the amount of tumor necrosis factor (TNF)-alpha in lung homogenates; by cell quantification in bronchoalveolar lavage (BAL) fluid; and by lung histology. Male C57BL/6 mice (n=25) were exposed to nine cigarettes per day, 7 days/week, for 60 days in a whole-body exposure chamber. The control group was sham smoked (n=10). After 60 days of CS exposure, a group of animals was sacrificed (n=5) and the others were divided into two groups: (a) CSv (n=10) supplemented with saline and olive oil (vehicles) for 60 days and (b) CSr (n=10) supplemented with vitamins C and E (50 mg/kg/day) for 60 days. These mice were then sacrificed; BAL was performed and the lungs were removed for biochemical and histological analysis. The results demonstrated that CAT activity was decreased in the CSv and CSr groups compared to the control group. SOD activity was higher in the CSv group than in the control and CSr groups. The CSv group showed a higher neutrophil count in BAL fluid, associated with more TNF-alpha in lung homogenates, than the control or CSr groups. Finally, emphysema in the CSv group was associated with fewer collagen and elastic fibers than in the control and CSr groups. These results indicate a possible role of vitamins C and E in lung repair after emphysema induced by long-term CS exposure in mice.
