RESUMEN

OBJECTIVE: To demonstrate a novel technique used to restore cervical patency in a patient with severe iatrogenic cervical stenosis. DESIGN: Surgical video case report. SETTING: A single academic institution. PATIENT(S): We highlight the case of a 35-year-old nulliparous woman with a history of primary infertility. Her past medical history was significant for focal, invasive, well-differentiated squamous cell carcinoma of the cervix, for which she underwent a loop electrosurgical excision procedure. During her infertility assessment, she was found to have an extremely stenotic cervix that was refractory to conventional treatment options. INTERVENTIONS: This video highlights our innovative laparoscopic transfundal technique used to restore her cervical patency. MAIN OUTCOME MEASURES: None, as this is a descriptive case report. RESULTS: Postoperatively, the patient had continued cervical patency for >1 year with successful fertility treatment resulting in pregnancy. CONCLUSIONS: To our knowledge, this is the first case report describing a laparoscopic transfundal approach used to reestablish cervical patency. This approach may be considered for patients with cervical stenosis who have not responded to standard conservative therapies.

A esquistossomose mansoni, endemia encontrada em diferentes áreas do território brasileiro, constitui um grave problema de saúde pública no Brasil e no mundo. Formas ectópicas da doença podem acometer o sistema reprodutor feminino, constituindo um tipo raro de infecção pelo S. mansoni. Paciente com 26 anos queixava-se de corrimento vaginal, dispareunia e dor à palpação do hipogástrio. No exame ginecológico foi observado pólipo endocervical, que foi biopsiado. O exame histológico evidenciou vários granulomas envolvendo ovos degenerados e viáveis de Schistosoma mansoni. Tratada com praziquantel, retornou assintomática após quatro semanas do tratamento. Corrimento vaginal e dispareunia podem ser causas secundárias de cervicite por Schistosoma mansoni. Pesquisa de ovos em esfregaço vaginal de rotina ou exame histológico devem fazer parte da avaliação ginecológica de pacientes de área endêmica, com o objetivo de rastrear forma ectópica da esquistossomose do trato genital feminino.


Schistosomiasis mansoni is found in different endemic areas of Brazil. It is a serious public health problem in Brazil and worldwide. Ectopic forms of the disease may affect the female reproductive system, representing a rare type of Schistosoma mansoni infection. A 26-year-old patient complained of vaginal discharge, dyspareunia and pain on palpation of the hypogastrium. Gynecological examination revealed an endocervical polyp. A biopsy was performed. Under microscopy, several granulomas surrounding degenerate and viable eggs of Schistosoma mansoni were seen. Treated with praziquantel, she was asymptomatic after four weeks of treatment. Vaginal discharge and dyspareunia may be secondary causes of cervicitis caused by Schistosoma mansoni. The search for eggs in routine vaginal smear or histological examination should be part of the gynecologic evaluation of patients from endemic areas, with the purpose of tracking ectopic schistosomiasis of the female genital tract.

RESUMEN

Fortunately cervical pregnancy, an ectopic gestation that frequently represents obstetric urgency, is a rare entity. We present here the case of a woman who developed cervical pregnancy despite no risk factor associated with ectopic pregnancy was identified. The patient came to the office because of primary sterility of 4 years of evolution. Among her background she reported previous cervical surgery due to benign pathology. She reached pregnancy after medical and surgical treatment. In the week eleven of gestation, the patient presented to the hospital with clinical symptoms of abortion. An ultrasound revealed abdominal pregnancy. Laparotomy was "white", cervix was found enlarged with the external orifice closed, suggesting cervical pregnancy. Conservative treatment consisted in cervical evacuation, endometrial curettage and vaginal tampon. She evolved satisfactorily. We do not know about any previous report of cervical-pregnancy associated with cervical surgery and sterility. We also recognize the need to increase the number of similar cases to generalize results. Thus, it is important to highlight in this case the absence of known risk factors for cervical pregnancy and the background of cervical surgery and sterility. Therefore, we recommend to search for antecedents of cervical surgery and sterility when cervical pregnancy is suspected.

RESUMEN

O nevus composto do colo uterino apresenta um achado muito raro. Relatos excepcionais de lesoes melanocíticas benignas e malignas da endo e exocérvice uterina têm sido publicados. Relatamos o achado ocasional do nevus composto da exocérvice uterina em uma paciente de 47 anos de idade que nao apresentava queixas ginecológicas. O diagnóstico foi sugerido pelo exame colposcópico e confirmado à histopatologia. Devido à possibilidade de transformaçao maligna dessas lesoes e à dificuldade de seguimento da paciente, o tratamento foi concluido com a realizaçao de histerectomia total abdominal.
